Arthroscopic Treatment of Lesser tuberosity Malunion - A Case Report - (상완골 소결절에 발생한 부정 유합의 관절경적 치료 - 증례 보고 -)

  • Coracoid impingement syndrome results from abnormal contact between the anterosuperior humerus and the coracoacromial arch. The coracoid impingement may occur from traumatic, idiopathic, or iatrogenic causes. Traumatic causes of coracoid impingement include fractures of scapular neck, coracoid process or lesser tuberosity. Coracoid impingement due to lesser tuberosity malunion is a rare disease and most of them were treated by open procedures. The authors present a case of coracoid impingement caused by malunion after isolated lesser tuberosity fracture which was treated by arthroscopic coracoplasty with percutaneous screw fixation.

Acromion Morphology in Coronal and Sagittal Plane; Correlation with Rotator Cuff Syndrome (관상면과 시상면에서의 견봉 형태와 회전근개 파열의 연관성)

  • Purpose: We evaluated the correlation of the anatomic parameters of the acromion those represent on the magnetic resonance image (MRI) of impingement syndrome. Materials and Methods: From June, 2004 to December, 2005, 71 cases were surgically proven to be impingement syndrome, and the anterior acromial hooking angle, the lateral acromial hooking angle (AAHA and LAHA) and the acromial hooking index (AHI: the sum of the AAHA and LAHA) were compared to 16 control cases. At the same period, 55 cases were surgically proven to be partial or full thickness rotator cuff tear, and age, gender and twelve anatomic parameters, including the acromial type, the acromial angle, the anterior covering, the acromial slope, the AAHA, the lateral acromial angle, the acromial torsional angle, the lateral acromial angulation, the LAHA, the lateral covering, the acromiohumeral distance and the AHI were assessed. Results: The AAHA and AHI were increased as impingement syndrome proceeded. The acromial type and acromial angle, and the AAHA, LAHA and AHI showed significant differences between the controls and the rotator cuff tear patients on univariant analysis. On multivariant analysis, gender was most strongly correlated with rotator cuff tear. Age, AAHA and the acromial angle showed similar correlation, respectively. Conclusion: The coronal acromial shape is correlated with rotator cuff tear, and it is important to correct the lateral acromial shape when performing acromioplasty.

  • 퇴행성 변화에 의한 회전근 개 파열은 보통 관절면측에서 파열이 먼저 시작되어 전층 파열로 진행되는 것이 보편적인 반면 점액낭면 파열은 견봉하 충돌 증후군에 의해 주로 발생하지만 충돌 증후군에 의해 관절면 또는 건내 파열도 발생할 수 있다. 따라서 연령에 따른 퇴행성 변화와 더불어 생역학적 기전 등의 내적 요인과 충돌 증후군 또는 외상 등의 외적인 요인이 복합적으로 작용하는 회전근 개 파열은 관절면과 점액낭면에 파열이 공존할 수 있다. 결론적으로 회전근 개 파열은 보통 퇴행성 변화가 가장 기본적인 원인으로 생각되나 하나의 원인을 독립적으로 생각하기 보다는 다양한 원인이 동시에 혹은 단계적으로 작용하여 임상적으로 의미있는 병적인 상태로 진행한다고 보는 것이 타당하리라 생각된다.

A Case Report of Prescribing Geumchuldodam-tang(Q$\acute{i}$nzhud$\check{a}$ot$\acute{a}$n-tang) for the Three Patients with Shoulder Impingement Syndrome (금출도담탕(芩朮導痰湯)을 투여한 어깨 충돌증후군 환자 치험 3례)

  • Objectives: The Purpose of this study is to investigate the clinical application of Geumchuldodam-tang(Q$\acute{i}$nzhud$\check{a}$ot$\acute{a}$n-tang) to three patients with shoulder impingement syndrome. Methods: Patients were hospitalized at Dept. of Oriental Rehabilitation Medicine, Bu-Chun Jaseng Oriental Medicine Hospital, diagnosed as shoulder impingement syndrome and treated mainly with herbal medicine, Geumchuldodam-tang(Q$\acute{i}$nzhud$\check{a}$ot$\acute{a}$n-tang). This study was measured by Numeric Rating Scale(NRS) score. Results: After taking Geumchuldodam-tang(Q$\acute{i}$nzhud$\check{a}$ot$\acute{a}$n-tang), pain of the patients was controlled. NRS scores were decreased. Conclusions: As seen in these three cases of shoulder impingement syndrome, Geumchuldodam-tang(Q$\acute{i}$nzhud$\check{a}$ot$\acute{a}$n-tang) has a positive effect to control pain with shoulder impingement syndrome.

Comparison of Isokinetic Strength between Stage 1,2 Impingement Syndrome and Rotator Cuff Tear (1, 2단계 충돌 증후군과 회전근 개 파열 환자의 등속성 근력 비교)

  • Purpose: The goal of this study was to evaluate differences in strength deficits between (i) patients with stage 1 or 2 impingement syndrome and (ii) patients with rotator cuff tears. Materials and Methods: We enrolled 43 patients with stage 1 or 2 impingement syndrome (group 1) and 21 patients with rotator cuff tears (group 2). The isokinetic strength of both groups was evaluated at $60^{\circ}/sec$ for external rotation, internal rotation, adduction and abduction. We measured the peak torque, total work, average power of bilateral sides, peak torque relationship to body weight, and the ratio between unilateral agonist and antagonist. Results: The isokinetic strength deficits assessed in ratio of peak torque to body weight for group 1 and group 2 respectively were; $28.48{\pm}23.76%$ and $29.12{\pm}32.81%$ for abduction (p=0.929), $7.20{\pm}13.98%$ and $18.94{\pm}19.52%$; for adduction (p=0.021), $16.88{\pm}13.76%$ and $25.80{\pm}24.07%$; for external rotation (p=0.221), and $14.1{\pm}25.67%$ and $29.02{\pm}35.06%$ for internal rotation (p=0.059). For average power and total work, group 2 showed a significantly greater deficit for adduction and internal rotation than group 1. Conclusion: Those with rotator cuff tears have more isokinetic muscle strength deficits than those with stage 1 or 2 impingement syndrome. Progression from stage 1 and 2 impingement syndrome to stage 3 may result in greatest changes in strength deficits for internal rotation and adduction.

Conservative Treatment of Impingement Syndrome and Rotator Cuff Tear (충돌 증후군 및 회전근 개 파열의 보존적 치료)

  • The conservative treatment for impingement and rotator cuff tear includes rest, nonsteroidal anti-inflammatory drugs (NSAIDs), local steroid injection and physiotherapy depending on the purpose to relieve the pain and inflammation, in addition, stretching exercise to recover flexibility and strengthening exercise to recover the function could be used. When these conservative treatments are divided into multiple steps, the first one contains pain relief, modification of daily activity and stretching exercise. Second step includes strengthening exercise of the anterior/posterior cuff and peri-scapular muscles and eventually. The third step includes training program to return to job, housework and hobby activities and maintain. Thus, the key of these step wise approach for the treatment of impingement and rotator cuff tear is exercise program. Understanding of various exercise program and apply to the patients properly is most important for the conservative treatment of impingement and cuff tear.
