  • 전 세계 IT 기업들은 중국 시장을 공략하기 위해 아낌없는 투자와 노력을 병행하고 있다. 한국 기업들 또한 아직까지는 저렴한 인건비 부분의 이점을 생각하고 진출을 하고 있는 상태이나, 추후 중국 현지 시장이 바로 세계 시장임을 아는 기업들은 현지 공략을 하기 위해 이미 준비를 마친 후 진출을 하고 있다. 하지만 해외시장 진출은 그 과정 자체가 녹록치 않다. 여는 국가처럼 중국으로 진출 시 기업들은 시간과 투자 방식, 사업진행 방향, 현지의 IT 인프라 및 인력 고용 등 여러 가지 당면과제에 맞닥뜨린다. 이를 해결하기 위해 기업들은 많은 시간과 자금을 비롯한 시행착오를 거치는 것 또한 사실이다. 그렇다면 가장 효율적이고 성공적인 해외시장진출의 첫 번째는 바로 사업환경이 잘 정비된 지역을 선택하는 것이 아닐까. 이번호에서는 중국의 여러 지역 중에서도 IT 기업의 진출에 최적화된 환경을 가진 연길(延吉)을 주도로 하는 연변조선족자치주를 살펴본다.

  • The stability and retention of mandibular complete denture have been a continuing problem for dentists and patients. Magnets have been investigated or used in clinical dentistry as retentive aids. The obvious advantages are that these attachments do not need spacial equipments or require frequent adjustment of replacement with use, and they likely transmit less lateral forces to the abutment.   • A few authors have reported good clinical results using double crown removable partial denture (RPD) with a few remaining teeth. Hybrid telescopic double crown is a good indication for a patient with poor periodontal condition and/or few remaining teeth after extraction, especially located in cross-arch position. In this case, there was a poor periodontal condition with teeth mobility. Several teeth with poor prognosis were extracted. Remaining anterior teeth was restored with fixed prostheses and edentulism was restored with Kennedy class I removable partial denture in maxilla. In mandible, it was hard to restore with clasp removable partial prostheses because of bilaterally isolated remaining teeth so that hybrid double crown removable partial denture with friction pin was suitable for this case. The objective of this report is to discuss the characteristics and the utility of hybrid double crown prostheses using a few remaining teeth in mandible.

Periodontal status in patients with osteoporosis (골다공증 환자의 치주조직 상태)

  • The purpose of this study was to evaluate the relationship between osteoporosis and periodontal condition in postmenopausal women. Twenty-eight patients who have been treated at Chonnam national university hospital with osteoporosis(osteoporotic group, mean $age62.2{\pm}6.6$) and 21 normal postmenopausal women with periodontitis(control group, mean $age60.4{\pm}3.7$) were examined. Bone mineral density(BMD) of lumbar spine(L2-L4) was measured by dual energy X-ray absorptiomemtry(DEXA). Percentage to peaked bone mass in osteoporotic group was 70.9% which had lower BMD than in control group(p<0.05). The number of present teeth were 23.6 in osteoporotic group and 23.1 in control group. Percentage of bleeding on probing was 41% in osteoporotic group and 37% in control group. probing depth and attachment level were shown 3.18 mm, 3.63 mm in osteoporotic group and 2.85mm, 3.11mm in control group, respectively. Probing depth and attachment level were significantly greater in osteoporotic group than in control group(p<0.05). The significant negative correlation was found between BMD level and periodontal attachment level(p<0.001, ${\gamma}=-0.56$). These results suggest that osteoporosis may be associated with periodontal breakdown.

