  • 치과보철학 분야에서 컴퓨터를 이용한 기술의 발달로 치료 계획, 수술, 보철물 수복에 이르기까지 CAD-CAM의 광범위한 적용이 가능해지고 있으며, 비용과 시간의 절약으로 효율적인 진료가 가능하다는 점을 인지한 많은 회사들의 투자로 다양한 프로그램과 기계들이 소개되고 있다. 그러나, 이렇게 활발한 제품 개발에도 불구하고 아직 실제 임상으로의 적용은 보편화되었다고 보기는 어려운데, 가장 큰 이유는 정확도에 대한 의심과 복잡하다고 느껴지는 초기의 숙련 과정에 있다고 하겠다. 제작회사들의 보고에 따르면 컴퓨터를 이용하여 디자인하고 절삭하는 것이 기존의 주조 기술보다 더 정교하다고 하였으나, 실제 임상에서는 미세한 오차를 간과한 데서 오는 최종 결과물과의 차이를 얘기하는 임상가들이 있는 것이 현실이다. 또, 그 사용 재료에 있어서도 초기 티타늄 금속 재료에 이어 심미적 요구를 위한 알루미나, 보다 향상된 강도를 가진 지르코니아에 이르기까지 다양한 재료들이 쏟아져 나오고 있다. 이에 본 논문에서는 다양한 CAD-CAM 시스템 중 대표적인 것 중의 하나인 Nobel Biocare사의 $Procera^{(R)}$ 시스템을 중심으로 지르코니아 수복물에 관한 최신 정보를 제공하며, 그 다양한 치과적 적용을 누구나 쉽고 정확하게 사용할 수 있는 임상 증례와 함께 소개함으로써 실제 임상 적용에 도움이 되고자 한다.

Full-mouth rehabilitation with implant-supported fixed dental prostheses for the edentulous maxilla and partially edentulous mandible: A case report (상악 완전 무치악 및 하악 부분 무치악 환자에서 임플란트 지지형 고정성 보철물을 이용한 전악 수복 증례 보고)

  • A conventional approach for the treatment of long-span edentulous areas is the use of removable dentures. However, placing implants in these areas results in superior functional outcomes by increasing the stability, support, and resistance of the prostheses and improving the masticatory efficiency. Treatment modalities utilizing implants can be further classified into either removable or fixed-type prostheses. Several factors such as the amount of alveolar bone resorption, inter-arch relationship, patient preferences, and socioeconomic status should be considered when determining the appropriate treatment approach. Monolithic zirconia has been considered a suitable material for implant-supported fixed dental prosthesis, because of the drastic improvement in its mechanical properties. It exhibits fewer incidences of fracture and chipping of the prostheses, and has greater bulk of material than metal-ceramic crowns and zirconia-veneered ceramics. Moreover, highly translucent monolithic zirconia is also available in the market, and its application is gradually increasing for anterior tooth rehabilitation. The present report describes a patient who underwent full-mouth rehabilitation with fixed dental prostheses (eight upper and three lower implant placements). All teeth, except bilateral mandibular canines and left mandibular first and second premolars, were extracted after the diagnosis of generalized chronic moderate-to-advanced periodontitis of the remaining teeth. The patient reported satisfactory esthetic and functional outcomes during the one-year follow-up visit.

  • With the evolution of the computer-aided design/computer-aided manufacturing (CAD/CAM) technology, the intraoral scanners are playing an increasingly important role, as they are the first step towards a completely digital workflow. The CAD/CAM double scanning technique has been used to transfer the information from provisional restorations to definitive restorations. In this case, a 67-year-old male with esthetically compromised anterior teeth, generalized severe attrition of teeth, and reduced vertical dimension was treated with full mouth rehabilitation including a re-establishment of the lost vertical dimension of occlusion assisted by the crown lengthening procedure. The provisional restorations were fabricated using an intraoral scanner and the CAD/CAM double scanning technique. After the period of adaption, the definitive monolithic zirconia restorations were delivered. The CAD/CAM double scanning technique successfully transferred the occlusal and morphological characteristics, obtained from the provisional restorations, to the definitive restorations.

  • For many years, ceramics have been used in fixed prosthodontics for achieving optimal esthetics. but, they have another use as well. Many studies today show ceramics can be used for biomaterials. In the beginning researchers made a start in the study of aluminium oxide and sapphire for biomaterial. The appearance of Zirconia began a new phase of research. Zirconia was introduced into implantology as an alternative to titanium, because of its white color, good mechanical properties and superior biocompatibility. But it is not easy to surface treatment in comparison with titanium. To overcome the limitation, interconnected porous bodies of zirconia were fabricated by sintering technique. And the technique of coating was developed. Therefore, some zirconia implants are currently available. It is thought that Research of biomaterials as a variety of puposes for the use of zirconia is looking very promising. The purpose of this paper reviews are to evaluation of zirconia as biomaterials.

