Optimum Parameter Design for Defibrillator (제세동기 최적 파라미터 설계)

  • In designing defibrillator, several parameters such as patient's transthoracic impedance, output energy level, peak current, and time duration of current waveform must be considered to generate optimum electrical shocks on the heart. Patient's transthoracic impendence depends on the physical and health condition of patient. In this study, before the development of a defibrillator, the range of above parameters value as circuit elements was determined to derive optimal waveform by predicting and analyzing the performance of designed circuit by means of simulation with the software, P-Spice. The efficiency of parameter design was verified through the performance test with the developed defibrillator.

고속철도에서의 응급의료체계 - 고속철도 역사와 차량내의 응급처치 장비 배치의 예를 중심으로

  • 국내에서 이용객수가 많은 6개 고속철도 역사와 운행 중인 고속열차(KTX)에 심장마비 환자에 대한 응급처치 장비로서 자동제세동기가 2009년에 설치되었다. 이와 관련하여 6개 역사의 역무원 및 고속열차 승무원과 일반대중에게 심폐소생술 및 자동제세동기 사용법과 일반대중 제세동기 사용허용을 교육하는 고속철도 자동제세동기 보급 및 교육사업도 시행되었다. 이를 통해서 국내 응급의료체계가 한 단계 발전하는 계기가 되었으며 국내 고속철도가 심장마비 환자의 응급 대처 능력을 갖추게 되어, 역사 및 고속열차 안에서 심장마비 환자가 발생하더라도 신속한 대응을 할 수 있을 것으로 기대할 수 있겠다. 본 연구는 이러한 역사 및 고속열차 내에서의 응급의료체계 향상을 위한 노력을 자동제세동기 배치를 통하여 알아보며, 이에 따라 다양한 교통편 및 다중이용시설의 사고, 재난 및 응급 대응이 현재까지 해 오던 건축물 안전, 법규 마련 뿐 만 아닌 인명 피해 감소를 위한 응급의료체계라는 매우 중요한 부분이 있음을 이해하고 이에 대한 시스템 개발 및 대처도 안전과 방재 측면에서 같이 다루어져야 함을 본 연구 내용은 강조하고 있다.

  • 본 논문에서는 개발된 biphasic 자동형 제세동기 시스템의 임상적 효용성을 동물실험을 통하여 검증하였다. 개발된 제세동기 시스템은 기존의 monophasic 제세동기와는 달리 낮은 에너지로 세동을 제거하는 biphasic 형으로, 이미 실험실 환경의 테스트를 통해 시스템의 안정성과 알고리즘의 탁월한 검출 능력이 확인되었다. 시스템의 임상적 안전성 및 효용성을 확인하기 위하여 8마리의 돼지를 사용하여 시스템의 적절한 세동 검출 및 세동제거능력을 실험하였고, 그 후 시스템의 효용성을 향상시키기 위한 연구로 같은 양의 에너지를 다른 전압 레벨에서 방전시켜 이에 따른 제 세동 효율을 조사하고, 또한 세동 신호의 규칙성과 방전시점에 따른 제 세동 효과를 평가하였다.

  • Virtual reality technology was initially developed for entertainment purposes such as games, movies, sports, and theme parks, but gradually expanded to a number of industries such as education, e-commerce, and health care,. Therefore, safety education content incorporating VR is also rising rapidly. However, the program activity on safety education in our country is still insignificant. Although rescue of patients with acute cardiac arrest using defibrillators is one of the most representative safety education, in 2014, defibrillators are rarely used in Korea to save the lives of patients and there are very few cases of using defibrillators. The use of automatic defibrillators and the importance of knowing how to use them for expanding accessibility are necessary. However, it is not easy to provide experience education only after completing the safety education in simple theoretical education. In this paper, we propose a automatic defibrillator training simulation system using virtual reality that can be freelu trained at any time.

Comparison of Educational Effectiveness Between Instructor-Centered Learning and Video Self-Instruction about Automatic External Defibrillator for High School Girls (여고생의 자동제세동기에 대한 강사중심학습과 영상자가학습과의 교육효과 비교)

  • This study aims to make a comparison of educational effectiveness between instructor-centered learning and video self-instruction(VSI) about automatic external defibrillator(AED) for high school girls in order to help select an effective teaching method concerning AED. A total of 59 high school girls (29 for the experimental group, 30 for the control group) in J Province participated in this study from July 7 to 14, 2010. The analysis was carried out by using an SPSS WIN 12.0 Version program. After the AED instruction, the knowledge increased more in the experimental group (1.72) than that in the control group (1.60); self-confidence increased more in the experimental group (1.58) than that in the control group (1.46); and accuracy of technical performance increased more in the control group (5.46) than that in the experimental group (4.20). The experimental group (5.68) was more satisfied with AED instruction than the control group (4.95). Since video self-instruction proved to be more educationally effective than instructor-centered learning in most areas, it is necessary to apply video self-instruction to AED instruction in the future. However, further researches are necessary to see if video self-instruction is effective.

  • Virtual training is a kind of training that proceeds as if it were a real situation. In recent years, there has been a growing demand for experiencing a situation in which a virtual reality technology has not been experienced directly in the real world due to the rapid development of the technology. Especially, safety education is very necessary in Korea where safety accidents are caused by many disasters. Therefore, simulation of disaster response training using virtual reality is more urgent than ever. Although the automatic defibrillator is the medical device that is most needed to rescue patients with cardiac arrest, few people know how to use it. Therefore, there are very few cases where the use of automatic defibrillators has saved the patient's life in Korea. The proposed Automated External Defibrillator virtual training system enables immersive and experiential training in real situations and effective training at low cost.

A Study on Awareness of Automated External Defibrillator, Usage Intention and Related Educational Experience in Workers at Public Institutions (공공기관 종사자들의 자동제세동기에 대한 인식과 시행의도 및 교육경험에 관한 연구)

  • This study was conducted to examine the awareness of workers in public institutions of automated external defibrillators, their usage intention and related educational experience in an effort to increase the efficiency of education and the rate of automated external defibrillator usage in emergency situations. The subjects in this study were 243 selected. male and female adults who worked in public institutions in Gangwon Province. A survey was conducted from June 13 to August 21, 2016. The statistical analysis was based on the frequency analysis and the $X^2$-test. There were significant differences among the public institution workers awareness of how to use an automated external defibrillator according to gender (${\chi}^2=6.32$, p<0.05) and academic credentials (${\chi}^2=5.91$, p<0.05). Additionally, there were significant differences in usage intention according to academic background (${\chi}^2=5.70$, p<0.05). The most common reason for having no intention to use an automated external defibrillator for patients with cardiac arrest was that the respondents did not know how to use it (31.5%). There were also significant differences in educational experience with automated external defibrillators according to age (${\chi}^2=6.15$, p<0.05), academic credentials (${\chi}^2=11.56$, p<0.01) and career (${\chi}^2=11.92$, p<0.01). The most common form of education that the workers received pertaining to automated external defibrillators was a combination of theoretical and practical education, which represented 55.9%. The findings of the study seem to suggest that workers in public institutions used by many people must receive compulsory standard education regarding automated external defibrillators and try to boost the revival rate of patients with cardiac arrest in real emergency situations.
