푸쉬업플러스와 데드리프트 운동 시 골반압박이 견관절과 요골반부 주위근의 근활성도와 체간 신전근 근력에 미치는 영향 (The Effect of External Pelvic Compression on Shoulder and Lumbopelvic Muscle sEMG and Strength of Trunk Extensor During Push Up Plus and Deadlift Exercise)

  • Background: Lumbopelvic stability is highly important for exercise therapy for patients with low back pain and shoulder dysfunction. It can be attained using a pelvic compression belt. Previous studies showed that external pelvic compression (EPC) enhances form closure by reducing sacroiliac joint laxity and selectively strengthens force closure and motor control by reducing the compensatory activity of the stabilizer. In addition, when the pelvic compression belt was placed directly on the anterior superior iliac spine, the laxity of the sacroiliac cephalic joint could be significantly reduced. Objects: This study aimed to compare the effects of EPC on lumbopelvic and shoulder muscle surface electromyography (EMG) activities during push-up plus (PUP) and deadlift (DL) exercise, trunk extensor strength during DL exercise. Methods: Thirty-eight subjects (21 men and 17 women) volunteered to participate in this study. The subjects were instructed to perform PUP and DL with and without the EPC. EMG data were collect from serratus anterior (SA), pectoralis major (PM), erector spinae (ES), and multifidus (MF). Trunk extensor strength were tested in DL exercise. The data were collected during 3 repetitions of all exercise and the mean of root mean square was used for analysis. Results: The EMG activities of the SA and PM were significantly increased in PUP with pelvic compression as compared with PUP without pelvic compression (p<.05). In DL exercise, a significant improvement in trunk extensor strength was observed during DL exercise with pelvic compression (p<.05). Conclusion: The results of this study indicate that lumbopelvic stabilization reinforced with external pelvic compression may be propitious to strengthen PUP in more-active SA and PM muscles. Applying EPC can improve the trunk extensor strength during DL exercise. Our study shows that EPC was beneficial to improve the PUP and DL exercise efficiency.

시각적 피드백을 이용한 전신 기울기 운동이 급성기 뇌졸중 환자의 체간 조절, 근력 및 균형에 미치는 효과: 무작위 대조군 예비연구 (Effects of Whole Body Tilt Exercise with Visual Feedback on Trunk Control, Strength, and Balance in Patients with Acute Stroke: a Randomized Controlled Pilot Study)

  • PURPOSE: This study was conducted to determine the effects of whole-body tilt exercise with visual feedback on trunk control, strength, and balance in patients with acute stroke. METHODS: The study included 18 patients with acute stroke who were randomly allocated to a Whole Body Tilt Exercise Group (WBTEG, N=9) and a General Trunk Exercise Group (GTEG, N=9). The WBTEG conducted whole-body tilt exercises with visual feedback, and the GTEG performed general trunk exercises. Both groups performed the exercises five times a week for 4 weeks. Outcomes were assessed using the trunk impairment scale (TIS), the trunk flexor and extensor strength test, the static balance test, and the Brunel Balance Assessment. RESULTS: After 4 weeks of intervention, both groups showed significantly improved TIS scores, muscle strength, and balance components (p<.05 in both groups). However, the improvement in TIS, muscle strength, and static balance in the WBTEG was significantly better than that in the GTEG (p<.05). CONCLUSION: Although both groups in this study showed post-intervention improvement, patients from the WBTEG who received visual feedback demonstrated more improvement. These findings indicate that whole-body tilt exercise with visual feedback may be effective at improving trunk control, trunk muscle strength, and balance in patients with acute stroke. Further studies are needed to gain a better understanding of the effectiveness of whole-body tilt exercise in patients with acute stroke.

필라테스 운동이 여중생의 요통과 체간 등속성 근력 및 평형성에 미치는 영향 (Effects of Pilates Exercises on Isokinetic Trunk Strength and Balance in Female Middle School Students with Lumbago)

  • Purpose: The purpose of this research was to investigate the effects of performing Pilates exercises for eight weeks on the isokinetic trunk strength and balance of female middle school students with lumbago. Methods: Twenty-four female students who met the test requirements were recruited and randomly divided into a control group (CG, n = 16) and a Pilates exercise group (Pilates group; PG, n = 8). The PG performed a Pilates program, which consisted of lumbar muscle strengthening exercises, for 60 minutes three times a week for eight weeks. Data analysis was conducted by two-way repeated ANOVA, and a Bonferroni test was carried out when significant differences appeared. The alpha level was set at 0.05. Results: Following the experimental treatment, the PG showed an increased trunk flexion peak torque of 17% and 13% at angular velocities of 180°/sec and 300°/sec, respectively. The intergroup comparison showed no significant difference at pre-measurement, while the PG increased about 31% (180°/sec) and 15% (300°/sec) higher than the control group at post-measurement. Following the experimental treatment, the PG showed an increased trunk extensor peak torque of approximately 29% and 21% at angular velocities of 180°/sec and 300°/sec, respectively. The intergroup comparison showed no significant difference at pre-measurement, while the PG increased approximately 27% (180°/sec) and 15% (300°/sec) higher than the control group at post-measurement. Both the CG and the PG showed a 20% and 50% decrease in pain index after the experimental treatment, respectively. After the experimental treatment, the PG had about 50% lower pain than the CG. The average error rate of static balance in the PG reduced by 19% from 3.28±0.45 to 2.65±0.36, confirming a significant improvement. Conclusion: Regular Pilates exercise for eight weeks improved the isokinetic trunk muscle strength of female middle school students with lumbago and relieved their pain. Pilates was also shown to be effective in improving balance.

