Prescribing Pattern of Clozapine and Clinical Factors associated with Discontinuation of Clozapine (클로자핀 처방 패턴 및 클로자핀 중단과 관련된 임상 요인)

  • Objectives: Clozapine is the drug of choice in treatment-resistant schizophrenia. However, its use is often delayed and a significant proportion of clozapine treated patients fails to respond and experience potentially dangerous side-effects. The aim of this retrospective study was to describe the clinical characteristics of patients started on clozapine and the rate and reason of discontinuation of clozapine. Methods: Medical records of 83 patients started on clozapine during the period of 2012-2016 were reviewed. Results: Clozapine started on patients in chronic phase; the mean age of start was 38.1 years old and the mean number of psychiatric admission was 6.5. A majority (80.7%) of the patients had been subjected to antipsychotic polypharmacy prior to clozapine and most (61.5%) of them were being treated with polypharmacy including clozapine. Overall, 39 (47.0%) subjects had continued clozapine whereas 15 (18.1%) discontinued it; 29 (34.9%) were lost to follow-up. The most common reason for discontinuation was side-effects (n=13) including six life-threatening cases, most of which occurred within 6 months of its start. Conclusion: This study demonstrated that there is some evidence of delays to clozapine use, high rates of polypharmacy and significant rate of discontinuation during the early phase of clozapine treatment.

Clinical study on a case of a patient with a glossal tremor caused by a side effect of drugs (약물부작용(藥物副作用)으로 인한 설부(舌部) 진전환자(震顫患者) 증례보고(症例報告))

  • The most common and unacknowleged something in the secondary parkinsonism is parkinsonism caused by drug abuse. Patient had a glossal tremor caused by a side effect of GIT regulators and antispasmodics taked for the purpose of cured cholecystitis. We thought that the cause of a glossal tremor is 'Pungdam(風痰)' and gave 'Chukdamtang(滌痰湯)'. As a result this patient was taken a quick turn for the better. This case is specific. Because parkinsonism is caused by GIT regulators and antispasmodics, and the symptom of side effect is a glossal tremor. We hope that this case arouse people's attention to a tremor caused by a side effect of drugs and is useful to medical cure and prognosis.

  • This is a case report of a patient with Tourett's disorder treated by Ukgan-san. We evaluated the patient Tourett's disorder by behavior and sound. His tic behaviors like eye blinking, shoulder shrugging, head jerking, facial frimace have been shown frequently, and His phonic tic symptoms like coughing, throat clearing, sniffing have been presented frequently, too. We diagnosed his condition as Wind-syndrome Caused by Hyperactivity of Liver-yang by patient's redish face, taut pulse, Left umbilical throb. So we treated the patient with Ukgan-san. and the score of Yale Global Tic Severity Scale(YGTSS) was 30 at the start of treatment. and the score of YGTSS was 3 at the end. We found that Ukgan-san was effective for the patient with Tourett's disorder.

Psychosomatic Intervention of Delirium (섬망에 대한 정신신체의학적 중재)

  • Delirium independently contributes to poor outcomes including prolonged hospital stays and increased risk of mortality. The reported prevalence of delirium in variety of medical and surgical hospital settings is from 15% to 70% ; delirium is, therefore, one of major reason for consultation that is required for psychosomatic interventions. This article reviews the psychosomatic interventions to treat delirium including 1) identification of risk factors and precipitating causes ; 2) non-pharmacological interventions, such as modifying treatment environment and educating patient's family and care-giver ; and 3) pharmacological approaches to control the various symptoms that are frequently presenting with delirium.

A Comparative Study Concerning the Psychopathologies between the Patients with Peptic Ulcer and Those with Essential Hypertension - Focused on Medically-Ill Out Patients - (소화성 궤양과 본태성 고혈압 환자의 정신병리에 관한 비교 연구 - 내과외래환자를 중심으로 -)

  • Objectives: The authors studied the anxiety, depression and psychological characteristics of medical out-patients, which are diagnosed as peptic ulcer disorder and essential hypertension. We also examined the state of psychiatric consultation. Methods: The subjects were composed of 56 patients with peptic ulcer disorder, 44 patients with essential hypertension and 116 controls. STAI, BDI and SCL-90R was administered to all subjects. Chart review, telephone interview and Self report questionnaire of etiology and severity of illness, drug compliance and so forth were performed in disease groups. Results: Considerable depression was noticed in 39.3% among the patients with peptic ulcer disorder, 27.7% in hypertension and 12.1% in control group by BDI. State anxiety was noticed in 44.6% among the patients with peptic ulcer disorder, 54.5% in hypertension and 18.1% in control group by State anxiety. Trait anxiety was noticed in 42.9% among the patients with peptic ulcer disorder, 34.1% in hypertension and 25.8% in control group by Trait anxiety. The higher SOM, BDI, STAI, the lower drug compliance and rapport. Psychological problems are considered of the most important etiology in 48.2% of peptic ulcer group and 45.7% of hypertensive group. But psychiatric consultation is made only in 1.8% of peptic ulcer group and 2.3% of hypertensive group. Conclusions: Anxiety and depression are common phenomena in medical outpatients. In comparison with the normal control group, peptic ulcer group showed significantly higher trait anxiety and depression and hypertensive group higher state anxiety. These characteristics are related to the drug compliance and doctor-patient relationship. These results suggested the needs of active psychiatric consultation.

