Attitude of Hospital Staff toward Hospice Care - The Program at Wonju Christian Hospital - (일 병원직원들의 호스피스 프로그램에 대한 인식)

  • Purpose : This study was conducted to describe the attitude of hospital staff toward the hospice program. The purpose of this study was to promote the extension and organization of hospice activities to include hospital staff. Method : This is a descriptive study using a survey method. The subjects for this study were the nurses, physicians, technicians, and support staff at Wonju Christian Hospital. Using a stratified sampling method based on position of staff, the researchers recruited 430 staff members as the sample for this study. Data collection was done through a questionnaire developed by the researchers. The data were analyzed using descriptive statistics and content analysis. Findings : 1)Ninety seven percent (n=417) of the subjects understood the concept of hospice care, and 97.4% (n=419) answered that dying patients should be respected. When considering the hospice team, the subjects of this study indicated that the team should include family members (n=245), physicians (n=77), pastors (n=41), and nurses (n=34), in that order of priority. 2) When asked to indicate priorities for systematic operation of a hospice program, the subjects indicated that the highest priority was "setting up a team for hospice service(31.4%)", followed by "setting up a hospice ward(28.6%)". Sixty seven percent(n=289) recognized the importance of the hospice activities provided by the volunteers, and 85.3%(n=367) intended to use the hospice service of the hospital. The highest merit of hospital-based hospice program is 'acomplishment of mission'. 3) The average score on attitude toward death was 2.84(maximum=4), and the best needful service except hospitalization is 'constructing of support system'. Conclusion : Most of the staff at Wonju Christian Hospital perceive the necessity for systematic hospice activities, and that to achieve this goal, administrative and structural support at the hospital governance level is the first necessity. The results of this study could be useful for any hospital which is in the first stages of setting up and promoting a hospice program.

Pollen morphology of subfamily Euphorbioideae (Euphorbiaceae) (대극아과(대극과)의 화분형태)

  • Pollen morphology of 20 species of Euphorbioideae and one from Crotonoideae was examined with light, scanning and transmission electron microscopy. The pollen morphology presented here did not support the monophyly of Euphorbioideae, and the unique characters of tribe Stomatocalyceae such as absence of aperture margins, and thick and continuous endexine suggest the exclusion of Stomatocalyceae from Euphorbioideae which is in accordance with previous phylogenetic studies and wood anatomical data. Aforementioned pollen features in this tribe support that it is closely related to Acalyphoideae. Two subtribes of Stomatocalyceae were well recognized in terms of pollen morphology: Hamilcoinae includes species with reticulate or microreticulate exine patterns, and Stomatocalycinae includes those with perforate patterns with small supratectal elements. Three subtribes of Euphorbieae, classified in terms of pollen morphology, were further divided into three different types including perforate in Euphorbiinae, microreticulate in Neoguillauminiinae and reticulate in Anthosteminae, strongly supporting the Webster's subtribal system. Hureae and Hippomaneae, having perforate sculpturing pattern with smooth margo, were similar in pollen morphology. Especially, the presence of small supratectal elements in Hura crepitans pollen suggests the close relationships with Excoecaria species of tribe Hippomaneae. According to the previous studies, unique intine thickness along the aperture margin in Euphorbieae was not reported elsewhere in Euphorbiaceae. However, in this study we found the same intine thickness along the aperture margin in Maprounea brasiliensis and Hura crepitans. We hypothesize that the similar intine thicknesses found in Euphorbieae and above two species are the evidence of close relationships among them.

