Applicability of American and European Spirometry Repeatability Criteria to Korean Adults (한국 성인을 대상으로 한 미국 및 유럽 폐활량 검사 재현성 기준의 유용성)

  • Background: The objective of this study was to evaluate the clinical applicability of the repeatability criteria recommended by the American Thoracic Society/European Respiratory Society (ATS/ERS) spirometry guidelines and to determine which factors affect the repeatability of spirometry in Korean adults. Methods: We reviewed the spirometry data of 4,663 Korean adults from the Korean National Health and Nutritional Examination Survey (KNHANES) Chronic Obstructive Pulmonary Disease Cohort (COPD cohort) and the Community-based Cohort Study VI-Fishing village/Islands (community cohort). We measured the anthropometric factors and differences between the highest and second-highest FVC (dFVC) and $FEV_1$ ($dFEV_1$) from prebronchodilator spirometry. Analyses included the distribution of dFVC and $dFEV_1$, comparison of the values meeting the 1994 ATS repeatability criteria with the values meeting the 2005 ATS/ERS repeatability criteria, and the performance of linear regression for evaluating the influence of subject characteristics and the change of criteria on the spiro-metric variability. Results: About 95% of subjects were able to reproduce FVC and $FEV_1$ within 150 ml. The KNHANES based on the 1994 ATS guidelines showed poorer repeatability than the COPD cohort and community cohort based on the 2005 ATS/ERS guidelines. Demographic and anthropometric factors had little effect on repeatability, explaining only 0.5 to 3%. Conclusion: We conclude that the new spirometry repeatability criteria recommended by the 2005 ATS/ERS guidelines is also applicable to Korean adults. The repeatability of spirometry depends little on individual characteristics when an experienced technician performs testing. Therefore, we suggest that sustained efforts for public awareness of new repeatability criteria, quality control of spirograms, and education of personnel are needed for reliable spirometric results.

Significance Evaluation of Lung Volume and Pulmonary Dysfunction (폐용적과 폐기능 환기장애에 대한 유의성 평가)

  • To In this study, we sought to evaluate related factors affecting lung volume and their significance in pulmonary function and ventilation disorders. As experimental subjects, 206 normal adult men and women who underwent a low-dose chest CT scan and a spirometry test were selected at the same time. The experimental method was to measure lung volume using lung CT images obtained through a low-dose chest CT scan using deep learning-based AVIEW. Measurements were made using the LCS automatic diagnosis program. In addition, the results of measuring lung function were obtained using a spirometer, and gender and BMI were selected as related factors that affect lung volume, and significance was evaluated through an independent sample T-test with lung volume. As a result of the experiment, it was confirmed that in evaluating lung volume according to gender, all lung volumes of men were larger than all lung volumes of women. he result of an independent samples T-test using the respective average values for gender and lung volume showed that all lung volumes were larger in men than in women, which was significant (p<0.001). And in the evaluation of lung volume according to BMI index, it was confirmed that all lung volumes of adults with a BMI index of 24 or higher were larger than all lung volumes of adults with a BMI index of less than 24. However, the independent samples T-test using the respective average values for BMI index and lung volume did not show a significant result that all lung volumes were larger in BMI index 24 or higher than in BMI index less than 24 (p<0.055). In the evaluation of lung volume according to the presence or absence of pulmonary ventilation impairment, it was confirmed that all lung volumes of adults with normal pulmonary function ventilation were larger than all lung volumes of adults with pulmonary ventilation impairment. And as a result of the independent sample T-test using the respective average values for the presence or absence of pulmonary ventilation disorder and lung volume, the result was significant that all lung volumes were larger in adults with normal pulmonary function ventilation than in adults with pulmonary function ventilation disorder (p <0.001). Lung volume and spirometry test results are the most important indicators in evaluating lung health, and using these two indicators together to evaluate lung function is the most accurate evaluation method. Therefore, it is expected that this study will be used as basic data by presenting the average lung volume for adults with normal ventilation and adults with impaired lung function and ventilation in similar future studies on lung volume and vital capacity testing.

