Measurement Accuracy of Blood Glucose Test on the Forearm (전완 부위 혈당검사의 정확도 분석)

  • The present study aimed to evaluate the accuracy of the blood glucose test with almost painless vacuum assisted auto-lancing technique. The blood glucose concentration measurements in the capillary blood sampled from the alternative (forearm) site were compared with the standard venous or finger sampling techniques. Capillary blood was sampled from both the left index finger and the forearm, followed by the venous blood sampling in 531 patients who visited the C University Hospital. Blood sugar test was performed for each blood samples for comparative analysis. Mean blood glucose concentration measured on the forearm was closer to the venous glucose level than that of the finger, but the difference was approximately 10 mg/dL small enough to ignore from the clinical point of view. The correlation coefficients of the measured blood glucose levels between finger and vein and between forearm and vein were 0.94 and 0.92, respectively (p<0.001). The blood glucose measurement on the forearm also well correlated with the finger, showing a correlation coefficient of 0.94, and both data were very much close to each other. Therefore, the blood sugar test on the alternative capillary blood sampling site such as forearm was accurate enough for clinical practice. The vacuum assisted auto-lancing technique on the alternative site would be of particular interest for the disease management of the chronic diabetes, since it induces almost no pain when sampling blood from the capillaries.

Acute postoperative myelopathy caused by spontaneous developed cervical disc herniation: Case report & literature review (수술후 자연발생 경추간판탈출에 의한 척수병증: 증례보고 및 문헌고찰)

  • Non-traumatic acute myelopathy caused by cervical disc herniation is rare. To date, no case has been reported to be caused by extrusion cervical disc herniation, unrelated to patient posture during surgery. Here, we report the case of a 65-year-old male patient with cervical myelopathy who underwent subsequent arthroscopic rotator cuff surgery under general anesthesia; non-cervical spine surgery. Ed. Patient showed acute postoperative tetraplegia in spite of optimal anesthetic management. He showed no limitation of neck movement at pre-operative airway evaluation, and had no history of trauma to the cervical spine. During surgery, there had been no overextension or twisting of the neck, including at the time of anesthetic induction by tracheal intubation. However, cervical disc herniation causing spinal canal cord compression was detected in the postoperative magnetic resonance imaging, which probably resulted in tetraplegia of the patient. Motor and sensory functions were recovered after 21 days of conservative treatment, including steroid pulse intravenous therapy without any surgical intervention. In this report, the disease is described after reviewing other reported cases; furthermore, we also discuss the pathophysiology of the disease. Based on our report, we propose that under general anesthesia, clinicians should pay attention to the possibility of pre-existing cervical disease, even in non-cervical spine surgeries of geriatric patients.

Water intake and oral disease symptoms in adolescents : a cross-sectional study conducted in Korea in 2021 (우리나라 청소년의 수분 섭취에 따른 구강질환 증상 : 2021년 청소년온라인행태조사 자료를 이용한 단면연구)

  • Objectives: Water constitutes a majority of the human body and is essential for health. In addition, water intake can prevent dental caries by improving salivary lubrication and self-cleaning. This study aimed to determine the relationship between the amount of daily water intake and the symptoms of oral disease in Korean adolescents. Methods: We used data from the Korea Youth Risk Behavior Web-based Survey (KYRBS), conducted in Korea in 2021, and identified the relationship between daily water intake and oral disease symptoms in Korean adolescents. KYRBS is a nationwide cross-sectional survey conducted by the Korea Disease Control and Prevention Agency (KDCA), and a total of 54,848 participants were included in this study. Results: After adjusting for confounding factors, logistic regression analysis showed that tooth pain was more often experienced by those who drank less than two cups of water per day (odds ratio [OR]: 1.14; 95% confidence interval [CI]: 1.01-1.30) than those who drank five or more cups of water per day. Conclusions: A low daily water intake is associated with tooth pain, a symptom of dental caries. The results of our study suggest that increasing water intake may reduce dental caries. Therefore, adequate water intake may help prevent dental caries.

