Pattern of Ovulation in Korean Women with Premature Ovarian Failure (한국인 조기난소부전 여성의 배란양상)

  • Objective: Significant portion of premature ovarian failure patients ovulate intermittently and even can achieve pregnancy. The clinical features and reproductive potentials of Korean women with premature ovarian failure have never been elucidated. The purpose of this study is to assess ovarian follicular growth and ovulatory potential. Methods: Complete medical records of fifteen women with idiopathic premature ovarian failure were reviewed retrospectively. Subjects with more than 6 ultrasonographic scans and 36 months of follow-up period were included. Once the ovarian follicle was found by ultrasonography, the follicular growth was serially monitored. The result of ovulation induction of 6 cycles in 4 patients were also evaluated. Rate of positive ovarian follicle by ultrasound, rate of natural follicular growth and response of ovulation induction were assessed. Results: Among the 15 patients, 6 women (40%) had ovarian follicle in random ultrasonography. But the chance of positive follicle per ultrasonography scan was 7% (8 out of 115 scans). One out of 6 women eventually ovulated during the follow-up scan. In remaining 5 subjects, size of follicles remained unchanged. None of 6 ovulation induction cycles resulted successful ovulation. Conclusion: Incidence of positive ovarian follicle in Korean premature ovarian failure patients is lower than that of Western counterpart. This result, however, could not be directly compared because of the differences of study design. The ovulation rate by medical ovulation induction in this group is 0%, which is worse than the reported success rate of Western studies.

Analysis of Gene Mutation and Expression Level of Follicle Stimulating Hormone Receptor in Premature Ovarian Failure(POE) Patients (조기 난소 부전증(Premature Ovarian Failure, POF) 환자에서 난포 자극 호르몬 수용체 유전자 변이 및 발현 양상에 대한 분석)

  • This study was investigated to analyze the inactivating point mutation and expression level of follicle-stimulating hormone(FSH) receptor mRNA. In first experiment, we analyzed the point mutation. Peripheral blood was collected from each patient. To screen individuals for the C566T mutation, PCR was performed for exon 7 of the FSH receptor gene in 10 patients. No inactivating point mutation of FSH receptor gene was identified in women with premature ovarian failure. To analyze the expression level of FSH receptor, mRNA expressions were examined by RT-PCR method using specific primers for the FSH receptor. The amount of FSH receptor mRNA expressed in POF patients was lower than that in the control group. But it was not significantly different. These finding suggests that lower expression of FSH receptor in premature ovarian failure patients might be the cause of the low response to the gonadotropin during the hyperstimulation in IVF-ET cycles.

  • Purpose : Premature ovarian failure, characterized by amenorrhea or oligomenorrhea, estrogen defieciency and elevated gonadotropins in woman less than 40 years of age, affects approximately 1% of woman and is associated with autoimmune phenomena or genetic abnormalities, but mostly idiopathic. This disease is loss of ovarian function seems not to be always permenent. Method : We investigated herb medication was effective in patient of premature ovarian failure. Results: Signs and symtoms of estrogen deficiency were improved and mensturation occurred monthly and the elevated during treatment of herb medication. As a result of our case recovering of ovarian function, we hope more investigtions about treating premature ovarian failure by oriental medicine will be gained.

A Clinical Study on One Case of a Spontaneous Pregnancy with Premature Ovarian Failure (조기난소부전 진단 후 한방치료를 통해 자연임신에 성공한 환자 1례의 임상증례보고)

  • Objectives: The purpose of this paper is to report the effects of herbal medicine on spontaneous pregnancy with premature ovarian failure. Methods: The 28-year-old patient who was diagnosed premature ovarian failure was treated with herbal medicine (Hyangbujapalmul-tang-gagam), acupuncture and moxibustion. Results: After the treatment, the symptoms of premature ovarian failure such as hot flushes, sweats were improved. The patient recovered menstruation and achieved spontaneous pregnancy. Conclusions: This case shows that Korean medical treatment including herbal medicine has effect on treating infertile female with premature ovarian failure.

Two Cases of Oriental Medicine on Premature Ovarian Failure by Checking Ultrasonography and Serum Hormone Assay (초음파기기를 통한 장부형상 및 호르몬검사로 확인한 조기난소부전 환자 치험 2례)

  • Objectives: The purpose of these cases is to report the effects of oriental medicine on two patients with premature ovarian failure. Methods: Two patients who had amenorrhea and hot flashes caused by premature ovarian failure were treated by oriental medicine such as herb medication. We estimated the effects of treatments by follow-up measurements of serum hormone level, menstrual cycle and symptoms. Besides, follicle and endometrium were checked by ultrasonography for academic research.Results After treatments, their symptoms of premature ovarian failure were improved and the menstrual cycle resumed. And the level of serum Follicle Stimulating Hormone decreased and the level of serum E2 increased.Conclusions Two cases shows that oriental medicine has effects on patients with premature ovarian failure by follow-up measurements of serum hormone level, menstrual cycle and symptoms.

  • Objectives: To report the Pregnancy and delivery in Premature Ovarian Failure after Korean Traditional Medicine Treatment. Methods: The patients who diagnosed as premature ovarian failure was treated with herbal medicine (Daeyeongjeon-gamibang). Results: After the treatment, symptoms of premature ovarian failure such as vaginal dryness, hot flush were improved. The patient got regular menstruation cycle and she was pregnant during the treatment. Conclusions: The case report shows that Korean medical treatment can be an effective for infertile female with premature ovarian failure.
