  • Multiple tooth loss can cause disharmonious occlusial plane, loss of vertical dimension and deflection of mandibular movement. Therefore, restoring proper vertical dimension and occlusion in the centric relation is an important treatment goal. Implant-assisted removable partial denture is a recently used clinical technique, because it increases patient satisfaction by improving retention, support, and stability of conventional denture. The objective of this article is to present a case report describing the fabrication of treatment denture and implant assisted removable partial denture using previously placed implants for a patient with partial loss of posterior support and anterior stop.

  • The mandibular 4 anterior incisor restoration has small teeth and a small space, so the choice of abutment is always a concern. The mandibular 4 anterior incisor extraction is caused by periodontal disease, interdental embrasure greatly open is advantageous of oral hygiene management. Try to make it small diameter with custom abutment, it is limited to the space for the screw. Rather than setting the post site of one body implant to a horizontal cross-section, it is advantageous to set it to a longitudinal cross-section, for interdental embrasure formation. When using an internal bone level implant, rather than using a two-piece abutment, using a one-piece abutment can more effectively secure space for interdental embrasure.

  • Implant-fixed prostheses and removable partial dentures are widely used as a traditional treatment method for partially edentulous patients who have lost multiple teeth. Recently, there has been increasing interest in implant-assisted removable partial denture (IARPD), which additionally obtains retention and support using a small number of implants. The IARPDs have higher retention, stability, and aesthetics than conventional removable dentures. It also has the effect of reducing the cost as well as the burden of surgery by placing a small number of implants at the edentulous site. In this case, the patient with a partial edentulous state that has multiple numbers of tooth loss in maxilla initially planned to be treated with implant fixed prosthesis. However, the treatment plan was changed to IARPD using Bar and Locator due to several factors. The clinical results were satisfactory on the aspect of esthetic and masticatory function.

  • The maxillary anteriors play an important role in esthetics. Therefore after extraction, it is crucial to preserve the hard tissue and soft tissue in order to promote esthetics of restoration. There are several challenges when restoring the maxillary anteriors via implant. Some of the challenges are be maintaining consistency with neighboring teeth in terms of shade, form, and texture : as well as having harmonious emergency with the gingival margin. In this case, a traumatized patient with crown-root fracture of the maxillary central and lateral incisors is presented. The cracked teeth were extracted, and implants were inserted with bone grafts to compensate the volume of damaged area of the maxillary anterior. Cantilever implant prosthetics were planned while precise adjustments to the gingival area were made using customized impression coping to perform the esthetic restorations. The final outcome of the treatment was satisfying in both esthetic and utilitarian perspective.

  • As dental implant treatment becomes popular for both partial and complete edentulous patients, old people with complex systemic diseases also tend to prefer implant-assisted-overdenture or implant-supported-fixed prostheses to conventional complete denture. In this case, 77-year-old female who was on medication for hypertension and osteoporosis and paralyzed on right side due to stroke visited for implant-assisted-overdenture on lower jaw. After oral and radiographic examination, root-assisted magnet overdenture on upper jaw and implant-assisted magnet overdenture on lower jaw are planned. Consequently, overdentures using self-adjusting magnetic attachment(SA) system on both jaws resulted in recovering satisfying function and retention, which is enable to insert and remove with only one hand.

Clinician should consider various treatment options to restore missing teeth, especially in anterior region.   • Peri-implantitis appears in almost 20% of patients who received implant treatment, and increase in its number is inevitable as time goes by. Although it can be treated by both non-surgical and surgical procedures, in cases which include severe bone loss, explantation and rehabilitation may be necessary. Careful treatment planning and considerations to prevent recurrent peri-implantitis should be taken into account. In the following case presented, a patient with chronic periodontitis and peri-implantitis was successfully rehabilitated after removal of several implants. Extraction and explantation of multiple teeth and implants were followed by full mouth reconstruction with fixed implant prostheses on the mandible and implant retained overdenture on the maxilla. Surgical and prosthetic measures to prevent recurrent peri-implantitis were taken into consideration.

  • Improving implant esthetics is very difficult, especially in cases where unaesthetic problems are related to implants in the maxillary anterior dentition. A 69-year old male patient was referred by a prosthodontist for periodic pus discharge and an unaesthetic implant prosthesis (maxillary right lateral incisor). The implant was placed too deeply and showed soft tissue volume deficiency and a long clinical crown. After a clinical and radiographic examination, implant submergence and alveolar ridge augmentation were performed to enhance the aesthetics instead of an explantation. The treatment plan was as follows: extraction the adjacent teeth with tooth mobility, secondary caries, and poor prognosis; placement an additional dental implant with hard and soft tissue grafting; fabrication a fixed bridge using implant abutments. A fixed esthetic prosthesis using implants was fabricated, and the patient was satisfied with the prosthesis. A ridge augmentation with implant submergence may be an alternative for solving the problems of unaesthetic implant restorations in the esthetic zone.

  • In the case of single tooth replacement, a fixed prosthesis or a dental implant is the treatment option commonly selected as first choice. However, any amount of sound tooth structure should be removed to prepare the abutment teeth for full coverage retainer. The adjacent tooth damage can be avoided placing a dental implant. However, depending on the patient's oral condition and any other circumstances, it may be impossible or delayed. In this case resin bonded fixed partial denture was selected as an alternative that can restore single tooth loss without much tooth structure removal to the young patients and the patients with gingival recession. We report these patients were satisfied with esthetic and function.
