Invasive Pulmonary Aspergillosis in a Whooper Swan (Cygnus c. cygnus) (큰고니에서 발생한 침습성 폐 아스퍼질러스 감염증 1례)

  • Aspegillosis in free-living birds can be occurred mostly under poor weather and climate. But, captive birds tend to be more susceptible to infection and diagnosis was made usually at post mortem. A two months old Whooper swan(Cygnus c. cygmus) dying suddenly was found in a zoo without prior clinical signs. At necropsy, numerous well- demarcated yellow to white firm nodules were scattered throughout the air sacs and the lungs. Microscopically, granuloma formations were observed in the lung and air sacs. The margin of granuloma was surrounded by connective tissue barrier and was infiltrated with lymphocytes, and also observed giant cell near the granuloma. By Periodic acid Schiff reaction, hyphae were detected in granuloma of lung and air sacs. This case was diagnosed as an invasive pulmonary aspergillosis caused by Aspergillus fumigatus infection in a Whooper swan at a zoo.

  • Streptococcus mitis is a Gram-positive, facultative anaerobic, alpha-hemolytic, and nonmotile cocci. S. mitis is a viridans streptococci and a normal commensal of throat, nasopharynx, and mouse and is associated with infective endocarditis, septicemia, and meningitis. S. mitis KCOM 1350 (= ChDC B183) was isolated from a human mandibular osteomyelitis lesion. Here, we present the complete genome sequence of S. mitis KCOM 1350.

  • Foreign body in the esophagus is not uncommom in the otolaryngological field, but esophageal perforation followed by pneumothrax due to esophageal foreign body is very rare. Authors recently experienced such a case developed in 1 year old male baby. This baby had been treated at local clinic for 2 weeks prior to admission under the impression of U.R.I.. Thereafter foreign body in the esophagus (fine wire pin) with left pneumothorax was detected by chest X-ray and the body was transfered to our hospital. Closed chest tube was inserted on left and under the general anesthesia, foreign body was removed by esophagoscopy. He was discharged on 16th postoperative day uneventfully.

  • Influenza, commonly known as "the flu", is an infectious disease caused by the influenza virus. We consider, in this paper, regression models as a prediction model of influenza disease. While most of previous researches use mainly the meteorological variables as a predictive variables, we consider social media information in the models. As a result, we found that the contributions of two-type of informations are comparable. We used the medical treatment data of influenza provided by Natioal Health Insurance Survice (NHIS) and the meteorological data provided by Korea Meteorological Administration (KMA). We collect social media information (twitter buzz amount) from Twitter. Time series model is also considered for comparison.

  • A canine patient exhibited partial anorexia and sudden respiratory distress. Diagnostic imaging and cytology of tracheal-lavage fluids revealed fungal pneumonia. Molecular identification and phylogenetic analysis detected Candida catenulata. Treatment with oral itraconazole for 3 weeks was effective. This is the first report of C. catenulata infection in a dog.

  • Necrotizing bronchial aspergillosis usually occurs in the immumocompromised host. Aspergillus invades bronchial epithelium and forms endobronchial mass or endobronchial stenosis. A 78-year-old male patient with diabetus mellitus complaining of dyspnea and cough was admitted to our hospital. Plain chest X-ray and chest computed tomogram showed a large endobronchial mass and total collapse of left upper lobe of the lung. Bronchoscopic biopsy of the endobronchial mass revealed chronic inflammation. To confirm the endobronchial mass, we performed sleeve lobectomy of left upper lobe of the lung. Histologically the mass was diagnosed as necrotizing bronchial aspergillosis. We report a case of necrotizing bronchial aspergillosis in an elderly man who has diabetus mellitus with review of the literature.

  • Lemierre syndrome is characterized by an acute oropharyngeal infection with secondary septic thrombophlebitis of the internal jugular vein and frequent metastatic infections such as septic pulmonary emboli and suppurative arthritis. In the preantibiotic era, this condition generally had a fatal outcome. The presentation is so distinctive that a clinical diagnosis is possible in most cases, and a cure is expected with the appropriate therapy in the majority of patients. We present a case report of Lemierre syndrome with a review of the relevant literature.

