스트레스.우울이 문제음주에 미치는 영향에 관한 탐색적 연구

  • 본 연구는 서울시 지역주민을 대상으로 스트레스, 우울, 문제음주 간의 인과관계를 검증하고자 시도되었다. 본 연구는 서울시 S구이며, 전체 17개동에서 만 19세 이상인 남녀 주민을 층화집락추출법을 통해 표집된 1234명을 대상으로 하는 면접식 단면조사연구를 수행하였다. 구조방정식 모형을 사용하였으며, 측정모형에서 우울, 알코올장애의 내생잠재변수와 외생잠재변수인 스트레스 및 인구학적 변수(성, 연령, 결혼상태)를 이론변수로 설정하여 이들 간의 상관관계를 추정하였다. 또한 문제음주를 최종내생변인으로 우울을 매개변인으로 설정한 상태에서 경로분석을 실시하여 각 변인들의 상대적 중요성에 대한 평가를 시도하였다. 문제음주에 영향을 미치는 변수는 여성보다는 남성이($\beta=-6.64$, p < .001), 연령이 낮을수록($\beta=-.08$, p < .001), 스트레스가 높을수록($\beta=-.08$, p < .001), 미혼이 기혼($\beta=-1.15$, p < .05)의 순으로 경로계수가 유의하였다. 우울에 미치는 영향력은 스트레스($\beta=-.56$, p < .001)와 문제음주정도($\beta=-.15$, p < .001)의 순으로 경로계수가 유의하였다. 이상과 같이 우울이 직접적으로 문제음주에 영향을 주기보다는 스트레스로 인하여 문제음주가 유발될 수 있고 문제음주를 경유하여 우울에 영향을 미치는 것으로 드러났다. 이상의 연구결과를 종합해 볼 때 문제음주는 스트레스와 우울과 관련성이 크므로 우선적으로 스트레스를 적극적으로 관리하는 것이 중요하다. 또한 알코올과 관련한 통제정책으로 절대적인 음주량을 감소시키는 것이 필요하며, 문제음주자를 조기 발견하여 정신건강에 문제로 발전하지 않도록 하는 예방프로그램의 개발이 시급하다 하겠다.

Relationship between Somatization and Mental Health of Registered Nurses (간호사의 신체화 증상과 정신건강의 관계)

  • Objectives : To identify the relationship between somatization, stress, depression, anxiety, and psychological symptoms risk for nurses working in the intensive care unit. Create clinical evidence of psychosomatic medicine research and complement the meaning of somatization. Methods : Seventy of the mental health checkups conducted by the National Mental Health Center among the nurses using tools including Perceived Stress scale, Fatigue Severity Scale, Patient Health questionnaire-15, Korean Beck Depression Inventory, Korean Beck Anxiety Inventory, and Symptom Checklist-90-Revision. Results : 12.9% of the patients experienced more than moderate somatization. There was no statistical relationship between somatization and psychological stress perception, but feeling of anxiety and decreased self-confidence were related to the level of somatization. The group with severe somatization experienced more depression and anxiety. The group with high physical fatigue also had no statistical relationship with psychological stress perception, but had an effect on the feeling of tension, stress, or decreased control. Physical fatigue level was increased by experience of depression, not by anxiety. For psychological symptoms the higher the level of somatization, the higher the obsession and hostility was explored. In the linear regression model, stress, depression, and anxiety accounted for 39.3% of somatization and 16.1% of physical fatigue symptoms. Conclusions : We can estimate the decrease in stress cognitive symptoms, accompanying depression and anxiety, compulsion and hostility as characteristics of somatization. The causal relationship between somatization and psychological symptoms cannot be confirmed in this study, but the interrelationships are observed, can be referred to mediation strategies.

  • 대한민국은 고령 사회로 진입하고 있을 정도로 노인인구가 빠르게 증가하고 있다. 이에 따라 각종 노인성 질환 역시 빠르게 늘어나고 있어 이것은 개인뿐 아니라 가족과 사회에 커다란 고통과 부담을 주고 있다. 그중에서도 우울증은 노인정신 질환 중에 가장 흔하며 노인자살의 원인이 되는 등 심각한 사회문제가 되고 있다.

