A Study on the Digital Therapeutics Based Community Care for Rehabilitation in Dysarthria in the Post-COVID-19 Era (Post-COVID-19 시대 마비말장애 재활을 위한 디지털 치료제 기반의 커뮤니티케어 방안)

  • The purpose of this study was to explore ways a digital therapeutics-based community care plans for the rehabilitation in dysarthria in a situation where non-face-to-face services are expanding in the COVID-19 era. To this end, a qualitative study was conducted on experts working in hospitals, speech-language pathology centers, and social welfare centers, and as a result of the study, 3 topics, 9 sub-themes, and 18 content units were derived. Based on the analysis results, the digital therapeutics-based community care model consisted of 9 types: remote diagnosis, telepractice, rehabilitation training program, peer supporters, clinical support, communication, psychosocial intervention, and care plan services. This study will be able to provide basic data for health care & welfare services using digital therapeutics and guidelines for establishing shared care plans based on multidisciplinary cooperation.

Type I Thyroplasty Using Prefabricated Hydroxylapatite Implant(VoCoM$^{\circledR}$) (미리 제작된 Hydroxylapatite 보형물을(VoCoM$^{\circledR}$) 이용한 제 1 형 갑상성형술)

  • Background and Objectives : $VoCoM^{\circledR}$ is a commercialized set composed of prefabricated hydroxylapatite implants and shims of various sizes which are specially designed for the type I thyroplasty. Even though a previously published preliminary report showed that $VoCoM^{\circledR}$ is a convenient and safe product for the type I thyroplasty, further investigations or experiences are yet to be reported. Authors aimed to evaluate the efficacy of $VoCoM^{\circledR}$type I thyroplasty, and its advantage and/or disadvantage. Materials and Method : Twenty three consecutive patients with unilateral vocal cord palsy enrolled for the study, who received type I thyroplasty with $VoCoM^{\circledR}$ between July 2001 and June 2003. Acoustic, aerodynamic and stoboscopic analyses were performed prior to surgery and 1 to 3 months after surgery. Speech language pathologists evaluated their voice quality by GRBAS scale, and patients themselves reported subjective changes of their voice by visual analog scale. Results : The average time for the operation was 80 min, which is about 30 min less than other methods. Preoperative jitter was 3.25$\pm$1.65% and improved to 1.94$\pm$1.79% postoperatively (p<0.05). Preoperative shimmer was 9.72$\pm$6.56% and improved to 5.61 $\pm$3.76% (p<0.05), Maximal phonation time increased from 4.41$\pm$2.99 to 7.98$\pm$4.35 sec (p<0.05) The postoperative stroboscopy revealed an effective medialization in 91.3% of the patients. The subjective phonetic improvements were reported in 21 out of 23 patients. GRBAS scale improved from 2.71$\pm$0.46 to 1.47$\pm$1.12(p<0.05). Additional medialization with $Gore-Tex^{\circledR}$ was easily performed in two revision cases. Previously inserted $VoCoM^{\circledR}$ implant was hard to remove because of the tight integration of soft tissues around the implant. Side effects such as extrusion or foreign body reaction are not observed. $VoCoM^{\circledR}$ was relatively expensive and costed more than 10 times of $Gore-Tex^{\circledR}$. Conclusion : Prefabricated hydroxylapatitie implant($VoCoM^{\circledR}$) provides a convenient, safe and efficient way of vocal fold medialization. However it is relatively expensive and hard to remove.

A Study on the Role of Models and Reformulations in L2 Learners' Noticing and Their English Writing (제2 언어학습자의 주목 및 영어 글쓰기에 대한 모델글과 재구성글의 역할에 관한 연구)

  • This study aimed to explore the role of models and reformulations as feedback to English writing in L2 learners' noticing and their writing. 92 participants were placed into three groups; a models group (MG), a reformulations group (RG), a control group (CG), involved in a three-stage writing task. In stage 1, they were asked to perform a 1st draft of writing, while taking notes on the problems they experienced. In stage 2, the MG was asked to compare their writing with a model text and the RG with a reformulated version of it. They were instructed to write down whatever they noticed in their comparison. The CG was asked to just read their writing. In stage 3, all the participants attempted subsequent revisions. The results indicated that all the participants noticed problematic linguistic features the most in a lexical category, and models and reformulations led to higher rate of noticing the problematic linguistic features reported in stage 1 and contributed to subsequent revisions. It was also revealed that the MG and RG significantly improved with their writings of MG and RG on the post-writing test. The findings imply that models and reformulations result in better performance in L2 writing and should be promoted in an English writing class.