A Comparative Study of Periodontal Conditions Following insertion of Distally Extending Cantilever Bridges or Removable Partial Dentures (후방연장 계속가공의치와 가철성 국소의치 지대치의 치주조직 상태에 관한 비교)

  • The purpose of the present study was to evaluate and compare the periodontal conditions of distally extending cantilever bridges(CB) and removable patial dentures(RPD). 78 patients treated with removable partial dentures were divided into 2 groups; bilateral free-ending (type 1) and unilateral free-ending(type 2). 43 patients treated with distally extending cantilever bridges were divided into 4 groups according to position of missing molars and number of including abutment tooth (or teeth) (type 1-4). Prosthodontic status of the abutment teeth were assessed; overcontoured crown, overhanged margin, embrasure closure, and others. Their Periodonted status were assessed with Plaque Index Gingial Index(GI) Sulcus bleedy Index (SBI), Tooth mobility(TM) & Posher depth(PPD). 35.9% of RPD group and 38.1 of CB grouphad disconfort on mastication with prosthetics. 16.7% of RPD group and 6.1% of CB group showed on opposing sites. Overhanged margins were 58.3% for RPD group and 86.6% for GB group. 45.5% of RPD group and 72% of CB group had embrasure closure on the abutment teeth. 30.5% of GB group had oversized pontic. Gb group showed much higher clinical parameters than RPD groups, but statistically not significant. Type 2 of RPD group showed lower GI and SBI, but statistically not significant. Type 3 of CB group showed the highest GI and SBI and type 2 showed the highest tooth mobility, but there is no differences between the same type of groups. In conclusion, only minor differences in periodontal conditions were recorded in RPD and CB group.

In Vitro Study Of Salivary Bacterial Adherence On e-PTFE Membrane According To Periodontal Status. (치주조직 상태에 따른 e-PTFE막 부착에 관한 연구)

  • The purpose of this study was to evaluate the bacterial adherence on e-PTFE membrane immersed in whole saliva from subjects with different periodontal status. Experiment involved 3 subject groups: 5 persons with healthy periodontium(probing depth below 3mm and no signs of gingival inflammation including bleeding on probing), 10 patients with gingivitis(probing depth below 3mm and apparent signs of gingival inflammation), and 10 patients with advanced periodontitis(probing depth over 7mm and apparent signs of gingival inflammation). Each disease group was included before and after scaling and root planing treatment. After obtaining whole saliva from each subject, e-PTFE membrane(Gore-Tex periodontal membrane : $GTPM^{(R)}$, W.L. Gore & Associates, Flagstaff, USA) specimens were immersed at room temperature in the saliva aliqouts for 1, 3, 7 days. The weight between pre - and post - immersion in saliva was measured with the analytical balance and the difference was recorded. The specimens were processed for SEM observation. The bacterial adherence on the membrane specimens was evaluated using the scanning electron microscope images. The obtained results were as follows : 1. There was no difference in the weight of bacteria adherent to e-PTFE membrane specimens according to the periodontal status and the immersion periods. 2. As the exposure time to saliva increased, the bacterial adherence to the membrane specimen significantly increased in all groups(P<0.005). 3. As the severity of periodontal disease increased, the bacterial adherence to the membrane specimens significantly increased(p<0.001). 4. After scaling and root planing, the bacterial adherence to the membrane specimens significantly decreased in gingivitis and periodontitis patient group(P<0.001). These results suggest that bacterial contamination on exposed barrier membrane surface be reduced through improvement of periodontal status and oral health environment before and after GTR procedure for the successful outcome.

PERIODONTAL AND PROSTHETIC FINDINGS IN PATIENTS TREATED WITH REMOVABLE PARTIAL DENTURES OR DISTALLY EXTENDING CANTILEVER BRIDGES (가철성 국소의치와 후방연장 계속가공의치를 장착한 환자의 치주 및 보철 상태)

