Clinical Factors Predicting the Pathologic Tumor Response after Preoperative Concurrent Chemoradiotherapy for Rectal Cancer (직장암에 수술 전 항암화학방사선 동시 병용요법 후 종양의 병리학적 반응에 영향을 주는 임상적 예측 인자)

  • Purpose: The objective of this retrospective study was to identify predictive factors for the complete pathologic response and tumor downstaging after preoperative concurrent chemoradiotherapy for locally advanced rectal cancer. Materials and Methods: Between the years 2000 and 2008, 39 patients with newly diagnosed rectal cancer without prior evidence of distant metastasis received preoperative concurrent chemoradiotherapy followed by surgery. The median radiation dose was 50.4 Gy (range, $45{\sim}59.4\;Gy$)). Thirty-eight patients received concurrent infusional 5-fluorouracil and leucovorin, while one patient received oral capecitabine twice daily during radiotherapy. Results: A complete pathologic response (CR) was demonstrated in 12 of 39 patients (31%), while T-downstaging was observed in 24 of 39 patients (63%). N-downstaging was observed in 18 of 28 patients (64%), with a positive node in the CT scan or ultrasound. Two patients with clinical negative nodes were observed in surgical specimens. The results from a univariate analysis indicated that the tumor circumferential extent was less than 50% (p=0.031). Moreover, the length of the tumor was less than 5 cm (p=0.004), while the post-treatment carcinoembryonic antigen (CEA) levels were less than or equal to 3.0 ng/mL (p=0.015) and were significantly associated with high pathologic CR rates. The univariate analysis also indicated that the adenocarcinoma (p=0.045) and radiation dose greater than or equal to 50 Gy (p=0.021) were significantly associated with high T-downstaging, while a radiotherapy duration of less than or equal to 42 days (p=0.018) was significantly associated with N-downstaging. The results from the multivariate analysis indicated that the lesser circumferential extent of the tumor (hazard ratio [HR] 0.150; p=0.028) and shorter tumor length (HR, 0.084; p=0.005) independently predicted a higher pathologic CR. The multivariate analysis also indicated that a higher radiation dose was significantly associated with higher T-downstaging (HR, 0.115; p=0.025), while the shorter duration of radiotherapy was significantly associated with higher N-downstaging (HR, 0.028; p=0.010). Conclusion: The circumferential extent of the tumor and its length was a predictor for the pathologic CR, while radiation dose and duration of radiotherapy were predictors for tumor downstaging. Hence, these factors may be used to predict outcomes for patients and to develop further treatment guidelines for high-risk patients.

Study of Clinical Research Acupuncture Treatment on Post-stroke Depression (뇌졸중후 우울증 침치료의 임상연구에 대한 고찰)

  • The aim of this study is to collect and analyze clinical studies on acupuncture therapies of Post-stroke depression, and to suggest a method on study about acupuncture therapies. In this thesis, electronic searches were performed. Also, 6 major journals associated with oriental medicine in Korea were searched manually. Through searching, 27 studies were selected and analyzed. There were 24 controlled studies and 3 non-controlled studies. Treatment methods were acupuncture, electronic-acupuncture, eye-acupuncture, abdominal-acupuncture and pharmaco-puncture. The percentages of these treatment method were 59.2%, 29.6%, 3.7%, 3.7% and 3.7%, respectively. Acupuncture points such as GV20, PC6, ST36, SP6, LV3, GV24, GV26, LI14, LI11, H7, ST40, GB20, GB34 and LI15 were used. In comparison of results before and after acupuncture treatment, there were statistically significant improvement in 26 studies. Based on the results from above, the studies on acupuncture for Post-stroke depression needs to be standardized and more appropriate high-quality study models should be introduced.

Four Clinical Cases of Tension-Type Headache Patients Treated with Different Types of Korean Medicine, including Gunoe-tang (구뇌탕(救腦湯)을 비롯한 한의치료로 호전된 긴장형 두통 환자 증례 보고 4례)

  • Objectives: The purpose of this case report is to describe the effect of Korean medicine therapy on patients with tension-type headaches. Methods: This study consisted of four patients with tension-type headaches. The patients were treated with Korean medicine therapies, such as herbal medicine (Gunoe-tang), acupuncture, and cupping, in addition to physical therapy. None of the patients took any analgesics during the admission period. To evaluate the effects of the treatment, the Numeric Rating Scale (NRS) for headaches was used. Results: After the treatment, the patients' complaints decreased, as shown by the results of the NRS. Conclusion: Treatment with Korean medicine may ameliorate tension-type headaches.

A Case Report on the Effect of Jeongjeongamiijin-tang-gamibang on Halitosis Patients (정전가미이진탕가미방으로 치료한 구취환자 증례보고)

  • Objectives : The aim of this study is to evaluate the long-term effect of Jeongjeongamiijin-tang-gamibang by measuring the symptom-free-period after treatment through sensory and subjective evaluation. Methods : We administered Jeongjeongamiijin-tang-gamibang to 10 patients diagnosed with stomach heat pattern. The asymptomatic period was measured from the end of treatment by sensory and subjective evaluation. Results : The total dose of medication administered to the patients was dose of 3 to 7 months and the mean was 4.1 months. Symptom-free period from the end of treatment was 24.5 to 69.4months, and the mean of treatment was 46.97 months. Conclusions : Jeongjeongamiijintanggamibang is effective for the treatment of halitosis and its effect is sustained for a long time.

