Treatment of Deep Venous Thromboses of Lower Leg with Thrombolysis (혈전용해술을 이용한 하지 심부정맥 혈전증의 치료)

  • Background: Deep venous thrombosis(DVT) is a curable disease when it is appropriately treated in the early stages of onset. The long term follow up of chronic DVT shows poor prognosis with serious complications such as venous valvular insufficiency, venous claudication, venous ulcer and leg swelling. Thrombolytic therapy is a very active treatment that delivers thrombolytic agents via catheter to the target thrombi. The aim of this study is to evaluate the effect of catheter directed thrombolysis using urokinase to acute DVT. Material and Method: We studied 5 patients, who were diagnosed as acute DVT and had no contraindication for selective hemolysis using urokinase. Result: All the patients were successfully recanalized. Total infusion time of urokinase was 2.0$\pm$0.6 days, and the amount was 5.9$\pm$2.45 million IU. In 4 patients, who were diagnosed as May-Therner syndrome, we performed the balloon angioplasty and inserted the stent at the stenotic portion. There were minor complications such as hematuria, hematoma at puncture site, and all of them are self limited. Conclusion: Catheter induced thrombolysis is an effective treatment in acute DVT.

Virulent Systemic Feline Calicivirus Infection in a Kitten (새끼고양이에서 발생한 고독성 칼리시바이러스(VS-FCV) 감염증)

  • A 5 month old Korean domestic short haired male kitten (weighing 1.7 kg) was presented with primary complaints of upper respiratory disease (URD) signs and skin ulceration and edema on face, feet and footpad with lameness. Diagnostic test revealed leukopenia, lymphopenia, pancreatitis and feline calicivirus (FCV) infection. Diagnosis of virulent systemic FCV (VS-FCV) was made on clinical signs, isolation of calicivirus via PCR and exclusion of other causes of acute upper respiratory disease. Therapeutic strategies were directed to lessen URD signs and to treat secondary bacterial infection and antiviral infection. One month after this therapy, skin lesions on face and feet and URD signs were much improved, although the lameness persisted mildly. To author's best knowledge, this is the first case reporting VS-FCV infection in a kitten in Korea.

Capture Myopathy in a Red-Necked Wallaby (Macropus rufogriseus) (붉은목왈라비에서 발생한 포획근병증)

  • This case report describes capture myopathy in an 8-month-old female red-necked wallaby (Macropus rufogriseus) that died after a 14-day history of depression, lameness and astasia following a bathing protocol for dermatitis. In a blood test performed the day after the wallaby fell down, serum chemistry showed a normal rangeof LDH but elevated CK, ALT and AST. Upon histopathologic examination after the animal's death, we found degeneration and necrosis of myofibers in skeletal muscle and myocardium. According to history that the wallaby was pressed by other individuals, suspected vitamin E insufficiency in the diet, physical stress during bathing as well as results of a blood test and microscopic examination, we diagnosed this case as capture myopathy. Although capture myopathy is a common problem in marsupials, this is the first reported case in Korea and understanding this case will help to manage future cases of capture myopathy in captive settings in Korea.

Tibial Tuberosity Advancement for Partial Rupture of Cranial Cruciate Ligament in Two Dogs (경골조면 전이동술을 이용한 개에서의 부분 전십자인대 단열치료 2례)

  • Two dogs (Case 1 weighing 27 kg, Case 2 weighing 42 kg) were referred with hindlimb lameness. On physical examination, there was moderate pain on stifle joint testing and mild cranial translation on cranial drawer testing in Case 1. This translation was obvious when the patient was under general anesthesia. Case 2 showed discomfort during hyperextension of the stifle joint, but no significant cranial translation under general anesthesia. Joint effusion was detected on radiography in both cases. Based on physiologic and radiographic examinations, cranial cruciate deficiency was suspected, so exploratory arthroscopy was performed. Arthroscopy revealed partial rupture of the cranial cruciate ligament (CrCL) in both cases. Case 1 showed unstable partial CrCL rupture, while Case 2 had stable CrCL rupture. Tibial tuberosity advancement (TTA) was carried out as planned. Seroma occurred one week postoperatively in Case 2, and was removed surgically. The patients returned to satisfactory weight-bearing ambulation 5 months after initial surgery. Based on the results of these cases, TTA appears to be a useful procedure in the treatment of CrCL partial rupture.