  • This study was perfomed to investigate the prosthodontic and periodontal status of the abutment teeth in distally extending bridges(DEBs) (78 cases) and removable partial dentures(RPDs) (43 cases) for 122 patients (55 males and 57 females) visiting department of Periodontology, Seoul National University Hospital. The average wearing periods were 67.8 months for DEBs and 66.4 months for RPDs. 38.4% of the patients in DEBs and 35.9% in RPDs complainted of chewing discomfort and 22.6% and 24.4% were not chewing on the prosthodontically - treated sides, respectively. In DEBs, when the restoration for 2nd molar supported by 1st molar & 2nd premolar was grouped to type 1, 1st molar supported by 1st & 2nd premolars was type 2, and 1st & 2nd molars supported by 1st & 2nd premolars was type 3, there was a significant differences only in the tooth mobility score among clinical parameters (type 2>type 1>type 3). In RPDs, when bilateral free-end case was grouped to type 1, and unilateral case was type 2, there was a significant difference in the Gingival index (type 1>type 2). In DEBs, 62.8% of restoration were overcontoured, 72% had interproximal space closures, 30.5% overextended pontics and 86.6% overhanging margins. In RPDs, 24.4% of the restorations were overcontoured, 45.5% had interproxinal space closures and 58.3% overhanging margins. From these results of this study, periodontal problems caused by prosthodontic defects were considered to be contributing factors in chewing discomfort.

  • Revascularization of the pulp in a necrotic, infected immature tooth with apical periodontitis was attempting several years. Revascularization of partially necrotic pulp in an immature tooth is based on the concept that vital dental stem cells can survive pulpal necrosis. Revascularization procedure obtains longer and thicker roots in teeth with necrotic pulp diagnosis. Pulp revascularization for immature permanent molars can be possibly applied on cases having difficulty to use conventional root canal treatment due to abnormally thin root canal wall or severe root curvature. Also, when an uncooperative patient does not agree with sedative treatment the revascularization can be useful. And a patient with disability who is barely cooperative can be another indication of this treatment. In this case report, pulp revascularization using triple-antibiotics, metronidazole, ciprofloxacine and minocycline, was applied on the immature first permanent molar infected by caries.

The association between oral health and suicide attempts in Korean elderly : The 7th Korea National Health and Nutrition Examination Survey (7th KNHANES, 2016-2018) (노인의 구강건강과 자살시도와의 관련성 분석 : 제 7기(2016-2018년) 국민건강영양조사를 바탕으로)

  • Objectives: This study investigated the association between oral health and suicide attempts in adults aged 65 years or older using the 7th Korea National Health and Examination Survey (KHANES, 2016-2018). Methods: Of the total 24,269 participants of the 7th KHANES (2016-2018), 1,488 adults aged 65 years or older who had an oral health examination were selected. Variations in suicide attempts with demographic characteristics and perceived oral health status were analyzed using the χ2 test, and the association between oral health and suicide attempts was analyzed using the logistic regression model. Results: The suicide attempt rate was higher among older adults with chewing (p<0.01) and speaking (p<0.05) difficulties. The odds ratio for a suicide attempt was 1.05 times higher among those with tooth loss (p<0.05). Conclusions: Our results showed that older adults with tooth loss had an increased risk of suicide. Thus, it is important to prevent tooth loss, in the first place, through proper oral health management. If tooth loss occurs, appropriate treatment should be prioritized to facilitate functional recovery. Finally, national support is required for older adults with limited access to dental care.

Studies on the Oral Disease Improvement Effects of Probiotics : A Review (프로바이오틱스의 구강질병 개선에 관한 연구 고찰)

  • This study is a review of research articles on the characteristics of probiotics. Probiotics are effective at ameliorating immune disease, alleviating glucose intolerance, and improving constipation and diarrhea. Furthermore, they have anticancer and antitumor effects, preventive effects against cardiovascular disease, antidiabetic effects, antioxidant effects, antibacterial effects, and they produce useful metabolites. It has been demonstrated that oral probiotics are effective in eliminating halitosis and forming a favorable oral ecosystem, by creating an environment that is not readily inhabited by harmful bacteria, such as Streptococcus mutans, which produces lactic acid and causes tooth decay, or Porphyromonas gingivalis, which causes gum disease.As a result, oral probiotics are being considered not only as therapeutics against diseases, but also as preventive agents for the maintenance of a healthy oral balance. In spite of some limitations, clinical trials are currently underway, and this study can provide evidence to support the use of probiotics to improve oral health conditions, including dental caries, periodontal disease, and Candida colonization.
