  • Conventional antipsychotics are commonly used to treat children and adolescents suffered from schizophrenia to other neuropsychiatric conditions. Regrettably, studies for typical antipsychotics report high rates of sedation, orthostatic hypotension, and extrapyramidal side effects. Over the past few years, atypical antipsychotics have been prescribed for use in adults with psychotic symptoms. Child psychiatrists have begun using these drugs to children and adolescents hoping safe and better alternatives to the conventional antipsychotics. However, there is not enough short-term and almost no long-term data about atypical antipsychotics for pediatric patients. Therefore, the purpose of this article is to review what is known about the use of the atypical antipsychotics in young patients. To do so, an appropriate approach to the use of these drugs in child and adolescent patients my be offered.

THE PSYCHOSOCIAL CHARACTERISTICS OF ADOLESCENT DRUG USER (약물사용 청소년의 심리-사회적 특성)

  • This study intended to investigate the characteristics of adolescent drug user in Korea. A questionnaire on adolescent drug use was administered to 1st or 2nd grade 898 students (465 males, 433 females) from 9 senior high schools in Seoul area. The extent of current using six representative drugs such as cigarettes, alcohol, chemical substance inducing hallucination(e,g., thinner, bond etc.), psychostimulant, hypnotic and minor tranqulizer was self-reported. Also their psychological distress, self-concept and individual-related variables such as academic grade, deviant behavior, drug-using peers, the perceived availability of drugs, the degree of family discipline, and the perceived parents' attitude for drinking and smoking were surveyed. Subjects were grouped into six(male) or five(female) subgroups (nonuser, simple experiencer, one kind of drug user, two kinds of drugs user, three kinds of drugs user(male only), minor tranqulizer user) to their extent of drug use. Then ANOVA procedure performed for group comparison of their psychological distress, self-concept and individual-related variables. The results showed that the most popular drugs among adolescents in Korea were cigarettes and alcohol. Minor tranqulizer was used by about 2.8% of male and 7.41% of female subjects. In general abusers showed lower academic grade, more deviant experiences, having more drug-using peers, higher degree of psychological distress, and lower postitive self-concept than nonuser group. According to the results of discriminant analysis, hostility and deviant behavior were reliable predictive variables that distinguished abuser group from nonuser group among the associated variables in this study. These results suggest that some common factors affect drug use of Korean high school students : high school students who have many common risk factors could be abusers more easily. These findings supported risk factor theory that the extent of drug use of adolescents might be associated with general maladjustment in everyday life.

Attitude to Drug Abuse in Adolescent in Jeollanam-do (전라남도 청소년의 약물남용 태도 조사)

  • 본 연구에서는 청소년의 일반적 비의도적 약물에 대한 인식과 태도를 확인하고자 하였다. 전라남도 22개 시군구 초중학교를 대상으로 무작위 추출 설문을 하였다. 약물남용예방 교육을 받은 경험 유무에 대한 결과는 438명 42.9%는 약물예방교육을 받은 경험이 있다고 하였으며, 582명 57.1%는 약물예방교육을 받은 경험이 없다고 응답하였다. 약물(술, 담배 포함)을 사용하여 기분이 좋아진다면 약간의 후유증은 무시해도 되는가에 대하여 86.2%가 그렇지 않다고 응답하였다. 술, 담배, 흡입제(본드, 가스) 등은 중독성이 없는지에 대하여 17.3%가 그렇다고 다소 높은 응답을 보였다. 담배는 피울수록 흡연량이 늘어나는가에 대한 질문은 사전 교육을 받은 그룹이 사전 교육을 받지 않은 그룹보다 '그렇다'고 높은 응답을 하였다. 만약 친구가 약물사용을 권유한다면 향후 어떻게 할 것인가에 대한 결과 88.9%가 정중히 거절할 것이라 응답하였다. 향후 약물 오남용 예방 교육이 있다면 참여하겠는가에 대한 결과 91.3% 가 '그렇다'고 응답하였다. 본 약물 오남용 예방교육은 효과적이였는가에 대한 결과 90.3%가 효과적이였다고 응답하였다. 따라서, 청소년의 특성을 이해하고, 올바른 건강 관련 행위 습득을 통해 성인기 건강의 기반을 형성할 수 있도록 꾸준히 노력해야 할 것이다.

Factors related to the Experience of Violence Damage among Multi-cultural and General Adolescents in Korea: Focusing on health risk behavior (한국 다문화 청소년과 일반 청소년의 폭력피해경험 관련요인 : 건강위험행위를 중심으로)

  • This study was to identify factors related to the experience of violence damage, focusing on health risk behavior of multi-cultural and general adolescents in Korea. Data from the 15th online survey of youth health behavior (2019) was used to analyze 35,574 adolescents. The data were analyzed using complex sample analysis by using SPSS/Win 22.0. The multi-cultural adolescents had significantly higher experience of smoking and habitual drug use and violence damage than those ordinary adolescents. In both multi-cultural and general adolescents, the more they did not live with their family, the more they had smoking experience and habitual drug use experience, the higher they experienced violence. The experience of violence victimization of general youth was significantly related to gender, school, economic status, and drinking experience. To prevent the violence victimization of adolescents, social support for adolescents who do not live with their families is required, and careful attention is needed to adolescents who have smoking and drug use experience. In addition, since multi-cultural youth are more vulnerable to health risk behaviors and violence damage than general youth, educational programs are needed to induce multi-cultural youth to practice correct health behaviors and prevent experiences of violence.

