The Effect of Ethanolic Extracts Mixture from Magnoliae cortex and Carthami semen on the' Progression of Experimental Periodontitis in Beagle Dogs (인위적으로 유발된 Beagle dog의 치주질환 진행에 후박과 홍화씨 추출물이 미치는 영향에 관한 연구)

  • 후박추출물은 치주병인균에는 항균효과가 있고, 치은섬유아세포에는 세포독성이 적은 것으로 알려져 있으며 홍아씨 추출물은 골아세포를 활성화시키는 효과가 있어 골조직재생에 효과가 있는 것으로 알려져 있다. 이 실험의 목적은 비글견에 인위적으로 치주염을 유도하고 후박추출물과 홍화씨추출물의 혼합물을 투여함으로써, 이 두 가지 혼합물이 치주염 진행에 예방효과가 잇는지, 또, 치료효과가 있는지 알아보는 것이다. 후박과 홍화씨를 구입하여 에탄올에서 추출하여 정제하고 진공상태에서 건조시켜 농축하였다. 후박추출물과 홍화씨 추출물을 1:5의 질량비로 혼합하고 250㎎씩 캡슐에 넣었다. 비글견의 상하악 소구치와 하악 제1대구치를 8주 동안 철사와 견사로 결찰하여 치주염을 유발하였다. 8주 후에, 실험군 비글견에게는 미리 후박추출물과 홍화씨추출물 혼합물을 매 12시간마다 하루에 1,500㎎ 씩 12주 동안 투약하였다. 대조군 비글견에는 투약을 하지 않았다. 투약을 시작한 후에 치은지수, 부착수준을 매 4주마다 측정하였다. 매 4주마다 치주낭에서 페이퍼포인트를 이용하여 혐기성세균과 호기성세균을 채취, 배양하여 각각의 집락 수를 세었다. 그 결과 실험군의 치은지수는 8주, 12주에서 대조군보다 유의하게 낮았고(p<0.05)), 임상적으로도 상당한 치은염의 개선을 보였다. 실험군의 혐기성세균 집락수는 감소하여 4, 8, 및 12주에서 대조군에 비해 유의하게 낮았고(p<0.001), 호기성 세균 집락수는 4, 8, 및 12주에서 대조군에 비해 유의하게 낮았다.(p<0.05)이 실험 결과, 후박 추출물과 홍화씨 추출물 혼합물이 치주질환의 예방 및 치료에 이용되어질 수 있을 것으로 생각된다.

  • Leukocyte adhesion deficiency is rare inherited defect on phagocytic function resulting lack of leukocyte cell surface expression of $\beta2$ integrin molecule that are essential for leukocyte adhesion to endothelial cells and chemotaxis. Clinical features of patients with leukocyte adhesion deficiency type I include recurrent necrotic infection of the skin mucous membranes, and intestinal tract with septicemia, and omphalitis arising from delayed umbilical cord separation. Oral manifestations are severe progressive periodontitis with alveolar bone loss, periodontal pockets, and partial and total premature loss of the deciduous and permanent dentitions. We report a case of leukocyte adhesion deficiency type I in a 5-year-old child with severe periodontitis. In order to prevent local and systemic infection, we controlled periodontal disease with periodic oral prophylaxis. Oral swabs and blood cultures were perfomed for suspected infection, so that optimal measures were taken through the use of appropriate antibiotics.

  • The purpose of this study was to investigate factors affecting perceived oral health status according to socioeconomic status and community periodontal index(CPI) and decayed, missing, and filled teeth(DMFT) using the 6th Korean national health and nutritional examination survey(KNHANES VI) and provide a basic data for plan of policy. The higher the age, the lower the household income and education level, the worse the subjective oral health had better oral health and there was a tendency that the respondents who had no oral exam within 1 year and experienced CPI or DMFT estimated their own health as worse. It is needed to make policy development to resolve the inequality of oral health.