Factor Influencing Depression in Eco Boomer (에코붐 세대의 우울에 영향을 미치는 요인)

  • The purpose of this study was identify the factor affecting depression in eco boomer. This study used the data from the $7^{th}$ Korea National Health and Nutrition Examinations Survey and identified factor affecting depression in adult aged 24 to 37. Data was analyzed using the IBM SPSS 25.0 program. As a result, there were significant differences according to sex, education level, house ownership, health insurance, economic activity status, status of workers, position of servant, working time system, employment condition, perceived health status, smoking, work related physical activity, suicidal plan, suicidal attempt, stress, frequency of meal, companion for meal. As a result of regression analysis, influencing factors of depression in eco boomer were sex, employment condition, perceived health status, smoking, stress, work related physical activity, suicidal plan, companion for dinner. According to the results of this study, depression of the echo boomer should be improved by considering sex, work related characteristics and life style of eco boomer.

Mental Health in Adolescents with Allergic Diseases-Using Data from the 2020 Korean Youth's Risk Behavior Web-based Study (알레르기 질환 청소년의 정신건강: 2020 청소년 건강 행태 온라인 조사 활용)

  • This study aims to understand the actual conditions of adolescents with allergic diseases and factors related to mental health. It is intended to provide basic data for disease management. As for the research method, big data of the 16th Youth Health Behavior Survey were analyzed secondary, and x2-test was used using the SPSS 25 program8, odds ratio and 95% confidence interval were calculated to determine the degree of association, and multi-logistic regression analysis was performed. Studies have shown that 37.1% of adolescents with allergic diseases have stress perception, 27.6% of depression experience rate and 12.5% of suicidal thoughts, and the likelihood of stress perception, depression experience, and suicidal thoughts among adolescents with allergic diseases was higher. As a result of this study, it was found that the presence or absence of allergic diseases in adolescents has a significant effect on mental health, and we intend to use it as basic data for prevention and research on diseases that can lead to mental illness and adolescent mental health.

Moderating Effects of Public Health Service Utilization Rate between Public Health Resources and Community Mental Health (지역사회 공공보건자원과 지역사회 정신건강의 관계에서 공공보건서비스 이용률의 조절효과)

  • This study aimed to identify the moderating effect of public health service utilization rate between public health resources and community mental health. The subjects included 144 local communities, from the data of 6th Community Health Plan and the 2015 Community Health Survey. Public health resources were measured by public health budget, public mental health budget and public mental health personnel; and mental health was measured by rate of perceived stress, rate of depressive mood and suicide rate. The hierarchical regression analysis was used to identify the moderating effects. The results were as followed. First, the effect of public mental health budget on rate of depressive mood was moderated by public health service utilization rate. Second, the effect of public mental health personnel on rate of depressive mood was moderated by public health service utilization rate. Third, the effect of public mental health personnel on suicide rate was also moderated by public health service utilization rate. Fourth, the effect of public health resources on mental health differed between the groups with high and low public health service utilization rate. In improving community mental health, the measures to improve the public health service utilization rate should be considered to ensure that reinforcing public health resources leads to the improvement of community mental health.

The Association Between Socioeconomic Changes and Adolescent Mental Health After COVID-19 Pandemic (코로나19이후 사회 경제적 변화와 청소년 정신건강의 연관성)

  • Objectives : The purpose of this study is to investigate depression and suicide ideation according to socioeconomic changes after COVID-19 among Korean adolescent. Methods : Data on the study population were obtained from the 16th Korea Youth Risk Behavior Web-based Survey (KYRBS). The KYRBS is a nationally representative sample of Korean adolescents (aged 12-18 years) that originally included over 103 questions in 15 domains of health-risk behaviors. In the 16th KYRBS, a total 54,948 students from 793 schools responded to the survey. Chi-square test and logistic regression analysis were conducted regarding depression and suicide ideation. Results : This study suggests that changes in the family household before and after COVID-19 pandemic are also affecting the mental health of the adolescents. The study shows that worse change of family household is significant associations with suicidal ideation and depression. Adolescents reporting worse (AOR 1.38; 95% CI 1.38-1.57) and much worse (AOR 2.07; 95% CI 1.87-2.29) were significantly more likely to report depression. Adolescents reporting worse (AOR 1.34; 95% CI 1.34-1.60) and much worse (AOR 2.01; 95% CI 1.76-2.29) were significantly more likely to report suicide ideation. Conclusions : In this study, it was confirmed that young people from socially disadvantaged backgrounds are at high risk of suicide ideation and more depression. The results of this study suggest that we should consider improving the screening and prevention of mental health problems for adolescents with poor socioeconomic changes of COVID-19.

