  • CPR is very important to paramedics, but the chest compression performed while on the move main stretcher is less accurate. The purpose of this study is to find out the difference between performing chest compression on the side of the main stretcher using C-step and on the patient's over the belly in order to increase the effect of CPR on the main object while on the move. As a result of the study, the appropriate depth (t=4.132, p=.000) and speed (t=7.177, p=.000) were shown in the group to which the C-step was applied, and the accuracy was higher (t=6.774, p=.000). In addition, it was found that there were few location defects (t=-5.197, p=.000) and too shallow errors (t=-2.948, p=.008) in the group to which the C-step was applied. In conclusion, mounting a C-step on the main stretcher seems to help improve the quality of chest compression, and it is thought that this will increase the efficiency of chest compression.

  • CPR with minimized hands-off time is one of integral factors for the improvement of the survival rate of patients. The purpose of this study was to examine the relationship of the installation of C-steps on the main stretcher to CPR accuracy and errors. The subjects in this study were 70 paramedics in 10 firehouses in the province of C(35 for control group, 35 for experimental group), and their CPR accuracy on the main stretchers was checked. As a result, it's found that the main stretchers equipped with the C-steps served to boost the accuracy of chest compression by creating suitable environments for that in terms of height and posture(t=65.104, p=.000), to improve the accuracy of artificial respiration by providing a proper amount of ventilation(t=5.207, p=.000), and to bolster the self-confidence of the paramedics about CPR(t=-10.612, p=.000). In conclusion, the mounting of C-steps on the main stretcher is expected to be of use for the improvement of the survival rate of cardiac-arrest patients by ensuring the precise performance of high-quality CPR.

  • The purpose of this study is to draw the improvement plan through the analysis of problems of main stretchers that are being used by the 119 EMS. In order to find out the problems, we used the literature review and analysis, survey questionnaire and we also made full use of KJ method (Kawakita Jiro method, affinity diagram), graph method, and priority matrix method to produce the improvement indicators. The problems of main stretchers are summarized as follows. they are being recognized as part of the emergency vehicle, they have the imperfection of performance verification criteria, and they cause the injuries of paramedics and patients accidents in operation. The indicators such as the ease of operation, the high performance, the multi-function, the driving performance, the durability and the lightweight, was produced to improve the problems. The results of the interconnection analysis and the applying priority matrix method on the indicators are the ease of operation ${\rightarrow}$ the multi-function ${\rightarrow}$ the driving performance ${\rightarrow}$ the high performance ${\rightarrow}$ the durability and the lightweight in order of importance.

  • This study identified the complaint ratio of musculoskeletal symptom by 119 EMTs and investigated the work risk extent through ergonomics evaluation about the patient transport works, which cause work-related musculoskeletal disorders (WMSDs) to 119 EMTs. For this, the complaint ratio of musculoskeletal symptom utilized questionnaire tool based on KOSHA Code H-30-2008 and the risk extent about the patient transport work evaluated by using ergonomics evaluation tools such as OWAS, RULA and REBA. According to the study result, 60.9% of 119 EMTs experienced musculoskeletal symptom. Among them, the symptom on back was the most common (36.1%). The work, which mostly causes WMSDs, has been found as patient transport work (48.4%). Among the patient transport motion, loading/unloading of ambulance cot to/from ambulance and the lifting of patient by stretcher were OWAS risk-level 3 and RULA/REBA risk-level 3 to 4. Among the patient transport environment, carrying patient on stairway using emergency mini-stretcher, moving patient in vehicle using spine board and piggy-back carrying or cradle carrying patient on stairway or slope way were OWAS, RULA, REBA risk level 3 to 4. It is suggested that immediate improvement in work postures for these works should contribute to prevention against WMSDs to 119 EMTs.

