목 적: 조기난소부전 환자의 10$\sim$20%에서 난소기능이 회복될 수 있다고 하며 간헐적으로 배란이 되고 심지어는 임신이 가능할 수 있다는 보고가 있다. 그러나 한국의 조기난소부전 여성의 임상적 특징과 배란가능성은 논의된 적이 없다. 이 연구의 목적은 한국인 조기난소부전 환자에서 자연 난포의 발생 빈도와 배란유도의 가능성을 평가하는 것이다. 연구방법: 특발성 조기난소부전으로 진단된 15명 여성의 전체 진료기록을 후향적으로 연구하였다. 환자별로 6번 이상의 초음파 검사를 시행하였고 36개월 이상의 기간 동안 추적관찰을 하였다. 또한 4명의 환자에서 6주기의 배란유도시도를 하였다. 조사항목은 초음파 검사에서 난포의 발견율, 자연 난포 성장률과 배란유도를 시행한 경우에 그 반응을 평가하여 결과를 분석하였다. 결 과: 15명 중 6명은 초음파상 난포가 발견되었나 (40%) 초음파 검사 당 난포 발견 빈도는 7%였다 (115회 중 8회). 6명 중 한 명은 초음파 추적관찰 상 배란이 확인되었고 나머지 5명은 크기에 변화가 없었다. 배란유도를 시도한 6명 중 성공적인 난포 성장이 관찰된 예는 없었다. 결 론: 한국인 조기난소부전 환자에서 난포의 발견율은 이전 서양 연구에 비하여 낮다고 생각되지만 관찰 방법에 차이가 있어 직접비교는 불가능하다. 배란유도의 성공율은 0%로서 이전 연구에서 발표된 수준보다 낮았다.

본 연구에서는 조기 난소 부전증 환자를 대상으로 난포 자극 호르몬 수용체 유전자의 돌연변이와 발현 양상을 분석하였다. 돌연변이 분석을 위해 환자의 말초혈액에서 genomic DNA를 분리하고 nucleotide 566을 포함하고 있는 exon 7에 특이적인 primer쌍을 이용하여 중합효소 연쇄 반응을 시행하였다. 전기 영동으로 반응산물을 확인한 다음, 돌연변이 여부를 조사하기 위하여 제한효소 절단분석 (Restriction Fragment Length Polymorphism, RFLP)을 시행한 결과, 대조군과 조기 난소 부전증 환자군 모두에서 돌연변이를 관찰할 수 없었다. 난포 자극 호르몬 수용체의 발현양상을 확인하기 위해 시험관아기 시술과정 중 난자 채취과정에서 얻어진 황체화 과립세포에서 total RNA를 추출하여 역전사 중합효소 연쇄 반응을 시행하였다. 반응 산물을 전기 영동하여 발현양상을 비교해 본 결과, 대조군에 비해 조기 난소 부전증 환자군에서 난포 자극 호르몬 수용체 유전자의 발현이 다소 낮은 것으로 확인되었다. 이상의 결과로 보아 조기 난소 부전증 환자에서 난포 자극 호르몬 수용체 유전자의 돌연변이는 발견할 수 없었으며 난포 자극 호르몬 수용체 유전자의 발현이 대조군에 비해 낮아 과배란 유도시에 생식소 자극 호르몬에 대해 저적응증을 보이며 난포형성과정에도 장애를 받는 것으로 사료된다.

본 증례에서는 조기난소기능부전 진단 후 여성 호르몬 대체요법을 받지 않고 에스트로겐 결핍증상과 속발성 희발월경의 양상을 보이는 환자를 적절한 변증론치(辨證論治)를 통하여 한약물 치료로 주기적인 월경의 양상과 호르몬 검사상 정상으로 개선시킬 수 있었다. 앞서의 고찰로 한약물 치료가 조기난소기능 부전에 있어 여성 호르몬 요법을 대체할 수 있는 새로운 방법이 될 수 있음을 암시해 준다. 향후 한의학계에서도 한약재의 식물성 에스트로겐으로서의 역할에 대하여 많은 연구가 필요할 것으로 사료되며 더 많은 증례의 축적으로 치료의 예후를 나타낼 수 있는 지표나 변증(辨證)에 대한 연구도 더 필요하리라 생각한다.

Objectives: The purpose of this paper is to report the effects of herbal medicine on spontaneous pregnancy with premature ovarian failure. Methods: The 28-year-old patient who was diagnosed premature ovarian failure was treated with herbal medicine (Hyangbujapalmul-tang-gagam), acupuncture and moxibustion. Results: After the treatment, the symptoms of premature ovarian failure such as hot flushes, sweats were improved. The patient recovered menstruation and achieved spontaneous pregnancy. Conclusions: This case shows that Korean medical treatment including herbal medicine has effect on treating infertile female with premature ovarian failure.

Objectives: The purpose of these cases is to report the effects of oriental medicine on two patients with premature ovarian failure. Methods: Two patients who had amenorrhea and hot flashes caused by premature ovarian failure were treated by oriental medicine such as herb medication. We estimated the effects of treatments by follow-up measurements of serum hormone level, menstrual cycle and symptoms. Besides, follicle and endometrium were checked by ultrasonography for academic research.Results After treatments, their symptoms of premature ovarian failure were improved and the menstrual cycle resumed. And the level of serum Follicle Stimulating Hormone decreased and the level of serum E₂ increased.Conclusions Two cases shows that oriental medicine has effects on patients with premature ovarian failure by follow-up measurements of serum hormone level, menstrual cycle and symptoms.

Objectives: To report the Pregnancy and delivery in Premature Ovarian Failure after Korean Traditional Medicine Treatment. Methods: The patients who diagnosed as premature ovarian failure was treated with herbal medicine (Daeyeongjeon-gamibang). Results: After the treatment, symptoms of premature ovarian failure such as vaginal dryness, hot flush were improved. The patient got regular menstruation cycle and she was pregnant during the treatment. Conclusions: The case report shows that Korean medical treatment can be an effective for infertile female with premature ovarian failure.
