Arthroscopic Ankle Arthrodesis (관절경하 족근관절 고정술)

  • Purpose : This study was conducted to analyze the results of arthroscopic ankle arthrodesis and to verify the advantages of the technique compared to open ankle arthrodesis. Materials and Methods : Between October 1992 and August 1996, the arthroscopic ankle arthrodesis had been performed in five patients(six ankle joints): two patients with seropositive rheumatoid arthritis(one patient surgically treated bilaterally), two with osteoarthritis and one with tuberculous arthritis. There were one man and 4 women. Average age was 48 years ranging from 38 to 65 years. Follow up period was average 45 months(range, $12\~80$). Results : All patients were successfully treated with ankle joint arthrodesis under arthroscopic control. The mean time to fusion was 10 weeks(range, $6\~15$). There was a $100\%$ fusion rate without any complication. Conclusion : The arthroscopic ankle arthrodesis was successful in all cases with less morbidity and short hospital stay. It was technically feasible with excellent predictability.

Arthroscopic Treatment for Residual Pain after Ankle Fracture (족근관절 골절 치료후 발생한 동통에 대한 관절경의 이용)

  • Purpose : The diagnosis of the causes of residual pain after ankle fractures and the treatment is not simple. The authors analyzed the clinical results of the patients with residual pain after ankle fracture fur whom ankle arthroscopy was undertaken, for the purpose of evaluating the efficacy of arthroscopic diagnosis and treatment. Materials and Methods : From January 1997 to June 1998, ankle arthroscopy was done for the seventeen patients suffering from residual ankle pain and limitation of motion after ankle fracture. Their symptoms were not improved despite conservative treatment. There were sixteen men and one woman, and their mean age was 37.4 years. Result : Radiological examination revealed loose bodies in two patients, and osteophytes at the anterior rim of the tibia in fourteen patients. By the classification of osteoarthritic change, three ankles were assigned to grade 0, eight to grade I, six to grade II, and none to grade III. On arthroscopic examination, soft tissue impingement was found In thirteen cases, loose bodies in five cases, and osteochondral lesions in four cases. For arthroscopic treatments, removal of soft tissue, abrasion of osteophytes, cartilage shaving and removal of loose bodies were done. Clinical results were evaluated using Evanski and Waugh score. Preoperative overall score of 58.7 points jumped to 74.6 points after the operation, and in fourteen patients$(82\%)$ the symptoms were improved. Conclusion : Ankle arthroscopy is considered to be a very useful diagnostic and treatment method fer the patients with residual complaints after ankle fracture.

Arthroscopic Ankle Arthrodesis Using Three Cannulated Screws (3개의 유관 나사를 이용한 관절경적 족근 관절 고정술)

  • Purpose: We evaluated the clinical and radiographic results of arthroscopic ankle arthrodesis using 3 cannulated screws for the treatment of arthritis of ankle. Materials and Methods: From May 2006 to February 2009, 17 cases of arthritis of ankle were treated by ankle arthrodesis using 3 cannulated screws under arthroscopy. There were 8 male and 7 female and the average age was 62.2 years. We evaluated them clinically using AOFAS ankle-hindfoot functional scale, VAS pain score and patient’s satisfaction. For the radiographic evaluation, we checked them by simple AP, lateral and mortise view. The average follow-up period was 24.2 months. Results: The ankle-hindfoot functional scale was improved from an average of 47.4 points preoperatively to an average of 82.5 points at the last follow-up. The visual analogue scale pain score was decreased from an average of 8.6 to 2.4. Patient's satisfaction had favorable results with excellent in 7 cases(41.2%), good in 8 cases(47.0%), fair in 1 case(5.9%) and poor in1 case(5.9%). All ankles were successfully fused and the mean period of fusion was 9.1 weeks. Conclusion: Arthroscopic ankle arthrodesis using 3 cannulated screws was good modality of ankle arthrodesis with good clinical results and high union rate in the case of advanced ankle arthritis.

Results of Arthrodesis in Ankle Arthritis with Deformity (변형을 동반한 족근 관절 관절염에 대한 관절 고정술 후 결과)

  • Purpose: We reviewed the results of ankle arthritis with advanced deformity treated with open arthrodesis. Materials and Methods: Seventeen patients who had painful ankle arthritis with advanced deformity underwent open arthrodesis using chevron osteotomy or transfibular approach and were followed for an average of 4.2 years (range, 1.5 to 9.7 years). The average age was 51.4 years and the most common cause was traumatic arthritis (13 patients). Postoperative outcome was evaluated using Mazur's grading system for ankle function. we also checked time to union, patient satisfaction, complications, position of arthrodesis and degenerative changes of adjacent joints. Results: Clinical score was improved to 76.4 points from 46.7 points. Average time to union was 4.3 months and the rate of satisfaction was 88%. Complications included 1 nonunion, 2 malunion, 1 superficial infection and 1 combined delayed union and malunion. There were 14 cases within $5^{\circ}$ valgus in frontal plane and 13 cases within neutral to $5^{\circ}$ dorsiflexion in sagittal plane. 3 cases in chevron osteotomy revealed valgus and plantarflexed position over $5^{\circ}$. Degenerative changes of adjacent joints was seen in 2 patients. Conclusion: Open arthrodesis for ankle arthritis with advanced deformity shows favorable clinical outcome. Transfibular approach shows more consistent results than chevron soteotomy for desired position of arthrodesis.

The Effect of Pharmacopuncture with Polygonum cuspidatum Sieb et Zucc. Solution on Collagen-induced Arthritis in Wistar Rats (호장근 약침이 흰쥐의 콜라겐 유발 류마티스 관절염에 미치는 영향)

  • 호장근(Polygonum cuspidatum Sieb et Zucc.)은 관절통, 만성 기관지염, 황달, 월경분순, 고협압 등의 치료제로 사용되고 있는 약물로서, 본 연구에서는 흰쥐의 류마티스 관절염 병태모델에서 호장근 약침이 류마티스 관절염에 미치는 영향에 대해 관찰하였다. 방법 : 본 연구에서는 bovine type II collagen으로 유발된 흰쥐의 류마티스 관절염 병태모델에서 인체의 족삼리(ST36)에 상응하는 부위에 호장근 약침액을 주입한 후, 체중변화, 족부종의 변화, 족근관절폭의 변화, cytokine의 변화, NOS 발현 양상 등을 관찰하였다. 결과 : 족부종 감소율은 고농도 호장근 약침군에서 높았고, 족근관절폭 감소율은 고농도 및 저농도 호장근 약침군에서 대조군에 비하여 유의하게 높게 관찰되었다. 족부 삼출물 내의 TNF-${\alpha}$ 함량은 고농도 호장근 약침군에서 높았고, IL-$1{\beta}$ 함량은 고농도 및 저농도 호장근 약침군에서 대조군에 비하여 유의하게 높게 관찰되었다. 대뇌 피질에서 NOS 양성 신경세포수는 고농도 및 저농도 호장근 약침군에서 대조군에 비하여 유의하게 낮게 관찰되었다. 결론 : 이상의 결과에서 호장근 약침은 type II collagen으로 유발된 흰쥐의 류마티스 관절염 병태모델에서 염증 반응을 억제하는 효과가 있는 것으로 사려된다.