  • This case report describes about recurrent herpetic stomatitis mimicking post-root resection complication. A 49 year-old male patient was diagnosed vertical root fracture of the mesiobuccal root of his left maxillary first molar (#26). The mesiobuccal root was resected following root canal treatment of the same tooth. 19 months later, the patient presented with pain on left hard palate after a barbecue party. Intra oral examination revealed a gum boil-like blister at the hard palate corresponding to the apex of the palatal root of #26. On clinical examination, there was bleeding on probing and the periodontal pocket depth was measured less than 5 mm with no tooth mobility. On a periapical radiograph, periodontal ligament space widening was observed. Tracing the sinus tract with gutta percha cone was attempted, however, it was impossible. Extending the field of vision, small multiple round ulcerations were observed at the palate front which caused pain to the patient. Therefore, the pain was considered a non odontogenic and the patient was referred to the department of oral medicine. The patient was diagnosed recurrent herpetic stomatitis and after 3 days of antiviral medication, the pain and ulceration were subsided.

Antibacterial Effect on Oral Pathogenic Bacteria of Phytoncide from Chamaecyparis Obtusa (구강병원균에 대한 편백 피톤치드의 항균작용)

  • Plant extract has attracted considerable interest in oral disease therapy. The present study was performed to observe the antibacterial effect on cariogenic Streptococcus mutans GS5 and Streptococcus sobrinus 6715, and periodontopathic Actinobacillus actinomycetemcomitans Y4 of phytoncide from Chamaecyparis obtusa Sieb. et Zucc employing the measurement of optical density, viable cell counts, and antibiotic sensitivity. The results were as follows: 1. Minimum inhibitory concentration of the phytoncide for S. mutans, S. sobrinus, and A. actinomycetemcomitans was observed to be 0.5%, 1%, and 0.2%, respectively. 2. Minimum bactericidal concentration of the phytoncide for S. mutans, S. sobrinus, and A. actinomycetemcomitans was determined to be 0.5%, 2%, and 0.2%, respectively. 3. The bacteria exposed to the phytoncide become more sensitive to antibiotics. The phytoncide enhanced significantly antibacterial activity of ampicillin against S. mutans and S. sobrinus. It also increased significantly the activity of penicillin and amoxicillin against S. sobrinus. In contrast, the phytoncide augmented the activity of amoxicillin and cefotaxime against A. actinomycetemcomitans but the increase was not statistically significant. The overall results indicate that phytoncide from Chamaecyparis obtusa Sieb. et Zucc used for this study has a strong antibacterial activity against cariogenic and periodontopathic bacteria and that it also has permeabilizing effect on certain antibiotics against these bacteria. Therefore, the phytoncide may be used as a candidate for prevention and therapeutic agent against oral infectious disease including dental caries and periodontal disease.

EVALUATION OF THE VIABILITY OF PERIODONTAL LIGAMENT CELL IN RAT TEETH USING SLOW CRYOPRESERVATION METHOD WITH MAGNETIC FIELD (자기장 저속 냉동보관법을 이용한 쥐 치아 치주인대세포의 활성도 검사)

  • The purpose of this study was to evaluate the viability of periodontal ligament cell in rat teeth using slow cryopreservation method with magnetic field through MTT assay and TUNEL test. For each group, 12 teeth of 4 weeks old white female Sprague-Dawley rat were used for MTT assay, and 6 teeth in TUNEL test. The Maxillary left and right, first and second molars were extracted as atraumatically as possible under tiletamine anesthesia. The experimental groups were group1 (immediately extraction), group 2 (cold preservation at 4$^{\circ}C$ for 1 week), group 3 (rapid cryopreservation in liquid nitrogen), group 4 (slow cryopreservation with magnetic field of 1 G), and group 5 (slow cryopreservation). F medium was used as preservation medium and 10% DMSO as cryoprotectant. After preservation and thawing, the MTT assay and TUNEL test were processed. One way ANOVA and Scheffe method were performed at the 95% level of confidence. The value of optical density obtained after MTT analysis was divided by the value of eosin staining for tissue volume standardization. In both MTT assay and TUNEL test, it had showed no significant difference among group 3, 4, and 5. And group 3 had showed higher viability of periodontal ligament cell than group 2. From this study, slow cryopreservation method with magnetic field can be used as one of cryopreservation methods.

