A Study on The Disturbed's Healing Games of Green Contents Concept (그린콘텐츠 개념을 적용한 정신장애 치료게임 연구)

  • Functional Games, which have developed in Korea until now, is only for the healthy people's education. There are less functional games for neglected people, specially for the disturbed's healing. I define 'Green Contents' as contents which is helpful to both of nature and human, especially to the week, and also they are not developed only for the commercial aim. Functional Games for the disturbed's healing should be developed as' Green Contents.' If functional games for the disturbed's healing will be developed as aspects of technology and contents development, they will have more effects to people, and also people will need them more. For this study, I analyze art therapy, which is a normal treatment to give a social nature and self respect to the disturbed. Also I developed a functional game program for the disturbed's healing as based on psychoanalyst's theories and suggest it's Road-map. This study will be useful for the developer who want to develop this kinds of games afterwards, and various functional games for the disturbed's healing as Green Contents should be more developed in the near future.

Effects of Horticultural Therapy on the Improvement of the Self-Esteem and Sociality of Mentally Retarded Persons (원예치료프로그램의 적용이 정신지체장애인의 자기주장 및 사회성 향상에 미치는 영향)

  • This study was conducted to evaluate the effect of phased horticultural therapy(HT) program on the experience of psychologic therapy and the development of job and social integration in the mentally handicapped persons. In this study, the mentally handicapped persons participated in HT program were chosen in disabled person's welfare institute of Jeonnam. HT program was performed twice a week with 1 hour activity for 4 months. As a results, the mentally handicapped persons participated in HT program showed high interest of horticulture and improvement of self-respect and the high satisfactory degree of HT program. Therefore, the mentally handicapped persons were showed the experience of both physical and mental therapy, improvement of self-esteem scale and sociality in HT program. Also, the application of HT program with continuously interest will be showed high improvement of physical, psychological and sentimental. In the course of this HT program progress, horticultural therapist and social welfare officer were showed the limitation of role. Therefore the leaders of group for successful HT program be required the comprehensive plan of more efficient HT program and induced technique of continuously up-phased improvements in HT program progress.

  • 과거 정신지체는 사회의 불가사의한 것이었으나 현재는 이들의 원인과 병리현상의 이해로 예방과 의학적 관리에 괄목할 만한 발전이 이루어져 왔다. 그러나 이들 정신지체인의 치과관리는 과거와 별다른 변화를 이루지 못한 것이 우리의 현실이다 이들에 관심을 갖고 치료하기 원하는 치과의사들도 아직 진단이나 치료방법에 이용이 제한되어있다 정신지체인 들에게도 정상화 개념을 도입해 보다 나은 건강관리 환경을 이루도록 시대가 바뀌어 가고 있다. 환자의 권리를 보장키 위해 수용시설 입소화. 물리적인 강제치료 등 기존의 일반적인 행위에서 탈피하여 이제는 행동조절의 차원에서 약물이용과 치과적용에 필요한 모든 지식의 습득이 필수적인 것으로 변화되고 있다.

  • Objectives The author aimed to investigate and understand the limitations of the biopsychosocial model for irritable bowel syndrome by reviewing the priorly reported etiologies in patients with irritable bowel syndrome(IBS). Methods The author reviewed all possible previous studies related to the investigation regarding IBS in the Pub-med & Kisep database from 1977 to 2003. Results : It was impossible that patients with IBS were diagnosed and traced to a single etiology. So, the biopsychosocial model was suggested to be a new paradigm for IBS. Biological, psychological and social factors are interplaying and contributing to the biopsychosocial model affecting each other, thus affecting the biopsychosocial model. Three factors were necessary for understanding the etiology, process of symptoms and clinical outcome of IBS by virtue of their key roles in the developing, precipitating and perpetuating of the illness. However, any treatment application for those three factors faces many obstacles to be overcome. Conclusion : The biopsychosocial model contributed relatively much to the explanation but little to the treatment application of patients with IBS. Therefore, because there seems to be little practical use at present, the author believes biopsychosocial aspects should be researched further.

