Evaluation of the Cause of Internal Jugular Vein Obstruction on Head and Neck Contrast Enhanced 3D MR Angiography Using Contrast Enhanced Computed Tomography (조영증강 전산화단층촬영술을 이용한 두경부 조영증강 삼차원 자기공명혈관촬영술에서 내경정맥 폐쇄의 원인 평가)

  • Purpose : To evaluate the cause of internal jugular vein (IJV) obstruction on contrast enhanced 3D MR angiography (CE-MRA) using contrast enhanced computed tomography (CE-CT). Materials and Methods : A total number of 30 patients were enrolled, who underwent both head and neck CE-MRA and CE-CT from 2005 to 2008. We defined obstruction group which had IJV obstruction and control group which had no IJV obstruction on CE-MRA. The following parameters were measured from axial images of CE-CT: 1) diameter of IJV; 2) distance between the styloid process and ipsilateral lateral mass of the atlas; 3) maximum area of lateral mass of the atlas. Each parameter was compared between obstruction group and control group. Results : The diameter of IJV and distance between the styloid process and lateral mass of the atlas at IJV obstruction side in obstruction group were $1.6{\pm}1.0\;mm$ and $4.1{\pm}2.1\;mm$ respectively, which resulted in statistical significance (p<0.01). The maximum area of lateral mass of the atlas at IJV obstruction side in obstruction group was $103.4{\pm}25.3\;mm^2$ which is significantly larger than in control group (p<0.05). Conclusion : We found that the cause of IJV obstruction on CE-MRA could be narrow space between the styloid process and the lateral mass of the atlas, which was related with asymmetric larger area of lateral mass of atlas.

The Value of Tc-99m DTPA Diuretic Renography for Assessment of Dilated Upper Urinary Tract in Children (소아의 상부 요관확장증에서 폐쇄 유무 감별에 있어 Tc-99m DTPA 이뇨 신장 신티그램의 유용성)

  • Purpose: The purpose of this study was to evaluate the accuracy of Tc-99m DTPA diuretic renal scans in children with dilated upper urinary tract. Materials and Methods: We reviewed diuretic renal scans of 14 pediatric patients (age range: 3 days to 4 years) with unilateral hydronephrosis diagnosed by ultrasonography. Diuretic renal scan was done using Tc-99m DTPA and standardized protocol. In 3 neonates, diuretic renal scans were performed within 1 week and 3-7 months after birth. Results: Six patients required Pyeloplasty and eight were managed conservatively. All 6 patients requiring Pyeloplasty were diagnosed as having ureteropelvic junction obstruction in the diuretic renal scan. In these 6 patients, post-operative renal scans at 3-12 months after surgery were converted to nonobstructive pattern in 5 and a nonfunctioning pattern in 1. In 3 patients who underwent diuretic renal scan within 1 week after birth, nonobstructive patterns of initial scan were converted to obstructive patterns in the follow-up scan. However, all patients with nonobstructive diuretic renal scans performed after the neonatal period did well on serial ultrasonography and showed favorable clinical outcome without progression to obstruction. Conclusion: Tc-99m DTPA diuretic renal scan with standardized protocol is useful in assessing suspected ureteropelvic junction obstruction in children as an initial diagnostic or post-operative follow-up modality. Nonobstructive or indeterminate scan results in the neonatal period requires follow-up scan to monitor development of the obstructive pattern.

  • Sleep plays an important role in maintaining overall human health. There is increasing interest regarding the impact of sleep related disorders on metabolic diseases. Obstructive sleep apnea (OSA) is a common health problem, and in the last decade, the emergence of increasing obesity rates has further led to a remarkable increase in the prevalence of OSA, along with more prominent metabolic diseases. Obesity is the strongest risk factor for OSA. However, OSA is also known to cause obesity, suggesting an interaction between OSA and obesity. Although the underlying mechanisms leading to OSA-induced metabolic diseases are probably multi-factorial and are yet to be fully elucidated, the activation of inflammation and oxidative stress and the dysregulation of appetite-regulating hormones have emerged as important pathophysiological components of metabolic dysfunction and obesity observed in patients with OSA. Here, we will review the current state of research regarding the association of OSA with metabolic diseases and the possible pathophysiological mechanisms by which OSA could lead to such diseases. This will enhance our understanding of the potential interactions between OSA and obesity and between OSA and metabolic dysfunction.