The Use of a Porcine Cancellous Bone Graft for Arthrodesis of Bilateral Antebrachiocarpal Luxation in a Dog (양측 전완앞발목관절 탈구의 관절고정술을 위한 돼지뼈 해면골이식의 적용)

  • A seven-year-old Yorkshire terrier 2.1 kg was admitted with a 2 months history of sever bilateral forelimb lameness and joint instability. Examination and radiographs revealed caudal luxation of bilateral antebrachiocarpal joint. Luxation of bilateral antebrachiocarpal (ABC) joint in a toy breed dog was treated with pancarpal arthrodesis. Modified external skeletal fixator (ESF) (type IIb) with polymethylmethacrylate (PMMA) was utilized in this case. Porcine cancellous bone graft (PCBG) was inserted to fill the joint space in carpus. The clinical and radiographic assessments of joint after the surgical procedure proved that PCBG can be used a promising alternative to Autogenous cancellous bone graft (ACBG).

  • Atherosclerosis has more than 60% of the causes of arterial occlusive diseases. The abdominal aorta and lower extremity arteries are the most common sites of occlusion. We have treated surgically 2 cases who had intermittent claudication and were diagnosed as simultaneous aortobifemoral and bilateral femoropopliteal obstruction by angiography, but had ineffective results from medical treatment or angioplasty. Simultaneously aortobifemoral bypass using Hemashield Y graft and bilateral femoropopliteal bypass using autologous greater saphenous vein were done. After operations, the symptom disappeared and there were no specific post-operative complications except abdominal wound dehiscence. In postoperative angiography, we had obtained good patency of bypass graft. We are following up patients through the out patient department without recurrence up to 16 months.

Use of Cross Pins and Temporal External Skeletal Fixator for Stabilization of a Tibial Physeal Fracture in a Korean Native Calf (한우 송아지의 정강뼈 성장판 골절에서의 교차핀과 임시 외부 골격 고정장치의 이용)

  • A one-month-old, male Hanwoo calf was presented to Chonbuk Animal Medical Center with non-weight bearing lameness on its right hindlimb. The radiograph and CT scan showed a Salter-Harris III fracture at the distal tibia. Following open reduction, the fracture was stabilized by cross-pins. A temporal external skeletal fixator was used as an additional support for 5 weeks. 5 weeks after surgery, bone fracture had successfully healed with no complication. At 2 years after surgery, the calf could walk almost normally although intermittent weight-bearing lameness after active exercise. This is a rare case that the combined use of temporal external fixation and cross-pins fixation was sufficiently effective for treating a distal tibial physeal fracture in a calf.

Minimally Invasive Arthroscopic-Assisted Reduction with TightRope® for Coxofemoral Luxation in a Korean Water Deer (Hydropotes inermis argyopus) (고라니에서 최소침습적 관절경과 TightRope®의 이용한 엉덩관절탈구 교정 증례)

  • A Korean water deer was rescued with non-weight-bearing lameness of the right hind limb. Clinical and radiographic examination revealed a craniodorsal coxofemoral luxation. The cause of the right coxofemoral luxation (CL) was unknown. We performed minimally invasive arthroscopic-assisted reduction with toggle pin fixation (mini TightRope$^{(R)}$). Craniodorsal CL in the water deer was reduced and stabilized successfully. At 60 days after admission, the water deer was successfully released back into the wild. This minimally invasive surgical technique can be used to correct coxofemoral luxation in wild animals.

Radiographic Changes of Bilateral Partial Avulsion Fracture of Tibial Tuberosity in a Dog (개에서 발생한 양측성 경골조면 부분 견열골절의 방사선학적 변화)

  • A 4-month-old intact male Jindo dog was presented for left hindlimb lameness. On radiographs, a small mineralized piece was identified beneath the apophysis of tibia with no or minimal displacement of apophysis bilaterally. Subsequently, the cranioproximal bony aspect of the tibia changed radiolucent to radiopaque. The left side recovered after surgical management, but the right side was self-limited with conservative management. The diagnosis was partial avulsion fracture of the tibial tuberosity which has been called 'Osgood-Schlatter disease' in dogs.

Potential Seizurogenic Effect of Tramadol in a Dog with Necrotizing Meningoencephalitis (괴사성 뇌척수막염을 가진 진돗개에서 Tramadol에 의한 잠재적 경련발생 증례)

  • A Jindo dog (8-month-old, intact male) was referred for hind limb lameness on the right side. The dog was diagnosed with a simple femoral fracture by radiological examination. After surgical fixation of the femoral fracture, tramadol: a narcotic-like synthetic analgesic was intravenously administrated for post-operative analgesia. After injection of the tramadol, generalized tonic clonic seizure was immediately occurred in the dog. Seventeen hours later, the dog died despite intensive care. We suspected that tramadol might induce the seizurogenic effect resulted in death. A necropsy was performed to examine the cause of the death. In consequence, the dog was diagnosed as necrotizing meningoencephalitis (NME) based on histopathological examination. We would be concerned that tramadol may be related to seizure activity in the NME patient. From this case, it is known that although tramadol has been proven to be a safe and effective agent for the control of pain in veterinary medicine, it would be carefully used to patient with history of neurological diseases including meningoencephalitis, hydrocephalus, and encephalopathy.
