An Analysis on Income-related Health Inequality of the Aged Applied to EQ-5D (소득계층에 따른 노인들의 건강 불평등 측정: EQ-5D 척도를 중심으로)

  • This study analysed the income-related health inequality of the aged in Korea applied to EQ-5D. Two decompositions were used in analysis. One was the decomposition of income-related health inequality into six different dimensions of EQ-5D, and the other was to decompose it by sub-group such as sex, region, existence of spouse. The results are summarized as follows. First, the health concentration index(CI) of the aged was .0254, which meant that there were pro-rich inequality in elderly people's health levels. The same patterns were also seen in the analysis of different dimensions of EQ-5D such as mobility, self-care, usual activities, pain/discomfort, and anxiety/depression. Second, mobility accounted for 35.8% of total EQ-5D score, most contributing to CI of the dimensions of EQ-5D. The CIs by the dimensions ranged from .0091 for mobility to .0013 for self-care. Third, The decompositions by sub-groups showed that the contributions of sex, region, and existence of spouse to health inequality was similar to each other, all of three sub-groups accounted for 10 % of inequality respectively. Fourth, the inequality within group was higher in female group, rural area, and the aged without spouse. The average health level of these groups was lower than that of the other ones, too. These facts indicated that old women, the aged without spouse, and the elderly in rural area were the most vulnerable groups in health problems. Therefore, it is necessary to pay more attention to health problems of these groups in the policy making of health security and social welfare services.

Integrative Review of Guidelines Related Symptom Management and Physical Activity for Developing of Self-Care Management Program for Cancer Survivors (암생존자의 자가관리 프로그램 개발을 위한 증상관리 및 신체적 활동 관련 가이드라인의 통합적 고찰)

  • The purpose of this study attempted to collect basic data for development of an integrated self-management program for cancer survivors who completed cancer treatment. Self-management programs are divided into symptom management and physical activity management. Symptom management includes fatigue, sleep disturbance, pain, depression and anxiety. PubMed, CINAHL and EMBASE were used for searching guidelines. Based on the guideline quality evaluation, the final 8 guidelines were analyzed. The structured table was used to extract the screening subjects, timing, contents, subjects for comprehensive assessment and contents, and summarized contents related to the physical activity and exercise in non-pharmacological approach. As a result, after the completion of cancer treatment, all cancer survivors should be screened regularly using reliable and validated tools. In the case of fatigue, physical activity was recommended as the primary intervention, but it was recommended for other symptoms as adjuvant therapy. Therefore, Cancer survivors should be encouraged to be active in their physical activity, and maintain the moderate intensity physical activity as long as they have no complication related to the cancer treatment. Motivation strategies for physical activity need to be developed and applied.

A Study on Wound Care Knowledge and Concerns of Patients at Discharge (퇴원하는 환자의 상처관리에 대한 지식과 염려에 관한 연구)

  • This study was done to examine patients' wound care knowledge and concerns prior to discharge from a tertiary hospital. The participants in this descriptive survey were 112 patients having wounds. During interview, a structured self-administered questionnaire was filled out. The participants were 71 males and 41 females. Wound types were surgical incision (52.7%), percutaneous wound (26.8%), pressure ulcer (9.8%) and diabetic foot and arterial ulcers (5.4%). Their wound care knowledge was 52.0% of correct answer and the mean of concerns (range 1-7) was 2.79. There was no significant correlation between their knowledge and concerns of wound care. The factors influence on wound care concerns were fear of wound care, wound pain, length of hospital stays, and perceived health condition. This findings showed that discharge patients with a wound had some incorrect knowledge and various concerns about wound care. They may help to direct patient teaching in discharge plan.

Survey on the Perceptions for Dental Implant Surgery and Maintenance Care (치과 임플랜트 상담 환자의 임플랜트 수술 및 유지관리에 대한 인식도 조사)

  • The purpose of this study was to obtain basic data for dental implants prosthetic maintenance care after analysis perceptions of dental implants. A question was used to questionnaire by 210 dental patients at five dental clinics in the Jinju-city. In conclusion, 1. The knowledge level about implant is 'Never'(35.7%). 2. A feeling of unrest during implant surgery is 'pain'(37.6%). 3. The expectation level about dental implants function is 70% of natural teeth(36.2%). 4. The main function of implant teeth is 'mastication'(70.5%). 5. The implant hygiene supplies for the dental implants maintenance is 'proximal brush'(60%). 6. The important maintenance attitude for the dental implants is 'regularly dental examination'(42.9%). 7. The interval regularly for implant examination is 'six monthly'(44.8%).

Development of community-based intensive health care program for the community dwelling elderly (재가노인을 위한 지역사회 중심의 집중건강관리프로그램의 개발과 적용)

  • The purpose of the present study was to develop a community-based intensive health care program for the community dwelling elderly to strength their functional status and to verify the effect on their geriatric syndrome. A one-group pretest-posttest design was used for the study. A total of 69 frail elderly, who lived in the area within 20 minutes by car, were committed themselves to the day care center(Sangikjae), and had the ability of verbal communication were selected from G city in Kyunggi province. The participants completed a set of questionnaires to measure the sub-score of frailty, fall, urinary incontinence, malnutrition, and mild cognitive disorder domain, using the Otasha-Kensin through the physical examinations and interviews. After 4 weeks of intervention, the outcome was measured to evaluate the effects of the program, and the data obtained were analyzed using descriptive statistics, paired t-test and McNemar test. The results showed that the sub-score of frailty, fall, urinary incontinence, and malnutrition domain were statistically significantly decreased after intervention except those of urinary incontinence and mild cognitive disorder domain, implying that the risk of frailty, fall, and malnutrition was decreased. These findings indicated that community-based the intensive health care program is effective for relieving geriatric syndrome of the community dwelling elderly.