  • Tooth movement is a result of mutual physiologic responses between the periodontal ligament and alveolar bone stimulated by mechanical strain. The PDL cell and osteoblast are known to have an influence on bone formation by controlling collagen synthesis and alkaline phosphatase activation. Moreover. recent studies have shown that the PDL cell and osteoblast release osteoprotegerin (OPG) and the receptor activator of nuclear factor ぉ ligand (RANKL) to control the level of osteoclast differentiation and activation which in turn influences bone resorption. In this study. progressively increased, continuous tensional force was applied to PDL cells. The objective was to find out which kind of biochemical reactions occur after tensional force application and to illuminate the alveolar bone resorption and apposition mechanism. Continuous and progressively increased tensile force was applied to PDL cells cultured on a petriperm dish with a flexible membrane The amount of $PGE_2$ and ALP synthesis were measured after 1, 3, 0 and 12 hours of force application. Secondly RT-PCR analysis was carried out for OPG and RANKL which control osteoclast differentiation and MMP-1 -8, -9, -13 aud TIMP-1 which regulate the resolution of collagen and resorption of the osteoid layer According to the results. we concluded that progressively increased, concluded force application to human PDL cells reduces $PGE_2$ synthesis, and increases OPG mRNA expression.

Additional use of autogenous periosteal barrier membrane combined with regenerative therapy in the interproximal intrabony defects: case series (치간부 골내낭의 치주재생치료에서 골막이식의 부가적 사용 증례)

  • Regenerative therapy in an interproximal intrabony defect is a challenge due to unaesthetic appearance after surgery. In this article, we introduce a case series of additional use of autogenous periosteal barrier membrane combined with bovine bone mineral and enamel matrix derivative (EMD) in interproximal periodontal intrabony defects to overcome an aforementioned shortcoming. During the periodontal regenerative surgery, autogenous periosteal membrane was additionally adopted besides xenograft material and EMD. Clinical and radiographic examinations were performed before surgery and 6 months after surgical treatment. All clinical parameters were improved and the intrabony defects were resolved on the radiography 6 months after surgery. Moreover, soft tissue esthetics such as the contour of interdental papilla was better than that of conventional regenerative therapy. Periodontal regenerative therapy using several graft materials and bioactive materials was effective in the treatment of periodontal intrabony defect. Moreover, using of autogenous periosteal barrier membrane combined with xenograft and EMD has additional effect for the treatment of an interproximal intrabony defect in terms of augmentation of interdental soft tissue volume.

  • Tooth impaction may lead to malocclusion, root resolution, cyst or aberrant changes in adjacent teeth. Clinical and radiographic examinations are used to locate the impaction, and appropriate treatment plans must be made to relocate the impacted tooth. When surgically exposing the impacted tooth, periodontal considerations to conserve maximum amount of soft and hard tissue are used. Oral hygiene instructions are emphasized to maintain sound periodontal health. Securing enough space for the impacted tooth and proper anchorage is important. Proper use of force and mechanics is crucial to prevent such complications as root resolution. Various patterns of orthodontic traction may be employed as situation permits. Most impaction cases can be managed with orthodontic traction to restore function and esthetics, provided that early detection and proper diagnosis and treatment planning are made.

Pontic site development with an implant submergence technique for unaesthetic implant in the anterior maxilla (상악 전치부 임플란트의 비심미성 개선을 위한 임플란트 침수(submergence)를 동반한 치조제 증대술)

  • Improving implant esthetics is very difficult, especially in cases where unaesthetic problems are related to implants in the maxillary anterior dentition. A 69-year old male patient was referred by a prosthodontist for periodic pus discharge and an unaesthetic implant prosthesis (maxillary right lateral incisor). The implant was placed too deeply and showed soft tissue volume deficiency and a long clinical crown. After a clinical and radiographic examination, implant submergence and alveolar ridge augmentation were performed to enhance the aesthetics instead of an explantation. The treatment plan was as follows: extraction the adjacent teeth with tooth mobility, secondary caries, and poor prognosis; placement an additional dental implant with hard and soft tissue grafting; fabrication a fixed bridge using implant abutments. A fixed esthetic prosthesis using implants was fabricated, and the patient was satisfied with the prosthesis. A ridge augmentation with implant submergence may be an alternative for solving the problems of unaesthetic implant restorations in the esthetic zone.
