Researching Quality Assessment of Systematic Reviews in the Journal of Chuna Manual Medicine for Spine & Nerves (척추신경추나의학회지에 게재된 체계적 문헌 고찰의 질 평가 연구)

  • Objectives This study amied to assess the reporting quality of systematic reviews(SRs) in the Journal of Chuna Manual Medicine for Spine & Nerves (JCMM). Methods SRs in JCMM from January 2000 to June 2023 were selected using the Research Information Sharing Service(RISS) and JCMM homepage. Two independent researchers reviewed SRs and evaluated their reporting quality using AMSTAR 2. Results A total of 51 SRs were included for assessment. The evaluation revealed 21 studies as having critically low quality, and 30 as having low quality the reporting of items in the studies ranged from a maximum of 81.2% to a minimum of 56.3%. On average, SRs reported 11.1 out of the total 16 items for AMSTAR 2 reporting quality. Conclusions To improve the quality of SRs published in JCMM, it is recommended to conduct SRs based on AMSTAR 2 and adhere to PRISMA 2020 guidelines.

Effects of Thermo-spinal massage treatment in a Patient with Rheumatism patient with Autonomic nervous system Dysfunction: A Case Report (자율신경 기능 이상이 동반된 류마티즘 환자에 대한 온열-척추 마사지 치료 효과 : 증례 연구)

  • Rheumatoid arthritis ("RA"), whose characteristics are chronic inflammation and chronic pain, accompanies autonomic nervous system(ANS) dysfunction. In particular, ANS dysfunction in patients with chronic musculoskeletal pain was found to have increased the local pain intensity while lowering the pain threshold, thereby negatively influencing pain. It is reported that thermo-spinal massage affects relief of chronic pain and recovery of ANS in patients with chronic musculoskeletal pain. Therefore, we report a case of rheumatoid patients with chronic pain and ANS dysfunction, who experienced recovery of ANS dysfunction and pain reduction by applying thermo-spinal massage treatment.

Investingation on the Relationship Scoliotic Curve and BMI, Cervical Lordosis Lumar Lordosis and Ferguson Angle in Spinal Scoliosis Patient (척추측만증 환자의 척추만곡도에 관한 고찰)

  • Objectives: To invesgate correlation between the scoliotic curve and BMI, cervical lordosis, lumbar lordosis and Ferguson angle in spinal scoliotic patient. Methods: The study was composed of 14 scoliosis patients beyond cobb'a angle $10^{\circ}$ (sample I group) and 15 scoliosis patients over cobb'a angle $10^{\circ}$ (sample II group). The patients were evaluated with X-ray findings of full spine AP and lateral views and statistically analyzed. Results: 1. Sample II group showed a significant decrease in BMI as compared with Sample I group(P<0.05). 2. Scoliotic curve had s negative relationship with $BMI({\gamma}=0.406)$ 3. Scolotic patients had a lower cervical angle than normal man. Conclusion: 1. The larger the scoliotic angle, the lower BMI 2. There are no concemed scoliotic curve and cervical lordosis, lumbar lordosis and Ferguson angle.

  • 허리는 요추(허리척추뼈), 척추골사이의 디스크(추간판), 척추와 디스크 주위의인대, 척수와 신경, 요추의 근육, 골반과 복부내의 기관들로 구성되어 있다. 우리 몸에서 고장이 잘 나는 곳은 움직임이 많은 곳이다. 사지도 움직임이 많은 관절에 고장이 잘 생긴다. 척추의 움직임이 많은 곳은 허리이다. 허리는 매우 불안정하여 전 인구의 약80%가 한번은 통증을 경험한다고 한다. 이러한 허리통증은 일반적으로 일상생활에서의 잘못된 자세와 생활습관에 기인하는 경우가 많다. 수술이나 치료 후에도 자세와 생활습관을 교정하지 못하면 재발의 경우가 있는 등 환자 자신이 허리통증에 대한 이해와 노력이 없으면 궁극적인 치료를 기대하기 어려우므로 환자 자신의 관심과 노력이 무엇보다 중요하다.이번 호에서는 인체의 신비 허리에 대해 척추전문에스병원 이승철원장과 경희대학교 동서신의학병원 척추센터 김기택 교수의 도움말로 알아본다.

  • The purpose of this study was to investigate the importance of continuous and detailed follow-up of patients after spinal surgery by reviewing the literature on epidural hematoma and the lower court ruling on lumbar MRI during the judgment on the negligence of postoperative follow-up. In the case of neglecting MRI examination or cooperation after surgery, delaying MRI examination after pain and symptom appeal after surgery, and returning home immediately after neurological symptom development after surgery, negligence in progress observation was recognized. In the case of the case where the negligence was not recognized even after the occurrence of the aftereffects by taking measures against the symptoms, and the case where the scope of the doctor's discretion for the execution of the test was recognized, It is hoped that this study will help prevent medical accidents and disputes related to follow-up after spinal surgery by increasing awareness of the importance of prompt MRI examination, diagnosis, surgical treatment, and power, especially in the case of new neurological symptoms.

Two Cases of Radial Nerve Palsy with Chuna Treatment on Cervical Vertebrae (경추 추나치료를 적용한 압박성 요골신경마비 환자 치험 2례)

  • Objective : The purpose of this study is to evaluate the korean medical treatment for compressive neuropathy of radial nerve, especially chuna treatment of cervical vertebrae. Methods : Two patients were treated with acupuncture and moxibustion, herbal medication, chuna treatment on cervical vertebrae. To evaluate the wrist drop and numbness of hand, Coding Result of arbitrary values and manual muscle test(MMT), visual analogue scale(VAS) were used. Results : After 2 weeks-treatment(in case 1) & 5 weeks-treatment(in case 2), the movement and power of wrist were restored to nearly normal range, also the numbness of hands was removed. All cases show the improvement in the movement of wrist and the numbness of hands. Conclusions : Korean medical treatment including cervical manual therapy as chuna treatment is remarkably effective for radial nerve palsy. But further studies are required to concretely prove the effectiveness of chuna treatment on central vertebrae for peripheral neuropathy.

Rotatory Vertebral Artery Syndrome in Foramen Magnum Stenosis (대공협착에서 발생한 회전척추동맥증후군)

  • Rotatory vertebral artery syndrome (RVAS) is characterized by recurrent attacks of vertigo, nystagmus, and syncope induced by compression of the vertebral artery during head rotation. A 60-year-old man with atlas vertebrae fracture presented recurrent attacks of positional vertigo. Left-beat, upbeat and count clock-wise torsional nystagmus occurred after lying down and bilateral head roll (HR) showing no latency or fatigue. Magnetic resonance imaging revealed foramen magnum stenosis (FMS) and dominancy of right vertebral artery (VA). The flow of the right VA on transcranial Doppler decreased significantly during left HR. The slower the velocity was, the more the nystagmus was aggravated. RVAS can be evoked by FMS causing compression of the VA. And the nystagmus might be aggravated according to the blood flow insufficiency.

  • A 68-year-old man presented with a bed sore with pus discharge on lower back. Radiographs showed extensive destruction of the L4 vertebral body. Magnetic resonance imaging (MRI) showed fluid collection with an enhanced wall at the defect of the L4 vertebral body extending into both psoas muscles. The primary diagnosis was neuropathic spondylopathy, but infective spondylitis was not ruled out. Initially, he was treated with antibiotics for two weeks. A follow-up MRI showed no improvement of the abscess, so surgical exploration was done. Charcot spinal arthropathy resulted in extensive vertebral body destruction that may be similar to infectious spondylitis, particularly in the case with fluid accumulation due to rupture of dura.
