당뇨병 맞춤 치료는 포괄적이고 과학적인 증거에 의해 당뇨병환자에게 각자 맞는 치료 관리 방법을 적용하는 것이다. 당뇨병 전문의들은 혈당치를 기준으로 환자들의 치료방법을 선택하고 인슐린의 분비상태와 효과에 대한 평가를 하지 않는 기존의 당뇨병 치료 모형을 보완하고, 한국인 당뇨병의 특성에 적합한 제 2형 당뇨병의 치료 모형을 개발하고자 시도해 왔다.

Feed Stuffs yearbook issue라는 책자에 실린 B.S. Pomeroy 박사의 부로일러 질병에 대한 예방치료법을 소개함과 아울러 필자의 약간의 설명을 첨가 하였다. 현재 국내에서 발생되고 있는 부로일러 질병은 장염, 마렉병, 난황염, 살모넬라균증, 뇨산침착증, 폐렴, 백혈병 등을 들 수 있는데 이에 대한 설명은 가금협회육용계 능력검정시(1971. 11.1$\~$12.31)에 안양가축위생연구소 의뢰한 가검물분석을 중심으로 하여 차후 설명하기로 한다.

Myofascial pain이나 filbrbomyalgia 등에 의한 만성 견관절통증은 보존적 치료로 치료적인 효과를 기대할 수 있는 질환이며, 수술적 치료가 필요한 다른 질환과의 감별을 요한다. 만성적인 견관절 동통의 치료는 증상의 원인과 악화요인을 확인하고 실질적으로 도움이 될 수 있는 적절한 치료법을, 단계적으로 적용하는 것이 효과적이며. 환자와 teamwork을 형성하여, 환자가 병증에 대해 충분히 이해하고 안심할 수 있도록 도와주는 것이 무엇보다 중요하다.

강력한 항레트로바이러스 치로(highly active antiretroviral therapy, HAART)의 도입으로 HIV 감영인의 기대여명(life expectancy)이 비감염인과 큰 차이가 나지 않을 정도로 향상된 것은 널리 알려진 사실이다. 하지만 평생 동안 약물을 복용해야 하며, 이에 따른 약물 부작용, 약제 내성, 경제적 손실, 불편함 등이 문제가 되고 있으며, 이를 극복하기 위한 다양한 치료법에 대한 연구가 진행 중이다. 이 글에서는 최근 발표되거나 진행 중인 새로운 치료 방법에 대해 간단히 알아보고자 한다.

천식은 기도가 수축하고 염증이 생기는 흔한 만성 호흡기 질환이다. 천식은 평소에도 기관지에 염증이 존재하므로 가벼운 자극에도 쉽게 기도가 좁아지게 되어 천명음(숨쉴 때 쌕쌕거리는 소리), 기침, 가슴이 답답함, 호흡곤란 등의 증상이 나타나게 된다. 만일 치료를 하지 않고 그대로 방치한다면 오랫동안 폐 기능이 감소된다. 아직까지 천식을 완치할 수 있는 치료법은 발견되지 않았지만, 올바른 방법으로만 하면 성공적으로 관리할 수 있는 질병이다.

현 상태에서 패혈증에서 최선의 치료는 조기, 목표지향치료, 저일호흡량을 포함한 폐 보호 기계환기, 그리고 activated protein C 등을 들 수 있으며, 코티코스테로이드 치료나 적극적인 인슐린 치료법에 대해서는 아직 치료효과가 검증되지 않은 상태이다. 전통적으로 시행되고 있는 조기에 광범위한 항생제 투여, 창상의 치료 등은 한상 강조되어야 할 치료 방법이다.

Recently we operated on two cases of PDA in premature infant. In both cases, indomethacin therapy had failed to close the PDA. The extremely small baby(body weight 540gm) died 28hrs postoperatively by unexpe ted intrathoracic bleeding probably due to coagulopathy related to septic condition and thrombocytopenia. The clinical course of the second case(body weight 1395gm) was complicated by ileal perforation sec- ondary to necrotizing enterocolitis. The baby underwent segmental resection of ileum with ileostomy on the 8th hospital day. On the 34th hospital day surgical closure of the PDA was done and the ile'ostomy was repaired simultaneously. Ventilator weaning was possible on the postoperative 6th day. The baby discharged on the postoperative 33th day with the body weight of 2050gm.

Skeletal muscles which are the largest part of human body may develop pain and dysfunction. The myofascial pain syndrome that has trigger points as a unique characteristic is a major cause of morbidity. Trigger points are focal, hyperirritable painful areas located in a taut band of skeletal muscle. They produce local area pain and a referred pattern pain and often accompany chronic joint dysfunction. Various modalities are used to inactivate trigger points in myofascial pain syndrome. Trigger-point injection has been shown to be one of the most effective treatment modality to provide prompt relief of symptoms. This review article presents general concept of myofascial pain syndrome and technique of trigger point injection.

Oral appliances are a primary treatment option for snoring and mild to moderate obstructive sleep apnea (OSA) and are considered as an alternative for patients with severe OSA who have failed continuous positive airway pressure (CPAP) or upper airway surgery. But it is true that OAs have varying degrees of effectiveness depending on different oral appliances and the efficacy of OAs are established in some but not all patients. New oral appliance, which is one of Adjustable Anterior Positioners, was developed by Dept of Oral Medicine, Dental School, Dankook University. This is a report for treating severe OSA patient with a failure of previous uvulopalatopharyngoplasty using a new Adjustable Anterior Positioner, followed by significant success of controlling OSA.
