Purpose: Depression is a common sign of suffering among the patients with Parkinson's disease (PD). Frequent and severe neuropsychiatric symptoms lead to high levels of distress in patients and their caregivers, which results in the high levels of caregiver burden. The aim of this study was to determine the predictors of caregiver burden in caregivers of the patients with Parkinson's disease (PD). Methods: The study included 183 consecutive PD patients and their caregivers. Patients were assessed using the Hoehn and Yahr scale, Mini Mental State Examination (MMSE), Beck Depression Inventory (BDI), and Schwab and England Activities of Daily Living Scale. Caregivers' depressive symptoms were evaluated using the Caregiver Burden Inventor (CBI), BDI, and World Health Organization Quality of Life Scale. Results: All of the patients reported one or more neuropsychiatric symptoms. Patients' and caregivers' depressive symptoms, caregivers' age and education, time for caregiving, and quality of life were significantly associated with the increased caregiver burden. After controlling the level of education as a potential confounding variable, depression in both patients and caregivers, time for caregiving, and quality of life explained 45.6% of the variance in caregiver burden. Conclusion: Substantial attention needs to be given to the early identification of depression in PD patients and their caregivers to improve caregivers' quality of life and burden.

Purpose: The purpose of this study was to describe the health care experiences among patients with Parkinson's disease. Methods: Of the qualitative research methods, Colaizzi's phenomenological method was used in this study. A total of nine patients, who were diagnosed with Parkinson's disease and receiving outpatient treatment, were selected as the subjects of this study. Subsequently, data were collected through individual in-depth interview. Results: The four categories obtained as a result were 'strenuous efforts to control my own body,' 'subject of health that no one can replace,' 'focus on the current while expecting a breakthrough in health management,' and 'human dignity that cannot be lost to the end.' Conclusion: The study results are expected to help health care providers deeply understand the experiences in health care among patients with Parkinson's disease and to provide source data for nursing intervention development that can be helpful in managing the health status of patients with Parkinson's disease.

Purpose: The purpose of this study is to explore the level of bone mineral densities of the femur neck and to identify factors affecting bone mineral density of the femur in Parkinson's disease (PD) patients. Methods: Participants were 121 PD patients visiting the outpatient clinic of D University Hospital in B City. Bone mineral density was measured at the femur neck by using a dual-energy x-ray absorptiometry. The serum vitamin D level, the amounts of milk intake, caffeine intake, cigaret smoking, and alcohol consumption, and the number of steps taken daily were measured. Past and present disease history, the medication history, and duration of the disease were also collected. The level of disability was obtained by neurologists using the Hoehn and Yahr stage and the Unified Parkinson's Disease Rating Scale. Results: Among PD patients, 57% had osteopenia and 15.7% had osteoporosis. The bone mineral density of the femur was associated with weight, serum vitamin D level, age, and working status. Conclusion: The identification of weight and serum vitamin D level as important predictive factors emphasizes the importance of balanced nutrition for PD patients.
Purpose: The purpose of this study was to investigate the factors affecting the quality of life of family caregivers of patients with Parkinson disease in Korea. Methods: A total of 162 family caregivers of patients with Parkinson disease who were visiting the D hospital in B city completed questionnaires, including general and disease-specific characteristics, family burden, patient-caregiver relationship satisfaction, family support and quality of life. Data were analyzed with t-test, ANOVA with $Scheff{\acute{e}}$ test, Pearson's correlation coefficient, and multiple regression analysis using SPSS 23.0. Results: The average score of the quality of life was $85.54{\pm}17.74$ (range: 26~130) among family caregivers of patients with Parkinson disease. Stepwise multiple regression analysis revealed that fifty percent of the quality of life was explained by family support, family burden, and patient-caregiver relationship satisfaction. Conclusion: Family support, family burden, and patient-caregiver relationship satisfaction were identified as factors affecting the quality of life of family caregivers of patients with Parkinson disease. Therefore, it is necessary to develop a program to enhance family support and reduce family burden to improve the quality of life of family members caring for patients with Parkinson disease.
Purpose: The purpose of this paper was to verify effects of the team approach rehabilitation program on balance, gait, and muscle strength of lower extremities of elderly people with Parkinson's disease. Method: Subjects of this paper were 40 elderly people with Parkinson's disease, 20 control and experimental groups respectively, who could walk independently and were less than the 2.5 Hoehn &Yahr stage. The team approach rehabilitation program was applied to the experimental group for 12 weeks. Results: There was significant decrease in second at timed up &go test (p= .008), but no significant difference in reach length at functional reach test (p= .201) with partial improvement of balance. There was no significant difference in second at 10-meter walk test (p= .070), but showed tendency of improvement of gait. And number of times at 30s-chair stand test, indicating muscle strength on lower extremities, increased significantly (p= .029), Conclusion: The team approach rehabilitation program has demonstrated its effectiveness on improving balance, and muscle strength of lower extremities for the elderly with Parkinson's disease.