The Analysis of Self-care Knowledge, Competence, Behavior, and Quality of Life in Community Diabetes Elderly (지역사회 당뇨노인의 당뇨 자가관리 지식, 자신감, 행위 및 삶의 질 분석)

  • This study was conducted to identify the level of diabetes self-care knowledge, confidence, behavior and quality of life in elderly people with diabetes. The participants were 205 elderly with diabetes who visited a citizen health center located in D city. Korea. The data were collected from April, 20th to August 31th, 2015, and were analyzed by the t-test and ANOVA. The results were as follows; 1) The mean scores for diabetes self-care knowledge, confidence, behavior, and quality of life were 6.99 (2.17), 71.27 (10.21), 62.78 (1.29) and 0.86, respectively. 2) There were significant differences in the self-care behavior according to the education level, duration of diabetes mellitus and subjective health, and the quality of life differeddepending on the level of subjective health(F=35.253, p=0.000). 3) Knowledge of diabetes diet was the lowest among the various aspects of diabetes self-care knowledge. Confidence for exercise was lowest, while confidence for medication and blood sugar check was highest. However, the participants exhibited good medication behavior, whereasthey did not carry out blood glucose checks in their self-care management behavior. The most uncomfortable aspect of the quality of life was found to be pain and discomfort. Based on the results of this study, it is necessary to develop guidelines and methods for dietary education reflecting the dietary habits of Koreans, and to investigatethe reasonsfor thelow performance of theblood glucose check. In addition, customized nursing interventionstaking into consideration the characteristics of elderly with diabetes need to be developed and verified.

Study on the Environmental Factors and Symptoms of VDT Syndrome (VDT 증후군의 환경적 요인과 증상에 대한 연구)

  • Purpose: Recently incidence of VDT syndrome has gradually increased as extensive use of computers. VDT syndrome reported by VDT workers include musculoskeletal disorder, neuropsychiatric disoders and eye symptoms such as eye strain, tired eyes, irritation and blurred vision. The environmental factors of VDT syndrome include electromagnetic waves, size, brightness and lighting of computer screen, height of a monitor and a worktable, working hours, kind of task, distance between screen and workers, indoor humidity and temperature, indoor air contamination and ventilation. In this study, we investigated the environmental factors related to body symptoms and health effects included in VDT syndrome. Methods: Study subjects were total 120 persons (54 male, 66 female) with age from 19 to 28. We surveyed the body symptoms and physical discomfort when doing an activity in a short distance such as reading book or paper, computer work. The questionnaire included main body symptoms, self-consciousness symptoms of eye, satisfaction of working environment, pain of the wrist when using keyboard and mouse. Results: Most of people (70%) felt physical pain from long time work of computer, paper, electrical apparatus. They mainly complained pain of neck and low back (57.1%), eye (45.2%) and head (31%). With the environmental factors, 78.3% of the subjects complaint pain of eye from inappropriate illumination. Most of the symptoms included 'eye fatigue'(38.3%), 'dryness of eye'(31.9%) and 'blurred vision'(23.7%). Subjects in this study complained discomfort of their chairs and most of them experienced pain in the wrist when using keyboard or mouse. Conclusions: When people use electrical apparatus or work with paper, people would get their eye fatigue and feeling of physical fatigue because of not harmonizing various environmental factors such as light, space, posture, worktable with theirselves. Therefore, workers should develop preventive method such as self-control of adequate break time to avoid fatigue while VDT work. Work environment should be changed to ergonomic design for optimal visual environment to prevent musculoskeletal disorder through constant research.

A Study on Patients' Concerns about Management of Cancer Pain and Related Factors (종양통증관리를 방해하는 환자의 염려와 관련요인 연구)

