Development of an Automatic External Biphasic Defibrillator System (Biphasic 자동형 제세동기 시스템 개발)

  • In this paper, an automatic external biphasic defibrillator that removes ventricular fibrillation efficiently with a low discharging energy has been developed. The system is composed of software including a fibrillation detection algorithm and a system control algorithm, and hardware including a high voltage charging/discharging part and a signal processing part. The stability of the developed system has been confirmed through continuous charging/discharging test of 160 times and the detection capability of the real-time fibrillation detection algorithm has been estimated by applying a total of 30 various fibrillation signals. In order to verify the clinical efficiency and safety, the system has been applied to five pigs before and after fibrillation inductions. Also, we have investigated the system efficiency in removing fibrillation by applying two different discharging waveforms, which have the same energy but different voltage levels.

Factors Influencing Nurses' Attitudes towards Nurse-led Defibrillation (간호사에 의한 제세동 시행에 대한 간호사 태도의 영향요인)

  • Purpose : This study examined factors influencing the attitudes towards nurse-led defibrillation in the emergency department (ED) and intensive care unit (ICU). Methods : This was a cross-sectional study. A total of 212 nurses in the ED and ICU from three general hospitals responded to the survey. Data were collected between September and October 2015 using structured questionnaires. Independent t-test, Mann-Whitney U test, one-way analysis of variance (ANOVA), Pearson's correlation coefficient, and hierarchical multiple regression were used to analyze factors affecting nurses' attitudes towards nurse-led defibrillation. Results : The mean score of nurses' attitudes towards nurse-led defibrillation was 61.37 of a possible 91. Hierarchical multiple regression showed that nurses' attitudes towards nurse-led defibrillation were significantly affected by their experiences regarding cardiopulmonary resuscitation (p=.003), knowledge regarding electrocardiogram interpretation and defibrillation (p=.007), and tenure in ED or ICU (p=.043). Conclusion : Results indicate that employers should set policies to support nurses in maintaining their careers in the ED or ICU and conduct systematic educational programs for them. This will increase nurses' confidence in performing defibrillation, which will lead to early defibrillation during in-hospital cardiac arrest. Consequently, it can be a strategy to increase the survival rate of cardiac arrest patients.

Knowledge, Attitude and Performance Ability of Automated External Defibrillator and Cardiopulmonary Resuscitation among Korean University Students (비보건계열 대학생의 자동제세동기와 심폐소생술에 대한 지식, 태도 및 수행능력)

  • This study examined the level of knowledge, attitude, and performance ability of automated external defibrillator (AED) and cardiopulmonary resuscitation (CPR) among university students who were not in health-related majors and to explore the relationships among these variables. In this study, 291 students were recruited from 3 universities in D city. Among them, 77.0% had ever seen or heard of AED and 61.9% did not know how to use it. The levels of knowledge, attitudes and performance ability differed significantly according to the CPR training experience and AED awareness (p<.002~.001). The performance ability showed significant correlations with knowledge (r=. 42, p<.001) and attitude (r=. 55, p<.001) of AED and CPR. These findings suggest that future AED and CPR training programs should be developed to promote a positive attitude towards the willingness to perform AED and CPR as well as to increase the participants' confidence by repeated training.

Optimal Value Detection of Irregular RR Interval for Atrial Fibrillation Classification based on Linear Analysis (선형분석 기반의 심방세동 분류를 위한 불규칙 RR 간격의 최적값 검출)

  • Several algorithms have been developed to detect AFIB(Atrial Fibrillation) which either rely on the linear and frequency analysis. But they are more complex than time time domain algorithm and difficult to get the consistent rule of irregular RR interval rhythm. In this study, we propose algorithm for optimal value detection of irregular RR interval for AFIB classification based on linear analysis. For this purpose, we detected R wave, RR interval, from noise-free ECG signal through the preprocessing process and subtractive operation method. Also, we set scope for segment length and detected optimal value and then classified AFIB in realtime through liniar analysis such as absolute deviation and absolute difference. The performance of proposed algorithm for AFIB classification is evaluated by using MIT-BIH arrhythmia and AFIB database. The optimal value indicate ${\alpha}=0.75$, ${\beta}=1.4$, ${\gamma}=300ms$ in AFIB classification.
