THE PRACTICE PATTERN OF PEDIATRIC DENTISTS IN KOREA (소아치과 개원의 진료 현황 분석)

  • Pediatric dentistry is differentiated from other fields of dentistry in that it provides comprehensive dental care for children and adolescents. In early days, pediatric dentistry used to be confined to the caries treatment and extraction of primary teeth However, the practice spectrum of pediatric dentistry has broadened to the orthodontic, surgical, esthetic and preventive treatments A survey that contained questions about practice patterns were mailed to 50 pediatric dentists, and 21 surveys were returned. Results were as follows: 1. The average number of patients per week was 82.4, and the average number of treated teeth per patient was 2.35. 2. Preventive treatments comprised 15.7%, restorative treatments 55.7%, endodontic treatments 15.6%, surgical treatments 10.5%, and orthodontic treatments 2.4%. 3. In restorative treatments, amalgam restoration comprised 3.8%, glass ionomer 5.5%, composite resin 63.0%, and stainless steel crown 27.7%.

Implant placement in severely atrophic mandible using alveolar ridge splitting procedure and small block bone graft: A case report of 4-year follow-up (심하게 위축된 하악 구치부에 치조제 분할술과 블록형 골이식술을 이용한 임플란트 식립: 4년 관찰 증례보고)

  • Various techniques have been developed, and the development of piezo electric devices have made it possible to widen the alveolar ridge even if the residual bone is dense or if there is a lack of cancellous bone between the cortical bones. In the operation of the mandibular posterior area, the flap is easily accessible to the ramus bone, from which high quality autogenous bone can be obtained, compared to other parts. A small autologous bone block can be used with particulated bone graft material using one screw for bone regeneration instead of a large autologous bone with two screws. The tapered implant design can minimize buccal bone fracture, even in severely atrophic mandibular areas. We report a case of 4 years following implant placement with ridge splitting and small autogenous bone graft in severly atrophic mandible. This report demonstrates a case of functional and aesthetic restoration in a patient through a collaboration.

TREATMENT OF ANTERIOR TEETH FRACTURE BY FORCED ERUPTION (치아 정출술을 이용한 전치부 외상치의 치험례)

  • There have been many treatment methods for traumatic subgingival crown fracture and intrusion without spontaneous eruption. The orthodontic forced eruption generally results in favorable clinical findings than crown lengthening with osteotomy and intentional replantation. In first two cases with subgingival crown fracture due to trauma, authors applied orthodontic forced eruption with axed appliance after root canal therapy and then restored them with composite resin. In another case with traumatic intrusive luxation, we observed spontaneous eruption of the corresponding tooth for about 6 months and then returning it to normal position by forced eruption with removable appliance, but root canal filling was conducted after apexification due to devitalization during forced eruption, and so clinically favorable results were obtained.

  • Cigarette smoking is a significant risk factor for periodontaldiseases and implant treatments. Smoking control is regarded as a key to the success of dental treatments as well as the well-being of the patients. The aim of this study was to reviewthe effects of smoking on periodontal health, and the results of periodontal therapy and implant treatments. Also, in vitro, microbiological, immunological and epidemiological relationships were studied. In vitro studies show that smoking interferes with normal healing process and increased tissue destruction. There is still controversy on the population of microbes of smokers. Smokers showed significantly less gingival inflammation and bleeding on probing compared with non-smokers. After periodontal treatments, a compromised clinical outcome was noted for smokers in terms of pocket depth reduction and gain in attachment levels. In conclusion, data from in vitro, epidemiological, cross-sectional and case-control studies strongly suggest that quitting smoking is beneficial to patients before periodontal and implant treatments.

