만성 관절 통증 여성 노인환자에 있어 관절염 태극권의 효과에 대한 pilot study (The Effects of Tai Chi for Arthritis on Chronic Arthritic Pain of Senior Female : A Pilot Study)

  • Objectives : Tai Chi for arthritis was developed in 1997 by Paul Lam, M.D. of family medicine in Australia. It is an exercise treatment program and a good approaching method for arthritis. However its application is limited to arthritis of knee joint only. Even there have been no clinical study in korean traditional medical society. The aim of this study is to investigate effect of the Tai Chi for arthritis on chronic pain of body. Methods : We designed this trial as objectives were (1) senior female over 60 years, (2) suffering for osteoarthritis and chronic body pain, (3) have no physical or mental dysfunctions (4) able to walk and exercise alone. Objectives mactched these criteria had been trained the Tai Chi for arthritis 2 hours a day for 21 weeks. Before and after training, pain of 8 joints (neck, shoulder, elbow, wrist, waist, hip, knee, ankle) were estimated with the VAS(Visual Analog Scale). Results : Training the Tai Chi for arthritis relieved multiple joint pain(especially neck, lower back, shoulder, wrist and knee) and these were statistically significant. Conclusions : This study is a pilot study to investigate effect of the Tai Chi for arthritis on chronic pain of whole body. Absence of control group, and other scale to test joint function except VAS were limitations of this study. It is the first clinical approach about Tai Chi in korean medicine field, also the first study of Tai Chi for pain of the whole body.

파킨슨병 환자를 대상으로 한 태극권 요법 -임상 연구 경험을 통해 본 문제점과 개선 방안- (Tai-Chi for Patients with Parkinson's Disease -the problem and improvement demand of clinical study experience-)

  • Objectives : This study was to analyse the reasons for the early termination of our clinical trials that were to know the influences of Tai-chi on non-motor symptoms of Parkinson's disease. Methods : We stopped the primary study of Tai-chi to observe the changes of non-motor symptoms of Parkinson's disease. So we carried out the survey to our participants of clinical trials. We conducted the survey about the patient's characteristics of usual exercise, experience of Tai-chi, appropriate time allocation, difficulties of proceeding the program and patient's opinion for the improvement of education. And the 4 questions about the contributiveness of Tai-chi for the Parkinson's disease, usability of supplied references, difficulties and time allocation of education were using the questionaries form of 5-point scale(Likert scale). Results : The results of survey showed that the benefits of Tai-chi program for the improvement of symptoms was $3.15{\pm}0.89$ and difficulty of lecture was $3.76{\pm}1.09$. 41% of participants answered that the motions of the Tai-chi is difficult to follow and 17% of participants felt the imbalance during the motion of Tai-chi. Conclusions : Participants replied that the Tai-chi is hard for the patients of Parkinson's disease and the time for the education was too short to keep doing exercise. Thus we have plan to educate the patients of Parkinson's disease with Tai-chi program consists of easier motion by developing ourselves or searching previous studies.

태극건강기공체조(太極健康氣功體操)가 신체(身體)·생리적(生理的) 기능(機能)에 미치는 영향(影響) (Effects of the Taeguk Gi Gong Exercise on Physical and Physiological Functions)

  • Objective : This The purpose of this study is to find out the effects of the Taeguk Gi Gong Exercise program on physical and physiological functions. Methods : The study is performed with one group pretest-posttest design. A total of 20 subjects are selected by convenience sampling, which are comprised of 11 researchers of a research center, and 9 nurses of a general hospital. The average age of the subjects is 26.3 years old. The Taeguk Gi Gong Exercise program is consisted of 30-minute main exercise and 10-minute finishing exercise. During the first two weeks, the 40 minute program was conducted in a group, 5 times per week. From the third week, it was conducted 3 times per week, twice in a group and once individually by watching video. All of the subjects were examined for the vital signs, blood sugar, body weight, and the flexibility of joint before and after conducting the program. Results : The results of the study are as follows: 1. After the Taeguk Gi Gong Exercise, the systolic blood pressure increased, and there is no change in the diastolic blood pressure, but all of them are not significant. 2. After the Taeguk Gi Gong Exercise, the pulse decreased, but not significant. 3. After the Taeguk Gi Gong Exercise, the blood sugar increased, but not significant. 4. After the Taeguk Gi Gong Exercise, the body weight reduced significantly. 5. After the Taeguk Gi Gong Exercise, the flexibility of waist significantly increased. 6. After the Taeguk Gi Gong Exercise, the flexibility of arm significantly increased(Left : t=-4.219, p=.000, Right : t=-2.74, p=.013). Conclusion : Based on the results described above, it is considered that the Taeguk Gi Gong Exercise program is clearly effective in decreasing body weight and flexibility of joint.