The First Formant Characteristics in Vocalize of One Soprano (소프라노 1인의 모음곡 발성 시 제 1 포먼트의 변화양상)

  • Background and Objectives : Vowels are characterized on the basis of formant patterns. The first formant(F1) is determined by high-low placement of the tongue, and the second formant (F2) by front-back placement of the tongue. The fundamental frequency(F0) of a soprano often exceed the normal frequency of the first formant. And the vocal intensity is boosted when F0 is high and a harmonic coincides with a formant. This is called a formant tuning. Experienced singers thus learned how to tune their formants over a resonable range by lowering the tongue to maximize their vocal intensity. So, the current study aimed to identify the formant tuning in one experienced soprano by comparing the first formants of vowel [i] in three different voice production : speech, ascending scale, and vocalize. Materials and Method : All voices recordings of vowel [i] in speech, ascending scale (from F4 note to A4 note), and vocalize(:Ridente la calam") were made with digital audio tape-corder in a sound treated room. And the captured data were analyzed by the long term average(LTA) power spectrum using the FFT algorithm of the Computerized Speech Lab(CSL, Kay elementrics, Model, 4300B). Results : Although the first formant of vowel [i] in speech was 238Hz, those of ascending scale [i] were 377Hz, 405Hz, 453Hz respectively in F4(349z), G4(392Hz), A4(440Hz) note, and 722Hz, 820Hz, 918Hz respectively in F5 (698Hz), G5(784Hz), A5(880Hz) note. In vocalize, first formants of [i] were 380Hz, 398Hz, 453Hz respectively in F4, G4, A4 note, and 720Hz, 821Hz, 890Hz respectively in F5, G5, A5 note. Conclusion : These results showed that the first formant of ascending scale and vocalize sustained higher frequency than fundamental frequency in high pitch. This finding implicates that the formant tuning of vowel [i] in ascending scale was also noted in vocalize.

Materials and Method: Twenty three patients with unilateral vocal cord palsy were included in the study, who received type I thyroplasty with $VoCoM^{(R)}$ between May 2000 and May 2007 in St. Mary's Hospital, The Catholic University of Korea, Seoul, Korea, Acoustic, aerodynamic and stroboscopic analyses were performed pre-and post-operatively, Subjective voice improvement was analysed by Voice handicap index, Results : Preoperative jitter was $4.68{\pm}2.46%$ and improved to $3.19{\pm}1.94%$(P<0,05), Preoperative NHR was $0.26{\pm}0.1$ and improved to $0.18{\pm}0,07$(P<0.05), Preoperative MPT was $6.16{\pm}4.9$secs improved to $9.55{\pm}4.67$secs(p<0.05), The postoperative stroboscopy revealed an effective medialization of vocal fold of all patients, Conclusion: Type I thyroplasty using $VoCoM^{(R)}$ is a efficient, safe and convenient way of vocal fold medialization at the expense of its high cost and difficulty in removal.

Analysis for Risk Factors and Effect of Vocal Hygiene Education in Patients of Vocal Polyp (성대 용종의 예후 인자와 음성 위생법 치료 효과 분석)

  • Background and Objectives Vocal polyp is one of the most common benign diseases of vocal fold caused by overuse of voice. Laryngeal microsurgery is the first treatment of choice for vocal polyp. However, surgery has many risks such as side effects of general anesthesia, injury of tooth and psychological burden. And we often experience reduction of vocal polyps without surgical procedure. The purpose of study is to evaluate the effect of non-surgical treatment such as vocal hygiene education and proton pump inhibitor (PPI) in patients with vocal polyp. Materials and Method We performed retrospective study for seventy-three patients of vocal polyp who treated with non-surgical modalities such as vocal hygiene education and PPI over three months. Treatment outcomes and risk factors such as age, sex, polyp size, position, symptom duration, presence of laryngopharyngeal reflux (LPR) symptoms, smoking history, voice abuse history and vocal hygiene education were evaluated by comparison between polyp size improved group and non-improved group. Results 5.5% of enrolled patients showed complete response and 23.3% showed partial response without surgery. Polyp size improved group significantly carried out more practice of vocal hygiene education treatment than the non-improved group (p=0.040). And the presence of LPR symptoms [hazard ratio (HR) 3.368, confidence interval (CI) 1.055-10.754, p=0.040] and not performing of vocal hygiene education (HR 3.664, 95% CI 1.078-12.468, p=0.038). Conclusion Vocal hygiene education can be a useful treatment option when making a decision to treat with vocal polyp.

