본 포항성모병원 정형외과에서는 1991년 5월부터 1995년 12월까지 척추불안정성과 관련된 요추부질환 중 연성고정술을 시행한 145례 중 문제점이 발생된 22례를 분석하여 다음과 같은 결론을 얻었다. 1. 총 145례 중 22례(15.1%)에서 연성고정술로 유발되는 문제점을 발견하였다. 2. 문제점으로 주위분절의 불안정성 증가 10례, 추간판 간격의 감소 8례, 척추경 나사못의 해리 3례, Graf band의 파열이 1례였다. 3. 추간판 간격의 감소 8례 중 초기 질환의 원 인으로 추간판이 파열된 경우가 6례로 의의있는 소견을 보였다. 이상과 같은 문제점이 관찰되었으나 주위분절의 불안정성 증가 및 척추경 나사못의 해리는 견고한 고정술에 비해 그 빈도가 낮은 것으로 보이며, 추간판 간격의 감소의 경우 역시 단순 수핵 제거술후 발생된 추간판 간격의 감소 보다 빈도가 적었으나, 보다 장기간의 추시 및 계속적인 관찰과 연구가 필요할 것으로 사료된다.

논은 농업생태계 생물다양성을 보전하는 중요한 서식공간으로 최근 생태학적 가치가 주목을 받으면서 중요성이 점점 커지고 있다. 저서성 대형무척추동물은 논에 우점하고 있으며 논생태계 생물다양성을 유지하는 중요한 역할을 하고 있다. 따라서 본 연구에서는 한국 남부 지방의 친환경 논을 대상으로 저서성 대형무척추동물 군집의 특성과 왕우렁이 분포에 영향을 미치는 주요 환경요인을 분석하였다. 연구 결과 저서성 대형무척추동물 군집의 다양도 지수(H')는 상주 지점(St. 12)에서 가장 높게 나타났고, 산청 지점(St. 18)에서 가장 낮게 나타났다. CCA 분석 결과, 김해와 영암에서 TDS, Salinity, EC의 값이 높게 나타났다. 그리고 군산, 익산에서는 수온(WT)이 높고 단위 면적당 왕우렁이 개체 수는 상대적으로 낮게 나타났다. 따라서 지리적인 특성과 수질 환경요인의 변화는 왕우렁이의 분포와 저서성 대형무척추동물 군집의 특성을 변화시킬 수 있다. 본 연구 결과는 농업생태계 생물다양성 보전과 생태계서비스 평가를 위한 기초자료로 활용될 수 있다.

1. 조사한 초등학교 5, 6학년 학생 중 약 10%가 등심대 검사상 양성 반응을 나타내었으며, 이들 중 정밀검사 받은 학생 대부분(88%)이 만곡을 가지고 있었다. 2. 척추측만증을 가진 학생들의 체격은 (신장, 체중)표본검사와 비교하여 차이점이 없음. 3. 어깨선 변형, 견갑후방돌출, 늑골변형, 요추부돌출이 등심대 검사의 주된 소견이며 좌측이 많음. 4. X-Ray 소견은 거의 대부분 경한 측만소견과 회전소견을 보이고 있음. 5. 증세는 요통이 제일 많았으며, 거의 대부분 모르는 사이 진행됨. 6. 자세 바르게 앉기와 등뼈 주변근육 및 골반 근육강화운동으로 척추측만증의 진행을 막을 수 있었다.

High precision of planning in the preoperative phase can contribute to increase operational safety during computer-aided spinal fusion surgery, which requires extreme caution on the part of the surgeon, due to the complexity and delicacy of the procedure. In this paper, an advanced preoperative planning framework for spinal fusion is presented. The framework is based on spinal pedicle data obtained from CT (Computed Tomography) images, and provides optimal insertion trajectories and pedicle screw sizes. The proposed approach begins with safety margin estimation for each potential insertion trajectory that passes through the pedicle volume, followed by procedures to collect a set of insertion trajectories that satisfy operation safety objectives. The radius of a pedicle screw was chosen as 70% of the pedicle radius. This framework has been tested on 68 spinal pedicles of 8 patients requiring spinal fusion. It was successfully applied, resulting in an average success rate of 100% and a final safety margin of $2.44{\pm}0.51mm$.