Implant assisted removable partial denture with implant surveyed prostheses: A 9-year follow-up (임플란트 서베이드 보철물을 이용한 임플란트 보조 국소의치의 9년 경과 관찰 증례)

  • Implant placement may be restricted by anatomical and/or financial limitations in restoring a completely edentulous arch, or the patients' unwillingness to have extensive surgical procedures. Implant assisted removable partial dentures (IARPD) in combination with anterior fixed implant prostheses can be proposed as an alternative treatment option for the restoration of a completely edentulous arch. In this case, a 56-year-old female patient who has a fully edentulous maxilla opposed by partially edentulous mandible was treated. The treatment option for the maxilla consisted of an implant-assisted removable partial denture supported by four anterior fixed implant prostheses. The mandible was restored with 8 anterior fixed partial dentures and posterior fixed implant-supported prostheses. Long-term follow-up and supportive periodontal treatment were performed for 9 years, and the patient was satisfied with the overall appearance as well as masticatory function.

Long-Term Observation for 10 Years after Dental Implant Surgery in Hemophilia A Patients (혈우병 A 환자의 치과 임플란트 수술 후 10년 추적 관찰)

  • Hemophilia is a congenital hemorrhagic disease in which blood clotting is impaired and hemostatic abnormalities occur when even one of the 13 factors required for blood clotting are deficient. Among the 13 factors, hemophilia A (factor 8), hemophilia B (factor 9), and hemophilia C (factor 10), depending on the deficient coagulation factor. A male patient in his 40s diagnosed with hemophilia A visited the dentist at Eulji University Hospital for problems with alveolar bone absorption and periodontal abscess caused by overall chronic tooth inflammation, and showed overall bone absorption and inflammation at the time of initial diagnosis. No complications or bleeding tendency were observed in the rest of the area except for #35 and #36 due to regular visits for 10 years, overall good oral conditions, and no side effects or complications were observed.

The risk factors for implant survival and marginal bone loss: a retrospective long-term study (임플란트 장기간 유지와 변연골 소실에 영향을 주는 요인들에 대한 후향적 연구)

  • Purpose: This study aims to investigate the risk indicators contributing to implant failure, and analyze the relationship between risk indicators and marginal bone loss (MBL) through long-term follow-up over 3 years. Materials and Methods: From 2003 to 2017, patients' medical charts with a history of dental implant surgery at Chonnam National University Dental Hospital were reviewed retrospectively. The patient's demographic variables, and clinical variables were recorded. Periapical radiographs were used to evaluated the changes in MBL around implants. And we analyzed implant survival rates. Multiple regression analysis with backward elimination was conducted to correlate the patient's clinical variables and implant failure and Pearson correlation analysis was performed to the correlated between implant long-term survival rates and MBL and initial stability. Results: In multiple regression analysis, there was a statistically significant negative correlation between abutment connection type (β = -.189, P < .05), with or without SPT (β = -.163, P < .05), diabetes (β = -.164, P < .05), osteoporosis (β = -.211, P < .05) and MBL. Anticoagulant medication influenced the long-term success rate of implants. PTV values at the second implant surgery showed a statistically significant negative correlation with long-term implant survival (P < .05). Conclusion: For the long-term success of the implant, the appropriate abutment connection type must be selected and the periodic SPT is recommended. Systemic diseases such as diabetes and osteoporosis and anticoagulant medication should be considered. Furthermore, since high PTV at the second implant surgery correlated with the long-term survival rates of the implant, initial stability should be carefully considered before undergoing the prosthetic procedure.

THE EFFECTS OF POLYTETRAFLUOROETHYLENE MEMBRANE AND MILLIPORE FILTER COMBINED WITH FIBRIN ON THE REGENERATION OF PERIODONTIUM IN DOGS (Polytetrafluoroethylene membrane 및 millipore filter를 섬유소와 병용사용시 성견 치주조직 재생에 미치는 영향)

