Comparison of landmark position between conventional cephalometric radiography and CT scans projected to midsagittal plane (3차원 CT자료에서 선정된 계측점을 정중시상면으로 투사한 영상과 두부계측방사선사진상의 계측정의 위치 비교)

  • Objective: The purpose of this study is to compare landmark position between cephalometric radiography and midsagittal plane projected images from 3 dimensional (3D) CT. Methods: Cephalometric radiographs and CT scans were taken from 20 patients for treatment of mandibular prognathism. After selection of land-marks, CT images were projected to the midsagittal plane and magnified to 110% according to the magnifying power of radiographs. These 2 images were superimposed with frontal and occipital bone. Common coordinate system was established on the base of FH plane. The coordinate value of each landmark was compared by paired t test and mean and standard deviation of difference was calculated. Results: The difference was from $-0.14{\pm}0.65$ to $-2.12{\pm}2.89\;mm$ in X axis, from $0.34{\pm}0.78$ to $-2.36{\pm}2.55\;mm$ ($6.79{\pm}3.04\;mm$) in Y axis. There was no significant difference only 9 in X axis, and 7 in Y axis out of 20 landmarks. This might be caused by error from the difference of head positioning, by masking the subtle end structures, identification error from the superimposition and error from the different definition.

  • Traumatic bone cyst (TBC) is an asymptomatic lesion seen most in adolescents. TBC is found incidentally on routine panoramic examinations and appears as a relatively well-demarcated unilocular radiolucency with scalloped margins. Histological examination reveals a vacant cavity of cancellous bone usually unlined or very occasionally lined with a thin connective tissue layer. The lack of lining epithelial membrane is common histological feature. The most affected site is between the mandibular canine and third molar. The involvement of the mandibular symphysis is rare. The etiopathogenesis of the TBC is unclear. TBC is treated with surgical exploration and curettage; new bone is formed in place of the lesion within 6 - 12 months of surgery. Diagnosis of TBC prior to surgical intervention has limitations in most of the cases. Both of our patients were diagnosed through radiological examination and biopsy. Neither patient had a history of trauma. After surgery, the panoramic radiograph and CBCT were used to confirm bone healing.

Three dimensional cone-beam CT study of upper airway change after mandibular setback surgery for skeletal Class III malocclusion patients (Cone-beam CT를 이용한 골격성 III급 부정교합자의 하악골 후퇴술 후 상기도 변화에 관한 연구)

  • Objective: Lateral cephalometric radiographs have been the main form of resource for assessing two dimensional anteroposterior airway changes. The purpose of this study was to evaluate the three dimensional volumetric change in the upper airway space in Class III malocclusion patients who underwent mandibular setback surgery. Methods: Three dimensional cone-beam computed tomographs (CBCT) and their three dimensional reconstruction images were analyzed. The samples consisted of 20 adult patients (12 males and 8 females) who were diagnosed as skeletal Class III and underwent mandibular setback surgery. CBCTs were taken at 3 stages - Baseline (1.8 weeks before surgery), T1 (2.3 months after surgery), and T2 (1 year after surgery). Pharyngeal airway was separated according to the reference planes and reconstructed into the nasopharynx, the oropharynx and the hypopharynx. Measurements at Baseline, T1, and T2 were compared between groups. Results: The result showed the volume of the pharyngeal airway decreased significantly 2.3 months after surgery (p < 0.001) and the diminished airway did not recover after 1 year post-surgery. The oropharynx was the most decreased area. Conclusions: These findings suggest that mandibular setback surgery causes both short-term and long-term decrease in the upper airway space.

