THE EFFECT OF LIP ADHESION ON ROTATION-ADVANCEMENT REPAIR IN UNILATERAL COMPLETE CLEFT LIP (구순접합술이 회전신전법에 의한 편측성 완전 구순열 수복에 미치는 영향)

  • The present study was carried out to evaluate the postoperative results of Millard rotation-advancement repair (MR) and lip adhesion followed by Millard rotation-advancement repair (LAMR) in unilateral complete cleft lip. Twenty patients with unilateral complete cleft lip underwent MR or LAMR at the Department of Oral and Maxillofacial Surgery of Chonnam University Hospital over a period of 6 years (January 1994 to December 1999) were analyzed. The surgical results following the operation were assessed on the basis of scoring, vertical lip length, and scar hypertrophy. The mean score was better in LAMR group ($74.74{\pm}1.09$, n=13) than in MR group ($66.50{\pm}1.14$, n=7) for both lip and nose segments. Scar hypertrophy developed in MR group with 28.6% and in LAMR group with 23.1%. No significant difference was noted in the ratio of lip length between LAMR and MR groups ($0.84{\pm}0.08\;and\;0.73{\pm}0.10$). These results suggest that LAMR is better than MR in repairing the unilateral complete cleft lip.

  • The goals of this study were to present presurgical naso-alveolar molding (PNAM) appliance in unilateral cleft lip and palate treatment and to evaluate the effects of PNAM appliance on alveolar molding. Samples were consisted of 4 unilateral cleft lip and palate infants (3 males and 1 female, mean age=23.2 days after birth) who were treated with PNAM appliances in Department of Orthodontics, Seoul National University Dental Hospital. Average alveolar cleft gap between the greater and lesser segment was 8.27mm and average duration of alveolar molding treatment was 9.7 weeks. These patients' models were obtained at initial visit (T1) and alter successful alveolar molding (T2). Seven linear and five angular variables were measured by using photometry and digital caliper. All statistical analyses were performed by SPSS win ver. 7.5 program. Paired t-test was used to compare the mean values. 1. The posterior part of alveolar segments are the stable structures during alveolar molding treatment period in infants. 2. Forward growth of the greater segment may be hindered by the action of alveolar molding. 3. The closure of cleft gap during alveolar molding were usually due to inward and backward bending of the anterior part of the greater segment and outward bending of the whole lesser segment.

Suspected Unilateral Renal Dysplasia in a Cat with Intermittent Hematuria (혈뇨를 보이는 고양이의 편측성 신장이형성 증례)

  • A 6-month-old, castrated male, Korean short-haired cat was referred with intermittent hematuria. Physical and laboratory examination including complete blood count and serum biochemistry profiles showed no specific findings. Ultrasonography revealed a cystic lesion of the mid-abdomen, which was approximately $2{\times}2cm$ in size. FNA cytology of cystic fluid showed numerous cholesterol crystals of varying size and shape and granulomatous inflammation. CT examination showed fluid-filled large cystic lesion in the left side of the abdomen. Taken together, this case was diagnosed as unilateral (left) renal dysplasia and cystic inflammation led by cholesterol crystallization in the left kidney.

  • Complications resulting from condylar fracture include occlusal disturbance due to loss of leverage from temporomandibular joint (TMJ). In general, closed reduction with active physical training has been performed, and under favorable circumstances, adaptation occurs in attempt to restore the articulation. The patient in this case report had unilateral condylar fracture accompanied with multiple teeth injuries, but he was left without any dental treatment for 1 mon which led to unrestorable occlusal collapse. Fortunately, delayed surgical repositioning of dislocated maxillary anterior teeth followed by consistent long-term physical training has been proved successful. Normal occlusion and satisfactory remodeling of condyle were obtained on 10 mon follow-up.