태극권 수련의 이론 전개에 대한 연구 (Philosophical Study on the Theory of Self-Discipline in Taijiquan)

  • Taijiquan is possible for us to use not only as a martial art, but also as a physical exercise and a way to discipline human mind, and so it is called 'moving-Zen' and is a martial art to discipline human body and mind against different diseases and stress of modern adult people caused by our modern advanced civilization. We can look at natural passion as one of the most fundamental categories in Philosophy as a minimum material unit comprising all nature. Taijiquan is an exercise with natural passion, flowing through all the body, leading the natural passion with our mind, and moving our body with the natural energy. Also, all the motions in Taijiquan were made based on Yin and Yang. Taijiquan is a discipling way to reach Taegeuk through Yin and Yang and to train ourselves together. The realization of Taegeuk is to reach Taegeuk through the discipline of Taegeuk and can lead to Taegeuk through the unification of the inside and the outside(內外合一), the mutually complementing(剛柔相濟), the circling of the heavenly body(周天) of Taijiquan. The strength and the weakness help each other, the mind and the body is unified, the mixture of the rapidness and the slowness is appropriate, the form and the consciousness are combined into one and move, naturally unified into one(形意結合), and progressive and regressive Junsakyung are combined and move, naturally forming Turoh(套路) where Yin and Yang are unified. The discipline of Taijiquan is not only for maintaining our health, but also for leading to Taegeuk, an ultimate entity, which had existed before the day when all things were generated as its essence and has existed since then.

아건강(亞健康) 치료[치미병(治未病)]에 미치는 태극침법의 효능에 대한 임상적 관찰 (Clinical Observation on Effectiveness of Taegeuk Acupuncture for Patients in Sub-health Status: a Case Series)

  • Objectives : This study aimed to report observed effectiveness and safety of Taegeuk acupuncture for patients in sub-health status. Methods : We analyzed clinical medical records of 36 outpatients who have visited the Pusan national university Korean medicine hospital between March and June, 2010. Patients received 6 sessions of Taegeuk acupuncture according to their constitutional types. Hepatic dullness sound and symptom reduction were used for the evaluation of clinical effectiveness of Taegeuk acupuncture. One traditional Korean medicine doctor performed all treatment procedures and assessments. Patients were allowed to continue their previous medication during acupuncture treatment. However, no additional traditional Korean medicinal interventions except the acupuncture were conducted during the whole treatment period. Results : After 6 sessions of Taegeuk acupuncture, mean hepatic dullness sound scores (0~100) were reduced from $97.3{\pm}9.0$ to $24.3{\pm}23.7$ with statistical significance (n=36, p<0.001). Mean symptom reduction scores (0~100) were changed from 0 to $53.3{\pm}29.6$ with statistical significance (n=24, p<0.001). 24 patients reported 44 cases of additional improvements of perceived health condition during or after the treatment course. No serious adverse events related to the Taegeuk acupuncture treatment were observed. Conclusions : Taegeuk acupunture may be a feasible, effective and safe treatment intervention for patients with chronic symptoms. Further well-designed studies are needed to confirm those observed improvements and evaluate benefits of Taegeuk acupuncture.

