THE EFFECTS OF TETRACYCLINE-CONTAINING GEL ON GINGIVAL CREVICULAR ENZYME ACTIVITY (치근활택술 및 테트라사이클린 젤의 부가적 사용시 치은 열구액내 효소활성도에 관한 연구)

  • 본 연구의 목적은 비외과적 치주치료시에 부가적으로 사용하기 위해 개발한 테트라싸이클린 젤의 효과를 임상지수, 치은 열구액양 및 치은열구액내 효소활성도 측정을 통해 평가함에 있다. 치주낭 깊이가 4mm 이상 6mm이하인 구치가 편악당 적어도 2개 이상 존재하는 중등도 치주염에 이환된 환자 23명을 선택하여, 한 편악에서는 치근활택술 만을 시행하였고, 다른 한 편악에는 치근활택술 후 테트라싸이클린 젤을 치은연하로 주입한 뒤 치근면 처리하였다. 사용된 임상검사 지수는 치태지수, 치은지수, 치주낭 깊이, 치은부착 상실도 및 탐침시 출혈로, 각각 치료시작 전과 치료 후 4, 12주에 검사하였으며, 치은열구액내 효소활성도 검사를 위해 치은열구액을 치료시작 전과 치료 후 1, 2, 4, 12주에 각각 채취하고 치은열구액 양을 조사한 뒤 Lactate dehydrogenase(LDH)와 Beta-glucuronidase(BG)의 활성도를 측정하였고, 효소활성도는 LDH는 340nm에서, BG는 550nm에서 각각 흡광도를 조사 분석하여 다음과 같은 결과를 얻었다. 1. 임상지수를 측정 비교한 결과 치근활택술 만으로 치료한 군과 치근활택술 후 테트라싸이클린 젤을 병용한 군 모두에서 임상적으로 치은건강의 현저한 향상을 관찰할 수 있었으나, 통계상 두 군간의 유의성있는 차이는 발견되지 않았다. 2. 치은열구액 양은 치근활택술 후 테트라싸이클린 젤을 병용한 군이 치근활택술만 시행한 군보다 치료 후 2주와 4주에 유의성 있게 감소하였다. 3. LDH와 BG 효소 활성도는 두 군 모두 현저히 감소하였고, 비록 통계학적인 유의성은 관찰할 수 없었으나 치근활택술 후 테트라싸이클린 젤을 병용한 군에서 감소량이 더 크게 나타났다.

Collagenolytic Activity Of Gingival Crevicular Fluid In Progressive Periodontitis (진행성 치주염에서의 치은열구액내 교원질분해효소 활성)

  • There were many reports that elevations in the levels of active and latent collagenase in gingival crevicular fluid(GCF) have been correlated positively with periodontal disease activity. To provide a simple diagnostic approach for testing GCF collagenolytic activity, the detection limit of enzyme activity was compared using radiofibril assay(Sodek et.al.1981) and spectrophotometric collagenolytic assay(Nethery et al. 1986). The detection limits of both assay for standard bacterial enzyme were similar and the radiofibril assay showed a little (1/2) lower detection limit for tad pole collagenase. To evaluate the relationship between periodontal tissue destruction and the collagenolytic activity, GCF was collected, and latent and active enzyme activities were measured by a spectrophotometric collagenolytic assay. Twelve subjects showing progressive lesions were selected according to the presence of immediate tissue destruction, frequent abscess formation, and increasing need for tooth extraction, and the absence of underlying systemic disease and previous antibiotic medication history within 6 months. Comparisons were made between sites with either: 1) inflammation with a previous history of progressive loss of periodontal tissue and bone support(2l progressive sites): 2) previous history of bone loss and periodontal destruction but now clinically stable(12 comparably stable sites); or 3) no loss of periodontal tissue and bone support(11 control sites including 5 gingivitis sites and 6 healthy sites). Active collagenase activity was the highest in the progressive sites and decreased in the order of the gingivitis sites, the stable sites, and the healthy sites. The total enzyme activity was $2{\sim}3$ fold higher in the progressive sites and the gingivitis sites, compared to the stable and the healthy sites. The ratio of active to total collagenolytic activity was twice in the progressive sites. Analysis of active collagenase level(5mU) and the ratio of active to total collagenolytic activity(0.8) as a diagnositic test indicates that these measurements have the sensitivity of 0.81 and 0.86, the specificity of 0.70 and 0.65, and the overall agreement of 0.75 and 0.73, respectively. Thus, this method has significant merits as a diagnostic tool to determine wherher the site is in a state of remission or progression.

The effects of low dose doxycycline regimen on gingival crevicular fluid enzyme actmty of diabetic patients with periodontitis and adult periodontitis patients (저용량 독시싸이클린 투여가 당뇨환자 마치 성인성 치주염 환자의 치은열구액내 효소 활성도에 미치는 영향에 관한 비교연구)

  • It was reported that low dose doxycycline(LDD) regimen could inhibit pathologically elevated collagenase activity in the gingival crevicular fluid of petients with adult periodontitis without producing typical antibiotic side effects. The purpose of this study was to evaluate the effects of LDD regimen(20mg) administered during non-surgical therapy on clinical index and gingival crevicular fluid enzyme activity in diabetics who are at high risk for periodontal disease. Forty-nine subjects having at least two sites with probing pocket depths greater than 4mm were selected. In this double-blind, placebo-controlled study, the patients were administered 20 mg doxycycline capsule or placebo capsule b.i.d. for 2 weeks. Clinical parameters of dental plaque, gingival inflammation, probing pocket depth and probing attachment level were assessed at week 0, 2, 4, and 8. Gingival crevicular fluid samples were collected at the same time to evaluate the activities of collagenase and elastase. Clinical parameters and elastase activity were significantly reduced in all groups compared to the baseline value after treatment. Significantly greater reduction in pocket depth and gain in attachment level was shown in the LDD-administered group compared to placebo group in both adult periodontitis and diabetic patients. Total collagenase activity was also reduced significantly in all groups after treatment, but the greater reduction was seen in the LDD-administered diabetics group compared to relevant placebo group(at 4, 8week). Significantly greater reduction in active collagenase activity was also seen in the LDD-administered group compared to placebo group in diabetic patients(at 2, 4, 8week). These results indicated that use of low dose doxycycline could be aueseful adjunct to instrumentation therapy in the management of diabetic patient with periodontitis as well as adult periodontitis patient.
