Dental caries is the most common disease in the oral cavity However, the mechanism and treatment of dental caries is not completely understood since many complex factors are involved. Especially the effect of pH on remineralization of early stage of dental caries is still controversial In this study, dental caries in dentin was induced by using lactic acidulated buffering solutions and the loss or inorganic substance was measured. Also decalcified specimens were remineralized by three groups of solution with different pH (group of pH 4.3, 5.0, and 5.5). Then, the amount and the area of inorganic substance precipitation was quantitatively analyzed with microradiograph. Also a qualitative comparison of the normal phase the demineralized phase, and the remineralized phase of hydroxyapatite crystal was made under SEM. The results were as follows, 1. In microradiograghic analysis, as the pH increased, the amount of remineralization in decalcified dentin tended to increase significantly As the pH decreaced, deeper decalcification, however, occurred along with remineralization. The group of pH 5.5 had a tendency to be remineralized without demineralization (p<0.05). 2. In SEM view, the remineralization in dentine caries occurred from the hydroxyapatite crystal surface surrounding the mesh of organic matrix, and eventually filled up the demineralized area. 3. 5 days after remineralization, hydroxyapatite crystal grew bigger with deposition of inorganic substance in pH 4.3 and 5.0 group, and the crystal in the remineralized area appeared to return to normal. After 10 days, the crystals in group of pH 4.3 and 5.0, which grew bigger after 5 days of remineralization, turned back to their normal size, but in group of pH 5.5, some crystals were found to double their size. In according to the results of this experiment, the decalcifying and remineralizing process of dentine is neither simple nor independent, but a dynamic process in which decalcification and remineralization occur simultaneously. The remineralization process occurred from the hydroxyapatite crystal surface.

The purpose of this study was to evaluate the diagnostic validity of an incipient occlusal caries using argon laser fluorescence. Extracted human premolars and molars with enamel carious lesion of occlusal surface were assessed using visual examination, visual examination with probing, argon laser fluorescence and histologic depth of carious lesion. The results in each of all the three detection methods were compared to the assessment of histologic depth of carious lesion using polarized microscope. The results from the present study can be summarized as follows; 1. There was highly correlation between the histologic depth of occlusal caries and all three detection methods(P<0.01). 2. The reproducibility(kappa value) of the visual examination, visual examination with probing and argon laser fluorescence between the histologic depth of occlusal caries was 0.189, 0.128, 0.472. The highest correlation was seen between detection of occlusal caries by argon laser fluorescence and histologic scores by polarized microscope. The results from this study indicated that argon laser fluorescence considered to be accurate and reliable method in detecting occlusal caries.

Objectives: The usage of fluoride varnish for a moderate to low caries-risk group has not been well validated. This study aimed to evaluate the preventive and therapeutic efficacies of fluoride varnish on the initiated root caries. Materials and Methods: Ten premolars were sectioned into quarters, further divided into two windows, one of which was painted with Fluor Protector (1,000 ppm fluoride, Ivoclar Vivadent). An initial lesion with a well-preserved surface layer was produced by pH cycling. Scanned line analysis using energy dispersive spectrometry determined the weight percentages of Ca and P in the demineralized layer. Scanning Electron microscopy and confocal laser scanning microscopy (CLSM) evaluated the varnish-applied root surfaces. Results: The mean lesion depth (SD) was 12.3 (2.6) ${\mu}m$ (single cycling) and 19.6 (3.8) ${\mu}m$ (double cycling). Double cycling extended the lesion depth, but induced no more mineral loss than single cycling (p < 0.05). The mean weight percentages of Ca and P between groups with and without varnish were not significantly different (p < 0.05). A CLSM showed varnish remained within 15 ${\mu}m$ of the surface layer. Conclusions: When a mild acid challenge initiated root tissue demineralization, the application of low-concentration fluoride varnish did not influence the lesion depth or the mineral composition of the subsurface lesion.