  • Pain management is a major issue in caring of cancer patients. Patients' concerns for reporting pain and taking analgesics are patient-related barriers to the management of cancer pain. Since such study has not been done at all in Korea, it is clearly needed to study on these problems. The purpose of this study is to attain basic data in order to improve cancer pain management in Korea. This is done by: 1) examining the extent of patients' concerns that might be barriers to the optimal pain management, and the extent of related factors (pain management hesitancy, adequacy of using analgesics, pain severity and pain interference); 2) identifying the relationship between patients' concerns and the related factors. The data has been collected from 180 cancer patients who were hospitalized in medical wards of one university hospital in Seoul, Korea during the period from November 1, 1997 to February 28, 1998. The data has been collected through interviews with (1) Barriers Questionnaire - Korean Version (BQ-K); (2) Hesitancy Experience Questionnaires (HQ); (3) Pain Management Index (PMI); (4) Brief Pain Inventory (BPI); and (5) Demographic Data. The data were analyzed by descriptive statistics and by t-test, One-way ANOVA, Pearson correlation using SPSSWIN program. The Results are as following: 1) The mean scores of Pain Management Concerns (PMC) by BQ-K were toward the moderate with a little high points(2.59). Most of the patients (99.4%) had some extent of concerns (over lout of maximum 5 points). Among the eight subscales of BQ-K, the Pain Management Concerns (PMC) about 'Fear of tolerance' was the highest (3.80) and 'Worry about side effects' was the least (1.40). 2) The extent of Pain Management Hesitancy (PMH) by HQ of wnom had pain on the day of the interview was a little higher than moderate score(5.53 out of maximum 10 points). 6.7% of the patients with experiencing pain used less adequate analgesics for the severity of pain than they were expected. 27.8% of them never used any analgesics at all. The mean score of pain severity by BPI was 16.59 (maximum: 40), and that of the interference with daily life by BPI was 32.03 (maximum: 70). 3) The patients who were older, less educated, and in low socio-economic status were likely to have more concerns. Pain Management Concerns (PMC) was positively correlated with Pain Management Hesitancy (PMH) (r=.75), pain severity (r=.44) and pain interference (r=.50). Those who were not using adequate analgesics had higher Pain Management Concerns (PMC) than did those who were using adequate analgesics (t=-5.42). The patients who had more Pain Management Concerns (PMC) tended to hesitate more to report pain and to use analgesics. They used more inadequate analgesics for the severity of pain and also had experienced more pain severity and interference with daily life. In conclusion, the patients' concerns for reporting pain and for using analgesics are major patient-related barriers to cancer pain management in Korea. The patients' concerns were correlated significantly with the level of the hesitancy experience, inadequate use of analgesics, the pain severity and the interference with daily life. Considering this, an educational program for cancer patients under the treatment with analgesics should be developed in order to solve these problems.

Physical Functions of Industrial Workers with Chronic Low Back Pain and Changes in Health-related Quality of Life according to Virtual Reality Exercise Program (가상현실 운동프로그램에 따른 산업체 만성요통환자의 신체기능과 건강관련 삶의 질의 변화)

  • This study performed a virtual reality exercise program for self-back pain control of industrial workers with chronic back pain and analysed their physical function and health-related quality of life relating to industrial chronic back pain. The subjects of the study were thirty chronic back pain patients working for K company and were divided into Nintendo Wii Sports Group made of 15 subjects and Physical Therapy Group made of 15. As variables for physical functions, back strength, flexibility, balance and pain were measured and for the health-related quality of life, SF-36 of each group was measured before and after the exercise program. To compare differences before and after the program, a paired t-test was used and to compare differences between both groups, an independent samples t-test was used. As a result of the test, it was discovered that balance and pain of the both groups were improved statistically significantly. Physical functions of the Nintendo Wii Sports Group were improved statistically significantly in comparison with those of the Physical Therapy Group. The health-related quality of life was improved statistically significantly only for the Nintendo Wii Sports Group. In conclusion, it is suggested that as the virtual reality exercise program had a positive influence on chronic back pain of industrial workers, it should be considered as a home exercise program for back pain control.

Influence of Health-Related Quality of Life (EQ-5D) on Oral Health Problems in the Elderly in the National Health and Nutrition Survey (국민건강영양조사에서 나타난 노인의 건강관련 삶의 질(EQ-5D)이 구강건강 문제에 미치는 영향)

  • The purpose of this study was to examine the influence of the health-related quality of life(EQ-5D) on oral health problems in an effort to provide some information on health promotion programs geared toward the elderly and on the right direction of related policy setting. Raw data from the 7th National Nutrition Survey were used, and systematic sampling was conducted on elderly subjects over 65 years. EQ-5D and chewing problems increased in the group without exercise ability, in the group with pain discomfort, and in the group with anxiety depression (p<0.05). EQ-5D and speaking problems increased in terms of odds ratio(OR) in the group without exercise ability, in the group with pain discomfort, and more in terms of articulating difficulties in the anxiety depression group (p<0.05). The results of this study show a direct correlation between health-related quality of life (EQ-5D) and the oral health of the elderly. As it's mandatory to promote the oral health of elderly people to improve their EQ-5D To promote rural societies should provide oral health education and institutions for continuous oral health care.

