Objectives : This study is designed to find out the improvement of eyelid ptosis and eyeball movement trouble caused by idiopathic oculomotor nerve palsy after diagnosing the patient to Ghan-shil (肝實), Bee, Shin-hur(脾腎虛) and treating the patient with acupuncture and herb medicine. Methods & Results : The changes in clinical symptoms of eyelid ptosis and eyeball movement trouble were described as the patient was treated with acupuncture therapy and herb medicine, Shae-ghan-san gagam(洗肝散加減) and Yukmi-jihwang-tang gagam(六味地黃湯加減). Conclusions : The patient was treated by acupuncture, herb medicine on idiopathic oculomotor nerve palsy for 6 weeks and showed good effect.

Objectives : There have been many studies of the effect of Bee Venom therapy about arthritis, but no one study was reported about its whole functional mechanism to musculo-skeletal system. This study was designed to investigate the effect, Indication, and side effect of Bee Venom therapy on musculo-skeletal disease by literature review of articles. Results : The effects of Bee Venom therapy to musculo-skeletal system are divided to Anti_inflammatory effect and Anti-nociceptive effect. Anti_inflammatory effect is achieved through competitive chemotaxis, immuno-regulation, increasing of cortisol secretion by stimulating hypothalamus-pituitary gland-adrenal cortex axis. Anti-nociceptive effect is achieved by Anti-inflammatory mechanism and it works similar to anti-nociceptive effect of the acupuncture acting on central and peripheral nociceptive transduction system. The Bee Venom therapy could cause severe side effect, for example, hypersensitivity and anaphylaxis, injury to central nerve system and cardiovascular system, peripheral blood system, and renal dysfunction. Conclusions : With its Anti-inflammatory and Anti-nociceptive mechanism, Bee Venom therapy is considered that has good effects to autoimmune disease, chronic inflammation of various musculo-skeletal disease and various pain syndrome. But the clinician must be careful for its side effects.

누난 증후군은 전형적인 얼굴 모양, 선천성 심질환, 저신장을 특징으로 하는 우성유전 질환으로 출생시부터 임상증상으로 진단하는데 어려움이 있다. 이에 최근 RAS-MAPK 경로 변이로 확진되는 경우가 많고, 유전 변이 유형에 따른 임상 증상이 차이가 나는 경우도 많다. 유전 변이 확진으로 말미암아 임상 증상에 대한 적극적인 치료와 경과 관찰이 중요하므로 저자들은 국내에서 드문 KRAS 유전 변이 환아를 출생시부터 임상증상으로 진단하고 확진한 경험을 하여 이를 보고하는 바이다.

Objectives : A case series was conducted to investigate the therapeutic effects of Hang-Am Plus (HAP) on the tumor response and HRQoL (Health Related Quality of Life) in advanced non-small cell lung cancer (NSCLC) patients. Methods : Three patients were given 1,000-2,000 mg of HAP, three times a day (daily total dosage of 3,000-6,000 mg/day) for 12 weeks. Results : After the 3 month administration with HAP, three patients showed stable disease (SD) condition according to the chest computed tomography (CT), and two of the patients reported a decrease in pain levels. Conclusions : The observed NSCLC cases suggest treatment with HAP may be related to the observed tumor growth inhibition and pain reduction.

종양 대치물을 이용한 사지 구제술은 조기에 안정성을 얻어 빠른 시간 내에 환자의 관절 운동 및 체중 부하를 가능하게 하는 장점이 있으며. 최신 종양 대치물은 규격화된 제품을 조립하여 만들 수 있게 되면서 종양 대치물의 길이 선택에 있어 유연성이 확보되었고 기능적 결과에서 좋은 결과를 보고하고 있다. 하지만 종양 대치물을 통한 재건술 후 광범위한 골 및 연부 조직의 결손, 긴 수술 시간, 삽입물 크기 등의 문제로 무균성 이완, 심부 감염, 기계적인 결함 등의 합병증이 높은 것으로 보고되고 있다. 본 증례를 통하여 대퇴 원위부 종양 대치물(MUTARS$^{(R)}$)의 잠김(LOCK)부 파손으로 치료한 경험을 보고하고자 한다.

이차성 대동맥장루는 아주 드물며 대동맥-장골동맥의 재건수술의 중대한 합병증으로 발생할 수 있다. 이 질환은 진성 대동맥장루와 paraprosthetic enteric fistula 로 분류할 수 있다. 이러한 합병증의 예방, 진단 및 치료는 추구해야할 문제로 되어 있다. 염증성 가성동맥류로 복부대동맥 및 장골동맥을 인조혈관으로 치환한 후 41개월에 내시경으로 Paraprosthetic enteric Fistula로 진단되어 수술을 시행한 39세 남자환자를 치험하였기에 보고한다.

심근경색 후 심실중격결손은 흔히 심인성 쇼크를 일으키며 심근경색 후 초기에 수술적 치료를 요하는 질환이다. 비록 지난 30년간 이 질환의 사망률은 감소하였으나 아직도 상대적으로 높은 편이다. 본 증례에서는 전반적인 심근허혈을 예방하기 위하여 심근경색 후 심실중격결손을 심박동 상태에서 교정하는 수술수기를 사용하였다. 이러한 접근방법은 대동맥차단이 필요없으며 우수한 심근보호를 제공한다.

바렛식도는 식도선암의 전구 병변으로 알려져 있으나, 우리나라에서는 보고된 예가 극히 드물다. 본 원에서는 81세 남자에서 식도선암 진단 하에, 부분식도절제술 및 식도-위 단단문합술을 시행하였다. 절제된 조직의 병리검사에서 식도 선암종 주위의 배상세포를 동반한 장상피화생을 관찰할 수 있었다. 이에 문헌고찰과 함께 보고하는 바이다.

악성 종양 환자에게서 졸레드론산과 같은 비스포스포네이트를 예방적으로 투여받은 경우 이와 관련한 비전형적 대퇴골 골절의 발생 위험성은 아직까지 명확하게 밝혀진 바가 없으며, 국내에 지금까지 보고된 예가 매우 드물다. 본 증례는 다발성 골수종 환자에서 고농도 졸레드론산을 지속적으로 투여받았던 환자에서 큰 외상의 병력 없이 우측 대퇴골 전자하 부위의 비전형적 골절이 발생한 경우로 폐쇄적 정복술 및 골수강 내 금속정 내고정술로 치료한 경험을 보고하고자 한다.

Objectives : The object of this study is to report a clinical effect of oriental medical treatments with chuna for foot drop caused by herniated intervertbral lumbar disc. Methods : The patient was diagnosed as lumbar bulging disc, and was treated by lumbar traction technique with other conservative treatments including acupunture herbal mixture. And we measured Visual Analog Score(VAS), Modified Bathel Index(MBI), Nurick's Classification, Range of movement of ankle joint. Results : After treatments, Visual Analog Score, Modified Bathel Index, Nurick's Classification, Range of movement of ankle joint were improved in case. Conclusion : Oriental medical treatments with Chuna manual therapy were associated with improvement of foot drop by herniated intervertbral lumbar disc.