AN EXPERIENCE OF UNILATERAL INCOMPLETE CLEFT LIP REPAIR BY USING BARDACH'S TRIANGULAR FLAP (Bardach 삼각피판법을 이용한 편측성 불완전 구순열의 수복 경험)

  • Tennison was the first to recognize and to preserve the Cupid's bow by lowering the peak in the margin of the cleft. Randall had modified the Tennison's repair based on accurate measurements. Bardach's technique evolved from the basic concept of triangular flap cleft lip repair described by Tennison-Randall method. Precise measurements are used to define the dimensions of the equilateral triangular flap, which is created on the cleft side and is inserted into an equilateral triangular defect on the noncleft side. Two symmetrical vertical distances on either side of the cleft are thus formed. It is essential that the incisions in the skin correspond precisely with those on the muscles and mucosa, and that all layers are sutured with the use of the triangular flap, thus preventing vertical scar contracture. This procedure produces a symmetric, balance lip with a well-defined Cupid's bow, a symmetric vermilion, and a properly aligned orbicularis oris muscle. We had treated three patients with unilateral incomplete cleft lip by using Bardach's triangular flap method. The operation scars could be reduced comparing to Millard method because Bardach's method did not use the columella base and the alar base incision. And the flap design was more simple and accurate comparing to Tennison-Randall method. On the other hand, the postoperative scars on the philtrum pointed as a disadvantage of triangular flap method were cosmetically acceptable because the three patients had incomplete cleft lip. We have experienced that Bardach's triangular flap is a recommendable technique for the repair of unilateral incomplete cleft lip.

THE MEDIAL SIGMOID DEPRESSION: Its Anatomic and Radiographic Considerations (하악골과 방사선사진상에서의 하악절흔 내측 함요)

  • An anatomic radiolucency called medial sigmoid depression can be observed on a number of panoramic and mandibular oblique lateral views in the ramus just below and anterior to the mandibular sigmoid notch. The radiolucency may be unilateral or bilateral. Seventy-eight mandibles of dry skulls were visually examined and radiographed by panoramic machine. The observable incidence, the location center, and the mean size of the medial sigmoid depression were studied. Additionally, the radiographic incidence of the depression was determined using 500 panoramic radiographs. Normal anatomic radiolucent areas in the jaws are encountered often in the radiographs taken for the usual dental practice purpose. Some of these radiolucencies can be misdiagnosed as pathologic entities. But in most cases, the size, location, and appearance of the radiolucent area, and the presence or absence of symptoms can lead one to the correct diagnosis before surgery is undertaken. And this can be secured if diagnosticians are aware of the common and varied appearances of the normal anatomic radiolucencies A depression in the ramus just below and slightly anterior to the most inferior aspect of mandibular sigmoid notch shows occasionally foramen-like or notch-like radiolucency on some panoramic and oblique lateral views. This radiolucency was named medial sigmoid depression by Langlais et a1. in 1983. Since there're no reports about this medial sigmoid depression after that, (The author retrieved the Mediline from 1983 to 1990: Cambridge scientific abstracts, (7200 Wisconsin Ave Bethesda MD 20814 U.S.A.) National Library of Medicine) the author studied the size and the location of the depression of the mandible specimens, and determined the relationship between the observable anatomic incidence and its reproducibility on the radiographs.

Effect of Presurgical Nasoalveolar Molding in Unilateral Cleft Lip and Palate Infants (편측성 구순구개열 신생아에 대한 술전비치조정형장치의 효과)

  • Cleft lip and palate, the most common craniofacial anomalies, are severe congenital defects that have an incidence of 0.28 to 3.74 per 1000 live births. Although there has been great improvement in the field of cleft surgery, surgical approach cannot be the single solution to resolve the various problems encountered in patients with cleft lip and palate. The concept of presurgical infant orthopedics (PSIO) for gradual closure of the cleft gap and simplified surgical performance was first introduced by McNeil in 1950. Recently, there are many attempts not only to approximate the alveolar segments but also to reshape the nasal cartilage. Three infants with unilateral cleft lip and palate were referred from the department of Plastic Surgery for presurgical nasoalveolar molding (PNAM). Maxillary appliances using resin with orthodontic wire were fabricated. Then these appliance was applied until patients underwent lip surgery. In all cases, the patients could wear the appliance all day since they were able to eat even with the appliance on, This resulted in significant improvements in the nasal symmetry were found. Our appliance, namely K-NAM, extends the wearing time within the limited period and as a result it is expected to maximize the treatment effects. Used properly, this appliance would play a major role in enhancing nasal symmetry with satisfactory results.

  • The author evaluated the effect of wunderer activator and karwetzky's u-bow activator for correction of anterior and posterior cross bite and mandibular shift to the Rt. or Lt. side. The results were as follows. 1. correction of anterior and posterior cross bite. 2. correction of mandibular shift to the unilateral side. 3. slightly down and backward rotation of mandible was occured.
