Disease-related Knowledge, Stress, and Quality of Life in Patients with Varicose Veins (하지정맥류 환자의 질병관련 지식과 스트레스 및 삶의 질)

  • Purpose: The purpose of this study was to investigate the disease-related knowledge, stress and quality of life for the patients with varicose veins. Methods: Data were collected via questionnaire from 138 patients with varicose veins in C University Hospital in Korea from November in Korea 2012 to August 2013. Results: Knowledge of the symptoms and diagnosis was the lowest. The scores from patients with disease-related knowledge tended to be low for both subjects over the age of 60 and subjects with low education (less than high school). The score for stress over complications was the highest. The level of stress is higher for women, subjects without spouses, subjects with higher severity of disease and patients who had been taking hormones in the past or are still taking hormones. The disease-related quality of life tended to be lower for women and subjects without spouses. 21% of the variance in quality of life was explained by the level of disease-related stress, disease severity and knowledge. Conclusion: Education and the management of nurses to improve disease-related knowledge for patients with varicose veins is highly recommended. Intervention, by reducing stress, is necessary to improve the quality of life for women, subjects without spouses, and subjects with higher severity of the disease.

The Effect of Supportive Nursing Care on the Knowledge Level and Compliance of Sick-Role Behavior in Patients with Coronary Artery Disease after Coronary Angiography (지지간호가 관상동맥조영술을 받은 관상동맥질환자의 질병관련 지식과 환자역할행위 이행에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effect of supportive nursing care on the level of knowledge and compliance with sick-role behavior in patients with coronary artery disease after coronary angiography (CAG). Methods: A quasi-experimental research was performed with 81 subjects with coronary artery disease who were admitted for CAG to a cardiovascular department. Among the selected subjects, 40 of them were assigned to experimental group and the rest of them were assigned to control group by convenience. Supportive nursing care was implemented twice by a researcher. Results: 1. The mean score of knowledge related to coronary artery disease was higher in the group who received supportive nursing care than that of the control group (t=2.259, p=.027). 2. The mean score of compliance of sick role behavior was higher in the group who received supportive nursing care than that of the control group (t=4.580, p=.001). Conclusion: The supportive nursing care after CAG was effective in increasing the knowledge level and compliance of sick-role behavior in patients with coronary artery disease. Further studies would be recommended to identify long-term effectiveness of supportive care on patients' outcomes.

동물복지법 발효후 외국산 케이지 A/S대란 올수도$\cdots$

  • 1980년대 산란 농가의 케이지는 대부분 A형이 주를 이루었으며, 1990년대에는 정부의 시설자금 지원이 본격화되면서 자동화$\cdot$대형화의 조류를 타고 외국산 직립식 케이지가 국내에 보급되기 시작하였다. 초기 직립식 케이지 설치로 평당 사육수수 증가가 가능했지만 자동화에 대한 지식 부족으로 질병과 생산성이 떨어지는 역효과도 있었다. 10년 이상 직립식 케이지에 적응하다 보니 노하우가 축적되어 대형 농장 위주로 직립식 케이지 설치가 증가하였고, 관리면에서도 안정을 보여주고 있다. 그러나 IMF이후 외국산 케이지를 설치하는 농가도 줄어들었고, 설상가상으로 정부의 시설자금 지원도 중단된 상태여서 외국산 케이지 보급이 소강상태를 보이다 최근에 와서 다시 설치가 증가하고 있다. 이와 관련하여 국내에 보급된 케이지 관리실태와 A/S현황을 알아보았다.

진단검사(Diagnostic test)의 정확도 추정

  • 수의사의 의사결정 과정에는 모든 의료행위에 내재된 본질적인 특성인 불확실성 (uncertainty)이라는 문제가 필연적으로 개입되는데 특히 진단검사와 관련된 신뢰도와 정확도 측면에서는 더욱 그러하다. 기존의 진단방법에 대체할만한 새로운 검사법이 개발되었을 때 실제로 이를 임상적 진단에 활용하기 위해서는 이 검사의 민감도와 특이도를 추정하는 작업은 필수적이다. 예컨대 표준검사법 (gold standard)이 제시되어 잇을 경우 비교적 간단한 계산으로 추정이 가능하지만 대부분의 질병에서와 같이 표준검사법이 없을 경우에는 상당한 통계적 지식을 요구한다. 또한 소위 표준검사법이라고 하는 검사도 실제로 질병에 이환된 집단과 이환되지 않은 집단을 완벽히 이분해주는 (dichotomize) 능력을 가진 것이라고는 볼 수 없기 때문에 추정과정에 신중을 기해야 한다. 본 원고에서는 다양한 상황에서의 진단검사의 민감도와 특이도를 추정하는 방법에 대하여 기술하며 본 원고에서 설명한 내용과 유사한 자료에 대하여 분석을 원하는 독자는 저자에게 연락을 주면 자세한 분석절차에 대하여 논의가 가능하다.

