TREATMENT FOR ROOT FRACTURE ON THE IMMATURE MAXILLARY PERMANENT CENTRAL INCISOR (미성숙 상악 영구 중절치에서의 치근파절 치험례)

  • Traumatic dental injuries in childhood and adolescent occurred more frequently than in adult. The time between the accident and the treatment is one of the most critical factors to prognosis, and because of the limited time available to examine and treat patients with traumatic dental injuries, if not treat appropriately, the result would be critical for the patient. In the previous studies, the prevalence and incidence of traumatic injuries were the most frequent at the age of 8 to 10 years, the majority of dental injuries involve the anterior teeth, especially the maxillary incisors, and males were more prevalent than females in an approximated proportion of 2:1. As the mean age of complete root formation is 10 years old, the maxillary permanent incisor involved in the most affected age group is usually immature, and the possibility of pulpal healing through excellent revascularization exists, more positive prognosis for pulp vitality would be expected. These are treatment cases of the immature maxillary permanent central incisor involved in the traumatic injury, and reports for progress and results of preserving the pulp vitality through the conservative treatment instead of the conventional endodontic root therapy.

Factors Influencing Prognosis of Traumatized Tooth in Primary Tooth Intrusion (유치 함입 시 외상 치아의 예후에 영향을 미치는 요인)

  • The purpose of this study was to investigate the characteristics of intrusion in primary dentition and to evaluate factors influencing complications of primary and permanent dentition during long-term follow-up period. 61 patients (84 teeth) were selected in this study. Medical records of 61 patients were reviewed and age, gender, cause of injury, site of injury, severity of traumatic injury, other injuries associated with trauma, treatment method, and complications of primary and permanent dentition were examined. Collected data were statistically evaluated using Chi-square test and Fisher's exact test. Intrusion in primary anterior teeth was predominant in boys over girls and fall was the most common cause of trauma. It was most common at home and occurred most in the primary maxillary central incisors. Severity had an effect on the incidence of sequelae in permanent successors (p = 0.014). The incidence of complications was significantly lower in patients with soft tissue injuries than in patients with other periodontal injuries (p = 0.000).

  • 치아 스케일링은 치아에 단단하게 결착된 치석을 제거하는 치과 시술로서 치아 우식 및 잇몸염증을 예방하는 중요한 시술이다. 특히 요즘에는 치료시간의 단축을 통한 효율성 증대의 목적으로 전기적 에너지를 미세한 진동에너지로 바꾸는 원리를 활용한 초음파 스케일링 기법이 많이 행해지고 있다. 하지만 치아 및 치석 확보에 따른 어려움으로 인해 스케일링 시술을 충분히 훈련하기란 쉽지 않다. 본 논문에서는 사용자가 가상현실을 통해 시각 및 촉각 피드백을 받으며 초음파 스케일링 시술을 훈련할 수 있는 치아 스케일링 시뮬레이션을 위한 알고리즘을 제안한다. 치아, 치석 및 잇몸의 볼륨모델과 스케이러 팁을 구성하는 각 부문의 관통깊이를 이용한 햅틱 랜더링 기법을 적용하여 스케일러의 모양에 따른 햅틱 피드백을 생성하였다. 그리고 치아의 손상을 줄이기 위해 스케일러의 팁 부문이 치아 표면에 되도록 평형을 이루어야 한다는 점에 입각하여, 치석을 구성하는 복셀들의 치아 디스턴스필드 값 비교를 통해 치석과 치아 사이의 접착면을 추출하고 스케일러의 팁 부분과 충돌하는 추출된 집착면의 각도를 고려한 스케일링 알고리즘을 구현하였다. 또한 수동 스케일링과는 달리 초음파 스케일링은 초음파의 진동에너지에 의해 점진적으로 치아와 치석 사이의 결속력이 감소된다는 점에 착안하여 치아와 치석의 접착면을 구성하는 지점 사이의 거리에 따른 결속력 감쇠 모델을 고안하였다.

  • 치과 진료는 대부분 인간의 시각에 의존하여 진단과 진료가 이루어진다. 하지만 미세한 치아 손상의 경우 의사가 육안으로 확인하기 어려울 수 있으며, 의사의 실력 및 피로 누적에 의해 조기에 발견을 하지 못할 가능성이 높아진다. 만약 미세한 치아 손상을 조기에 발견하지 못할 경우 시간이 지남에 따라 손상 부위가 커지고, 이를 치료하기 위해 더 큰 비용과 고통이 따르기 때문에 치과에서의 조기 발견이 매우 중요하다. 한편, 인류는 디지털 헬스케어 기술을 개발하여 보다 정확한 진단을 할 수 있게 되었고, 의료 서비스를 효율적으로 제공할 수 있게 되었다. 이에 본 논문에서는 위의 문제를 해결하기 위해 디지털 헬스케어를 사용하여 빅데이터와 딥러닝을 활용한 지능형 전자 루페에 대해 탐구하였다.

  • Traumatic injuries to immature permanent teeth are common and the results can be destructive. Although Hertwig's epithelial root sheath is usually sensitive to trauma, it may resist damage from trauma thereby retaining its vitality and continuing to calcify a root under favorable conditions. This case report describes two cases of trauma to immature permanent incisors. The first case presents an avulsed maxillary central incisor which has been replanted. The other case shows completely avulsed mandibular central incisors which have not been replanted. However, both cases subsequently show continued growing roots separated from the main roots. This report highlights the ability of the immature pulp tissue to continue to form dentin and the robustness of Hertwig's epithelial root sheath to initiate root development despite a traumatic injury.

  • Traumatic intrusion is a type of injury that involves axial displacement of a tooth toward the alveolar bone. Its occurance is relatively rare compared to other types of luxation in permanent dentition. It is more common in boys than in girls, and most common etiology of intrusion is fallen down. Various complication may occur following traumatic intrusion, such as pulp necrosis, root resorption, pulp obliteration and marginal bone loss. In addition, traumatic intrusion is commonly combined with hard or soft tissue injuries. Therefore, it is difficult to establish proper treatment plan. Choice of treatment for an intruded tooth by trauma include waiting for spontaneous re-eruption, orthodontic repositioning, and surgical repositioning. In this case, we repositioned the intruded central incisor using orthodontic traction, in a six-year old girl, which failed to re-erupt spontaneously.
