TREATMENT OF A PERMANENT INCISOR COMPLETELY INTRUDED BY TRAUMA : A CASE REPORT (외상으로 완전 함입된 영구전치의 치험례)

  • Traumatic intrusive luxation, an occurrence common in the primary dentition but one that occurs rarely in the permanent dentition, has a poor prognosis. There have been many treatment approaches such as allowing the tooth to reerupt spontaneously, surgical repositioning and immediate luxation, surgical luxation, and orthodontic repositioning; but all have their own drawbacks. Meanwhile, Turley et al. (1987) have proposed surgical and orthodontic combination therapy to treat intrusion. Surgical and orthodontic combination therapy means to apply the orthodontic traction force immediately after surgical luxation. If ankylosis occurs, orthodontic force may be applied after re-luxation repeatedly. But in cases of complete intrusive luxation, it would be not feasible to bond an orthodontic button or bracket on the tooth directly. Thus, in this case, traction of the tooth was attempted after surgically repositioning it close to the probable original socket site to promote better healing.

THE EFFECTS OF EGF ON PROLIFERATION AND ATTACHMENT OF HUMAN PERIODONTAL FIBROBLASTS (표피성장인자가 치주인대 섬유 모세포의 증식과 부착에 미치는 영향)

  • In considering the healing process of injured periodontal tissue, healing rate would be influenced by the cellular activity of periodontal fibroblasts(PDLFs). In addition, the reattachment among PDLFs should be induced for healing process. The purpose of this study was to evaluate the effects of epidermal growth factor(EGF) on the proliferation and attachment of PDLFs and to verify the efficacy of EGF as a storage media or a pre-replantation conditioner of traumatically avulsed tooth. Human recombinant epidermal growth factor(hrEGF) and human periodontal fibroblasts from first premolar were prepared. At first, MTT assay was done to evaluate the toxic effect on human periodontal fibroblast and the maximum cellular growth of EGF. Cellular proliferation rate was then compared between control group and 10ng/ml EGF added group. Also, western blot was done to evaluate the expression of fibronectin in both groups. The results were as follows: 1. From MTT assay, EGF showed no toxic effect on PDL fibroblasts. The highest proliferation was shown at 10ng/ml EGF. 2. In 10ng/ml EGF added group, the degree of proliferation of PDLFs was significantly higher than that in control group. 3. Fibronectin expression of EGF added group was also significantly higher than that of control group. From this study we could conclude that EGF enhanced the regeneration rate of periodontal fibroblast, which could be used as a pretreatment agent or a storage media for traumatically avulsed teeth.

Decoronation and implant restoration of ankylosed tooth resulted from anterior avulsion: A case report (전치부 탈구로 인한 유착치의 치관절제술 및 임플란트 수복 증례)

  • Dental trauma is common in active children and adolescents. Among them, in the case of ankylosis due to avulsion, diagnosis through clinical examination is important, and the treatment is important for esthetic prosthetic restoration after adulthood. This case is a case of esthetic prosthetic restoration of maxillary anterior teeth through decoronation and implant. After that, space was maintained for prosthetic restoration using a decoronated crown. After becoming an adult, precise implant placement and esthetic prosthetic restoration were possible using guide surgery. For soft tissue support, the temporary crown were replaced by changing the contours of the temporary crown, and the final esthetic prosthesis was fabricated by digital wax-up.

DEVELOPMENTAL DISTURBANCES OF SUCCESSIVE PERMANENT TEETH BY TRAUMA TO THE PRIMARY TEETH (유치의 외상에 의한 계승 영구치의 발육 장애)

  • Trauma to the primary teeth may have an effect on the successive permanent teeth. Thus, the treatment plans should be focused on safety of their permanent teeth. The degree of developmental disturbances in those permanent teeth depends upon many factors including the age of the children, developmental conditions of the affected teeth, types and severities of trauma and time interval from the moment of trauma to the initial treatment the patients received. The complications of the effected permanent teeth vary from the mild enamel hypoplasia to the premature finishing of teeth development. In this study, children whose ages were 33 months, 5 years, and 26 months presented the developmental disturbances to their permanent teeth, which resulted from the trauma to the primary teeth: the intrusion, the avulsion, and the subluxation. There showed a mild complication in the case of subluxation resulting in enamel hypoplasia, but, the more severe complication of the root dilacerations occurred in the case of intrusion. Furthermore, the 5-year-old patient whose primary dentition was near in transition period to the permanent one, the more complicated problem such as stop of root development of the permanent teeth was resulted from the avulsion. Thus, one should assume that the types of trauma and the age of the children at the moment of trauma have different effects on those successive permanent teeth. In conclusion, when the trauma that causes changes in the position of primary teeth happened, it is important to arrange an appropriate treatment procedure considering the types of trauma and the developmental conditions of the permanent teeth.

