  • In closed calcaneus fractures, the occurrence of FHL interposition in subtalar joint is relatively rare and difficult to diagnosis preoperatively. But careful physical examination and significant radiologic findings could be useful tools to diagnosis preoperatively. This findings is shearing off a superior medial fragment made up of the sustentaculum talus connected to a significant portion of the medial wall and medial aticular surface of the posterior facet.

  • Intraosseous lipoma is rarely reported in the literature, despite the fact that lipocytes are found as normal components of medullary bone. It is a primary bone tumor that originate from within the medullary cavity. The incidence of intraosseous lipoma has been reported as less than one per thousand bone tumor. The authors experienced a patient who had the findings of bilateral intraosseous lipoma of the calcaneus. We report a case of bilateral intraosseous lipoma of the calcaneus with brief review of literature.

  • The arthroscopic ankle arthrodesis is one of the good method of the ankle arthrodesis. Mechanical distraction of tibiotalar joint is an absolute necessity for adequate visualization and operative instrumentation. However, there are several disadvantages. Thus we performed arthroscopic ankle arthrodesis using calcaneal traction in three cases and achieved following results. 1. The procedure is easy and the operation time is shorter than the other arthroscopic ankle arthrodesis. 2. The risk of neurovascular damage and infection is reduced by using calcaneal traction. 3. It does not require the expensive distraction device. 4. The adequate visualization for operative instrumentation is obtained.

  • Calcaneus fracture with a subtalar dislocation are extremely rare. A case of a joint depression type calcaneus fracture with a lateral dislocation of the calcaneal posterior facet and tuberosity is presented. We treated it with open reduction and internal fixation with Steinmann pins and K-wires through limited posterior approach and obtained satisfactory radiographic and clinical outcome.

  • There may be complications after comminuted or intraarticular calcaneal fracture regardless of the initial treatment. Transcalcaneal talonavicular dislocation is rarely reported severe form of calcaneal fracture. We experienced a neglected transcalcaneal talonavicular subluxation case, who had been treated for intraarticular calcaneal fracture conservatively. Subtalar distraction bone block fusion was done for calcaneal malunion with talonavicular subluxation. Inspite of successful subtalar fusion, pain was persisted because of talonavicular re-subluxation with arthritis and calcaneocuboid arthritis. So, second operation, the talonavicular and calcaneocuboid fusion, was done. After union achieved, the patient's foot pain was improved. Calcaneal malunion combined with talonavicular subluxation and unstable transverse tarsal joint, such as this case, initial triple arthrodesis could be considered.

  • 편평족은 내측 세로궁이 소실되어 족저부가 편평하게 되는 변형의 총칭으로, 소아에서 흔하 고 보호자들의 관심이 많은 질환이다. 이학적 검사상 후족부의 외반과 전족부의 외전소견을 보이며, 감별질환을 위해 정확한 이학적 검사와 방사선학적 검사를 필요로 한다. 원인으로는 유연성 편평족, 종골 외반, 선천성 수직 거골, 부주상골, 족근골 결합등이 있으며 정상적 유 아의 경우 시간경과에 따라 저절로 호전되는 유연성 편평족이 대부분이다. 따라서 병적 편평족이 아닌 경우 편평족의 원인과 양호한 자연경과를 보호자에게 설명하는 것이 중요하다. 본 논문에서는 소아 편평족의 감별질환을 위한 각 질환에 있어서의 이학적 소견과 검사소견 그리고 이들의 치료에 대해 살펴 보고자 한다.

  • Osteogenic sarcoma is the most common primary malignant tumor of bone in which tumor cells form neoplastic osteoid or bone or both. Classic osteogenic sarcoma usually involves the metaphysis of the more rapidly growing long bones (distal femur, proximal tibia). Osteogenic sarcoma of the foot is rarely noted and only a few well documented cases have been reported. Osteogenic sarcoma of foot can clinically, radiographically, and histologically mimic several benign lesions and tumor-like lesions, so it sometimes leads to late diagnosis and delayed treatment. We experienced a case of primary osteogenic sarcoma on left calcaneus in 66-years-old female and report it with a review of references.

  • $10\sim15%$ of tuberculosis infections is extrapumonary. 10% of the extrapulmonary tuberculosis involves the musculoskeletal system. The spine is most frequently affected about 50%. Foot is infrequently sites, with the incidence in the literature varying from $3\sim7%$. Especially isolated calcaneal tuberculosis is very rare. The authors report one case of a 71-year-old female with calcaneal tuberculosis.

  • Open reduction and internal fixation is currently considered as a gold standard of treatment in most of the intra-articular calcaneal fractures. Among various different approaches, extensile lateral approach is the most popular since it provides good exposure to the subtalar joint. However, wide skin incision followed by extensive soft tissue dissection leading to increased risk of wound breakdown is the most serious drawback. Sinus tarsi approach, a minimal invasive technique to approach the subtalar joint and reduce the intra-articular calcaneal fractures, provides good clinical outcome and less wound complications compared to the extensile lateral approach. This article introduces the surgical technique and review of the literature regarding the sinus tarsi approach.

Use of Subtalar Arthroscopy in Intra-Articular Calcaneus Fractures (관절 내 종골 골절에서 거골하 관절경술의 이용)

  • Displaced intra-articular calcaneal fractures are difficult to treat because of complex anatomy and high soft tissue complications. Various surgical approaches have been introduced to solve these problems, but the treatment remains complex. Recently, clinically and radiographically superior results were reported using a subtalar arthroscopy in reducing the posterior facet in both percutaneous and open approaches. In the percutaneous approach, the arthroscopically assistant percutaneous approach must be selected carefully for mild-to-moderately displaced fractures because of the limited view. In the open approach, there is little evidence of the utility of subtalar arthroscopy. Therefore, intraoperative arthroscopy should always be used in conjunction with fluoroscopy to achieve reduction and assess the internal fixation placement.