  • The clinical manifestation of pericardial disease is similar to that of coronary artery disease and aortic disease. Therefore, a timely and accurate diagnosis is necessary. The pericardium is a 2-layered membrane that envelops the heart and great vessels, and there are numerous anatomic variations and pathologic conditions. Large or unusually located pericardial recesses can be easily mistaken for abnormal findings. Additionally, primary pericardial diseases resulting from infections, tumors, and injuries are possible; further, diseases can quickly spread along the pericardium. Echocardiography is generally the first imaging tool used to evaluate the pericardium. However, it has limited windows and poor resolution. Besides, the evaluation of postoperative echocardiography is sometimes limited. Currently, CT and MR imaging are useful for evaluating pericardial diseases. Detailed knowledge of the pericardium is important for interpreting the images and clinical results.

PCR in diagnosis of pneumocystosis of rats (중합반응을 이용한 흰쥐 페포자충증의 진단)

  • Polymerase chain reaction (PCR) is a powerful technique to detect scanty amount of DNA from living organisms. The present study intended to develope specific primers for PCR diagnosis of pneumocystosis and to evaluate diagnostic efficacy by preparation of template DNAs from invasive BAk fluid and also to screen serum or blood as a non-invasive specimen. Albino rats of Wistar or Fischer strains were experimentally infected by Pneumocwstis ccrinii. Extracted DNAs or cell Iysates of their blood, bronchoalveolar lavage fluid, and lung homogenate were used as the tenlplate DNA. Primers were synthetic oligonucleotides among 16s rDNA sequences. All of the primer combinations gave PCR products, but the primer pair of #24 and #27 gave best quality product of 666 bp. The sensitivity of PCR with Iysates of BAk fluid was 57.7% but it increased to 84.6% with extracted DNAs. None of BAL Iysate or DNA was positive among 13 microscopically negatives. The serum DNAs were positive only in 2 cases out of 20 morphologically positive rats. DNAs of human, rat, other parasites, yeast, and microorganisms were negative. The findings suggest that the present primers are specific but simple Iysate of BAL fluid is not sensitive. PCR may be used as a routine diagnostic method of pneumocystosis if simple and rapid preparation of non-invasive clinical specimens are available.

Prevalence and Expression Pattern of Cytokines in Porcine Respiratory Disease Complex (PRDC) (돼지호흡기복합증후군(Porcine respiratory disease complex, PRDC)에 대한 발생상황의 분석 및 cytokine의 변화)

  • Porcine respiratory disease complex (PRDC) is a common respiratory disease in nursery and grow-finishing pigs. A complex of viral and bacterial agents is known to be involved in the etiology of PRDC. The purpose of this study was to investigate common etiologic agents associated with PRDC in the field and compare detection methods for identifying these agents. To understand the mechanism of polymicrobial infection in PRDC, changes in the expression of cytokines were investigated. In 461 pig samples examined, most of the affected pigs ranged from 3 to 10 weeks old (73.4%), and 348 (75.4%) samples were confirmed as polymicrobial infection. Of the polymicrobial-infected cases, two (50.3%), three (32.2%), four (13.8%), five (3.2%), and six (0.5%) agents were detected. Two- or three-agent infections were the most common, with PRRSV/PCV-2 (44.6%) the most common two-agent infection. PRRSV/PCV-2/H. parasuis (11.0%) was the most common three-agent infection. Comparison of two detection methods (PCR and IHC) in the polymicrobial cases showed that 78.4% were PCV-2 positive with the PCR method, and 26.2% were PCV-2 positive with IHC. SIV was 7.8% by the PCR method and 3.7% positive by the IHC. This result indicates that the PCR method is more useful than IHC for detecting causative agents in PRDC. In the analysis of cytokines in the two- and three-agent infected samples, interleukin (IL)-$1{\alpha}$, IL-2, IL-4, IL-6, IL-10, and INF-${\alpha}$ showed the same expression pattern. All cytokines were suppressed, except IL-6. These findings indicate that changes in cytokine expression could be used to understand the mechanism of polymicrobial infection in PRDC.