The Effects of a Case Management Program of Customized Home Visiting Health Service for Clients with Arthritis (보건소 맞춤형 방문건강관리사업 관절염 사례관리 효과)

  • Purpose: The purpose of this study was to analyze the effects of a case management program on range of motion, pain, depression and self efficacy in community-dwelling older women with knee arthritis. Methods: The research design was one-group pre and post-test design with 40 participants with knee arthritis who agreed to participate in the 12-week case management program. Data were collected four times, before and after 4, 8 and 12 weeks. Results: Repeated measures ANOVA revealed significant differences between before and after the service in range of motion, pain, depression and self efficacy. Significant improvements from the baseline to 4 weeks after the service were observed in the measurements of range of motion, pain, depression and self efficacy. There were no significant differences in range of motion, pain and depression between 4 and 12 weeks after the services. Conclusion: This study represents the first effort showing that a case management program can have positive outcomes on range of motion, pain, depression and self efficacy for older women with knee arthritis. More research using a control group pre-post test design considering time lapse is needed in order to examine more accurately the effects of case management programs for knee arthritis.

  • A common characteristic of many patients whose illness is getting worse is that they miss the treatment red flag. When high subjective symptoms appear or there is no strong pain, this problem arises because it is reluctant to visit the hospital. Gingivitis causes bleeding from the gums in the early and mid-term, and shows mild symptoms of tooth collapse. When treatment is done at this point, it shows a very high effect. However, when you miss the timing of treatment you will have a situation where you can't eat food by causing serious problems in the health of the gums and oral cavity. In this paper, the patient's periodontal image is photographed with a smartphone and transmitted in real time. This is done by the doctor in charge. Then, we propose a design of a patient management system that provides information on the current situation to the patient so as not to miss the timing of treatment.

Design of 3-Sectored Oxygen Chamber with Automatic Control Function based on Embedded System (임베디드시스템 기반 자동제어 기능의 삼단분리형 산소챔버 설계)

  • In this paper, considering the oxygen sensitivity and preference pattern of the patient, the profile of the oxygen therapy of the individual is specified. And, we design a three-compartment, single compartment oxygen chamber that automatically adjusts the oxygen pressure according to the specified profile. Hyperbaric oxygen therapy is a method of providing patients with 100% oxygen higher than atmospheric pressure for therapeutic purposes. However, there is a disadvantage that the ear pain is caused by the pressure difference depending on the individual. Based on the embedded system, the proposed system creates a patient-tailored oxygen therapy profile by DB of patient's preference patterns and treatment records of oxygen therapy. If only the patient's name is entered, the oxygen chamber system can adjust the oxygen pressure automatically according to the profile pattern to maximize the oxygen treatment effect.

Sports Injuries in Athletes with Disabilities (장애인선수들의 스포츠상해에 관한 연구)

  • The purpose of this study was to analyze sports injuries in athletes with disabilities during the 2015 National Games in Korea. We conducted a survey to assess sport injuries in 171 male athletes with disabilities who participated in this study. The shoulder was most frequently injured, followed by the fingers and low back. Contusions were the most common type of injury, and were usually caused by collisions with teammates or contact with equipment. Injuries often occur during enthusiastic play in both competition and practice sessions. Injured athletes are emergently treated with medicated spray and patch application by the head coach. Warm-up and cool-down exercises are recommended to prevent injury, and physical therapy is effective after an injury occurs. Based on these findings, an injury prevention training program and athletic trainers should be provided for athletes with disabilities to ensure appropriate care and rehabilitation.

Effect of Oral Hygiene Controllability on the Subjective Oral Malodor (구강위생관리능력이 구취 자각증상에 미치는 영향)

  • In our study, we investigated self-evaluation of tongue coating, self-rating intensity of oral malodor, habit related to tooth brushing and tongue scraping. Investigation was carried out on 714 Korean college students by method of self-reporting to prepared questionnaire. The obtained data were processed and analyzed with SAS program. The results of this study are as follows. 1. Students who have frequently experienced tongue coating have tendency to have stronger oral malodor than the other students. 2. The self-reporting prevalence rate of oral malodor is 81.1%. 3. Students who brush teeth shortly and irregularly have tendency to have stronger oral malodor than the other students. 4. Students with strong oral malodor have tendency to neglect scraping tongue. 5. Students with low index of tooth brushing or low index of oral hygiene controllability have tendency to have stronger oral malodor than the other students. 6. Mean scales of tooth brushing index(p<.0001), tongue scraping index(p=0.0439) and oral hygiene controllability index(p<.0001) among students in the department of Dental Hygiene and Dental Technology are significantly higher than those among general students.
