The effects of auricular acupressure on pain and quality of life in patients with lung cancer (이압요법이 폐암 환자의 통증과 폐암 관련 삶의 질에 미치는 영향)

  • This study examined the effects of auricular acupressure on the pain and quality of life of lung cancer patients. Forty-four participants, and were divided into an experimental group (n=22) and a placebo control group (n=22). The patients in the experimental group received vaccaria seed to the four auricular acupressure points. In contrast, the patients in the placebo control groups received auricular acupressure on cancer-related points unrelated to pain. The intervention was implemented for six weeks, with five consecutive days a week. The pain intensity, pain pressure threshold, and quality of life score were used to validate the effects of the treatment. The pain intensity of the experimental group showed a significant decrease compared to the placebo control group after the intervention (Z=-2.78, p=.006). The pain pressure threshold of the experimental group showed a significant increase compared to the placebo control group after the intervention (Z=-2.69, p=.007). The quality of life among lung cancer patients showed a significant increase after the intervention compared to the placebo control group (t=3.20, p=.003). Therefore, auricular acupressure can be used as a proven nursing intervention method for lung cancer patients from cancer-related pain and the quality of life.

  • The aim of palliative radiotherapy (RT) is to control cancer-related local symptoms with minimal radiation reaction. About one third of all radiation treatments are given with palliative intent. Indications for RT in symptom palliation are as follows: Pain from bone metastasis, pressure symptom from brain and spinal cord, obstruction of bronchus, esophagus, superior vena cava and malignant cancer bleeding from bronchus, urinary tract, uterine cervix and rectum. In hospice palliative care, RT is very effective for symptom palliation and improvement of quality of life without influence on survival.

  • High spinal block is a rare complication during epidural block, but it may result in serious events. 56-year-old man with gall stones was scheduled for cholecystectomy under general anesthesia. After operation, lumbar epidural catheterization was done at $T_{8-9}$ interspace for postoperative pain control. At the recovery room, initial bolus drug (0.1% bupivacaine 10 ml containing fentanyl $100{\mu}g$) was administered via epidural catheter and observed carefully. 15 minutes later, hypotension and bradycardia ouccurred. Hartman' solution was administered rapidly and ephedrine 5 mg was injected. 30 minutes after drug administration, loss of consciousness and respiratory arrest developed. Tracheal intubation was done immediately. Cardiovascular and respiratory functions were monitored continuously. The location of intrathecal catheter was confirmed by cerebrospinal fluid (CSF) seen in syringe after aspiration of catheter. The patient recovered gradually and was placed in the ward 4 hours after drug administration, without any problems. He was discharged 1 week later in good health.

Respiratory Depression during Oral MS-Contin Administration for Pain Management of Gall Bladder Cancer (말기 담낭암 환자의 통증조절을 위해 MS Contin 투여중 발생한 호흡억제 -증례 보고-)

  • MS-Contin is an oral controlled-release preparation of morphine sulfate that has been used widely in the management of advanced cancer pain. It prolongs plasma concentration of morphine with no observable accumulation properties following repeated dosing, thereby promoting uninterrupted sleep and hopefully improving patient's quality of life. The common side effects of MS Contin are nausea, vomiting, drowsiness and constipation. But these symptoms are usually mild and respiratory depression is a rare problem. We experienced respiratory depression during oral administration of MS contin for the pain management of advanced gall bladder cancer of 76 years old male patient with metastasis at liver, intestine and cervical lymph node. After we increased the dosage of MS Contin from 160mg to 220mg per day, due to abdominal pain, we observed morphine reaction of MS Contin overdose such as pinpoint pupil, deeply slow respiration below 8/minute, and drowsiness. After intravenous bolus injection of 0.4 mg naloxone followed by continuous administration of 0.2 mg/hr for 4 hours, the patient regained consciousness. The administered route of morphine was changed to intravenous PCA (patient controlled analgesia). There was no aspiration sign as confirmed by chest x-ray. The patient was comfortable and delayed no signs of respiratory depression until now.

  • Botulinum toxin has been used for treating strabismus, blepharospasm, cerebral palsy, cervical dystonia, hyperhydrosis, facial wrinkle and chronic migraine under US Food and Drug administration approval. Also it has been tried spasticity-induced pain, post-herpetic neuralgia, myofascial pain and aphthous ulcer as off-label use. In this study, we reviewed recent studies that suggested effects of botulinum toxin on snoring and sleep apnea.