Influence of varying cement types and abutment heights on pull-off force of zirconia restorations (시멘트의 종류 및 임플란트 지대주 높이가 지르코니아 수복물의 제거력에 미치는 영향)

  • Purpose: The purpose of this study is to evaluate Ti-base abutment's three different heights and three different cement types on the pull-off force of zirconia-based restorations. Materials and Methods: A total of 90 fixture lab analogs were embedded in auto polymerizing resin bloack. 90 Ti-base abutments heights of 3 mm, 5 mm, 7 mm were scanned and zirconia restoration were prepared from scanned files. Zirconia restoration were cemented with three different types of cements (temporary, semi-permanent, permanent) following manufacturer's instructions. All 90 specimens were placed and tested in a universal testing machine for pull-out testing. Retention was measured by recording the force at load drop. Statistical analysis was performed using Kruskal-Wallis test for detecting whether there are any statistical significance along cement types or abutment heights. After that, Mann-Whitney test was used for figuring out differences regarding abutment height and the comparison between 3 cements. Results: Temp bond showed significantly lower pull-off force compared to Fujicem regardless of any abutment height. However, there were significant differences between Cem-implant and Fujicem in abutment height of 3 mm and 7 mm, but there was no significant difference in 5 mm. Temp bond and Cem-implant had significant differences only in abutment height of 5 mm. Conclusion: Although Ti-base abutment height did not influenced zirconia restorations' retentiveness, cement types showed significant differences.

Full mouth rehabilitation with vertical increase in patient with severe tooth wear using monolithic zirconia prosthetic restoration (과도한 치아마모를 보이는 환자에서 수직교합고경의 증가를 동반한 단일구조 지르코니아 보철물 전악수복)

  • Gradual teeth wear is normal physiological progress, severe tooth wear can causes dysfunction, occlusal plane disharmony and esthetic problems. If additional space is needed for prosthetic restoration due to severe attrition, full mouth rehabilitation with increase of occlusal vertical dimension may be considered. In this case, 72 year old male patient with severe worn dentition, irregular gingival zenith and deep anterior bite was treated by full mouth rehabilitation for regaining the space for restoration, improving relationship and esthetic of anterior teeth. Provisional restoration obtained by systemic analysis, diagnosis and re-evaluation for a sufficient time was replicated to final restorations through double scanning technique using monolithic zirconia blocks. Satisfactory functional and esthetic outcomes were obtained.

Comparative Evaluation of the Survival Rates of Zirconia Crown and Stainless Steel Crown for Proximal Caries in Primary Molars: a Retrospective Study (유구치 인접면 우식증 수복 시 지르코니아 전장관과 기성금속관의 생존율 비교 평가: 후향적 연구)

  • Due to increasing demand for aesthetics, zirconia crowns have become a popular choice for treating primary molars. However, there is limited literature available comparing the survival rates of zirconia crowns with those of other restorative materials. The objective of this study was to compare the 36-month survival rates of zirconia crowns and stainless steel crowns for proximal caries, as well as to analyze failure types associated with each crown type. Electronic medical records and radiographs of 1,061 primary molars from 498 patients treated with 2 types of prefabricated crowns at Chosun University Dental Hospital and 2 private dental clinics between 2017 and 2019 were collected and analyzed. The survival rate of zirconia crowns was found to be lower compared to that of stainless steel crowns. Regarding the groups without pulp treatment, the survival rate of stainless steel crowns was significantly higher than that of zirconia crowns. However, in the groups that received pulp therapy, no significant difference in the survival rates was observed between the two preformed crowns. Notably, abnormal root resorption or periapical lesions were identified as the primary cause of restorative failure in stainless steel crowns, whereas loss of restoration was the predominant cause in zirconia crowns. This study holds valuable implications for clinicians when selecting preformed crowns for primary molars.

Flexural strength and reliability of highly translucent colored zirconia (고반투명 유색 지르코니아의 굽힘강도와 신뢰도에 대한 연구)

  • Purpose: The purpose of this study was to evaluate the flexural strength and reliability of highly translucent colored zirconia for all ceramic restoration. Materials and Methods: Bar-shaped specimens (25 × 4 × 2.5 mm) were prepared from highly translucent monolithic zirconia. Three experimental groups were set up according to color (shade A0, A1, and A3). For each group, 20 specimens were prepared. Flexural strength was determined using a 3-point flexural test and results were analyzed with one-way ANOVA test. Weibull statistical analysis provided 2 parameter estimates: Weibull modulus and characteristic strength. X-ray diffraction (XRD) analysis was performed. Results: There was statistically significant difference between uncolored (Shade A0) and colored (shade A1 and A3) (P < 0.05), but there was no difference between colored groups (P > 0.05). The uncolored group had higher reliability compared with colored study groups. On x-ray diffraction analysis of each group, typical peaks of tetragonal phase appeared in all groups. Conclusion: Within the limitations of this in vitro study, coloring highly translucent zirconia had significant effect on flexural strength and reliability. Therefore, clinicians should be careful when using highly translucent colored zirconia to prevent breakage of veneering ceramic and enhance aesthetics.

  • Although the finishing of ceramic restorations affects overall results of prosthetic procedures, the importance of finishing procedures has been underestimated. Finishing procedures have influence on the esthetics, and strength of ceramic restorations, and attrition of opposite tooth. For achieving the appropriate results of these factors, the overall understanding related to the finishing procedures of ceramic restoration is necessary.