Recently the effect of CPP-ACP on early caries remineralization and prevention has hashbeen extensively researched. However, there has been a lack of research on micro-shear bond strength of a sealant applied on a CPP-ACP treated surface. Therefore, the purpose of this study was to evaluate the effect of CPP-ACP on the micro-shear bond strength of a sealant bonded with 3 different bonding systems. From the sound human 3rd molars, the 150 specimens were prepared and randomly assigned to six groups. Group I: Immersion in artificial saliva for 2 weeks + 35% phosphoric acid Group II: Immersion in artificial saliva for 2 weeks + 35% phosphoric acid + dentin bonding agent Group III: Immersion in artificial saliva for 2 weeks + self-etching adhesive Group IV: CPP-ACP application for 2 weeks + 35% phosphoric acid Group V: CPP-ACP application for 2 weeks + 35% phosphoric acid + dentin bonding agent Group VI: CPP-ACP application for 2 weeks + self-etching adhesive Sealant was applied and the micro-shear bond strength was measured. From the result of this study, it can be assumed that the CPP-ACP pretreatment can weaken the bond strength of a sealant if the enamel surface is conditioned with self-etch adhesive.
Dental caries is the most common oral disease. There are many factors contributing to its development, but complete understanding and prevention are not fully known. However, it is possible to remineralize the early enamel curious lesion by fluoride containing remineralization solution. Recently the pH-cycling model has been used to examine the effect of fluoride solution on remineralization of artificial caries in vitro as it can closely simulate the conditions encountered in vivo within a carefully controlled environment. The aim of this study was to evaluate the remineralizing effects of supersaturated buffer solutions under pH-cycling model. The specimen with 3mm-diameter was made using mature bovine incisors which has no caries and has sound enamel surface. Early curious lesions were produced by suspending each specimens into demineralization solution at pH 5.0 for 33 hours and the specimen whose surface hardness value ranged from 25 to 45 VHN were used. The pH cycling treatment regimen consisted of 5 min soaks of three treatment solutions four times per days for 15 days and the continuous cycling of demineralization and remineralization were carried out for 15 days. Following the pH-cycling treatment regimen, the specimens' surface microhardness were measured by the Vickers hardness test (VHN) and analyzed by ANOVA and Duncan's multiple-range test. 1. The surface microhardness value of supersaturated solution, Senstime, and Gagline groups were increased after pH cycling, and that of supersaturated solution was significantly Increased compared to saline group(P<0.05). 2. The surface remineralization effect of fluoride containing solutions was accelerated by saliva under pH-cycling mode 3. The pH cycling model was considered appropriate to mimic the intra-oral pH changes when evaluating demineralization and remineralization in vitro. Under the results of above study, salivary remineralization effect can be improved by fluoride containing remineralization solution. The aim of this study is to compare the differences of the demineralization resistance of resin infiltration and 1.23% acidulated phosphate fluoride in bovine teeth with artificial caries. We applied 1.23% Acidulated phosphate fluoride (APF) gel and $Icon^{(R)}$ caries infiltrant on the artificial bovine enamel carious lesion and then demineralized all samples. The depth of demineralization was measured by using Confocal Laser Scanning Microscope (CLSM) and observed the roughness and irregularity of the enamel was observed by Scanning Electron Microscope (SEM). In this experiment with demineralization resistance on smooth artificial carious lesion, less depth of demineralization, roughness, and irregularity of enamel was observed in APF gel and $Icon^{(R)}$ group than in the control group. There was no significant difference between the depth of demineralization of 1.23% APF gel and $Icon^{(R)}$ caries infiltrant group. However, resin infiltration is beneficial as less roughness and irregularity was observed on the enamel surface than when 1.23% APF gel is applied.

Streptococci are Gram-positive, facultative anaerobes and have no catalase activities. Among mutans streptococci containing ${\alpha}-type$ hemolytic activity, S. mutans is a causative agent for dental caries. As well as acid production yielding the demineralization of tooth enamel, adherence and colonization of S. mutans to the teeth are also important for its virulence. These early colonization are accomplished by the bacterial fibrillar protein, Antigen I/II (Ag I/II) and glucosyltransferase (GTF). Therefore, Ag I/II and GTF are reasonable targets for the development of vaccine against S. mutans GS-5. The ag I/II and gtfD genes from S. mutans GS-5 were cloned and sequenced. Sequence analyses showed the nucleotides sequence of cloned genes had high homology to the sequences previously reported. The sequence alignment of 280 nucleotides between the cloned Ag I/II and the available sequence of the corresponding S. mutans GS-5 showed the perfect match. Comparing with the sequence of gtfD from S. mutans UA159, the corresponding nucleotide sequence of S. mutans GS-5 showed some mismatches and the mismatches introduced changes in four residues out of 105 amino acids, yielding four missense mutations.

