재발성 혈관절증은 인공 슬관절 치환술의 합병증 중 상당히 드문 합병증으로 본 연구에서는 인공 슬관절 치환술 시행 후 발생한 혈관절증이 수차례 재발하여 관절경 수술을 받았음에도 증상이 지속된 환자를 동맥색전술을 시행하여 치료한 증례를 보고하고자 한다.

슬관절 관절경 수술 후 발생한 화농성 관절염의 발생률은 대략적으로 0.04%-3.4%로 보고되고 있다. 관절경하 반월연골판 봉합술 후 발생한 화농성 관절염은 더욱 드물어 해외에서는 몇몇 사례가 보고되어 있으나, 국내에서는 아직까지 보고된 바 없다. 저자들은 관절경적 반월연골판 봉합술을 시행한 환자에서 발생한 수술 후 화농성 관절염 2예를 경험하였으며, 이를 문헌고찰과 함께 보고하고자 한다.

Purpose: To evaluate the efficiency of the electronic foot function index (eFFI) through a prospective, random based, multi-institutional study. Materials and Methods: The study included 227 patients ranging in age from 20 to 79 years, visited for surgery in different 15 institutes, and agreed to volunteer. The patients were assigned randomly into a paper-based evaluated group (n=113) and tablet-based evaluated group (n=114). The evaluation was done on the day of hospital admission and the method was changed on the second day of surgery and re-evaluated. PADAS 2.0 (https://www.proscore.kr) was used as an electronic evaluation program. Results: There were no differences in age and sex in both groups. The intraclass correlation coefficient (ICC) evaluation revealed an eFFI ICC of 0.924, showing that both results were similar. The evaluation time was shorter in the tablet-based group than the paper-based group (paper vs tablet, $3.7{\pm}3.8$ vs $2.3{\pm}1.3minutes$). Thirty-nine patients (17.2%) preferred to use paper and 131 patients (57.7%) preferred the tablet. Fifty-seven patients (25.1%) found both ways to be acceptable. Conclusion: eFFI through tablet devices appears to be more constant than the paper-based program. In addition, it required a shorter amount of time and the patients tended to prefer the tablet-based program. Overall, tablet and cloud system can be beneficial to a clinical study.

최근 학문간의 장벽이 무너지고 융합학문이 큰 이슈가 되면서 다양한 결합이 시도되고 있다. 의학분야중 특히 정형외과 분야는 기계공학의 고체 및 바이오공학과의 융합연구로 매우 매력도가 높은 분야이다. 이 글을 통해 정형외과 분야 중 슬관절 분야가 기계공학도에게 왜 흥미로울 수밖에 없는가에 대해서 몇 가지 내용들을 소개하고자 한다.

This paper reports a rare case of the symptomatic third metatarsal (MT3) - lateral cuneiform (LC) in a 55-year-old male who presented with complaints of severe intermittent pain in his right foot. Plain radiographs and computed tomography scans revealed sclerosis and irregularity at this joint. The intraoperative findings demonstrated a fibrocartilaginous coalition. The pain had improved one year after removing the MT3-LC joint by en bloc and arthrodesis.

Lipoma arborescens or synovial lipomatosis is a rare disorder that is characterized by mature fat infiltration of the hypertrophic synovial villi, most frequently affecting the supra-patellar pouch of the knee. This paper presents a case of lipoma arborescens of the ankle joint bilaterally in an adult patient with involvement of both the intra-articular synovium and the synovial sheath of the tendons around the ankle.

신경초종은 Schwann 세포에서 기원하는 양성 종양이다. 총상 신경초종은 총상형이나 다결정성의 성장 형태를 보이는 신경초종의 드문 아형이다. 총상 신경초종은 대부분은 피부와 피하조직에 고립성 병변(solitary lesion)을 보이지만 드물게 심부조직 내에 위치하기도 한다. 저자들은 매우 드물게 후경골 신경 및 원위부 분지 신경에서 발생하였으며, 심부조직에 위치하여 골 변형을 동반한 다발성 총상 신경초종이 재발한 1예를 경험하였기에 이를 보고하고자 한다.

주로 고에너지 손상이나 관통상에 의해서 발생하는 가성동맥류는 진단이 지연되면 심각한 결과를 초래할 수 있다. 저자들은 계단에서 넘어져 다친 후 처음에는 단순 타박 및 피부 열상으로 진단되었다가 증상이 악화되어 수상 2주일 후 시행한 자기공명영상 검사와 컴퓨터 단층촬영 혈관 조영술에서 경골 동맥의 경골비골동체의 가성동맥류로 뒤늦게 진단된 63세 남자를 치험하였다. 경한 둔상에 의하여 경골비골동체에 가성동맥류가 발생한 예는 아직까지 국내에 보고된 바 없어 이에 저자들은 문헌고찰과 함께 보고하고자 한다.