Design of Sound Synthesis System using Audio Compression Method (오디오 압축 방식을 적용한 사운드 합성 시스템의 설계)

  • 현재 상용화된 사운드 합성 기기에서 널리 쓰이고 있는 PCM 방식에서의 문제점은 고음질의 음을 얻기 위해서 많은 메모리 용량을 필요로 하는 것이다. 이 논문에서는 이 문 제를 해결하기 위해 MPEG 오디오 압축 방식을 적용하여 샘플된 음을 압축하고, 실시간으 로 이를 복호화 해서 음을 합성해내는 사운드 합성 시스템을 설계하였다. 사운드 합성 시스 템은 마이크로프로세서, 음원 DSP, MPEG 오디오 복호화기로 구성되며, 44.1Khz의 샘플링 주파수로 32개의 음을 동시에 합성할 수 있도록 설계되었다. 설계 과정에서 각각의 기능 요 소를 C언어로 기술하여 사운드 합성 시스템에 대한 소프트웨어 모델을 작성하였다. 이것을 통해 미리 전체 시스템의 동작을 시뮬레이션하고, 압축 방식을 적용함으로써 발생될 수 있 는 여러 가지 문제점에 대한 해결 방안을 제시하였다. 시뮬레이터로 시스템의 동작을 검증 한 후, DSP와 MPEG 복호화기를 포함하는 사운드 합성 시스템을 VHDL로 설계하여 시뮬 레이션을 통해 하드웨어가 정상적으로 동작함을 확인하였다. MPEG 오디오 압축 방식을 이 용함으로써 메모리 용량 측면에서는 약8:1의 감소 효과를 얻을 수 있다.

The Applicability of Schema Theory to Scientific Texts

  • The primary purpose of this study is to investigate the applicability of content and formal schemata for processing the scientific texts which encompass the human knowledge of the physical world. In general, schema theory is based on the culture-oriented background of a text. From this point of view, the problem as to whether both content and formal schemata are applicable to the comprehension of a scientific text deserves a focal attention in terms of information processing modes. The results of empirical study indicate that whereas the universality of general knowledge content about the natural world attenuates the tenets of schema theory, the rhetorical organization of scientific texts encourages the application of the schema-based approach; the reader's familiarity with the structural patterns of a text facilitates his reading comprehension.

  • 본 논문에서는 효율적인 CIM 구축을 위해 필요로 하는 표준항목과, 이에 관련된 국제표준동향, 국내의 표준화 여건에 대해 요약하고, 설문조사를 통한 국내전문가 의견 수렴결과를 취합하였다. 설문의 조사내용은 CIM이 아직 보편화도지 않은 상항을 고려하여 FA기기 및 요소기술을 중심으로 작성하였으나, 설문결과의 분석은 시스템통합이란 관점에서 분석하여 국가적으로 시급히 추진해야 할 표준항목을 도출하였으며, 국내 여건을 고려한 표준화 추진방안을 고찰하였다.

Perceptions of social presence and aggressive behavior in cyberspace (사이버 공간에서 사회적 실재감의 지각과 공격행동)

  • This study investigated the effects of social presence as means of self-awareness and interpersonal-awareness on the cyber-aggressive behavior. The cyber-aggressive behavior (flaming and direct aggression use of character) should be differ from existence of social presence and type of social presence of internet users. To test hypothesis, an experiment was executed a field study on cyberspace, an on-line game, "fortress 2 blue forever". I made a chat-room in the game site to conduct an experiment to 107 person who entered the chat-room and blinded ignorance of this situation made by researchers. As the subjects enter the chat-room, he chats with 3 researchers who were waiting before he gets in. The social presence was operated with 3 phases by the contents of the chat (①control group; nothing, ②experimental group 1; reaction about other people ③experimental group 2; reaction about other people + self-exposure by an exchange information of their home region). The studies show that, subjects of the control group behaved more aggressively than other subjects of the experimental groups(both flaming and direct aggression use of character). Meantime, I compared experimental group 1 with experimental group 2 to investigate difference between the type of social presence. As the result, subjects of the experimental group 1 behaved more aggressively than experimental group 2 (only flaming, there's no difference in direct aggression use of character).