Factors Affecting Social Interaction Anxiety of Nursing Students (간호대학생의 사회적 상호작용 불안에 영향을 미치는 요인)

  • There is increasing tension and anxiety about being evaluated by others such as anxiety, which is felt when others' evaluation is expected or actual, and this is called social interaction anxiety. The purpose of this study was to develop a nursing intervention program that can reduce social interaction anxiety among nursing college students by analyzing factors affecting social interaction anxiety in nursing college students. The subjects of this study were four nursing college students in B city and C city. The survey data were analyzed using frequencies, percentages, means, t-test, ANOVA, Pearson correlation coefficient, and stepwise multiple regression with the SPSS WIN 21.0 program. The results of this study show that social interaction anxiety was positively correlated with fear of negative evaluation (r=.67, p<.001), fear of positive evaluation (r=.56, p<.001), and intolerance of uncertainty (r=.44, p<.001). Factors affecting ambivalence over emotional expressiveness in terms of social interaction anxiety was interpersonal relationship (β=.19, p<.001), fear of negative evaluation (β=.43, p<.001), and fear of positive evaluation (β=.34, p<.001). The explanatory power of these variables was 58.1%. Therefore, it is thought that nursing college students need to provide a support system to maintain good interpersonal relationships, to reduce sensitivity to positive and negative evaluation, and to accept and positively evaluate their own ability.

Postoperative Quality of Life in Patients with Papillary Thyroid Cancer (갑상선 유두암환자의 수술 후 삶의 질)

  • The purpose of this study was to determine postoperative quality of life(QoL), thyroid specific symptoms(TSSs), self care compliance, anxiety and depression in patients with papillary thyroid cancer and to identify factors influencing their postoperative QoL. 154 patients were surveyed using structured questionnaires and data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation, and multiple regression with SPSS/WIN 18.0 program. The mean score of postoperative QoL in the subjects was 2.72 and the postoperative QoL score of social/family well being subscale showed the lowest score. Most of the subjects suffered from TSSs such as fatigue, cold intolerance, and mood swings. The most frequent activity for self care compliance was taking thyroid hormone(100%) and OPD follow up was the second activity(99.4%). Anxiety score was 45.3 indicating a medium level however 63% of the subjects were evaluated as depression status. Postoperative QoL in thyroid papillary cancer patients showed significantly negative correlations to TSSs, anxiety, and depression (r=-.573, p<.001; r=-.739, p<.001; r=-.742, p<.001). The factors influencing postoperative QoL were TSSs, anxiety, and depression, which explained about 64.9% of the variance. Thus to improve postoperative QoL in patients with papillary thyroid cancer, health care providers should relieve negative emotions related to long term cancer management, develop the support system and provide practical information to apply patients' physical, and psychological symptoms control.

Sexual Knowledge, Sexual Attitude and Reproductive Health Promotion Behavior among North Korean Women Defectors: alienated women's sex (북한이탈여성의 성지식, 성태도, 생식건강 증진행위: 소외된 여성의 성)

  • This study examined the reproductive health-related experiences of North Korean women defectors and their sexual knowledge, sexual attitudes, and reproductive health promotion behavior. Convergent parallel mixed methods were used. From April to July 2020, 8 North Korean women defectors in Seoul metropolitan area were recruited by snowball sampling. In addition, 175 participants in the Seoul metropolitan area, Chungcheong, and Gyeongsang Province were recruited. Content analysis was conducted to analyze qualitative data, and the SPSS 23.0 program was used. Three themes were derived: scared of sex due to ignorance', 'the embarrassing and annoying sex only for duty', and had to manage myself for sex under the limited situation.'. The subject's sexual knowledge was higher in the older and married groups, and the sexual attitude was more liberal in the younger group. Reproductive health promotion behavior was more active in the high economic level, and sexual knowledge and sexual attitude showed a positive correlation. Therefore, it is necessary to provide support systems and resources from the community to encourage reproductive health promotion activities. In addition, in terms of humanitarian aid, prioritization of the reproductive health of North Korean women is required.