삼킴 장애 환자는 뇌졸중, 치매, 외상성 뇌손상, 파킨슨병, 암이 주요 원인으로 급속히 증가하고 있다. 특히 고령화 사회가 되면서 더욱 삼킴 장애 환자는 늘어날 것으로 전망하고 있다. 고령 환자의 삼킴 이상의 진단을 위해 가장 많이 사용하고 있는 검사법으로는 비디오 조영 삼킴 검사(VFSS)이다. VFSS는 진단에 있어서 숙련된 전문의가 필요하기 때문에 대학병원 급에서 주로 시행하며, 고령 환자에게는 분석 결과를 상담받을 때까지 오랜 시간을 소요해야하는 문제점들이 있다. 본 논문에서는 삼킴 장애 진단을 위한 의료영상 관리 및 라벨링 시스템에 대해서 기술한다. 이를 구현하기 위해 서버에서 대용량 멀티프레임 영상을 성능 저하 없이 핸들링 하고 라벨링 데이터 생성을 위한 라벨링 툴을 구현하였다. 차후 라벨링 데이터를 생성하고 학습을 통하여 삼킴 장애 진단을 위한 인공지능 모델을 개발하고자 한다.

Objective : This study was designed to assess the heart rate variablitv of patients with Parkinson's disease according to Hoehn-Yahr stage. Methods : Subjects were voluntarily recruited by newspapers and internet. All the subjects are confirmed as idiopathic parkinson's disease by a neurologist. The patients were grouped by Hoehn-Yahr(H-Y) stage. Heart rate variability was measured for 5 minutes after 5 minutes' bed rest. Subjects were comprised of 3 groups which were H-Y stage I (n=14), H-Y stage H(n=9), H-Y stage III(n=7). We compared the heart rate variability of each groups by one-way ANOVA test. We also studied the relationship between UPDRS score and Heat rate variability components (SDNN, RMS-SD and TP) by pearson correlation analysis and simple linear regressin analysis Results: Age, duration was matched among groups. SDNN, RMS-SD, TP were significantly decreased as H-Y stage increased(p<0.05). Especially there was significant difference between H-Y stage I and III group(p<0.05). But other components(Heart rate, LF, HF, LF/HF ratio) were not showed significant difference. SDNN, RMS-SD and TP were significantly decreased as UPDRS scores increase(p<0.05). Conclusion : This study suggests that the function of the autonomic nervous system decreases as Parkinson`s disease progresses.