A Study on the Subjectivity of Pain Management of Nurse (간호사의 통증관리에 대한 주관성 연구)

  • The purpose of this study is to analyze the structural pattern of nurses' pain management with patients who experience pain. As a research method, the Q methodology, which is useful for an objective view of a highly abstract concept, was applied. The data collecting activity of this study was from August, 21st, 2000 to November, 24th 2000. The Q-population, the previous thesis and a literature review were done. Questions about pain management by the nurse on the patients, caregivers, nursing students, doctors, nurses, and others were asked in documentary work and in-depth interviews. In all, 223 units of the Q-population were formed, and the last 35 units of the Q-population were extracted. The data on the P-sample was collected from 41 nurses who worked in the medical and surgical units of a hospital that belonges to C university in Seoul. The research results were constituted in 3 types. Type I was the 'pattern of judging by objectivity'. The statement on which most of the people highly agreed for those patterns was shown by 'If patients said that they are suffering from pain, we sufficiently performed an assessment about the etiology, location, duration and degree'. For type I, the same pain was found in different locations according to the patients, so the etiology of the pain should be identified first place. Since ways of coping are different according to pain etiology, it was thought that it is important to assess sufficiently the pain etiology, location, duration, and degree. Therefore, when patients complain of pain, the pain etiology should be identified and assessed; according to the result, pain management should be performed systematically. Type II was the 'pattern of accepting by subjectivity'. The statement on which most of the people highly agreed for those patterns was shown as 'If patients said that they are suffering from pain, the medical treatment should be performed rapidly and speedily.' For type II, when the patient complains of pain, treatment should be performed quickly in order to prevent the condition getting worse, and it is thought that activity is a reasonable duty. Further, by trying to show empathy after pain is admitted and by understanding and coping rapidly with the pain of patients, an attitude which matched the altruistic morals of nurses is being shown. Type III was the 'pattern of worrying about', and the statement on which most of the people highly agreed for those patterns was shown is 'When there is a pain, to help patients to tolerate the pain to the highest degree.' In type III, the pain is a subjective expression, so there is a difference according to every individual. Therefore, actually if there is no measurement of pain, it could be exaggerated so nurses should help patients to tolerate it to the utmost. Even if there is a way to remove pain without an analgesic drug, nurses were reluctant to perform pain management as they possibly could. Through these research results, pain management of nurses was classified in 3 types, and structural characteristics in each type were discovered. Based upon the characteristics according to the type, an individualized pain management intervention strategy should be established and the follow up work performed.

Oral Health-Related Quality of Life of the Elderly under Visiting Health Care (방문건강관리사업 대상 노인의 구강건강 관련 삶의 질과 관련 요인)

  • The aim of this study was to investigate the relationship between the oral health status of elderly under visiting health care and their oral health-related quality of life. The subjects were 300 elderly under visiting health care program over 65 years old in Asan city and they were interviewed. The result showed that average Geriatric Oral Health Assessment Index (GOHAI) score was 37.36 and the lowest scored question was 'never satisfied with the oral condition'. In GOHAI related to oral health behaviors, unmet dental treatment experience was statistically significant (p<0.001). In overall GOHAI, number of remaining teeth (p<0.001) and the age had statistical significance and men had higher score in pain and discomfort (p<0.05), under age of 74 limited function factors score was high (p<0.01). The number of remaining teeth had statistical significances in most factors except pain and discomfort (p<0.001). The factors affecting GOHAI were age (p<0.05), number of remaining teeth (p<0.001) and perceived health state (p<0.001). In order to improve oral health-related quality of life of elderly receiving visiting oral health care services, it is necessary to increase denture support by securing budgets and developing oral health promotion programs at the local government level.