  • 이 연구에서 우리는 건강 교육 프로그램을 통해서 청소년 건강 복지를 향상시키는 방법을 추구해본다. 흡연, 음주, 약물 남용의 패턴은 전형적으로 틴에이지 시기에 형성되어진다. 청소년들은 다양한 심리 사회적 이유 때문에 substance 사용을 시작한다. 정책 수립자들의 우선 과제는 청소년들을 교육시키는 것이어야한다. 이 연구에서는 청소년 substance 사용 행위가 종단적 관점에서 연구되어진다. 청소년 substance 사용 행위 예방 교육 프로그램으로부터 얻어진 종단 데이터가 이 연구에서 사용되어진다. 건강 교육 프로그램을 도입했던 학교들에서 청소년 substance 사용 행위가 더 작은 율로 중가되는 것이 보여지고, 그 효과성이 검증되어진다.

Methamphetamine and Club Drug Use among Indiana Students in Grades 6-12

  • 연구목적: 본 연구는 미국 인디애나 주에 소재하는 중학교와 고등학교 학생들 사이의 methamphetamine, MDMA, GHB, and Rohypnol과 같은 약물사용 특성을 파악하고 청소년 약물사용과 관련된 성과 학교 환경에 대한 인지된 안전도 특성을 분석하였다. 연구방법: 본 분석에 사용된 데이터는 Indiana Prevention Resource Center (IPRC)에서 2007 년도에 수행한 음주, 흡연, 약물사용 설문조사(ATOD) 응답자료로서 인디애나 주에 거주하고 있는 6-12 학년 사이의 학생들을 편의추출하여 조사가 가능했던 158,632명의 응답을 얻었다. 연구결과: 로지스틱 회귀분석에서 조사한 4개 약물을 사용하는 학생들의 관심도는 교실에 있을 때, 복도에서 혼자 있을 때, 방과후 활동을 위해 학교에 남아있을 때 불안전하다고 느끼는 약물 미사용자보다 더 높은 경향을 보였다. GHB, MDMA, Rohypnol을 사용한다고 응답한 학생이 여자보다 남자에서 더 많았고 상관관계분석 결과, 메탐페타민을 비롯한 네 가지 약물 사용의 상호간 상관도가 모두 유의미하게 높았다.

The Factors Related to Substance Use among Korean Adolescents: Focusing on Sexual Experiences and Risky Sexual Behaviors (한국 청소년의 약물사용 관련요인: 성경험과 위험 성행동을 중심으로)

  • The purpose of this study is to investigate factors related to substance use focusing on the sexual experiences and risky sexual behaviors among Korean adolescents. This study conducted with a secondary data analysis using the Seventh Korea Youth Risk Behavior Web-based Survey 2011 in South Korea. The study participants selected 75,643 Korean adolescents who were recruited anonymously through national web-based survey. Data were analyzed using descriptive statistics, rao-Scott ${\chi}^2$, and logistic regressions considering complex samples. Results indicated that substance use was significantly associated with sexual experiences such as sexual intercourse with the opposite sex (adjusted OR=5.691; 95% CI=4.223-7.670), sexual intercourse with the same sex (adjusted OR=21.180; 95% CI=14.704-30.507) and risky sexual behaviors such as sexual assaults (adjusted OR=9.936; 95% CI=7.217-13.680), date rape (adjusted OR=16.979; 95% CI=11.648-24.751), sexual intercourse after drinking (adjusted OR=6.564; 95% CI=3.986-10.810), unexpected pregnancies (adjusted OR=18.628; 95% CI=8.476-40.936), and sexually transmitted diseases (adjusted OR=21.872; 95% CI=13.920-34.367) in Korean adolescents. Findings of this study show that adolescents who use substances are more likely to engage in risky sexual behaviors. Therefore, intervention program should be designed to consider both substance use and sexual problems and to include the effective approaches at the family and school level.

PHARMACOLOGICAL TREATMENT IN PERVASIVE DEVELOPMENTAL DISORDERS (전반적발달장애의 약물치료)

  • Pervasive developmental disorder is one of the most severe clinical disorder in child psychiatry and is associated with deviancies in multiple areas of development. Medication does not cure pervasive developmental disorder and its effectiveness is generally nonspecific. But psychopharmacological treatment can be important for some children with pervasive developmental disorder and can make many young autistics more amenable to behavior modification and education. Haloperidol, the most widely studied antipsychotics, was statistically and clinically superior to placebo, and furthermore, was known to facilitate the positive functioning such as, discrimination learning and imitative communication, without side effects. However, administration of haloperidol is associated with drug related dyskinesia, and it warrants the introduction and use of the other novel drugs. Several biochemical studies suggest that subgroups of children with pervasive developmental disorder show hyperserotonemia and increased endogenous opioid level as compared with controls. Psychopharmacological trials were conducted according to these findings(ex : fenfluramine, naltrexone), with mixed results till now. These and another drugs that have been used in children with pervasive developmental disorder and their effectiveness are reviewed.

The Relationship Between Substance Use and Delinquent Behavior, Risk and Protective Factors : Korean-American Adolescents in USA (청소년 약물사용과 비행과의 관계 및 보호요인과 위험요인에 관한 연구 - 재미 한인 청소년을 중심으로 -)

  • The subjects for this study on delinquent behavior, substance use and risk/protective factors consisted of 301 Korean-American students in grades 6 to 12. Findings were that (1) the delinquent behavior rate of adolescents using alcohol, tobacco or other substances was higher than that of non-using adolescents, increasing the odds of delinquent behavior from 3 to 45 times. (2) The probability of delinquents in the substance users group was higher by 7 to 44 times than in the non-users group. (3) The relationship with delinquent behavior of risk factors and substance use was much stronger than the relationship with delinquent behavior of protective factors and substance use. (4) The strongest predictor of delinquent behavior or substance use was the peer-individual domain.