노년기 여성을 위한 보건소 운동프로그램이 체중, 신체질량지수, 일부 체력 요소 및 혈압에 미치는 효과 (Effect of a Public Health Center Exercise Program on Physical Fitness Factors, Body Weight, Body Mass Index and Blood Pressure in Elderly Women)

  • Purpose: The study examined the effect of exercise public health center exercise program on strength, balance, body composition and blood pressure in healthy elderly women. Method: Thirty healthy elderly women were divided into three 12-week exercise groups: Tai-Chi (n=10), silver dance (n=10), and band exercise (n=10). The results were analyzed by two-way repeated ANOVA and paired t-test using SPSS 12.0 program. Results: Body weight and body mass index were significantly decreased in the Tai-Chi and silver dance groups. In addition, there was a significant increase in balance time and reduced diastolic blood pressure in Tai-Chi participant. Grip strength was significantly increased in Tai-Chi and silver dance participants, but there was no effect in the band exercise group. Agility was significantly increased in the band exercise group. Conclusion: Tai-Chi and silver dance may be beneficial to improve fitness of elderly people, although the details remain to be elucidated.

태극권24식(太極拳24式)이 건가(健康)에 미치는 영향(影響)에 대한 소고(小考) (A study on the Effect of Health of the 24-Form Taegeukkwon)

  • Taegeuk is the source and union of the two primary aspects of the cosmos, Eum(陰, Yin) and Yang(陽). The Neo-Confucian philosophers of Chinese Song(宋) dynasty associated Taegeuk with the supreme rational principle of the universe. Taegeuk is a powerful blend of self-discipline, self-defense and healing. It is an art which has grown over the millennia to include hundreds of forms and variations, and can easily take a lifetime to master. Taegeukgwon based on the Taoist philosophies of Eum(陰, Yin) and Yang(陽) is an ancient Eastern form of exercises, characterized by slow reversing movements. There is strong research evidence validating Taegeuk as a safe form of exercise having potential for health promotion and rehabilitation. Today, Taegeukgwon has spread worldwide. The Internal Training of Taegeuk is one of the exercises for regiment. As exercise, Taegeukgwon is designed to provide relaxation in the process of body-conditioning exercise and is drawn from the principles of Taegeuk, notably including the harmonizing of the Eum-yang(陰陽, Yin-yang), respectively the passive and the active principles. It employs flowing, rhythmic, deliberate movements, with carefully individually prescribed stances and positions. In practice, two masters teach the system exactly alike. As a mode of attack and defense, Taegeukgwon resembles Kungfu and is properly considered a martial art. It may be used with or without weapons. Freehand exercise to promote health was practiced in Korea. This training proved to have effect on hypertension, enervation, impotence, premature ejaculation, seminal emission, chronic gastrointestinal diseases, and arteriosclerosis. There have been many schools of Taegeukgwon. Depending on school and master, the number of prescribed exercise forms varies from 24 to 108 or more. The objective of this paper is to reconsider Taegeukgwon and its effect on the body and mind-conditioning in Hwangjenaegyeong(黃帝內經).

"영추(靈樞).구침십이원편(九鍼十二原篇)"을 중심으로 한 태극침법(太極鍼法) 분석 (A study on the analysis of Taegeuk Acupuncture focused on "Young Chu.GuChimSibYiWon")

  • Objective : This study was conducted to underpin the theory of Taegeuk Acupuncture systematically and increase the utility of clinical trial and a range of application. Method : This study analysed ByungHaeng Lee's theory, who is the originator of Taegeuk Acupuncture, by focusing on "Young Chu GuChimSibYiWon" Result & Conclusion : 1. ByungHaeng Lee's thought about Viscera and Bowels is following medicine of Internal Classic. Even though JeMa Lee's thought about Viscera and Bowels has same name with ByungHaeng Lee's, but the functional meaning and related part is totally different. So, four constitution types in Taegeuk Acupuncture are different from four constitution types in Four-Constitution Medicine. Therefore, Taegeuk Acupuncture isn't related with Four-Constitution Medicine 2. On the authority of "Young Chu GuChimSibYiWon", we can extend 4 therapeutic acupuncture point combinations of Taegeuk Acupuncture to ten therapeutic acupuncture point combinations which tentatively named 'Five Viscera Source Point Acupuncture'. 3. To consider an example of eight constitutional acupuncture, Taegeuk Acupuncture thoroughly says constitutional acupuncture. Especially, JaeGyuKim's Taegeuk Acupuncture has specific diagnosis standard that after treating a patient with acupuncture, he checks an oppressive pain at the precordial region and a voiced sound at liver and then he makes sure constitution of patient. So we can consider JaeGyuKim's Taegeuk Acupuncture as constitutional acupuncture. However, it has to change the name of four constitution types to taegeuk greater yang person, taegeuk lesser yang person, taegeuk greater yin person, taegeuk lesser yin person so as to avoid confusion. 4. Taegeuk Acupuncture is markedly simple in comparison with other acupuncture, so it needs to extend the utility of clinics by enlarging treatment acupuncture points.