수술 침습도에 따른 등장성 요부선전운동이 술후 체간 선전근력에 미치는 영향 (The Effort after Isotonic Lumbar Extension Exercise According to Surgery Invasiveness)

  • The purpose of this study is to analyze, durability of motor effort after finishing rehabilitation program and the influence from the isotonic lumbar region extension exercise after an operation according to the different operation way in invasive degrees. We selected randomly 80 patients who have no complication and musculoskeletal system diseases with finishing the 12 weeks' rehabilitation program after getting laser discectomy, but fail to conservative treatment, about Lumbar HNP, and divided into minimal invasive groups 38 and invasive groups 42 for study. As the results, in minimal invasive group, after finishing cure and 6 months later, the degree of hold muscle of women is much better than that of men. but in Invasive group, that of men is better than that of women, in case of women, the $0^{\circ}$ and $72^{\circ}$ in lumbar flexion angle have weaken or no improvement than before beginning exercise. and muscle force in $72^{\circ}$ in lumbar flexion angle increased to all men and women than before beginning exercise in Minimal invasive group. but in invasive group. it decreased. When analyze the lapsed time in Minimal invasive group, the groups who start the exercise within 3 months are better than the groups who start the exercise above 3 months after an operation the hold muscle degree all angles after finishing cure and 6 months later. Meanwhile, in invasive group, the groups who start the exercise within 3 months are better than the groups who start the exercise above 3 months after an operation about the hold muscle degree in 36-72 of lumbar flexion angle but 0-36 are not good after finishing cure and 6 months later. When analyze BMI in Minimal invasive group, the normal groups are better than the overweight groups about hold muscle degree in all angles after finishing cure and 6 months later.

만성뇌졸중 환자의 웰니스 증진을 위한 신체기능과 낙성전조동작의 관련성 연구 (A Study on the Relationship between Body Function and Prelusive Movement to Falls to Promote Wellness in Chronic Stroke Patients)

  • 이 연구의 목적은 만성 뇌졸중 환자의 낙상 전조 동작과 다양한 신체 기능과의 관련성을 알아봄으로써 뇌졸중 환자의 낙상 연구에 관한 기초 자료를 제공하고자 실시하였다. 연구 대상은 뇌졸중으로 편마비 진단을 받고 N병원에서 입원 치료를 받고 있는 환자 중 연구에 동의한 환자 75명을 대상으로 실시한 결과 낙상 유무에 따른 일반의학적 특성에서 성별, 연령, 신장, 체중, BMI, 진단명, 마비 유형, 유병 기간에서는 차이가 없었으나 보행 보조 도구 사용(독립보행, 네발 지팡이, 외발 지팡이)에서는 비낙상군이 1회 낙상군과 다발성 낙상군과 비교하여 차이가 있는 것으로 나타났다. 낙상 전조 동작에 영향을 주는 요인을 알아본 결과 일어서기에서는 STS, FM-하지 운동 기능 순이었으며, 일어서기 시도에서는 FM-하지 운동 기능, STS, 연령, TIS-동적 균형, OLST-마비측 순이었다. 일어선 직후 균형 잡기에서는 FM-하지 운동 기능이 영향을 주었고, 서서 균형 잡기에서는 FM-하지 운동 기능, OLST-마비측, TIS-협응력 순이었다. 살짝 밀기에서는 FM-총합이 영향을 주었으며, 서서 눈 감고 균형 잡기에서는 STS, 연령, OLST-비마비측, FM-하지 운동 기능, TIS-정적 균형, OLST-마비측, TIS-동적 균형 순이었다. 360도 회전하기에서는 TIS-협응력, FM-하지 운동 기능, FM-상지 운동 기능 순이었다. 이상의 결과로 볼 때, 만성 뇌졸중 환자의 낙상 전조 동작과 다양한 신체 기능과의 관련성을 확인함으로써 낙상을 예방하기 위한 마비측 하지 근력 강화와 더불어 정상적인 보행 주기 및 체중 부하에 입각한 집중적인 균형, 체간조절, 보행 대칭성 훈련 등이 필요하며, 외적인 자극에 대한 최적의 운동 반응을 유도할 수 있는 고유수용성 감각 훈련이 병행되어야 할 것이다.