Pharmacotherapy for Patients Complaining With Somatic Symptoms (신체증상을 호소하는 환자의 약물치료)

  • Patients complaining with somatic symptoms are very common in clinical practice, and are often consulted to medical department. But it is difficult to treat well. The treatment of somatic symptom disorder is multi-modal as none of the methods on their own provide a satisfactory outcome. The treatment of somatic symptoms disorders is complicated by lack of boundary, conceptual clarity, and overemphasis on psychosocial causation and effectiveness of psychological treatments. In clinical practice all classes of psychotropics are used to treat somatic symptoms disorder. Drugs such as tricyclic antidepressants, serotonin reuptake inibitors(SSRI), serotonin and noradrenalin reuptake inhibitors (SNRI), atypical antipsychotics are studied. The evidence indicates that these drugs are effective in somatic symptom disorders. All classes of antidepressants seem to be effective against somatic symptom disorders. SSRIs are more effective against hypochondriasis and body dysmorphic disorder, and SNRIs appear to be more effective than other antidepressants when pain is predominant. The author suggest that psychiatrists should know how to treat patients complaining with somatic symptoms by using not only psychotherapeutic approach but also pharmacological treatment. It will be helpful to reduce suffering and increase quality of life of these patients.

Mediating Effect of Therapeutic Relationship with Mental Health Professionals in the Relation between Empowerment and Recovery of Community Dwelling Patients with a Mental Illnesst (지역사회 정신장애인의 임파워먼트와 회복 : 치료적 관계의 매개효과)

  • This study was conducted to identify the mediating effects of the therapeutic relationship with mental health professionals between empowerment and recovery in mental illness. The participants were 168 adults in two community mental health centers and three rehabilitation centers in Jeonbuk. The data were collected by structured questionnaires from April 15 to April 30, 2015 and analyzed using descriptive statistics, Pearson's correlation coefficient, and multiple regression analysis with the SPSS 19.0 statistical program. The mean score of empowerment for mental illness, recovery and therapeutic relationship were $60.4{\pm}10.85$, $101.8{\pm}20.70$ and $74.5{\pm}14.30$, respectively. There were significantly positive correlations between recovery in empowerment and therapeutic relationship (r=0.33, p<0.001), as well as empowerment and recovery (r=0.51, p<0.001), while there were significantly positive correlations between therapeutic relationship and recovery (r=0.55, p<0.001). Therapeutic relationship had a partial mediated effect between empowerment and recovery, which was found to be significant by the Sobel test (z=3.706, p<0.001). The results of this study can be used as basic data for development of a practical recovery-enhancing program to promote therapeutic relationships and improve empowerment in relation to therapist of the mentally disabled.

The Effects of a Brief Intervention for Insomnia on Community Dwelling Older Adults (지역 사회 노인을 대상으로 한 불면증 단기 치료 프로그램의 효과)

  • Objectives: Insomnia is one of the major concerns in the elderly population. Cognitive behavioral treatment for insomnia is the first line treatment option, but there are some limitations including time and cost burdens and the requirement for sufficient cognitive resources to obtain a proper treatment effect. The Brief intervention for insomnia (BII) is a treatment that focuses on behavioral aspects of insomnia in primary care practices. The purpose of this study was to evaluate the effects of BII in community-dwelling older adults. Methods: A total of 47 older adults with insomnia were enrolled from community centers between May 2016 and January 2018. They participated in the BII program for three weeks. We gathered sleep-related participant information with using the Pittsburgh sleep quality index (PSQI), the Sleep hygiene index, and a sleep diary. Clinical efficacy was evaluated by comparing total sleep time (TST), sleep latency (SL), waking after sleep onset (WASO), and sleep efficiency (SE) before and after the treatment. Results: There was significant improvement in sleep-related features after BII. Global score and sleep quality from the PSQI, freshness, and WASO from the sleep diary showed statistically significant improvement. Conclusion: We found BII showed positive clinical efficacy in community dwelling older adults, especially from the perspective of subjective sleep quality and WASO. This finding implies that BII can be effectively applied for the managment of elderly insomnia patients in a community setting.

  • Psychosomatic medicine is also known as consultation-liaison psychiatry. Pediatric consultation-liaison psychiatry is composed of all consultation, liaison, diagnostic, therapeutic support and research activities carried out by psychiatrists and other mental professionals in pediatric ward to provide mental health services to physically ill pediatric patients. As the differences in the basic concepts of disease models between psychiatry (psychosocial model) and pediatrics(biomedical model) exist, active communication between the child psychiatrist and pediatric medical staffs is required. Although the general guidelines are similar, there are specific considerations for consultation in children and adolescents. Much work is still needed to identify empirically supported treatments which are effective for managing a board range of psychosocial difficulties in children and adolescents.
