A Case of Enuresis and Renal Failure by Tethered Cord Syndrome with Sacral Lipoma (계류 척수 증후군에 의한 야뇨증과 신부전 1례)

  • The tethered cord syndrome is characterized by sensory and motor disturbances of the lower extremities and incontinence. We report a 12-year-old boy with sacral lipoma and a tethered cord syndrome, whose chief complaint is enuresis. This complication of tethered cord syndrome was most likely due to a renal failure and hydronephrosis secondary to a neurogenic bladder. Thus, the spinal lesions have to be considered in patients with enuresis.(J Korean Soc Pediatr Nephrol 2002 ;6 : 109-13)

VOLATILE CONSTITUENTS OF COOKED SQUID (피등어꼴뚜기의 자숙취에 관한 연구)

  • In this study, gas chromatographic analysis was carried out on volatile constituents of cooked squid for the object of obtaining information on the characteristic flavor of the cooked squid meat. The results obtained are as follows: 1) Methanol was the most effective solvent for the extraction of volatile constituents of squid meat. 2) Twenty five and thirty two peaks were detected from the condensate collected in cold traps which were immersed in ice water and dry ice-acetone, respectively. In these compounds, five kinds of volatile organic acids such as acetic acid, butyric acid, iso-valeric acid, valeric acid, and caproic acid were identified. 3) Eleven peaks were detected from the head space vapor collected in cold trap which is immersed in liquid nitrogen. Volatile amines identified in these components are as follows; methylamine, trimethylamine, dimethylamine, ethylamine, and iso-propylamine.

Studies on Varietal Resistance of Rice to Striped Rice Borer, Chilo suppressalis Walker (이화명충 (Chilo suppressalis W.)에 대한 벼품종 저항성에 관한 연구)

  • Several selected rice varieties Tong-il (Suweon #213-1), Jinheung, IR747, Suweon #229 and so forth were evaluated for their resistance to the striped rice borer, Cltilo suppressalis Walker. The resistance appears to be non-preference and antibiosis in nature. Under laboratory and greenhouse conditions, resistance to striped rice borer was manifested by the low larval weight, percentage of pupation and number of eggs recorded. In the cage test, varieties Suweon #240 and Shenshuraku appeared to be non-preferred by the borer moths for oviposition. Under field condition, variety Tetep received the lowest percentage of infested tillers.

  • Normal lumbar vertebrae function only when soft tissues are in position, constituting vertebral body, discs and facet feints. Considering the mechanism of supporting bodily weight, the widest movement of vertebral column reaches a lumbar sacral joint to cause structural changes. The feet is proved that lumbago is the damage of lumbar vertebrae accompanied with the change of soft tissues surrounding lumbar vertebrae, rather than simple pain in a certain lesion. It is based on the mechanism of vertebral body and intervertebral discs in the anatomical structure of the lumbar region. In my opinion, it is necessary to prove more accurately the cause of lumbago, escaping from the conventional cause of the abnormality of disc.

Clinical Experience of Management of Lower Leg Pain due to Chloroma of Lumbosacral Bone in Chronic Myelogenous Leukemia (만성골수성 백혈병 환자에서 발생한 요천추부 Chloroma로 인한 좌하지통증의 치료경험 1예 보고)

  • Chloroma is a localized myeloblastic tumor which may develop during the course of myelogenous leukemia or as a presenting sign of the disease. A 47-year-old female diagnosed as chronic myelogenous leukemia in her hematologic remission period complained of left lower leg pain. The lumbar-spine series showed multiple osteolytic changes in the left lateral border of the lumbar spine. An inhomogenous soft tissue mass involving left lateral aspects of lumbar vertebrae was identified by CT-scanning. At the first pain attack, lumbar epidural steroid and local anesthetic injection could abolish her pain and the patient could go a few days without pain. The following radiation therapy could also improve the symptom and retain the pain free interval. One month later, a second pain attack occurred and lumbar and caudal epidural steroid and local anesthetic injections could result only in an incidental relief of pain. Radiation and chemotherapy were started but failed to relieve pain. A neurolytic block was considered but the patient's general condition was aggravated and even verbal communication with her became impossible.

Ultrasound-guided Intervention in Lumbar Spine (요추부 초음파 유도하 중재술)

  • This review was described to investigate the feasibility of using ultrasound as an image tool for interventions of lumbar spine. This article will first provide an overview of lumbosacral spine surface anatomy and sonoanatomy. A detailed understanding of anatomy is critical for interpretation of ultrasound and procedural performance at spine. Fluoroscopy is most commonly used in interventional spine procedures, but radiation exposure is the major concern when obtaining fluoroscopic images. Ultrasound is radiation-free, is easy to use, and can provide real-time images with high accuracy. Also this device can be used in virtually any clinical setting. Ultrasound guidance offers a reliable alternative to fluoroscopy or computed tomography for lumbar interventions and can be safely performed without radiation exposure.

