Left to Right Ratio of Autopneumonectomy State in Pulmonary Tuberculosis Patients (폐결핵 환자의 편측 전폐 파괴에 대한 좌우 비교)

  • Background: There are several causes that the total destruction of unilateral lung can occur. Pulmonary tuberculosis of these causes may lead to destruction through chronic inflammation and fibrosis. In such circumstances, the left to right ratio is supposed to be different. We performed the study with 224 cases for total destruction of unilateral lung, who were admitted at National Kongju Tuberculosis Hospital for recent 4 years. Method: On admission chest film, radiologic pattern was classified into 4 types. And we analyzed patients' age, sex, duration of illness, sputum AFB smear and culture. Result: 1) The male to female ratio was 2:1 and 74% of the patients were between 31 and 60 years of age. 2) One hundred and ninety eight cases(88%) had the duration of illness over 3 years. Namely, most of patients had long history. 3) Sputum AFB smear and/or culture were positive mostly (80%). 4) As for the radiologic pattern, 181 cases (81%) had the total destroyed lung in left and partial lesion in right lung, 31 cases (14%) total destroyed lung in right and partial lesion in left lung, 11 cases (5%) total destroyed lung in left only, and 1 case total destroyed lung in right only. 5) In the radiologic pattern, there is no significant difference in terms of age, sex, duration of illness and sputum examination. Conclusion: From these results, we found that the total destruction of unilateral lung in pulmonary tuberculosis occurred generally in the left side.

  • Postpneumonectomy syndrome is a rare condition characterized by dyspnea due to an extreme mediastinal shift and bronchial compression of the residual lung after a right pneumonectomy or a left pneumonectomy with the right aortic arch, Severe fibrosis of the lung such as tuberculsosis (TB) - destroyed lung can cause similar clinical features in the absence of pneumonectomy. We report a unique case of postpneumonectomy syndrome without pneumonectomy, which was successfully treated with pneumonectomy and mediastinal repositioning with tissue expanders.

Effects of Flushing, Preservation and Reperfusion in the Canine Transplanted Lung Tissue (관류, 보존 및 재관류 과정이 이식된 개의 폐조직에 미치는 영향)

  • Background: Due to the paucity of suitable donor organs for lung allotransplantation, a number of techniques have been developed to improve the lung preservation. Ultrastructural studies of the morphologic changes of the flushing, preservation and reperfusion injury in donor lungs have rarely been reported. Methods: Adult dogs (n=46) were matched as donors and recipients for the single lung transplantation. The donor lungs were preserved after flushing with preservation solution and transplanted after 20-hours of preservation at $10^{\circ}C$. Ultrastructural features of the lung were examined after flushing, preservation and 2 hours after lung transplantation (reperfusion) respectively. Results: Electron microscopy after flushing showed focal alveolar collapse and mild swelling of type I epithelial cells. After preservation both type I epithelial cells and endothelial cells were swollen and destroyed focally. The endothelial cells showed protrusion of tactile-like structures into the lumina, blebs or vacuoles of the cytoplasm After reperfusion the lung tissue showed fibrin material in the alveoli, prominent type I epithelial cell swelling with fragmented cytoplasmic debris and marked endothelial cell swelling with vacuoles or tactile-like projections. The alveolar macrophages showed active phagocytosis. Scanning electron microscopic examination of the pulmonary parenchyma showed focally alveolar collapse and focal consolidation after the preservation and more prominent changes after the reperfusion procedure. The lungs preserved with low potassium dextran glucose solution, with additional prostaglandin $E_1(PGE_1)$ and verapamil(VP) showed relatively well preserved ultrastructures compared with those which were preserved with modified Euro-Collins or University of Wisconsin, and with additional $PGE_1$ and/or VP. Conclusion: The ultrastructural changes associated with flushing were mild in severity, the donor lungs were injured during the preservation, and further damage was occurred during the reperfusion. The reperfusion injury resulted in prominent pulmonary parenchymal alterations with a pattern of acute lung injury.

Prognostic Factors Affecting Postoperative Morbidity and Mortality in Destroyed Lung (파괴폐의 술후 합병증과 사망에 영향을 미치는 예후 인자)