Effects of the Essential Oil from Modified SuHeXiang Wan (Storax Pill) in Mice after Inhalation, Oral Administration, and Inunction (가감소합향원(加減蘇合香元) 정유향기(精油香氣)의 흡입(吸入), 경구투여(經口投與) 및 피부도찰(皮膚塗擦)에 따른 진통효과(鎭痛效果) 비교(比較))

  • 소합향원 (蘇合香元) 가감방(加減方) 정유향기의 진통효과가 투여방법에 따라 어떤 차이가 있는지를 비교, 검토함으로서 본 약재의 새로운 적용방법을 탐색해 보고자 한다. 방법 : 가감소합향원에서 추출한 정유향기를 실험동물을 대상으로 7일간 경구 (50mg/kg, 100mg/kg), 흡입 (매일 3시간씩 1일 2회), 또는 피부도찰 (10mg) 방법으로 투여하여 페닐퀴논-유도 writhing test, 아세트산-유도 writhing test 및 hot-plate test를 실시하였다. 결과 : 가감소합향원 정유향기는 페닐퀴논-유도 writhing test에서는 피부도찰이 가장 효과적 이었으며 그 효과는 양성 대조약물인 아세트아미노펜 보다 약간 더 우수하였다 (p<0.05). 아세트산-유도 writhing test에서도 피부도찰법이 가장 좋은 효과를 나타내었으며 양성대조 약물인 아미노피린보다는 좀 더 좋은 효과를 보여주었다 (p<0.05). 그러나 hot-plate test에서는 경구투여가 가장 좋은 진통효과를 나타내었는데 고용량 (100mg/kg)에서는 양성 대조약물인 아세트아미노펜보다 유의적으로 더 효과적이었으나 저용량 (50mg/kg )에서는 이보다 다소 약하였다. 이상의 연구결과, 가감소합향원의 정유향기는 피부도찰시에는 비특이적 화학자극제인 페닐퀴논이나 아세트산에 의한 통증의 완화에 보다 효과적이며, 경구투여시에는 중추신경계에 영향을 미치는 hot plate에 의한 열자극성 통증에 더 유효함을 의미한다. 결론 : 가감소합향원의 정유향기액은 향기흡입보다는 피부흡수나 경구투여방법으로 진통의 목적으로 응용될 수 있을 것으로 사료된다.

The Comorbidity of Attention Deficit Hyperactivity Disorder and the Effect of Methylphenidate on it (주의력 결핍 과잉 운동 장애 환자들의 약물치료 효과 및 Comorbidity에 관한 연구)

  • The objectives of this study were to evaluate the effect of methylphenidate on attention deficit hyperactivity disorder (ADHD) and the comorbidity of the disorder, using child attention problem checklist to 56 (male: 38, female: 18) patients from, March 992 to February 1993. The results were as follows: Among 56 subjects, ADHD alone were 20 (335.71%) subjects, and with one additional diagnosis were 31 (55.35%) subjects and with two additional diagnosis were 5 (8.93%) subjects. There was significant improvement on symptoms in the scores rated by teachers in 7th and 28th day after menthlphenidate administration compared to baseline score (P<0.05) and 28th day score showed significant improvement compared to 7th day score (P<0.05) and 28th day score showed significant improvement compared to 7th day score (P<0.05). There was significant improvement on symptoms in the scores rated by parents in 28th day after methylphenidate administration improvement compared with 7th day score. In single administration of methylphenidate in Sunday morning score compared to afternoon score (P<0.05). In the administration of significant improvement on symptoms compared with the Sunday morning rating score of parents (P<0.05) and the 28th day comparison was also showed significant improvement on symptoms in the scores rated by teachers compared with the scores rated by parents on symptoms (P<0.05). Among group comparison, all groups showed significant improvements (P<0.05) except conduct disorder & oppositional defiant group.

  • Backgrounds: It was thought that sport was related with mentality. Compared to psychology, psychiatry has so far paid rather little attention to athletes. In this overview, I attempted to introduce the ideas of various observers of sports with the view of clinical psychiatry and summarized basic principle role of psychiatrist. Results and Discussions : To understand the athletic frame of mind, it is helpful to observe athletes in their psychologic development and when we do so we can quickly see trends that are not apparent by observing mature athletes alone. Psychobiology of sport would discuss some of the endogenous neurochemicals(hormone, neurotransmitter) that may be part and parcel of the athlete's psychiatric status. The psychological problems that frequently interfere with training and competition may be divided into five categories 1) choke, 2) slump, 3) injury, 4) interpersonal problem, 5) training dilemma. Psychiatric problem also arise in athletes like general people. various method for psychiatric patients like psychotherapy, group therapy, family systems perspective, and pharmacotherapy could been applied to sports psychiatry. Conclusions Although the contribution of sports psychiatry to sport is clinical one, psychiatrist's interest in role and conflict of athlete would enhance the molding of humanistic athletes who enjoy their life and sports.

  • The neuropsychiatric sequelae of traumatic brain injury are effects on complex aspect of cognition, emotion and behavior. They include problems with attention and arousal, concentration, executive function, intellectual changes, memory inpairments, personality changes, affective disorders, anxiety disorders, psychosis, apathy, aggression, and irritability. There are many useful therapeutic approaches available for people who have been brain injuries. Although a multifactioral, multidisciplinary, collaborative approach to treatment is proposed, for purposes of exposition the author have divided treatment into psychopharmacological, cognitive, behavioral, psychological, and social interventions.

  • Psychogenic cough is barky, croupy, and explosive and disappears during sleep, and is not affected by antitussive drugs. Physical and radiological examinations of the respiratory tract and other routine laboratory test are normal. It is helpful to recognize this condition early in its course so that appropriate treatment can be instituted before iatrogenic problems resulting in multiple hospitalizations and chronic steroid use occur. We experienced two child and adolescent cases that showing typical psychogenic cough so reported it with the literatures review.