보행 종료(gait termination)는 인간의 보행개시(gait initiation)나 율동성 보행(rhythmical walking)에 비해 관심을 적게 받고 있다. 파킨슨 환자들이 보행종료와 방향전환 시 가지는 어려움은 이들 환자들이 보행 시 넘어지게 되는 노출 요인으로 보고되어져 왔다. 보행종료에 대한 기전의 이해는 효과적으로 걸음을 멈추지 못해 넘어지게 되는 위험요인을 가진 사람들에게 적용되어질 수 있다. 이 논문에서 우리는 보행종료에 대한 최근문헌을 고찰하고 우리 실험실에서 진행되어온 파킨슨환자의 보행종료에 관한 일련의 실험들의 결과를 요약하였다. 본 연구는 율동적인 보행상태에서 완전한 멈춤으로의 전환에서 일어나는 동역학의 변화를 검사하기위해 시행되었다. 보행속도가 증가함에 따라 따르는 다리(trailing limb; 보행종료시점에서 뒤에 위치하는 다리)의 유의한 가속력 감소가 나타나고 멈출 때 이끄는 다리(leading limb; 보행종료시점에서 앞에 위치한 다리)에서 유의한 감속력의 증가가 나타난다. 보행종료 시, 이끄는 다리의 뒤쪽에 질량중심(center of mass) 을 유지시키기 위해 하지 신전근의 활성도가 오래 지속되어져야 한다. 예측하지 못한 상황에서 보행종료 시, 보행 중이나 예측되어진 상황에서 멈출 때보다 이끄는 다리 아래에서 감속력이 빠르게 발생하는 경향을 보였다. 대상자들을 보행속도에 따라 두 군으로 나눌 때, 계획되지 않은 보행종료를 하는 동안 보행속도가 느린 군이 빠른 군보다 전방 가속력을 급속하게 증가시키는 것으로 나타났다. 이러한 결과들은 보행속도가 느린 대상자들이 따르는 다리로 힘을 생성하기 보다는 주로 이끄는 다리에 의존하여 보행종료를 하는 것을 보여준다. 따르는 다리에서 발생되는 힘과 근육활성도의 조절을 측정하기 위하여, 파킨슨 환자와 연령에 의해 짝짓기된 정상 환자군에게 표적 조건(target condition)을 추가하였다. 파킨슨 환자군은 표적에 다리를 정확히 놓기 위해 몇 발자국 전부터 보행 속도를 줄였다. 파킨슨 환자들은 보행종료를 촉진하기 위해 필요한 근육들의 활성도를 증가시키는데 큰 어려움을 가지고 있다고 생각되어진다.

Purpose: The aim of this study was to evaluate the discriminating nature of $^{123}I-FP-CIT$ SPECT in patients with parkinsonism. Methods: $^{123}I-FP-CIT$ SPECT images acquired from the 18 normal controls; NC ($60.4{\pm}10.0$ yr) and 237 patients with parkinsonism ($65.9{\pm}9.2$ yr) were analyzed. From spatialIy normalized images, regional counts of the caudate, putamen, and occipital lobe were obtained using region of interest method. Binding potential (BP) was calculated with the ratio of specific to nonspecific binding activity at equilibrium. Additionally, the BP ratio of putamen to caudate (PCR) and asymmetric Index (ASI) were measured. Results: BPs of NC $3.37{\pm}0.57,\; 3.10{\pm}0.41,\; 3.23{\pm}0.48$ for caudate, putamen, whole striatum, respectively) had no significant difference with those of essential tremor; ET ($3.31{\pm}0.64,\; 3.06{\pm}0.61,\; 3.14{\pm}0.63$) and Alzheimer's disease; AD (3.33 $\pm$0.60, 3.29$\pm$0.79, 3.31$\pm$0.70), but were higher than those of Parkinson's disease; PD (1.92$\pm$0.74, 1.39$\pm$0.68, 1.64$\pm$0.68), multiple system atrophy; MSA (2.36$\pm$1.07, 2.16$\pm$0.91, 2.26$\pm$0.96), and dementia with Lewy body; DLB (1.95$\pm$0.72, 1.64$\pm$0.65, 1.79$\pm$0.66)(p<0.005). PD had statisticalIy lower values of PER and higher values of ASI than those of NC (p<0.005). And PD had significantIy lower value of PCR, higher ASI and lower BP in the putamen and whole striatum than MSA (p<0.05). Conclusion: Dopamine transporter image of $^{123}I-FP-CIT$ SPECT was a good value in differential diagnosis of parkinsonism.