A study on hypertension relevant nutritional knowledge and dietary practices in Chinese college students studying in South Korea (재한 중국 유학생들의 고혈압 관련 영양지식과 식생활 실천에 관한 연구)

  • Purpose: The purpose of this study was to investigate the effects of hypertension relevant nutrition knowledge on practices for prevention of hypertension in Chinese college students studying in South Korea. Methods: We conducted a survey of 276 Chinese students studying more than three months in Korean universities who were aged 19 and older about hypertension and sodium relevant nutrition knowledge, intake of salty processed food, hypertension prevention practices, and stages of behavior change for hypertension. Results: The average score on the questionnaire for hypertension relevant nutritional knowledge was 40.62 out of 50 points, and the average score for sodium relevant nutritional knowledge was 24 out of 30 points. Kimchi was the most frequently eaten salty processed food. The average score for hypertension prevention practices was 3.10 out of 5 points. The behavior change stages for prevention of hypertension were contemplation (47.1%), action (32.2%), and pre-contemplation (20.7%). The students received high scores on nutrition knowledge and showed significantly higher scores on the action stage than on pre-contemplation or contemplation. Nutritional knowledge of hypertension and sodium showed positive correlation with hypertension prevention practices, whereas negative correlation with salty processed food intake. Conclusion: Development of an education program for Chinese students in Korea on hypertension and sodium relevant nutritional knowledge is needed so that they can practice for prevention of hypertension.

The Problem of Individuality and Intrinsic Norms in Canguilhem's Philosophy of Life (캉길렘의 생명철학에서 개체성과 내재적 규범의 문제)

  • George Canguilhem(1904-1995) is one of the rare French philosophers of the 20th century to develop an approach that was shaped by a medical education. For him, medicine is considered as "a technique or an art at the junction of many different sciences, rather than a proper science." The thesis that medicine is a technique is presented not at a practical level, but on an axiological horizon which reflects the totality of humanity. This character of medicine became a motive that concretized Canguilhem's philosophical thinking. Medical knowledge is not an application of physiology, but is derived from clinical observations which are based on the personal experiences of each patient. If medicine were based on scientific knowledge and its practice the very application of this pure knowledge, the patient might be a passive object. However, the patient doesn't remain passive, but reacts to the menace of disease according to attitude that the patient developed over the course of his or her life. Canguilhem characterizes this point as 'normativity', the core of individual life, which eludes positivist medicine. Here appear the essential contents of his vitalism. Although they emphasized the activity of individual living being, other modern French vitalists didn't consider this dimension of norms. Since the normativity in Canguilhem concerns the subjectivity of the first person, it avoids a mechanical form of explanation. Thus Canguilhem's originality is found in his derivation of the essence of medicine from individuality, values and norms.

Factors Influencing Diabetes Educational Needs in Patients with Diabetes Mellitus (당뇨병환자의 교육요구도 영향요인)

  • This study was carried out to identify the perceived educational needs of diabetes mellitus type 2(DM 2) patients and the factors influencing their diabetes educational needs. The descriptive and correlation study design was used. The data was collected from January 2, 2012 to February 29 10, 2012 and a total of 346 diabetes patients responded in a structured instrument regarding their characteristics, diabetes knowledge and needs for diabetes education. The data was analyzed using ANOVA, t-test, Scheffe test, Pearson correlation, and stepwise multiple regression. The mean score of the diabetes educational needs was 3.03 (range: 1.09-4.00). Of the factors significantly influencing the diabetes educational needs, age (${\beta}$=-.25, p<.001), diabetes education participation of the family (${\beta}$=.21, p=.003), and self blood sugar test (${\beta}$=-.13, p=.045) explained the 11% variance of the diabetes educational needs. These three factors need to be considered when implementing diabetes education programs for diabetes management.

A Study on the Correlations among Knowledge, Anxiety and Compliance of Patient Role Behavior of Outpatients with Myocardial Ischemia (허혈성 심질환자의 질병관련지식, 불안 및 환자역할행위 이행과의 관계)

  • Purpose: This study is to provide effective care to each individual with myocardial ischemia by finding out how the patient's knowledge related to the disease and anxiety level affect his/her compliance of patient role behavior. Methods: The subjects of study were 100 patients diagnosed as ischemic heart disease at a university hospital located in Gyeonggi Province. The data were collected from May 19, 2008 to August 30, 2008. A cross-sectional descriptive survey was done using a structured questionnaires. Results: The average score of knowledge level about myocardial ischemia was $22.24\pm5.07$(out of 31), anxiety level was $54.68\pm9.15$(out of 80), and compliance of patient role behavior was $34.28\pm6.00$(out of 50). A significant correlation between the patient's level of knowledge and his/her compliance of patient role behavior was found(r=0.239 p=0.017). The patients who are females, singles, without occupation, have a long term being diagnosed, under-educated, tend to be excellent in their compliance. Conclusion: The patient with a higher level of knowledge is better in his/her compliance of patient role behavior. Therefore, it is suggested that we need to develop nursing intervention to increase knowledge level as well as consistent educational support.

Attitudes, Perception and Preventive Behaviors for Health and Safety among College Students (건강과 안전에 대한 예방적 실천 행위에 영향을 미치는 요인)

  • Background: The purpose of this study was to analyze the correlation between risk perception and attitudes and preventive behaviors among college students with poor health and safety consciousness. Methods: A survey was conducted in students at three universities in the metropolitan area from May 11 to 18, 2018. Four kinds of instruments were used for this study, questionnaires about attitudes, perception, prevention education and preventive behaviors. Results: The safety-related attitude depended on age: those aged <21 years had the best attitude. Education experience significantly affected perception. Preventive behaviors were more strongly and positively correlated with attitudes than with perception. The factor most significantly affecting preventive behaviors were attitudes to preventive health. Conclusions: To allow the students to change their awareness and behaviors related to sickness and accidents, first it is necessary to give education on a continuous basis so that they can obtain perception to cope with sickness and accidents; second, it is necessary to develop behavioral guidelines related to accidents in each area so that they can practice and make it a habit through attitudinal changes.