  • Purpose: This study aimed to compare the effect of chest compression and the resulting ventilation volume in walking cardiopulmonary resuscitation (CPR), straddling CPR, and mechanical CPR while moving manikins to main stretchers. Methods: We compared the chest compressions in terms of compression depth, number of incomplete releases, complete release depth, compression rate, duration between peak time of previous compression and peak time of current compression, and respiration. We analyzed the compression comparatively with the ventilation volume in three different types of CPR. Results: The chest compression depth was significantly improved during straddling CPR as compared to walking CPR, during which women were unable to achieve sufficient chest compression depth. A constant chest compression depth was maintained during mechanical CPR. Conclusion: High-quality chest compressions were difficult to achieve in moving spaces. Further, walking CPR may be helpful in men, but straddling or applying automatic chest compressions in women would result in more effective CPR. Our findings demonstrate the limitations and trends in administering CPR in men and women, which may be useful in devising better education and training methods in the future.

  • The purpose of this study is to perform the effectively advanced airway management in an emergency patient with cardiac arrest and trauma by comparing the appearance of influence in a change of visual-field height upon endotracheal intubation according to a change in position with intubation of paramedics in a moving ambulance and by analyzing the appearance of the influence in a patient's change in position with intubation upon speed in endotracheal intubation. Research subjects were randomly extracted 60(30 people for control group, 30 people for experimental group) people as the paramedics who are working at 13 fire stations in C Province. Data analysis was carried out ${\chi}^2$-test, independent t-test, paired t-test by using SPSS WIN 14.0 Version. As a result of research, to improve speed of the advanced airway management, it is considered to be likely effective in a patient's sniffing position and in the endotracheal intubation in the upper space of the main stretcher. The self-confidence in intubation after experiment increased significantly. Thus, the continuous education(training) on the intubation position and method within ambulance is considered to be likely needed for improving efficiency of the advanced airway management.

  • Purpose: Authors suggest a new alternative of preparedness capabilities for health care institutes to react to the new types of communicable diseases and the pandemic situations through the quality improvement and advancement of Negative Pressure Isolation Stretcher (NPIS). Methods: We employed FOCUS-PDCA, a representative quality improvement model. First, we identified and materialized necessary components of existing NPIS to be improved. Second, we fabricated an advanced NPIS prototype. Third, we carried out a performance test by the accreditation institute and an empirical evaluation at the communicable diseases trauma center, to locate the improvement pain points of the prototype. Finally, we draw a conclusion on quality improvement points to be considered during the production stage. Results: The suggested, NPIS outperforms the existing one in terms of communicable disease controls, patient treatments and the safety during the patient transfer, and the ease of use. Additionally, the suggested NPIS turns out to be a proper alternative to satisfy the requirements of long range transfer of patients with communicable disease. Conclusion: Authors developed a quality improved, NPIS prototype. There are several points that still remain to be improved. Additionally, we expect that the concurrent use of the suggested NPIS with the existing one will fortify the preparedness capabilities on reacting to the communicable diseases.

  • The purpose of this study was to investigate the treatment status and its related factors of the newly detected rural hypertensives through community health promotion program. A questionnaire survey and blood pressure measurement were performed to 6,977 residents of a rural area, and 282 hypertensives detected by blood pressure measurement were selected as subjects of the study. The study employed the health belief model as a hypothetical model. The major results of this study were as follows: The proportion of person experienced treatment among hypertensives was 12.0%. Treatment experience rate was significantly related with age and educational level(p<0.01). That is, if they were older, lower educational level, the treatment experience rate was higher. The major reasons of no treatment were 'they had not hypertensive symptoms ' (45.6%), 'their blood pressure was not high so much that they received treatment ' (43.2%). The chief facilities for treatment were public health institutions(57.9%) such as health center and health subcenter, and hospital/ clinics(29.8%). The treatment experience rate was higher when they had higher perceived severity for hypertension, lower perceived barrier to treatment, although statistically not significant. Treatment experience rate was significantly related with cues to action and health education experience(p<0.05). That is, if they had hypertension related symptoms such as headache previously, patients suffered from hypertension complication and health education experience for hypertension, the treatment experience rate was higher. In multiple logistic regression analysis for treatment experience, having a cerebrovascular patient in their acquaintance and the experience of health education for hypertension were significant variables. On consideration of above findings, it would to be essential to provide knowledge about hypertension and its treatment, and severity of hypertension complications through health education.