  • With the medical progress that has given spinal cord injured(SCI) individuals greater longevity and better overall health, chronic pain is emerged as major challenge in treating this population. According to past reports, estimates of prevalance of severe/disabling chronic pain in SCI patients have ranged from 18% to 63%. In etiologies of chronic pain in SCI patients, psychic or psychogenic pain categories should be included and more recent data have demonstrated that the persistant pain is directly related to higher levels of psychosocial distress and impairment. Recently, neurophysiological classification of the SCI pain syndrome into three etiologic groups(a; mechanical pain, b; radicular pain, c; deafferentation pain) is more frequently adopted for the classification of chronic SCI pain syndrome. The deafferentation pain is most common of the pain syndromes associated with SCI. After cervical epidural anesthesia for the surgical intervention of decubitus ulcer on the hip of two SCI patients, there were much reduction of existing chronic deafferentation character pain.

A Effectiveness of Butorphanol and Nalbuphine as Utilized with Ketorolac in Patient Controlled Analgesia after Total Abdominal Hysterectomy (복식 전자궁 절제술 후 통증자가조절을 통하여 Ketorolac과 함께 투여한 Butorphanol과 Nalbuphine의 효과)

  • Background: This study was designed to know the dose requirement, analgesic efficacy and side effects of butorphanol and nalbuphine when administered with ketorolac by patient controlled analgesia (PCA) after total abdominal hysterectomy. Methods: Forty women who underwent total abdominal hysterectomy received ketorolac (bolus dose 2.4 mg, lockout interval 10 min) with either butorphanol (bolus dose 0.1 mg) or nalbuphine (bolus dose 1 mg) using PCA pump postoperatively. Results: Total amounts of 48 hr consumption were 8.7 mg (butorphanol)and 61.5 mg (nalbuphine). There were no significant differences between two groups in total ketorolac infusion doses, VAS score and side effects. Conclusions: Both butorphanol and nalbuphine were useful for PCA for postoperative pain control. We may suggest that ketorolac 180 mg with butorphanol 9 mg or nalbuphine 70 mg would be useful for 48 hr postoperative pain control.

  • 반복적인 투구동작은 성장기의 주관절에 손상을 주기 쉬우며 투구 시 통증을 유발할 경우, 투구를 중지하고 관절의 능동적 휴식을 통해 유연성을 회복하고 점차 근력강화운동을 시행해야 한다. 유소년기 Little league elbow의 주된 병변인 상완골 내상과 골단염을 포함한 대부분의 손상은 보존적 요법으로 치료가 가능하며, 굴곡구축 등의 운동제한을 동반한 통증이나 관절 내 유리골편 등의 감입으로 인한 증상이 있을 경우 수술적 치료를 요하며, 성인의 경우 내측 측부인대의 만성 불안정성 둥에 대해서 인대 재건술 등의 수술적 치료가 고려된다. 투구 시 주관절의 손상은 대부분의 경우 과 사용으로 인한 것이며, 특히 성장기에 손상의 예방이 무엇보다 중요하다. 따라서 부모와 코치 그리고 경기 관계자 둥이 성장기 소년의 근력과 관절의 상태가 성인과 다름을 이해하고, 운동의 강도와 횟수를 적절히 조절하며 손상시 조기치료를 받을 수 있는 환경을 조성하는 것이 필요하다.

  • Neurolytic splanchnic nerve block is effective for treatment of intractable upper abdominal cancer in. The possibility of neurologic complication cannot be completely precluded. A 53 year-old female patient with hepatoma and lung metastasis was submitted for splanchnic nerve block with alcohol. Splanchnic nerve block was performed under radiologic control by image intensifier without any difficulty during procedure. One day after the block, she complained of numbness with sensory deficit in bilateral inguinal area. Neurologic examination revealed that asymmetrical anesthesia, hypoesthesia at $T_{12}-L_2$. She was discharged 70 days after the block with partial improvement. The possibility of alcohol spreading to the low theracic and upper lumbar somatic nerves cannot be excluded.

Effect of surgical site instillation after laparoscopic urologic surgery (비뇨기과 복강경 수술 후 국소마취제 점적의 효과)

  • The aim of this study was to evaluate the effect of Ropivacaine on pain following laparoscopic surgery. Between March 2015 and June 2017, all 97 patients with renal mass who elected to receive laparoscopic nephrectomy were reviewed, retrospectively. Normal saline (0.9%) was used as a control in 45 patients while Ropivacaine was used for local anesthesia in the instillation group (n=52). Pain score (visual analog scale) of the shoulder and upper abdominal area between the two groups was significantly different between recovery time and 24 h (p<0.05), with no significant differences between 48 and 72 h (both p>0.05). In the instillation group, pain score of shoulder and abdominal pain in the two groups according to the amount of CO2 used was not significantly different between recovery time and 24, 48, and 72 h (all p>0.05). Ropivacaine reduced shoulder and upper abdominal pain that occurred within 24 h after surgery. Effective pain control with Ropivacaine is needed in the early postoperative period when a large amount of CO2 is used in laparoscopic surgery.