  • Objectives : The 5th lumbar - 1st sacrum facet joint is unstable area from an anatomical viewpoint, so that it is clinically major causes of low back pain. The purpose of this study is to assess the difference of the Van Akkerveeken measurement and intervertebral disc angle, Ferguson angle betwoon the 5th lumbar - 1st sacrum facet syndrome patients and sample group patients.Methods : Van Akkerveeken measurement, intervertebral disc angle and Ferguson angle were measured in 30 patients who had 5th lumbar - 1st sacrum facet syndrome and 31 sample group patients.Results :1. Van Akkerveeken measurement of 5th lumbar - 1st sacrum facet syndrome patients is statistically larger that of than sample group patients.2. Intervertebral disc angle of the 5th lumbar - 1st sacrum facet syndrome patients is statistically larger that of than sample group patients.3. Ferguson angle is the 5th lumbar - 1st sacrum facet syndrome patients is statistically larger than that of sample group patients.Conclusions : These results suggest that Van Akkerveeken measurement and intervertebral disc angle is able to be used for diagnosis of facet syndrome.

The Case Report on Improved Change of Lumbosacral Radiculopathy Using Acupuncture Therapy and Cox Technique - In the Point of Needle Electromyogram - (침치료 및 Cox 수기요법을 이용한 요천추 신경근병증 환자의 치험 2례 - 침근전도 검사상의 호전을 중심으로 -)

  • Objectives : We have evaluated the effects of conservative treatment on two patients who were diagnosed to lumbosacral radiculopathy. Methods : Two patients were diagnosed as lumbosacral radiculopathy through needle electromyoram. We used acupuncture therapy, Cox technique to the patients. We measured visual analog scale(VAS) and needle electromyogram before and after treatment. Results : After treating acupuncture therapy, Cox technique in the cases, We find out that the patients were improved, and changed of needle electromyogram result. Conclusions : These results suggest that Cox thechnique and acupuncture therapy were effective to lumbosacral radiculopathy and reduced the low back pain and the leg myasthenic.

A Study on Correlation Coefficients between Lumbosacral Balance and Low Back Pain (요천추부의 정렬과 설문을 통한 요통 자각도의 상호 연관성에 관한 연구)

  • Objectives : This study was designed to investigate the correlation coefficients among Oswestry low-back pain disability index(ODI), Roland-Morris disability questionnaire (RMD), visual analogue scale(VAS), lumbar lordosis angle(LLA), Cobb's angle and Ferguson's angle(FA). Methods : We measured LLA, Cobb's angle, and FA of 42 students. Then we researched ODI, RMD and VAS of all students, and analyzed correlations coefficient among all of them. Results : 1. There was significant correlation among VAS, RMD, ODI. 2. There was significant correlation between ODI and Cobb's angle. 3. There was no significant correlation between LLA, FA, Cobb's angle and VAS. 4. There was no significant correlation among LLA, FA, Cobb's angle and RMD. Conclusions : According to above results, there was no significant correlation between lumbosacral balance and low back pain except between ODI and Cobb's angle. On the other hand, there was significant correlation among RMD, ODI and VAS.

Comparison of Three Existing Methods for Predicting Compressive Force on the Lumbosacral Disc (들기작업 설계와 평가를 위한 요천추의 Compressive Force 예측모형 비교연구)

  • The main objective of this study is to compare three representative methods predicting compressive forces on lumbosacral disc : LP-based method, double LP-based method and EMG-assisted method. Two subjects simulated lifting tasks performed in the refractories industry, in which vertical and horizontal distance, and weight of load were varied. To calculate the L5/S1 compressive forces, EMG signals from six trunk muscles were measured and postural data and locations of load were recorded using the Motion Analysis System. The EMG-assisted model was shown to reflect well all three factors considered here. On the other hand, the compressive forces of the LP-based model and the double LP-based model were only significantly affected by weight of load. In addition, lowly positive correlation was observed between compressive forces of the EMG-assisted model and lifting index(LI) of 1991 NIOSH lifting equation. From this results, it can be concluded that compressive forces on L5/S1 by the EMG-assisted method should be used as biomechanical criterion in order to evaluate risk of jobs precisely, and LI can not evaluate risk of lifting tasks fully.