  • Pulmonary function test (PFT) is a test method to determine respiratory disease. In order to obtain accurate PFT results, it is absolutely necessary to induce the inspector and cooperate with the patient. This study was to observe the importance of understanding and posture of the patient in spirometry. In 2016, 110 healthy experimenter performed spirometry; 1) only heard the explanation, 2) watching video and inspector,s demonstration, 3) twisting legs and bending shoulder. FVC, $FEV_1$, $FEV_1/FVC$, $FEF_{25-75%}$, PEF were measured by spirometry. FVC, $FEV_1$, $FEV_1/FVC$, $FEF_{25-75%}$, PEF were significantly increased before and after the understanding the test method. There was a significant difference in FVC, $FEV_1$, and PEF in the false posture. Reproducibility was significantly different in the experimenter' comprehension and false posture. This study provides accurate understanding of the patient and correct posture should be maintained during the examination to obtain correct and reproducible results of PFT.

Pulmonary function is related to basic physical fitness and physical activity in college students (대학생의 폐활량에 대한 기초체력과 신체활동량의 관계)

  • The purpose of this study was to analyze the correlation between pulmonary function, basic physical fitness (PF), and physical activity (PA), and to compare the differences by gender in Korean college students. Measurements of body composition, basic PF, PA (questionnaire), and pulmonary function tests of forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) using a Quark pulmonary function test were carried out on 312 healthy participants (150 males [mean age: 19.29±1.72 years] and 162 females [mean age: 19.05±1.17 years]). The pulmonary function of male students was related to right-handedness, left-handedness, and back strength, and the pulmonary function of female students was related to all basic PF. The pulmonary function of male students was related to all PA variables, whereas the pulmonary function of female students was related to middle-intensity PA. The findings of this study suggest that male students need to increase PA, and female students need to improve basic PF to sustain a healthy pulmonary function. Understand gender differences for pulmonary function-related factors and the gender-specific educational efforts are needed to improve and maintain pulmonary capacity in college students.

The Effects on the Pulmonary Function of 20's Females Proprioceptive Neuromuscular Facilitation Respiration Pattern Convergence Exercise by Sitting Position (앉은 자세각도에 따른 PNF를 융합한 호흡운동이 20대 여대생의 폐활량에 대한 비교)

  • The purpose of this study was to determine whether PNF respiratory convergence exercise increases the pulmonary function of normal adults by $45^{\circ}$ leaning sitting position. Twenty normal adults in their 20s were randomly assigned to an experimental group (n=10) or control group (n=10). Over the course of four weeks, the experimental group participated in PNF respiration exercises by $45^{\circ}$ leaning position for 30 minutes three per week. And the control group participated in PNF respiration exercises by supine position for 30 minutes three times per week. Subjects were assessed pre-test and post-test by measurement of pulmonary function. Our findings show that the experimental group had significant improvements in expiratoy reserve volume and vital capacity, maximal voluntary volume. In this study, the experimental group showed greater improvement in pulmonary function than the control group, which indicates that the PNF respiratory exercise by $45^{\circ}$ leaning sitting position is effective at increasing the pulmonary function of normal adults.

The Effects of Danjeon Breathing Exercise on Vital Capacity, Physical Fitness, Anxiety and Depression among Older Adults (단전호흡운동 프로그램이 노인의 폐활량, 체력, 불안 및 우울에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of Danjeon Breathing Exercise (DBE) on vital capacity, physical fitness, anxiety and depression among older adults. Methods: This study employed the nonequivalent control group pretest-posttest design. The experimental group participated in a 12-week DBE program. Vital capacity and physical fitness were measured with a health measurement system (HELMAS). Anxiety and depression were measured with SCL-90-R-K. Data were collected from 37 community-dwelling older adults (experimental group=21, control group=16) in the Seoul metropolitan area. The data were analyzed by $x^2$-test, Mann-Whitney test and ANCOVA. Results: The experimental group taking DBE reported significant increases in flexibility than the control group, but differences in vital capacity and balance were not significant. Older adults taking DBE showed significant decreases in the anxiety and depression levels. Conclusion: A DBE program may be a useful nursing intervention for older adults to improve their flexibility and to reduce anxiety and depression.