Relationship among Incivility, Burnout, Coping and Satisfaction to Clinical Practice experienced by Nursing College Students in Clinical Practice. (간호대학생이 임상실습에서 경험하는 무례함, 소진, 대처와 임상실습만족도의 관계)

  • The purpose of this descriptive study was to identify the relationship among incivility, burnout, coping, and satisfaction to clinical practice experienced by nursing college students in clinical practice. A set of self-reported questionnaires, which included demographic characteristics, incivility, burnout, coping, and satisfaction to clinical practice, was completed by 160 nursing college students. Data was analyzed using t-test, ANOVA, correlation analysis, and multiple regression analysis by SPSS ver. 18.0. The mean age was 21.5 years. The mean scores, in Likert scales, were 1.18. 3.41, 2.48 and 3.30 out of 5 for items of incivility, burnout, coping, and satisfaction, respectively. According to our analysis, significant positive correlations were observed among incivility, burnout, and coping; significant negative correlations were observed between incivility and satisfaction, as well as between burnout and satisfaction. The predictors on satisfaction to clinical practice were coping, incivility, and burnout. Our model explained 26.6% (F=9.868, p<.001) of the variance. Based on the findings of this study related to main impediment, it is necessary to build prepared practice environments, such as interpersonal relations capacity empowerment and supporting system. Therefore, various nursing curricular programs are needed to promote satisfaction through multilateral collaboration with clinical practice among nursing college students.

Literature Review of Empowerment Intervention Studies for Patients with Chronic Physical Health Conditions in Korea (만성적인 신체질환을 가진 환자에게 적용된 임파워먼트 중재 연구 고찰 - 국내를 중심으로)

  • This study was a systematic review examining the effect of empowerment intervention studies among patients with chronic physical health conditions in Korea. The PICOT-SD (Participants, Intervention, Comparison, Outcomes, Timing of outcome measurement, Setting, Study design) strategy was applied. A total of 19 studies published between January 2000 and April 2020 were selected. Study participants were rheumatoid arthritis patients, hemodialysis patients, stroke patients, diabetes mellitus patients, chronic pulmonary disease patients, hypertensive patients, cancer patients, chronic kidney disease patients, and kidney transplantation patients, etc. A total of 17 studies were found to have presented a conceptual framework. Empowerment skills were participation, dialogue, experience, interaction, support, and problem solving, etc. The most frequently used dependent variables were self-care (n=13), empowerment (n=10), self-efficacy (n=7), and depression (n=5). Self-care, empowerment, and self-efficacy showed significant improvement among patients in at least 60% of the included studies. Based upon this review, there is evidence that empowerment intervention studies are an effective method to improve the health of patients with chronic physical health conditions. This study further suggests the development of an empowerment intervention program to verify the effects of the various variables identified in this study.

Needs for Hospice Care among Families of Children with Cancer for Outpatients (외래통원 암 환아 가족의 아동호스피스요구)

  • The purpose of this study was to analyze the need for child hospice care in families of children with cancer for outpatient. The participants were 83 parents of children with cancer. This survey was conducted from January 2011 to March 2011 at four hospitals in Daegu. Data were collected through self-report questionnaires and analyzed by descriptive statistics, t-test and ANOVA using the SPSS/WIN Program. Needs for hospice care for the participants were high. The need for "emotional care of children" showed the highest, "control of secondary physical problems", "acceptance of the family's difficulty", "management for terminal physical symptoms", "spiritual care for preparing for death". With respect on the demographic characteristics of the participants, there were statistically significant differences in hospice care needs, among to the religion, sibling, relatives, whether of the cancer. The above findings indicate that needs for hospice care for the participants were high about emotional care, especially as it is related to children's anxiety. Therefore hospice care, based on emotional part, should be provided systematic hospice care with specialized multidisciplinary child hospice care team, child hospice center.

THE STRATEGIC CONSEQUENCES OF A SOUTH KOREAN NUCLEAR SUBMARINE -RISKS AND REWARDS FOR THE US-ROK ALLIANCE- (한국의 핵추진잠수함 확보를 위한 도전과 과제 -한미동맹 측면에서의 전략적 효용성을 중심으로-)