고혈압 환자에서 경락도인태극권의 혈압강하 효과 (Antihypertensive effect of Meridian Tai Ji exercise treatment in hypertension patients)

  • Objectives : The purpose of the study was to determine the effects of a 4 weeks Meridian Tai Ji exercise treatment on blood pressure reducing in hypertensive patients. Methods : We measured the blood pressure of 3 patients who were visited in the Gwacheon Health center from 21th November 2007 to 17th December 2007. Eligible participants had systolic blood pressure ${\geq}$ 120 mmHg or diastolic blood pressure ${\geq}$ 80 mmHg with antihypertensive drug. Blood pressure measurements were after the patient had been in rest for at least 10 minute. Twelve sessions of Meridian Tai Ji exercise treatment over 4 weeks were performed in the patients. Blood pressure were measured twice before and after each session. Results : After 4 weeks, blood pressure reduction was observed in the treatment patients, with an average decline of systolic blood pressure up to 5.67 mmHg and diastolic blood pressure up to 0.17 mmHg. But, it was not statistically significant. Blood pressure increased between measurements taken before and after an Meridian Tai Ji exercise treatment session, although individual responses were quite variable. No significant difference was found in the immediate effect. Conclusion : Twelve sessions of Meridian Tai Ji exercise treatment do not seem to control hypertension. Further research is required to determine whether Meridian Tai Ji exercise treatment can enhance clinical management of hypertension if it is used in combination with more enrolled participants, over longer periods, or randomized controlled trial.

전통사상(專統思想)과 서원건축배치(書院建築配置)의 구성(構成) 원리(原理) (Composition Principle of Seo-Won Architecture from the View of Its Disposition and Korean Traditional Thought)

  • The Korean academics of classical learning, Seowon which from the middle of Joseon Dynasty was complexly reflected in "the illustration of Taiji(太極圖說)" Five-Elements school(陰陽五行說), "Zhou Yi(周易)" and a theory on spherical heaven and square ground(天圓地方) which based on orientalism. Also the theory of Xiangshu Xue(象數學) was a significant factor to decide the size(number of facade module) of Seowon architecture. So, in this study, how the oriental thought was adopted and reflected in existing 21 Seowon in South Korea. The size of Seowon architecture was adopted a theory of combination with heaven, earth and human(天地人三合論) that based on the theory of Xiangshu Xue on "the illustration of Taiji" and "Zhou Yi". "Zhou Yi" was the central thought of Confucian culture in Joseon Dynasty, with which Seowon space was divided into two, ancestral rites space and lecture space. It coincides with balance of yin(陰) and yang(陽), Five-Elements(五行) and four seasons(四季節). In lecture space, lecture hall is relevant with the water(水) and winter, and front tower structure or outer three-door is the fire(火) and summer. Also, central garden means the soil(土) and center. Thus, the size and spatial composition was planned with the philosophy, "the illustration of Taiji", Five-Elements school and a theory on spherical heaven and square ground. Yin and yang has an idea of the heaven and earth, and Five-Elements has an idea of direction and season with which spatial composition of Seowon could be set. And the numeral meaning on the theory of Xiangshu Xue established an ideal background for spatial composition of Seowon architecture.