  • Objectives The purpose of this study is to investigate recent clinical studies on effect of acupuncture for rhinitis in other countries. We have analyzed the studies on effect of acupuncture for rhinitis within randomized controlled trial(RCT), reviewed for 11 years (from 2001 to 2011). Methods The search database includes PubMed. To narrow the search, the following key search terms were used: 'rhinitis, acupuncture'. The search was limited to the publication date from 2001 to 2011. Results Total 20 studies were selected: RCT(13), review(5), case report(1), and cost effectiveness study(1). In RCT, 9 studies used acupuncture only, and 8 studies of them have revealed that acupuncture is effective on rhinitis. In review, 3 studies are systematic review, 2 studies are no systematic review. Among systematic review, 2 studies have revealed partial effectiveness of acupuncture for rhinitis, but they suggested the necessity of further studies. Conclusions Domestic studies on effect of acupuncture for rhinitis are necessary. For more accurate results, controlled, large scale and well designed studies are recommended.

Influence of Trunk Control Using Pelvic Movements upon the Foot Pressure in Patients with Hemiplegia (골반동작을 이용한 체간조절이 편마비 환자의 족저압에 미치는 영향)

  • Purpose: The purpose of this study was to examine effect of trunk control using pelvic movements upon the foot pressure in patients with hemiplegia. Methods: Twelve males with hemiplegia were the procedure executed turnk control using pelvic movements. The foot pressure were measured using Parotec-system. Results: The data were analysed with paired t-test. First, there was a significant increase in external and internal sensors of dynamic foot pressure change of the hindfoot before and after therapy. Also there was significant increase in hallux (p<0.05). Second, there was a significant increase of affected side in support phase(p<0.05) and decreased of affected side in overlapping phase(p<0.05). Third, there was a significant increase in foot floor contact time and impulse pressure rate between affected and non affected side(p<0.05). Conclusion: The trunk control with pelvic movement had an significant effect on the legs by increase activities of hip flexors and abductors. Also had an effect on ankle dorsiflexion and plantar flexor by biomechanical movement.

Ehrlichia canis Infection in a Dog (개 엘리키아 감염 1 증례)

  • 3-year-old female Pitbull terrier dog that had been moved to Republic of Korea was diagnosed with Ehrlichia canis infection. Abnormal findings on a complete blood count (CBC) and serum chemistry profile were moderate anemia, mild thrombocytopenia, hyperproteinemia and hyperglobulinemia. Serologic screening test by a commercial ELISA kit for Ehrlichia canis was positive, and serum antibody titer was markedly high (> 1 : 10240). The morula of Ehrlichia organisms was not detected in buffy coat blood smears. Polymerase chain reaction (PCR) was done using the peripheral blood and the result was negative. Based on the serologic test results and the clinical signs, the dog was diagnosed as ehrlichiosis. The dog responded well to doxycycline and was uneventfully recovered.

  • Epidermoid cysts are rare benign lesions in the oral cavity that may be either congenital or acquired. The cysts are usually slow-growing and asymptomatic until becoming secondarily infected or large enough to interfere with mastication and speech. Consequently, diagnosis is often delayed. The condition is also uncommon in newborns and infants. Most of the lesions occur in the floor of the mouth and rarely in the upper lip. This report describes the case of a 29-month-old girl with a palpable mass in the inner mucosa of the upper lip. The lesion was surgically enucleated using an intraoral approach and histopathologically diagnosed as epidermoid cyst.

A Case of Pyogenic Liver Abscess Caused by Staphylococcus epidermidis in a Healthy Child (건강한 소아에서 발생한 Staphylococcus epidermidis에 의한 화농성 간농양 1례)

  • Staphylococcus epidermidis is a normal inhabitant of skin, throat, mouth, vagina, and urethra. It is not usually pathogenic, particularly in immunocompetent hosts. This report describes a case of a pyogenic liver abscess caused by Staphylococcus epidermidis in a healthy 12-year-old boy. He was admitted to Seoul St. Mary's Hospital with intermittent fever for 6 days. Findings on abdominal computed tomography (CT) showed a mass measuring $7.0{\times}6.5$ cm in the right hepatic lobe. Culture of the abscess resulted in growth of Staphylococcus epidermidis as a causative organism. The patient was successfully treated with intravenous administration of antibiotics and percutaneous drainage of the abscess.

The Effects of Constraint-Induced Movement Therapy on Improvement of Hand Function in Hemiplegic Side (Constraint-Induced Movement Therapy가 편마비측 손기능 증진에 미치는 영향)

  • Purpose: This study examined the effect of constraint-induced movement therapy (CIMT) on improving the hand function in hemiplegic side. Methods: Ten subjects without a control group were given CIMT to the hemiplegic side for 3 weeks. The effects of their hand function and sensibility were examined using a MAL and two point discrimination test. Repeated ANOVA was carried out for an analysis of the effects of the application of CIMT before and after treatment. Results: The participants showed significant improvement in their functional aspect with CIMT while there were no significant changes in the time domain variables. There was significant improvement in the quantitative and qualitative aspect of MAL, as well as significant improvement in the two-point discrimination function in all fingers. Conclusion: CIMT can enhance the motor function and sensory function of the hand in hemiplegic patients.