  • Background: Postoperative morbidity and mortality in destroyed lung are relatively high. We tried to identify the prognostic factors affecting postoperative morbidity and mortality in destroyed lung through a retrospective study. Material and method: The retrospective study was undertaken in 112 patients who had undergone pneumonectomy or pleuropneumonectomy for destroyed lung at Severance Hospital from 1970 to 2000. We analyzed the correlation between postoperative morbidity and mortality and etiology, duration of disease, preoperative FEV1, presence or absence of peroperative empyema, operation timing, the side of operation, duration of operation, and operation type. Result: There were 55 men and 57 women, aged 20 to 81 years (mean 44 years). Etiologic diseases were tuberculosis in 86 patients(76.8%) including tuberculos empyema in 20 and tuberculous bronchiectasis in 4, pyogenic empyema in 12(10,7%), bronchiectasis in 12(10.7%), and lung abscess in 2(1.8%). Postoperative morbidity were 25%(n=28) and postoperative mortality was 6%(n=7). The presence of preoperative empyema(p=0.016), pleuropneumonectomy(p=0.037) and preoperative FEV1 of less than 1.75 L(P=0.048) significantly increased the postoperative morbidity, If operation time was less than 300min, postoperative morbidity(p=0.002) and mortality(p=0.03) were significantly low. Conclusion: Postoperative morbidity and mortality in destroyed lung were acceptable. Postoperative morbidity and mortality were significantly low when operation time was less than 300 min. Preoperative existence of empyema, pleuropneumonectomy and preoperative FEV1 of less than 1.75 L significantly increased postoperative morbidity.

  • 자동차용 배터리는 초기 용량의 80% 이하가 되면 교체하게 되며, 근간 폐배터리의 수가 폭발적으로 증가할 것으로 예측되고 있다. 폐배터리의 폐기로 인한 환경 파괴를 방지하고 자원을 재활용하기 위해서 자동차에서 나오는 폐배터리를 에너지저장장치(ESS)로 재사용 하는 것에 대한 관심이 높아지고 있다. 폐배터리를 ESS로 재구성하기 위해서는 폐배터리 모듈의 그레이딩을 통해 비슷한 성능의 모듈끼리 모아서 구성하는 것이 매우 중요하다. 배터리 모듈 간의 불균형은 전체 시스템의 성능을 저하시키며, 따라서 비슷한 성능과 잔존 수명을 가진 모듈을 골라내는 일은 폐배터리의 재사용에 있어서 첫 번째 선결 과제가 된다. 본 연구에서는 폐배터리의 상태 및 잔존수명평가를 위해 배터리 모듈의 임피던스 스펙트럼을 측정할 수 있는 장비를 개발하였다. 폐배터리 모듈에 AC 섭동을 인가하고 이를 측정하여 임피던스 스펙트럼을 계산할 수 있는 하드웨어와 소프트웨어를 개발하였다. 개발 장비는 60V이하의 폐배터리 모듈의 임피던스 스펙트럼을 0.1Hz에서 1kHz까지 측정 가능하며, 측정 결과를 바탕으로 커브 피팅을 통해 등가회로의 파라미터도 계산할 수 있다. SM3에서 얻어진 폐배터리 모듈을 이용하여 측정한 임피던스 스펙트럼을 상용장비인 BIM2로 측정한 결과를 비교하였고, Reduced Chi-Square를 이용한 분석결과 두 데이터가 거의 일치함을 알 수 있었다.

  • This study aims to analysis the environmental impact on waste catalyst recycling technology using entire life cycle assessment. Environmental impacts consist of the five categories of impacts: global warming, resource depletion, acidification, eutrophication, and photochemical oxide production. The waste catalyst recycling presently have a GWP 3.53 ton $CO_2$ equivalent/ton, a ADP 0.017 ton Sb equivalent/ton, a AP 0.051 $SO_2$ equivalent/ton, a EP 0.0092 $PO{_4}^{3-}$ equivalent/ton, a 0.0019 ton $C_2H_4$ equivalent/ton. The smelting reduction process is the greatest contributor to all categories of environmental impacts in waste catalyst recycling. Electricity used in the smelting reduction process is the major contributor of all impact categories.

  • 본 연구는 국제환경규제에 따라 폐자동차의 재활용율 향상을 위한 재활용 부품 우선순위를 도출하고, 폐시트 등 부품 재활용에 의한 환경성을 평가하기 위하여 폐차 해체시스템 전과정평가를 수행하였다. 연구결과 차피의 고철을 재활용 할 경우에는 지구온난화와 오존층파괴에 큰 환경이득을 얻을 수 있으나 폐시트를 폴리올 원료로 재활용할 경우에는 많은 자원의 사용으로 오히려 소각으로 인한 환경부하보다 지구온난화와 오존층파괴, 광화학산화물생성 등의 부하를 증가시키는 것으로 나타났다. 그러나 폐차의 95% 이상을 재활용 및 회수하기 위해서는 분해시간 및 시장성, 기술현황 등을 종합하여 고려하여야 하며 재활용이 곤란한 유리와 같은 다른 부품과 함께 시트의 물질재활용도 반드시 포함되어야 될 것으로 사료된다. 처리와 재활용에 따른 환경성을 비교한 결과 재활용이 필요한 부품은 시트와 유리가 가장 시급하며, 배터리, 혼합플라스틱도 재활용시 환경친화적 공정개발이 필요한 것으로 도출되었다.