Effect of Aquatic Exercise on Body Composition, Fitness and Pulmonary Function of Spasticity Cerebral Palsy (수중운동이 경련성 뇌성마비인의 신체구성, 체력 및 폐기능에 미치는 영향)

  • The purpose of this study was to evaluate the effect of aquatic exercise on body composition, fitness and pulmonary function. Subjects were seven spasticity cerebral palsy(18-20years). The aquatic exercise program consisted of 11 items, and performed for 30-40mins in a bout, 4 times a week at the intensity of HRmax($40-65\%$) for 12 weeks. And the change of body weight, %fat, fat mass, hand grip, sit up, sit and reach, close eyes foot balance and side step have been measured before and post exercise 12 weeks. Paired t-test was performed for data analysis at the 0.051eve1 of significance. The following results as; First, body compositions(body weight, $\%$fat and fat mass) were decreased significantly after 12 weeks(p< .05). Second, muscle endurance, balance and flexibility were increased significantly after 12 weeks(p< .05, p< .01), respectively. Third, vital capacity was increased significantly after 12 weeks(p< .05). These results suggest that the aquatic exercise programs are applicable to the spasticity cerebral palsy with abnormal movement.

The Relationship between Physical Characteristics and Walking Ability in Elderly: A Cross-Sectional Study (노인들의 보행 능력과 신체적인 특성 간의 상관관계: 단면 연구)

  • This study aimed to investigate the relationship between physical characteristics and walking ability in the elderly population. Subjects were 77 elderly (38 men and 39 women) who are capable of walking independently with and without walking aids. Correlation and stepwise multiple linear regression analyses were used to analyze the relationship between physical characteristics (age, gender, height, weight, body mass index, muscle mass, waist/hip ratio, heart rate, vital capacity, flexibility, maximum oxygen consumption, one-leg standing time, and strength of knee flexor and extensor) and walking velocity of subjects. Age, height, vital capacity, one-leg standing time, and strength of knee flexor and extensor showed significant correlations with walking velocity of subjects (p<.05). Further, the strength of knee flexor explained 27% of the variance, and up to 32% of the walking velocity could be explained when the strength of knee extensor were added to the model. The findings suggest that walking velocity of elderly depends on the strength of lower limb's strength and a variety of physical characteristics.

Effect of Implementing Candle-Blowing Respiratory Exercise Program on Functional Improvement of Forced Breathing Volume among Adults (촛불끄기 호흡운동 프로그램의 시행이 성인의 노력성 호흡량의 기능향상에 미치는 영향)

  • This study examined the effect of the respiratory training centered at forced breathing exercise of blowing out a candle on the improvement of adults' respiratory ability, which means the increased muscle strength of body trunk. After the four-week candle-blowing breathing training, the forced vital capacity (FVC) increased statistically significantly (p<.05). The forced expiratory volume in one second (FEV1) did not show a statistically significant difference before and after the four-week intervention(p>.05). The peak expiratory flow rate (PEF) statistically significantly increased after the four-week blowing-out-the-candle training (p<.05). This study examined the relationship between forced breathing training and pulmonary function of healthy adults including FVC, FEV1, and PEF, which means the increased muscle strength of body trunk, by implementing blowing-out-the-candle breathing exercise centered at forced respiratory exercise and obtained significant results. Further studies that use a sample of patients with advanced respiratory system disease for whom physical therapy is absolutely necessary will be required in the future.

Recovery of Pulmonary Function according to the Operative Sites after General Anesthesia (전신마취를 이용한 수술에서 수술부위에 따른 폐기능의 회복)

  • Background: After general anesthesia, decrease of functional residual capacity and lung compliance, ventilation/perfusion imbalance, and transpulmonary shunting can provoke hypoxemia during postoperative periods. Diaphragmatic dysfunction may be the main cause of these physiological abnormalities. Thus, we evaluated the change of pulmonary function after general anesthesia according to the operative sites, which could suggest clinical course and critical period of respiratory care of postoperative patients. Method: Preoperative portable spirometric evaluation and arterial blood gas analysis were performed at sitting or most-sitting position just previous day of surgery. Pulmonary function tests were also as same condition from postoperative day 1 to day 5. Results: 1) For thoracic surgery, FEV1 and FVC were not recovered at day 5, but FEV1/FVC was not decreased. $PaCO_2$ was slightly elevated at postoperative one day. 2) After upper abdominal surgery, postoperative day 5 did not show the recovery of FEV1 and FVC, but mild hypoxemia was developed at postoperative day 1. 3) Pulmonary function was recovered as preoperative value at postoperative day 5 in lower abdominal operation, but mild hypoxemia was also noted at postoperative day 1. 4) Surgery of peripheral areas did not show significant pulmonary function change and hypoxemia and hypercapnia from postoperative day 1. Conclusion: Surgery involving diaphragm provoke significant postoperative pulmonary function change after day 5. For the operation of peripheral sites adequate respiratory care during operation and postoperative period within 24 hours could prevent patients from respiratory complication.