Analysis of Vegetation Structure with The Types of Abandoned Channels in The Mangyeong River (만경강 폐천 유형별 식생구조 분석)

  • 본 연구는 현재와 과거의 하도 지형자료 분석을 통하여 만경강에서 나타나는 폐천(Abandoned channel)을 파악하고, 유형을 폐쇄형, 개방형, 육화형 폐천으로 구분하여 각각의 본류구간과 함께 식물상 및 식생분포 특성을 비교하였다. 만경강의 경우 1918년 하도 형태가 경지정리 및 치수사업에 따른 제방축조 등 인위적인 절단(Engineered cutoff)으로 8개소 이상의 구간에서 하도 선형이 크게 변경됨으로써 폐천이 형성되는 것으로 나타났다. 현재 폐천부지는 육화형태의 농지 이용 및 개방형 또는 폐쇄형의 습지로 유지되는 것으로 조사되었다. 폐천 유형에 따라 나타난 식물상은 폐쇄형 56종류, 개방형 31종류로 조사되었다. 육화형의 본류의 경우 보에 의한 영향으로 달뿌리풀 (Phragmites japonica)과 저수로 검정말(Hydrilla verticillata) 등의 수생식물이 우점하는 정체수역이 나타났다. 개방형 폐천은 갈수기에도 수심이 깊은 수역이 확보되어 본류에 비하여 출현종수가 높았다. 폐쇄형 폐천은 연꽃(Nelumbo nucifera), 애기부들(Typha angustifolia) 등의 다양한 수생식물이 출현하여 본류에 비해 안정된 습지 생태계를 유지하였다. 만경강의 개방형과 폐쇄형 폐천은 수생식물의 증가와 전형적인 습지생태계를 유지하고 있었으며 본류의 정체수역이 관찰되었다. 본래 하천의 연속성을 향상하기 위한 유수생태계의 복원은 하천의 건강성을 향상하기 위한 방안이 될 것이다.

  • This paper presents the diagnostic method for fault prevention in metal clad switchgear(MCS) through comparison of signals before and after detecting the partial discharge using electromagnetic detection technique. Electromagnetic waves detected by antennas of the inside and outside of MCS are analyzed and compared by frequency spectrum analysis method which can estimate an insulation abnormality and normality of MCS. As a result of the experiment by the proposed method, we can detect the insulation abnormality as partial discharge in MCS and these results can be applied to preventive diagnosis of MCS.

A Case of Localized Subaortic Stenosis Associated with Aortic Regurgitation (대동맥판폐쇄부전을 동반한 국소성)

  • The localized form of subaortic stenosis shows a spectrum of pathological lesions varing from dis- crete membrane to a thickened flbromuscular collar. Aortic valve is commonly involved late in the pro- cess resulting in regurgitation. Because of the likelihood o the progressive obstruction and aortic regurgitation, early elective oper- ation should be considered for the patient with subaortic stenosis. We experienced a case of localized fibromuscular subaortic stenosis associated with aortic regurgi- tation. Excision of the fibromuscular ridge and septal myectomy-myotomy relieved the subaortic seen- osis. Regurgitant aortic valve was repaired by peeling away the Hbrotic tissue on the cusps and subcommissural annuloplasty at the each commissural area. On postoperative echocardiographic examination, the systolic pressure gradient between left ventricle and aorta decreased markedly and the aortic regurgitation was not detected at all.

  • Anterior papillary muscle rupture caused by occlusion of the first diagonal branch is reported to be very rare. A 66-year-old woman was transferred to our hospital for acute myocardial infarction and cardiogenic shock. Echocardiography and angiography revealed severe mitral regurgitation caused by rupture of the anterior papillary muscle and the complete occlusion of the first diagonal branch. In an emergent setting, she had been successfully treated by mitral valve replacement and coronary artery bypass grafting. She was discharged on postoperative twelfth day without any event.

  • Preprocessing in speech recognition system is useful, for it reduces some of the complicated procedures required for the final recognition. In this paper, we suggest a new preprocessing algorithm for detecting the intervals of implosive stop sounds. Implosive stop sounds follow vowels in Korean language, and its characteristic is included in the region of vowels. When an implosive stop is pronounced, the velum is quickly colsed, thus its energy decays abruptly and the closure lasts for about 50 to 150 msec. The enegy difference between adjacent frames is chosen as a parameter which represents well the above features.

  • Active contour model or snake is widely used for segmentation method in the area of the image processing and computer vision. The main problem in the active contour model is that results are very dependent to the closet points of the numbers and the location in initial step. Especially, in case of balloon-like active contour model, the small region which consist of intial closet points are expanded until the edge is reached. It is a serious problem because the considered region are huge with limited points. To solve this problem, in this paper, we propose the method that the number of closet points could be change based on the distance between points.