Evaluation of the Volume Changes of Grafted Bone Materials in Sinus Augmentation Procedure Using Dental Cone-beam CT (치과용 Cone-beam CT를 이용한 상악동 골 이식 후 나타난 골 이식재의 부피 변화평가)

  • To evaluate the 3-dimensional features of the grafted bone with maxillary sinus augmentation and to assess the relation between the degree of bone resorption and the type of bone grafting source and implant, and the operation method. A retrospective chart review of patients receiving sinus augmentation procedures for implant positioning was conducted: radiographic analysis of the volume and area of bone grafts was performed. The volumetric remodeling.measured at 6 months after implant positioning as the percentage of residual bone graft.was correlated to type of graft materials and operation methods. 53 dental implants positioned with sinus bone graft in 15 patients at Wonkwang University Sanbon Dental Hospital. Computed tomography scans,taken implant positioning after 6 months, showed greater resorption values for the group of $Puros^{(R)}$ graft alone from the group of mixed with $Puros^{(R)}$ and various bone graft (P<0.05, respectively). And resorption values according to operation methods have statistically significant difference(P<0.05). Volume change ratio of bone graft showed greater values for the group of delayed implant position from the group of simultaneously implant placement with bone graft. Retrospective data analysis shows that the method of graft with $Puros^{(R)}$ alone may occur greater resorption of graft materials than mixed graft material with $Puros^{(R)}$ and other graft materials. The group of simultaneously implant placement with bone graft also display smaller resorption ratio of bone graft than the group of delayed implant placement.

  • To insure accuracy in dental roentgenographs, it is important that the patient's head position and angulation of the x-ray tube. In 1907 Cieszynski applied the rule of isometry to dental radiography and established that in the production of an accurate image of a tooth, the central ray must be porjected perpendicularly to a plane bisecting angle formed by the longitudinal axis of the tooth and the film plane. Proper exposure of the film is a part of the production of a good dental radiograph, and correct processing also makes an essential contribution to the quality of the radiograph. The author analysed the failure of exposure and processing results, and recommended followings. 1. The patient head must be positioned that occlusal plane are parallel with the horizontal floor. 2. Central ray must be projected to the objective tooth and supporting structure and projected as perpendicularly to tooth axis and film surface as possible. 3. In processing, the temperature of the solution and the processing time must be correct.

AN EXPERIMENTAL STUDY ON THE TOOTH ROOT RESORPTION FOR DIGITAL RADIOGRAPHY (디지털 방사선 촬영술을 이용한 치근 흡수 판독에 관한 실험적 연구)

  • The purpose of this study was to quantitatively evaluated experimental tooth root resorption for digital radiography. For this study, experimentally three root sites were used, and radiograms were taken with standardized apparatus. Digital imaging system were consisted of NEC PC-980l(computer), TRINITRON(monitor), SONY XC-711 CCD camera. The display monitor had a resolution of 512X512 pixels. The obtained results were as follows: 1. In the difference of the four X-ray film of the contrast correction, the contrast difference was one gray scale variation at mean value. 2. Viewing of the view box of the periapical radiographs, experimental tooth root resorption of the periapical area of the first premolar, middle of mesial surface of the first molar mesial root, middle of lingual surface of the first molar distal root were recognized by increased diameter. 3. On the analysis by histogram, the periapical area of the first premolar, the middle of mesial surface of the first molar mesial root were each recognized tooth root resorption of the 5,6,7 pixel, 2,4,5 pixel by increased diameter. 4. On the analysis by histogram, the middle of lingual surface of the first molar distal root was each recognized tooth root resorption of the none, 3,6 pixel by increased diameter.

Early caries detection using optical coherence tomography: a review of the literature (광간섭단층촬영술을 이용한 치아우식증의 발견)

  • Early detection of carious lesions increases the possibility of treatment without the need for surgical intervention. Optical coherence tomography (OCT) is an emerging three-dimensional imaging technique that has been successfully used in other medical fields, such as ophthalmology for optical biopsy, and is a prospective candidate for early caries detection. The technique is based on low coherence interferometry and is advantageous in that it is non-invasive, does not use ionizing radiation, and can render threedimensional images. A brief history of the development of this technique and its principles are discussed in this paper. There have been numerous studies on caries detection, which were mostly in vitro or ex vivo experiments. Through these studies, the feasibility of OCT for caries detection was confirmed. However, further research should be performed, including in vivo studies of OCT applications, in order to prove the clinical usefulness of this technique. In addition, some technological problems must be resolved in the near future to allow for the use of OCT in everyday practice.