  • 고도화 및 가시화되고 있는 북한의 '잠수함발사탄도미사일(SLBM: Submarine Launched Ballistic Missile)'위협에 대응하기 위한 효과적인 전략수단으로써 핵추진잠수함의 필요성에 대한 국민적 관심이 고조되고 있다. 핵추진잠수함의 전략적 가치에 대한 논의가 활발히 진행되고 있는 가운데, 주변국과의 갈등과 국제사회의 비핵화 규범의 미 준수 논란 등 핵추진잠수함 확보과정에서 야기될 수 있는 대·내외의 정치·외교적 파장에 대한 우려의 목소리 또한 높아지고 있다. 그러나, 핵추진잠수함의 필요성 및 확보와 관련한 지금까지의 대부분의 논의들은 한국의 '내부적 논의(Just our own scenario)'에 그치는 한계를 보이고 있다. 전략무기체계로써의 상징성과 그에 따르는 대외적 민감성을 고려 시 일방적이고 독자적인 핵추진잠수함 확보노력은 과정상의 시행착오와 불확실성을 더욱 가중시켜 정책적 실패로 귀결될 수 있는 위험성을 내포하고 있다. 특히, 한반도 평화와 아태지역의 안전보장이라는 공동의 전략적 목표를 공유하고 있는 동맹국인 미국의 공감대와 지지가 뒷받침되지 않은 독자적인 핵추진잠수함 확보노력은 큰 난항이 예상되며 자칫 서로간의 '전략적 신뢰(Strategic Trust)'를 무너뜨려 '한미동맹의 결속력(Alliance Cohesion)'을 약화시키는 요인으로 작용할 수 있다. 미국의 동의와 지지에 기반한 핵추진잠수함 확보를 위해서는 한국의 핵추진잠수함 확보가 동맹의 전략목표 및 미국의 전략적 이해관계에 미칠 수 있는 긍정적, 부정적 효과에 대한 충분한 검토와 논의가 선행되어야 한다. 한미동맹의 공동의 전략목표와 미국의 전략적 이익에 상충하는 한국의 핵추진잠수함 확보시도는 성공 가능성이 낮기 때문이다. 본 연구에서는 현실화되고 있는 북한의 핵위협에 대응하고 지역안전보장에 기여할 수 있는 미국과의 연합방위력 증강차원에서의 한국의 핵추진잠수함의 전략적 효용성을 분석하였다. 더불어, 한국의 핵추진잠수함 확보과정에서 야기될 수 있는 대·내외의 기술적, 정치·외교적 사안들을 살펴본 후 한미동맹 차원에서의 정책적 해결방안을 제시하였다. 연구목적을 위해 유사한 역사적 사례연구를 통해 교훈을 도출하였으며, 미국 오바마 1기 행정부에서 미국의 아태지역 및 대북정책을 주도한 전 미국 국무부부장관 제임스 스타인버그(James Steinberg) 및 여러 미국 내 한반도 전문가들의 의견을 수렴하였다. 본 연구가 한국의 핵잠수함 확보를 위한 한미간 발전적 논의의 시발점이 되기를 기대한다.

A Study on the Divorce Experienced by Marriage Immigrant Women (결혼이주여성 이혼경험 연구)

  • The purpose of this study was to analyze the life experiences of fifteen marriage immigrant Asian women who went through running away from their home countries, marrying with Korean men, divorcing from their husbands, and coping with many difficulties after their divorce in Korean society. In order to conduct this study grounded theory methods have been employed. The central phenomenon digged out from this study was 'resistance to baffled reality' (i. e. dislocation). The causal conditions which brought about the central phenomenon were 'escaping for survival' and 'experiencing the gap between reality and expectation. 'The intervening conditions included 'getting to know the reality of their husbands,' 'losing hope,' and 'not being able to pull themselves together.' The contextual conditions consisted of 'being treated as maids,' 'becoming victims of family violence,' 'making up their minds to survive,' 'securing future life,' 'being marginalized,' and 'being aware of themselves as strangers.' The action/interaction strategies on the central phenomenon were 'building support systems,' 'building up will for new life,' and 'reconstructing social identity.'The final outcome was 'arranging places of new settlement.' The divorce was classified as four types: 'coping and growth,' 'emancipation and settling down,' 'being overwhelmed by livelihood,' and 'continuous wandering.' Based on these results, this study provided a few political and practice suggestions to prevent family violence and divorces among multi-cultural families, and also to bumper the impacts of divorce.