The newly developed equipments for the early detection of carious lesion are LFD (laser fluorescence device), Ultrasonic diagnostic system, CLSM(confocal laser scanning microscopy), QLF(quantitative light-induced fluorescence) and DIFOTI (digital imaging fiber-optic trans-illumination) system. In this study, DIFOTI system and LFD were used for the detection of early enamel caries. Twenty five primary teeth extracted from twenty one children at around the dentitional exchanging period were selected as samples. The results obtained from DIFOTI imaging and LFD measurement were compared with those of CLSM and comprehensive evaluations were made for the diagnostic capacity of each device. In vitro test, 40 sample teeth with their buccal & lingual surface formed by a window of $2{\times}3mm$ in diameter were immersed in artificial demineralizing solution for the period of 4, 8, 12 and 16 days. The results obtained from the experimental groups (DIFOTI, LFD) were compared to control group (CLSM) and we have reached to the following conclusions. 1. The sensitivity and specificity of DIFOTI system operated in oral environment was 88.2% and 76.9% respectively. 2. The sensitivity and specificity of LFD measured in oral environment was 76.5% and 69.2% respectively. 3, Regression analysis on the light transparent rate of DIFOTI showed its decrease according to the length of primary enamel decalcification performed in vitro(r=-0.96, p<0.05). 4. No statistically significant difference between LFT measurement and the length of in vitro decalcification was found in regression analysis (p>0.05). 5. The correlation coefficient of DIFOTI image transparent rate and the lesion depth of CLMS was -0.6988 (p<0.05), whereas no statistically significant difference was found for LFD measurement.

This study was aimed to assess the new trial for minimal cavity preparation in composite restoration combined with resin infiltration, focusing at application sequence. 32 human primary molars with early carious lesions around small cavity were selected and randomly divided into two groups, according to the sequence of cavity preparation (P), composite filling (F) and resin infiltration (I) as IPF and PFI group. Each group was assessed about amount of tooth reduction, features of resin infiltration, and marginal leakage around restoration. Amount of tooth reduction evaluated using micro-CT was decreased compared with the original lesion size in both groups. Features of resin infiltration were verified under confocal laser scanning microscopy. In both groups, infiltrant resin was found on all around the composite and maintained in spite of extent of decalcification even after artificial caries induction. Marginal micro leakage assessed with silver nitrate immersion and micro-CT was found more frequently in PFI group. The technique combining resin infiltration and composite restoration might ensure better adhesion prognosis as applied by the sequence of resin infiltration, cavity preparation, and composite filling. This new trial was thought meaningful in minimizing the cavity size and contributing to minimal invasive dentistry.

The aim of this study was to examine efficacy of the commercially available 10% CPP-ACP cream (Tooth mousse, GC Co., Japan) and/or 0.05% NaF solution on the remineralization of artifical caries-like lesion in the bovine teeth enamel. Sixty bovine teeth were embedded in orthodontic resin and flattened. The enamel surface in 3 mm diameter was exposed with nail varnish. Specimens were stored in demineralizing solution and divided 5 groups; Group 1 (No treatment), 2 (0.05% NaF solution 1 min), 3 (Tooth mousse 3 min), 4 (After 0.05% NaF solution, Tooth mousse treatment), and 5 (After Tooth mousse treatment, soaking in 0.05% NaF solution during 1 min). After treatment by groups, all specimens was stored in artificial saliva for 30 min. After the process described as above was performed during 10 days without pH cycling, surface hardness (Vickers Hardness Number, VHN) was tested and analyzed by paired t-test and one-way ANOVA test with SPSS 14.0. In intragroup comparison between surface hardness of pre and post-treatment, group 3, 4, 5 showed statistically significant increase (P < 0.05). In intergroup comparison among surface hardness increase of all groups, difference of group 5 between pre and post-treatment ($15.80{\pm}12.21$) was the highest, and followed by group 4 ($14.27{\pm}11.73$), 3 ($4.05{\pm}5.18$), 2 ($1.15{\pm}6.83$), 1 ($0.78{\pm}6.21$). Tooth mousse can be a good alternative agent for the fluoride, and the combination use with fluoride might have the additional anticariogenic effect.
