A Study on Regional Variations of Medical Service Utilization of Hospital Inpatients in Korea (우리나라 병원급 이상 의료기관 입원환자의 지역별 의료이용 변이에 관한 연구)

  • 우리나라의 병원급 이상 의료기관을 이용한 입원환자의 지역별 의료이용의 변이와 의료자원의 공급, 도시규모, 소득수준이 지역별 의료 이용의 변이에 미치는 영향을 살펴보기 위해 연구를 수행하였다. 본 연구에서는 이를 위해 2005년 환자조사 자료를 이용하였다. 지역별 의료이용의 변이에 성, 연령과 같은 지역별 인구구조의 차이에 의한 효과는 직접표준화 방법을 이용하여 보정한 다음 지역별 의료이용의 변이 정도를 파악하였다. 연구결과 병원급 이상 의료기관을 이용한 입원환자의 표준화 퇴원율과 표준화 재원일수는 차이가 있었다. 표준화 퇴원율에 영향을 미치는 요인으로는 도시규모, 인구10만명당 병상수가 주요 요인으로 나타났다.

Analysis of regional variation in the lifetime physician diagnosis rate of atopic dermatitis (아토피피부염 평생의사진단율의 지역별 변이 분석)

  • The purpose of this study is to analyze temporal and spatial variations of atopic dermatitis and to identify major factors. Data utilized in the study were collected by the Community Health Survey, KOSIS and so and on from 2009 to 2013. This study was analyzed using descriptive statistics and Geographically weighted regression model. As a result, regional diagnosis rate of atopic dermatitis was increased by 5 years, and difference related to geographic location was so large. The regional characteristics that contribute to the diagnosis of atopic dermatitis were as follows: older adults population ratio, ratio of basic living security received people, depression experience rate, high risk drinking rate, number of wastewater discharge business, number of tobacco retail business, number of fast food restaurant business. This study is meaningful in that it provided basic data on health policy direction and provided information on prioritization of health business in each region.

Development of Variation Marker of Myzus persicae by Altitude (고도에 따른 지역별 복숭아혹진딧물 집단 변이 마커 개발)

  • This study focused on the green peach aphid, Myzus persicae, as an indicator pest in Chinese cabbage cultivation to develop a genetic marker. We hypothesized that M. persicae gene flow is related to climate change. Genetic variation was analyzed using five local populations collected at different altitudes (157 m, 296 m, 560 m, 756 m and 932 m above sea level) in Hoengseong, Pyeongchang, and Gangneung areas, plus a laboratory strain used as an outgroup. There were no differences in ecological characteristics among strains. Esterase isozyme pattern and inter-simple sequence repeat (ISSR) PCR results showed significantly different bands between laboratory and wild, local populations. However, there was no difference among local populations. Partial fragments of ribosomal RNA (rRNA) and mitochondrial cytochrome oxidase I (mtCO I) were amplified and their nucleotide sequence was analyzed. Single nucleotide polymorphisms (SNPs) were detected in internal transcribed spacer-2 (ITS-2) and mtCO I regions among the five local populations. These SNPs can be use to discriminate different populations of M. persicae to monitor gene flow.

Analysis on geographic variations and variational factors in expenditures for hypertension (고혈압 의료비 지역 간 변이 및 변이 요인 분석)

  • This study is to investigate how the expenditures for hypertension is affected by socioeconomic, health care resources, and health behavior factors with a special emphasis on geographic variations and to provide the data about regional management for hypertension. To analyze, we combined a unique data set including key indicators from Medical Service Usage Statistics 2012 by Region by National Health Insurance Corporation, Annual Community Health Survey 2012 by Korea Centers for Disease Control and Prevention and other government organizations at the 247 small administrative districts. We found that the average expenditures of hypertension in 249 small districts is 62,000 won and coefficient of variation is 30.0. Major factors of differences in hypertension expenditure is population density, marital status, household income, number of hospital per 100 thousand, medical expenses outside the jurisdiction, drinking rate, moderate and over-intensity physical activity, and hypertension diagnosis rate. The results of decision tree was that there were significant differences between regions in hypertension diagnosis rate, household income, marital status, number of hospital per 100 thousand, obesity rate, drinking rate. This study concluded that determinants of geographic variations in hypertension spending are not only health resources and socioepidemic characteristics but health behaviors.

  • The purpose of this study was to analyze regional medical utilization variation of hospital inpatients and to suggest policy for the allocation of medical service in Korea. We analyzed the relationship among medical service, city size, income level and regional medical utilization variation of hospital inpatients. The patient survey report of 2005 year in Ministry of Health and Welfare was used for this study. To adjust on the factor age and sex, we used direct standardization method. Findings of the research were summarized as follows ; First, standardization discharge rate on patient was different in the district. city type, and number of beds. Second, standardization length of stay of patient was different in region, city type, and number of beds.