A STUDY ON SIALOGRAPHIC IMAGE OF NORMAL PAROTID GLANDS BY PANORAMIC VIEW (Panorama 촬영술에 의한 정상 성인 이하선 조영상에 관한 연구)

  • This study was performed to determine the postitional relationships of two lobes of parenchyma and to analysis the anatomical feature and its variations of duct on the panoramic views of the normal parotid glands in adults. Materials included 66 panoramic views and anterioposterior views of sialograms of selected persons and the radiograms of the gland experimentally reproduced on dry skull with lead foil and the reference images of computed tomograms of normal persons. Results were as follows : 1. On panoramic view of sialogram, the superficial lobe was revealed with totally being superimposed with the mandibular ramus and condyle and its tail portion superimposed with mandibular angle area, the deep lobe was revealed between the posterior border of the ramus and the mastoid process, and the isthmus was begin from the marked furcation off main duct and superimposed partially with the medial part of the deep lobe. 2, The mean length and the lateral extension of parenchyma was 63.18±8.05mm and 21.78±4.87mm respectively on panoramic view and showed no statistical relationship between them. 3. The main duct was generally perpendicular to the posterior border of ramus at middle portion and its configurations revealed 57,58% of curvilinear type, 21.21% sigmoid type, 15.15% reverse sigmoid type. 4, The interlobular ducts of the deep lobe showed relatively well defined features between the mandibular ramus and the mastoid process.

Reproducibility and reliability of head posture obtained by the outer canthus indicator (Outer Canthus Indicator를 이용한 두부 자세 기록법의 재현성)

  • Objective: The purposes of this study were to evaluate the reproducibility and reliability of head posture obtained by registering outer canthus as a soft tissue landmark with the Outer Canthus Indicator (OCI). Methods: Twenty-one adults with normal facial morphology were enrolled in this study (mean age $27.5\;{\pm}\;1.72$ years). To register initial head posture, height of the outer canthus from the ear rod plane was measured using OCI. Head posture was reproduced by moving the head upwards and downwards until the outer canthus was in a straight line with the indicator set at a registered height. After the head posture is reproduced by two operators after two days, lateral photographs were taken. Computerized photometric analyses of the photographs were performed. Results: The head rotations around the transverse axis were $0.69\;{\pm}\;0.43^{\circ}$, $0.98\;{\pm}\;0.65^{\circ}$ from each of the two operators. Standard errors were $0.09^{\circ}$ and $0.14^{\circ}$ each, which were similar to results from past research findings. There were no significant differences between the data from the two operators (p > 0.05). There were no correlations between the head rotation around the horizontal and vertical axes (p > 0.05). Conclusions: The present study suggests that OCI-registered head posture may minimize errors from vertical head rotation in cephalometry and photometry.

  • The purpose of this study was to evaluate the characteristics of craniofacial skeleton on orthognathic surgical cases with skeletal Class III malocclusion. For this study, 74 students at the dental college of Chosun University volunteered as a normal occlusion group. They had well-balanced faces and good occlusions with acceptable Class I molar relationship. They had not received orthodontic treatment and had no signs or symptoms of temporomandibular joint dysfunction. 45 malocclusion patients enrolled for orthognathic surgical treatment with skeletal Class III malocclusion at the Department of Orthodontics, College of Dentistry, Chosun University. On the basis of this study. the results of this study were as follows: 1. Skeletal Class III malocclusion was largely due to the overgrowth of mandible in man and the undergrowth of maxilla in woman. 2. The mandible was antero-inferiorly overgrown by large MP-HP angle and large genial angle in orthognathic surgical cases with skeletal Class III malocclusion. And also, upper incisors were severely labioversioned, but on the other hand lower incisors were linguoversioned. 3. In female, lower-third facial height was characteristically shortened in comparison with middle-third facial height and also, lower facial throat angle was small in male.