  • The ultimate goal of periodontal therapy is the regeneration of the periodontium that have been destroyed as a result of periodontal disease. This study were done in order to determine the healing status of periodontium under Polytetrafluoroethylene and millipore fillter combined with fibrin and the effect of the guided tissue regeneration procedures were performed as follows : 1) flap operation using PTFE membrane(control group) 2) flap operation using PTFE membrane which was fixed with fibrin(experimental group 1) 3) flap operation using millipore filter which was fixed with suture(experimental group II) 4) flap operation using millipore filter which was fixed with fibrin(experimental group II) After 1, 2, 4, 8, 12 weeks, dogs were sacrificed by perfusion technique and tissue block was excised including the tooth and prepared for light microscope with H-E & Masson’s trichrome staining. The result were as follows : In control and experimental group, there is no siginificant difference on epithelial cell down growth within 1st week, but more epithelial cell downgrowth in millipore or millipore combined with fibrin group. In this experiment, there were no significant difference in new cementum and alveolar bone formation whether PTFE membrane was fixed with suture or fibrin. In control and each experimental group, bone maturation appeared in 4 weeks, bone width increased bucco-lingually in control and experimental 1 group especially. Both control group and experimental group showed mild mew cementum formation on root surface and irregular arrangement of collagen fiber at 4 weeks, that showed obvious increased cementum formation at 8 weeks, and that was observed the functional arrangement of collagen fiber between new cementum and new alveolar bone at 12 weeks.

Initial changes of dental plaque, gingivitis and decalcification in Korean orthodontic patients with fixed appliance (한국인 고정식 교정 환자의 치태, 치은염 및 탈회의 초기 변화에 관한 연구)

  • Intraoral filled type of orthodontic appliance can cause reversible or irreversible damages such as gingivitis, periodontitis, enamel decalcification, dental caries, root resorption, and pulpal changes. Such adverse effects are brought by increase in dental plaque as well as oral flora. Such an increase causes gingival inflammation and enamel decalcification. The purpose of this study is to get klowledge on initial changes in dental plaque, gingivitis, and enamel decalcification after bonding fixed orthodontic appliances according to time flow, gender, and sides(right/left) of premolar region. For control group, 48 students of dental college, Yonsei university(26 males, 22 females) were chosen; for experimental group, 73 orthodontic patients(36 males, 37 females) who will be treated with fixed appliances were chosen. All the subjects had no systemic disease, juvenile periodontitis and all the females had passed their ,menarche. Tooth brushing instruction was given to all the subjects prior to the experiment. For control group, plaque index, gingival index, and decalcification index were measured twice at 3 weeks interval ; for experimental group, the same was done prior to, 3, 6, 9 weeks after bonding fixed appliances. The following results were obtained: 1. In plaque index 3 weeks after placement of appliances, and it showed gradual increase afterwards. 2. In gingival index3 weeks after placement of appliances, and afterwards it showed increase at a faster rate than plaque index. 3. Enamel decalcification began to show between 3 and 6 weeks after bonding fixed appliances. Decalcification index began to increase 6 weeks after appliance placement, but there was no statistical significance. 4. When the comparison was made between two sides of premolar region, the right side showed greater index in plaque and gingival index of experimental group.

Difference of perception of the duties of dental hygienist between dentists and dental hygiene students in an area (일개 지역 치과의사와 치위생학과 학생 간 치과위생사의 업무에 대한 인식의 차이)

  • Although dental hygienists have performed chair-side assisting and other dental cares as well as preventive dental cares in Korea, medical technician law confines duties of dental hygienists as closed narrative. The aim of this study was to investigate difference in perception of duties of dental hygienists in dental clinic between dentists and dental hygiene students. A total of 245 copies of questionnaires were distributed to dentists and students by post-mail. Only 42 dentists and 30 students in an area replied these questionnaires about the present and future duties of dental hygienists after providing written consent. Both groups agreed that intra and extra oral X-ray taking, education about oral health behavior, instruction after dental treatment, chair-side assisting, consulting for patients, scaling, initial impression taking, management of dental materials and equipment, sterilization of equipment, and receiving dental bills are duties of dental hygienists. However, they had different perceptions about various dental treatments as duties of dental hygienists even if they were under instructions of a dentist, including infiltration anesthesia, filling in cavity, intramuscular injection, FC change, canal irrigation, orthodontic treatment including separating, ligature bracket bonding and removing, setting crown and bridge, making individual, removing implant screw, and so on. These findings demonstrated that there were different perceptions about duties of dental hygienists between dentists and dental hygiene students, especially on dental treatment.