  • The purpose of this study was to examine how region-specific characteristics affect the occurrence of cardiac arrest. To analyze, we combined a unique data set including key indicators of health condition and cardiac arrest occurrence at the 244 small administrative districts. Our data came from two main sources in Korea Center For Disease Control and Prevention (KCDC): 2010 Out-of-Hospital Cardiac Arrest Surveillance and Community Health Survey. We analyzed data by using multiple regression, geographically weighted regression and decision tree. Decision tree model is selected as the final model to explain regional variations of cardiac arrest. Factors of regional variations of cardiac arrest occurrence are population density, diagnosis rates of hypertension, stress level, participating screening level, high drinking rate, and smoking rate. Taken as a whole, accounting for geographical variations of health conditions, health behaviors and other socioeconomic factors are important when regionally customized health policy is implemented to decrease the cardiac arrest occurrence.

A Study on Small Area Variations of Hospital Services Utilization in Some Acute diseases -Focused on Gastric Diseases and Acute Appendicitis (일부 급성질환의 지역간 입원의료이용 변이에 관한 연구 -위장질환과 충수염질환을 중심으로-)

  • This study aims to provide an important base resource for the effective medical supply system construction by figuring out the characteristics and changes of admission medical use of gastritis in 160 middle treatment areas classified by 2008 patient examination resources, and by figuring out factors affecting on changes. As a result, in terms of admission rate according to sex and age standardization per small area, there were differences as EQ was 15.1 and CV was 0.4. Top 10 variations were mainly in rural areas. It showed significant positive relations between admission and the number of bed, doctor and special medical equipment per 10,000 population. It is recommended that the government set a priority in the management of regional bed supply and the distribution of efficient medical resources.

Analysis on Geographical Variations of the Prevalence of Hypertension Using Multi-year Data (다년도 자료를 이용한 고혈압 유병률의 지역간 변이 분석)

  • As chronic diseases have become more prevalent and problematic, effective cares for major chronic diseases have been a locus of the healthcare policy. In this regard, this study examines how region-specific characteristics affect the prevalence of hypertension in South Korea. To analyze, we combined a unique multi-year data set including key indicators of health conditions and health behaviors at the 237 small administrative districts. The data are collected from the Annual Community Health Survey between 2009 and 2011 by Korea Centers for Disease Control and Prevention and other government organizations. For the purpose of investigating regional variations, we estimated using Geographically Weighted Regression (GWR) and decision tree model. Our finding first suggests that using the multi-year data is more legitimate than using the single-year data for the geographical analysis of chronic diseases, because the significant annual differences are observed in most variables. We also find that the prevalence of hypertension is more likely to be positively associated with the prevalence of diabetes and obesity but to be negatively associated with population density. More importantly, noticeable geographical variations in these factors are observed according to the results from the GWR. In line with this result, additional findings from the decision tree model suggest that primary influential factors that affect the hypertension prevalence are indeed heterogeneous across regional groups. Taken as a whole, accounting for geographical variations of health conditions, health behaviors and other socioeconomic factors is very important when the regionally customized healthcare policy is implemented to mitigate the hypertension prevalence. In short, our study sheds light on possible ways to manage the chronic diseases for policy makers in the local government.

A Study on Variation and Application of Metabolic Syndrome Prevalence using Geographically Weighted Regression (지리적 가중 회귀를 이용한 대사증후군 유병률의 지역별 변이에 관한 연구 및 적용 방안)

  • In this study, regional variations and factors associated with prevalence of metabolic syndrome were grasped using GWR (geographically weighted regression) and methodologies for the efficient management of metabolic syndrome were then set up to resolve health inequalities. Based on the National Health Screening Statistical Yearbook published by the National Health Insurance Service (NHIS), community health survey (KCDC) and other governmental institutions, indicators of social structural and mediation factors related to the regional prevalence of metabolic syndrome were collected. First, the existence of indicators to measure variations in metabolic syndrome were confirmed with the collected data by calculating the EQ (extremal quotient) and CV (coefficient of variations). The GWR, which is able to take spatial variations into consideration, was then adopted to analyze the factors of regional variations in metabolic syndrome. The GWR analysis revealed that severity and management of the main causes need to be prioritized in accordance with the prevalence of metabolic syndrome. Consequently, the order of priority in management of regional prevalence of metabolic syndrome was established, and plans that can increase the effectiveness of management of metabolic syndrome were confirmed to be feasible.

Quantity and Quality of Seed Protein and Its Regional Variation in Barley (보리 단백질의 특성과 지역간 변이)

  • To breed good quality and high yielding varieties, and to evaluate quantity and quality of seed protein in barley, protein content of 139 cultivars, amino acid (AA) composition of 6 cultivars were analyzed. Total protein contents of cultivars tested showed variation from 12.0 to 15.7% in covered barley, 9.2 to 15.4% in hulless barley and 11.4 to 15.9% in malting barley. Varietal mean protein contents of covered barley were higher than any other barley. Barley grains harvested from northern area showed higher protein contents than those from southern area. Relationship between protein content and heading date showed negative correlation in covered barley and no significantly different in malting and hulless barley. Total amino acid (AA) showed variation from 9.51 to 11.47%, also essential amino acid (EAA) was 2.81 to 3.25%. Comparatively, percent of EAA was higher in hulless barley, but lower in covered barley. First limiting EAA was lysine, and other limiting EAAs were threonine and methionine in 6 barley cultivars tested.
