A Case of Epidermal Cyst Using Surgical Method After Bufonis Venenum Pharmacopuncture Anesthesia (섬수약침 마취후 수술적으로 제거된 표피낭종 증례)

  • Objectives : The aim of this study is to report the effect of surgical method after Bufonis Venenum Pharmacopuncture Anesthesia on epidermal cyst. Methods : A 49-year-old male was presented with a $1.5{\times}1.5(cm)$ sized epidermal cyst on the upper back. Local anesthesia was achieved with Bufonis Venenum pharmacopuncture. After 5 minutes, 2.5cm incision was made using 11th blade. To minimize bleeding, the laser($CO_2$ Hani-maehwa laser-surgical mode) was excised to the cyst wall. After incision, cyst were separated and removed using adson forceps and iris scissors. Then buried intradermal suture and simple interrupted suture were performed. Yeonkyopaedok-san was administered for 5 days for anti-inflammatory effects. After 10 days, the suture was removed after confirmation of skin adhesion. Results : Cyst size measured $1.0{\times}1.0{\times}1.0(cm)$. Until the suture is removed, Adverse effects were not reported. Conclusions : Bufonis Venenum pharmacopuncture applies to local anesthesia. Korean medicine cautery method applies to laser($CO_2$ Hani-maehwa laser). It can be considered that To expand the application of surgical treatment in korean medicine.

A Case Report of 2 Types of Epidermal Cyst Surgically Removed after Bufonis Venenum Pharmacopuncture Anesthesia (섬수 약침 마취 후 수술로 제거한 2가지 유형의 표피낭종(epidermal cyst) 증례)

  • Objectives : The study is to report cases of open/closed types of epidermal cyst removed by surgical method with Bufonis Venenum pharmacopuncture. Methods : 2 patients were diagnosed each open/closed epidermal cyst. Local anesthesia was achieved with Bufonis Venenum pharmacopuncture. Incision was made using 15th blade and the CO2 Hani-maehwa laser. After cyst was removed, simple interrupted suture was performed. Eunkyo-san and Yeonkyopaedok-san were administered respectively for 7 days for anti-inflammatory effects. Results : Local anesthesia was maintained until epidermal cysts were clearly removed. The suture was removed after confirmation of skin adhesion. Adverse reactions were not reported. Conclusions : Epidermal cysts were removed surgically after Bufonis Venenum pharmacopuncture anesthesia and the risk of infection can be prevented by administering herbal medicines that have anti-inflammatory properties. It is difficult to visually diagnose closed type of epidermal cyst and then suggests that imaging equipment such as ultrasound is required.

Clinical Efficacy of the Ultrasonography in the Diagnosis of the Palpable Soft Tissue Masses (촉지되는 연부조직 종괴의 진단에서 초음파 검사의 유용성)

  • Purpose: To determine the clinical efficacy of the ultrasonography (US) for the diagnosis of the soft tissue masses comparing to the clinical diagnosis through the physical examination. Materials and Methods: 83 consecutive patients with palpable soft tissue masses were presented retrospectively. On the basis of the pathological diagnosis after the operation, the diagnostic accuracies were evaluated the cases diagnosed by history taking and physical examination, and the cases diagnosed by US before the operation. Results: The diagnostic accuracy of the cases using US was higher(86.7%) than that of the cases using physical examination and history taking. The accuracy of US of the epidermal inclusion cyst was 71.4%, ganglion cyst 85.7%, lipoma 88.6%, respectively. Accurate diagnosis were made 54 cases with 65.1% sensitivity in the cases diagnosed by history taking and physical examination. In this case, its accuracy of the ganglion cyst was 56.0%, and the lipoma was 67.9%. Conclusion: US is a useful diagnostic tool for the diagnosis of the palpable soft tissue masses before the operation.

Clinical Evaluation of Tissue Biopsy for Children with Neck Mass; A Single Center Study (조직검사를 시행한 소아 경부 종류의 임상적 고찰 : 단일 기관 연구)

  • Purpose : Neck masses, in pediatric population, derive from a multitude of congenital, inflammatory, or neoplastic diseases. The majority of these masses represent benign conditions. However, thorough clinical evaluation is required to rule out malignant diseases. We evaluated the causes, clinical characteristics and outcomes of children with neck masses who underwent tissue biopsy. Methods : A total of 28 medical records of children with neck mass who underwent tissue biopsy at Chungnam National University Hospital, from January 2000 to March 2004 were retrospectively analyzed. The methods of biopsy were ultrasonography guided core biopsy(CB), fine needle aspiration biopsy(FNAB) and excisional biopsy. Results : Out of 28 patients, half were boys. The most common location of the mass was the posterior cervical area(N=19, 67.9%). Laboratory findings of peripheral blood and serologic studies were nonspecific. In 25(89.3%) cases, CB or FNAB was initially performed for neck masses. Among them 10 cases(40%) were reactive hyperplasia, 8(32%) inflammatory granulation tissues, 4(16%) necrotizing lymphadenitis, and 3(12%) acute suppurative inflammations. Initially, excisional biopsy was performed for diagnosis in 3(10.7%) cases. Diagnosis of these cases was thyroglossal duct cyst, dermoid cyst and lymphoblastic lymphoma, respectively. Conclusion : Most neck masses in children were benign. CB and FNAB were safe methods for tissue sampling, without need for general anesthesia.

Postoperative Radiotherapy in Malignant Tumors of the Parotid Gland (이하선 악성종양의 수술 후 방사선 치료)

  • Purpose : This study was performed to analyze the factors affecting local control in malignant tumors of the parotid gland treated with surgery and postoperative radiation. Materials and methods : Twenty-six patients were treated for malignant tumors of the parotid gland from 1986 to 1995 at Department of Therapeutic Radiology, Chonnam University Hospital. Age of the patients ranged from 14 to 72 years (median : 55 years). Histologically 10 patients of mucoepidermoid carcinoma, 7 of squamous cell carcinoma, 4 of acinic cell carcinoma, 4 of adenoid cystic carcinoma and 1 of adenocarcinoma were treated. Total parotidectomy was performd in 15 of 26 patients, superficial in 7, subtotal in 4. Facial nerve was sacrificed in 5 patients. Postoperatively 4 patients had residual disease, 4 had positive resection margin. Radiation was delivered through an ipsilateral wedged pair of photon in 11 patients. High energy electron beam was mixed with photon in 15 patients. Electron beam dose ranged from 900 cGy to 3800 cGy (median 1700 cGy). Total radiation dose ranged from 5000 cGy to 7560 cGy (median : 6020 cGy). Minimum follow-up period was 2 years. Local control and survival rate were calculated using Kaplan-Meier method. Generalized Wilcoxon test and Cox proportional hazard model were used to test factors affecting local control. Results : Five (19$\%$) of 26 patients had local recurrence. Five year local control rate was 77$\%$. Overall five year survival rate was 70$\%$. Sex, age, tumor size, surgical involvement of cervical lymph node, involvement of resection margin, surgical invasion of nerve, and total dose were analyzed as suggested factors affecting local control rate. Among them patients with tumor size less than 4 cm (p=0.002) and negative resection margin (p=0.011) were associated with better local control rates in univariate analysis. Multivariate analysis showed only tumor size factor is associated with local control rate (p=0.022). Conclusion : This study suggested that tumor size is important in local control of malignant tumors of parotid gland.
