  • For individuals with cleft lip and palate, the removable partial dentures (RPDs) have been an important treatment option. Some modifications from conventional prosthetic treatment may be necessary to achieve satisfactory functional and esthetic results in cleft patients. In case of cleft palate patient with periodontally compromised and only posterior few remaining teeth, removable partial prosthesis connected to telescopic crown can be the alternative treatment option. When connected to the RPD, telescopic crowns increase the prosthetic stability and retention, optimize favorable force transmission to the long dental axis, and improve esthetics. And the cross arch stabilization of double crown denture helps to stabilize both divided maxillary ridges in CLP patient. This case present one adult CLP patient using an RPD connected with hybrid telescopic crowns with friction pins to improve not only retention and stability but also aesthetics.

  • Regenerative therapy in an interproximal intrabony defect is a challenge due to unaesthetic appearance after surgery. In this article, we introduce a case series of additional use of autogenous periosteal barrier membrane combined with bovine bone mineral and enamel matrix derivative (EMD) in interproximal periodontal intrabony defects to overcome an aforementioned shortcoming. During the periodontal regenerative surgery, autogenous periosteal membrane was additionally adopted besides xenograft material and EMD. Clinical and radiographic examinations were performed before surgery and 6 months after surgical treatment. All clinical parameters were improved and the intrabony defects were resolved on the radiography 6 months after surgery. Moreover, soft tissue esthetics such as the contour of interdental papilla was better than that of conventional regenerative therapy. Periodontal regenerative therapy using several graft materials and bioactive materials was effective in the treatment of periodontal intrabony defect. Moreover, using of autogenous periosteal barrier membrane combined with xenograft and EMD has additional effect for the treatment of an interproximal intrabony defect in terms of augmentation of interdental soft tissue volume.

  • Centric occlusion-maximum intercuspation (CO-MI) discrepancy is one of main causes of evoking premature contact and resultant mandibular shift. These non-physiological conditions can induce temporomandibular disease, periodontitis, and non-carious cervical lesion. Therefore, if CO-MI discrepancy exists in patients who need extensive prosthetic rehabilitation, it must be corrected and then physiological occlusion must be restored. This report describes the treatment procedure of removing CO-MI discrepancy and prosthetic rehabilitation in a patient with 3.5 mm discrepancy, multiple caries and periodontitis. Proper mandibular position and modified opening & closing movement were confirmed by ARCUSdigma II and transcranial radiograph.

  • Purpose: In this paper, we introduce a wide clinical application of root resection therapy and additional benefits on the next prosthetic or dental implant treatment procedures in the age of dental implants. Materials and Methods: Additional clinical benefits of root resection were monitored by clinical and radiographic evaluation. Results: No complication was reported in all the cases, and the favorable results of root resection were maintained for long period. Conclusion: In compromised multirooted teeth, variety clinical treatment modality should be considered for enhancing the reconstruction of alveolar ridge around root resected in preparation for next implantation.

  • The purpose of this study was to examine association between oral health status and perceived general health symptom. We analyzed 14,231 subjects who participated in Korea National Health and Nutrition Examination Survey (2007~2009). All individuals were examined by a questionnaire about socioeconomic history, smoking and drinking habit, the frequency of daily tooth brushing, the presence of regular dental visit, and EuroQol-5D. Dental survey was conducted to find the decayed, missing, and filled teeth (DMFT) index and community periodontal index. Subjects with perceived problem with mobility had higher DMFT index (odds ratio, 1.18, p<0.001). Subjects with with perceived problem with pain/discomfort had higher DMFT index (odds ratio, 1.16, p<0.001). Self-rating general health symptom was not associated with periodontitis (p>0.05). Perceived general health was associated with DMFT index. It is recommendable that we can use the perceived general health to predict oral health status.

  • This study is a review of research articles on the characteristics of probiotics. Probiotics are effective at ameliorating immune disease, alleviating glucose intolerance, and improving constipation and diarrhea. Furthermore, they have anticancer and antitumor effects, preventive effects against cardiovascular disease, antidiabetic effects, antioxidant effects, antibacterial effects, and they produce useful metabolites. It has been demonstrated that oral probiotics are effective in eliminating halitosis and forming a favorable oral ecosystem, by creating an environment that is not readily inhabited by harmful bacteria, such as Streptococcus mutans, which produces lactic acid and causes tooth decay, or Porphyromonas gingivalis, which causes gum disease. As a result, oral probiotics are being considered not only as therapeutics against diseases, but also as preventive agents for the maintenance of a healthy oral balance. In spite of some limitations, clinical trials are currently underway, and this study can provide evidence to support the use of probiotics to improve oral health conditions, including dental caries, periodontal disease, and Candida colonization.

  • A 20-year-old male patient with Down syndrome presented with rampant caries and chronic periodontitis on full-mouth. Patient have many oral diseases despite of no abnormalities of medical history, no involuntary tremor, no excessive masticatory pressure, and no limitation of exercise. Also he had no experiences of oral health education of prostheses and oral health management. Therefore, A number of fixed prostheses and denture treatments were performed to caregiver and patient with education for oral hygiene and management. After delivery final prostheses, they had repeated follow-up for checking management.

  • The purpose of this study were to report the reasons for removal of fixed prostheses and survival rate of abutment teeth. A total 192 fixed prostheses removed at Chosun university Dental Hospital and 308 abutment were investigated. The most frequent reason of removal was periodontal problem(30.7%), followed by caries(29.7%), then periapical problem(18.8%) and defective margins(14%). In metal ceramic crown, periodontal problem was the most frequent reason of removal. In complete cast crown, caries was the most frequent reason of removal. Periapical and periodontal problem was the most frequent reason people under forties and over fifties, respectively. Of the 308 abutment teeth, vital and non-vital teeth were 135(43.8%) and 173(56.2%), respectively. Of 135 vital abutment teeth, 39(28.9%) were extracted and of 173 non-vital abutment teeth, 85(49.1%) were extracted. Each risk factor for removal of fixed prostheses and extraction of abutment should be evaluated more clearly for prediction of prognosis of final prostheses and abutment teeth.

  • Matrix metalloproteinase(MMP)-9 is considered important in tissue destruction in periodontitis. The aim of this study was to investigate the influence of smoking on MMP-9 in the gingival crevicular fluid(GCF). GCF samples in upper incisors area from 332 male subjects were collected after the informed consent. The dental examination included the assessment of oral hygiene, gingival inflammation and probing pocket depth. A quantitative assessment of MMP-9 levels in GCF was performed utilizing and immunological procedure. The mean MMP-9 concentrations found in GCF of smokers(30.86 ng/ml) and quit-smokers(29.82 ng/ml) differed from non-smokers(11.33 ng/ml), adjusted by age, gingival index and Community periodontal index(p<0.001). Smoking seems to influence MMP-9 in GCF regardless of gingival inflammation and age. It means smoking can destruct the periodontal tissue for itself.