TRAUMATIZED TOOTH STABILIZATION USING VACUUM-FORMED SPLINT IN A CEREBRAL PALSY PATIENT (뇌병변 장애 환자의 외상 치아에서 vacuum-formed splint를 이용한 교합 안정술)

  • Reposition and splinting has been widely recommended when clinicians treat traumatically dislocated tooth. This case represents stabilization of traumatized tooth in a cerebral palsy patient who failed resin wire splint because of parafunctional oral habit and lack of cooperation. Clinically, mobility of traumatized tooth decreased due to stabilization using vacuum-formed splint with posterior occlusal block in 2 weeks. Vacuum-formed splint may be a simple and effective stabilization technique for traumatized tooth in a cerebral palsy patient.

  • The loosely structured periodontal ligament and low mineralized bone surrounding erupting teeth provide minimal resistance to an extrusive force. The maxillary central incisors are the most frequently avulsed due to trauma. The prognosis of replanted teeth is affected by extra-alveolar period and conditions. Implant-supported restorations are contraindicated in adolescent patients with poor prognosis traumatized teeth. Autotransplantation of premolar to the anterior region provides a viable treatment alternative. This case report describes the autotransplantation of premolars and esthetic rehabilitation in a teenage patient with traumatized incisors, poor oral hygiene, and severe crowding. Comprehensive and long-term treatments were performed to improve complex and challenging oral problems.

  • This study three-dimensionally and histologically assessed the effect of Emdogain$^{(R)}$ on periodontal healing in replanted teeth in three beagle dogs which were 1 to 2 years old, weighing from 10 to 13 ㎏. Twelve maxillary and mandibular incisors were intentionally extracted and replanted after 15(Group I), 30(Group II) and 60(Group III) minutes of air dry storage with and without the application of Emdogain$^{(R)}$. The following conclusions could be drawn from the present investigation. 1. Increased incidence of inflammatory healing response in periodontal tissue was proportional to air dry time in Both control and experimental group(P<0.001). 2. Treatment Group showed lower inflammatory root resorption, especially there were statically significant in Group I, III(P<0.001, Group II : P=0.093). 3. Inflammatory root resorption were mainly occurred in apical third(14, 15-16 section)(P<0.05, 0.001), and cervical third(1 section)(P<0.05). 4. In view of horizontal section of long tooth axis, Main root resorption area were 1, 4, 5 line area(bucco-lingual area)(p<0.01).

Effectiveness of Dental Emergency Education for School Nurses (학교보건교사를 위한 치과 응급처치 교육의 효과)

  • School nurses can play an important role in improving the prognosis of traumatized teeth of school children when they are informed about the immediate and proper dental first aid steps. The purpose of this study were to assess the awareness of school nurses concerning the management of traumatized teeth and to determine if a lecture on dental trauma management could improve school nurses'knowledge on this topic. School nurses in Gwangju and Jeonnam province completed a questionnaire before and after the lecture on the first-aid knowledge with particular focus on the following three categories. Questionnaires are composed of questions asking general information and about knowledge and proper attitude in case of the specific situation on dental trauma. According to previous studies, school nurses' knowledge tended to be higher compared with the teachers. Improvement of knowledge in school nurse was observed in emergency actions, in visiting time, replantation, carriage method, vaccine prescription related with luxation, and in possibility of fragment reattachment related with subluxation and fracture. In conclusion, it is recommended that periodical education are needed for improvement of school nurses'knowledge on proper management of dental trauma emergency.

Pulp vitality and coronal discoloration following traumatic injuries (치아외상 후 치아 변색과 치수 생활력의 변화)

  • Coronal discoloration is a common sequela to traumatic injuries. In subluxation cases, although the injury is not strong enough to rupture the apical vessels, discoloration may appear by tearing thin walls or occluding small capillaries. In absence of infection pulpal regeneration can occur, and as a result discoloration may completely or partially subside. But judging pulpal status by coronal discoloration can be dangerous and it may lead to unnecessary treatment. This case presents coronal discoloration and recovery following traumatic injury of maxillary anterior teeth. In diagnosing traumatized teeth routine cold tests or electric pulp tests are known to be unreliable, but with the aid of ultrasound doppler imaging, assessing pulp vitality of traumatized teeth can be more accurate.

A SURVEY ON THE AWARENESS OF ELEMENTARY SCHOOL TEACHERS ABOUT THE FIRST AID TREATMENT OF TRAUMATIZED TEETH (어린이의 치아손상 대처방법에 대한 초등학교 교사들의 인식도 조사)

  • For the tooth traumas of children, instant and appropriate treatment is one of the important factors to determine prognosis of traumatized teeth. Especially in case of avulsed tooth, prognosis is greatly affected by instant and appropriate emergency treatment. The objective of this survey was to investigate elementary school teachers? knowledge of first aid treatment for tooth traumas of children. We asked for 520 elementary school teachers in Busan to answer questionnaires and 455(87.5%) were returned. The result of the survey supports that knowledge of tooth is little understood except general information about the treatment of tooth traumas. And it also shows that practical treatment is poor and instruction of medical treatment is not generally known Furthermore most teachers agreed that expert education for prevention and cure of tooth traumas is necessary (81.8%) and mouth protector is also necessary (71.9%) for the prevention. The result leads to the conclusion that educations on the prevention and emergency treatment of tooth traumas of elementary children who are physically active are needed.