차세대 웹 기술인 시맨틱 웹(Semantic Web)이 각광을 받음에 따라, 시맨틱 웹과 관련된 다양한 기술들이 활발히 연구되고 있다. 특히, 시맨틱 웹의 핵심 사례인 링크드 데이터(Linked Data)는 웹으로 접근 가능한 형태의 URI로 시맨틱 데이터를 표현하고 서로 연결함으로써, 오픈 데이터로서 상호운용성 극대화 및 데이터의 통합을 용이하게 한다. 이는 바이오인포매틱스 기술 발전과 결합하여 웹에서 바로 이용 가능한 많은 의학 링크드 데이터 생산과 이를 활용한 다양한 의학용어 검색 서비스 개발 결과를 가져왔다. 하지만, 기존 의학용어 검색 서비스들은 각 기관이 보유하고 있는 의학정보를 기반으로 서비스되기 때문에 다양하고 정확한 정보를 얻기 위해서는 많은 시간과 노력을 투자해야 한다. 그래서 본 논문에서는 척추 분야 의학 관련 링크드 데이터를 활용해 정확도가 높고 확장이 용이한 척추정보 검색 서비스를 제안한다.

Recently, various types of pedicle screws have been developed considering the anatomical structure of the spine. The purpose of this study was to evaluate the pullout stiffness and strength of two types of commercial pedicle screws. The design of two type screws were single pitched thread (ST) pedicle screw and dual pitched thread (DT) pedicle screw, respectively. The tests were conducted in accordance with the ASTM standards using polyurethane (PU) test blocks which has anatomical structure of the spine. There was no significant difference in pullout stiffness between two types of screw. However, DT exhibited higher pullout strength than ST (p<0.05). Pedicle screw with dual pitched thread showed higher pullout strength without decrease in pullout stiffness compared to the standard pedicle screw. In conclusion, dual pitched thread design of the pedicle screw is considered to be more suitable than the single pitched thread for the anatomical structure of the spine.

흉요추 이행부의 안정성 압박골절은 통증의 소실과 후만각 증가 억제에 의한 척추의 안정화이다. 이의 원인을 후방인대의 손상에 의한 손상으로 보고 프로로테라피(Prolotherapy)로 이의 치료를 시행 할 수 있다고 생각되며 한방적인 방법으로 화침을 적용할 수 있었다. 최근 저자는 흉요추 이행부의 안정성 압박골절에 의한 요배통 환자를 발견하고 화침치료를 통해 증세 회복에 도움이 되었기에 치료내용과 경과를 문헌고찰과 함께 보고하는 바이다.

The main objectives of this paper are to determine optimum cutting conditions using experimental design method to manufacture pedicle screws. Generally, titanium alloys are known as difficult-to cut materials. In the machining of titanium alloy, high cutting temperature and strong chemical affinity between the tool and the work material are generated because of Its low thermal conductivity and chemical reactivity. Such phenomenon cause increase of tool wear and deterioration of surface quality. Thus, in this paper, required experimental investigations are performed to evaluate the machinability of titanium materials With tungsten carbide tools Required simulation and experiments are performed, and the results are investigated.

본 증례는 45세 남자환자로서 흉부X-선상 우측폐야에 종괴모양이 발견되어 내원하였다. 입원당시 우측제7 흉신경부위의 피부분절에 감각둔감이 있었고, 컴퓨터 단층촬영 및 자기공명영상에서 우측 후종격동에 2개의 종양이 각각 제6 및 제7번 흉추 신경공을 통하여 척주관내로 확장되는 모양이었다. 자기공명영상에서 척수 침범소견은 없었다. 우측개흉술후 제7번 늑골두와 신경공주위의 척추경을 절제\ulcorner 후 종양을 제거하였다. 종양은 신경초종양으로 확진되었으며 수술후 환자는 별다른 문제없이 퇴원하였다.

The purpose of this study is to verify the relations with the fatigue life and stress distribution on the screw thread for thoracic and lumbar spine. These screws are now developed for Korean in this research group. The fatigue life of single screw was found longer than that of up-left-screw, but shorter than that of up-right-screw. The fatigue life of screw used was associated with the angie between bending and thread direction. In the case of upper left and right screw were fractured in the region of one pitch and two pitch of the thread valley from the shank of pedicle screw, respectively.