Association between Depression and Sociodemographic Factors of Elderly Welfare Facility Users in a City (일도시 노인복지시설 이용자들에서 우울증과 인구사회학적 요인과의 연관성)

  • Objectives : As the prevalence of elderly depression increases, it becomes urgent problem to provide preventive and management measures. But in practice, it is difficult to detect depression in early stage. The purpose of this study is to evaluate the association between sociodemographic factors and depression in elderly welfare facility users in a city. Methods : In this research, the severity of depression and sociodemographic factors(gender, age, education, co-morbid disease, housing type, number of children, number of family members living with the subjects) was evaluated through PHQ-9 and interview. Using the data, the associations of depression and sociodemographic factors were analyzed. Based on PHQ-9 depression cut-off value(10 points), the subjects were divided into two groups, and the difference of the variables between groups were analyzed statistically. Results : 'Own house'(YES/NO), education(less than elementary school graduation/more than elementary school graduation) were sociodemographic factors which showed significance difference in mean PHQ-9 scores. Group with Over PHQ-9 10 points showed less 'having their own house' and less education level than group with less PHQ-9 10 points. Conclusions : With this research, it is expected that the risk factors for the elderly depression can be understood and the measures for early detection and invention of elderly depression would be provided.

The Effect of Spiritual Well-being on the Mental Health of the Cho-Sun Tribal Women Residing in P.R. of China (중국거주 조선족 여성의 영적 안녕정도가 정신건강에 미치는 영향)

  • Background: Spirituality has been an important part of Transpersonal Psychology and is believed to have a large effect on the mental health because it has been systematized. The aim of this study was to determine the level of spiritual disposition on human beings along with its effects on one's mental health. Materials and Methods: The study targeted 400 women residing in Youn-Gil city of JiLin Prov., which is a district of the Cho-Sun tribe in China. Their spiritual well-being was studied using the Spiritual Well-being Scale-Korean Version. The spiritual well-being scale consists of 2 sub-scales of religious well-being and existential well-being. The study was evaluated using a lie scale, psychotic trend, and a combined anxiety-depression scale. The results were considered to be factors of one's mental health. The correlation between the spiritual well-being and each tendency was analyzed by regression analysis. Results: The total score of the Cho-Sun tribal women according to the spiritual well-being scale was 68.29 which was much less than the 100.65 of Korean Christian women. There was no significant correlation between the spiritual well-being and the Lie trend. However, it was found that 86%(344) of Cho-Sun tribal women scored above 70 in the Lie trend with a mean score of 74.57 which is higher than normal populations. Regarding the correlation between the spiritual well-being and psychotic trend, the psychotic trend became significantly higher when the religious well-being was at a high level. On the other hand, the psychotic trend became significantly lower when the existential well-being was at a high level. Regarding the correlation between the spiritual well-being and anxiety, the anxiety was significantly higher when the religious well-being was at a high level. However, the anxiety level was significantly low when the existential well-being was at a high level. Regarding the correlation between the spiritual well-being and depression, the depression level was somewhat significantly high when the religious well-being was at a high level. However, the depression level was significantly low when the existential well-being was at a high level. Conclusion: This study evaluated the effects of spiritual well-being on a person's mental health among Cho-Sun tribal women in Youn-Gil city of JiLIn Prov., P.R. of China. The results found that the religious well-being, which is a sub-scale of spiritual well-being, had negative effects while the existential well-being had positive effects on the mental health. These results proved that a person's religious disposition had negative effects on their mental health in a communitarian society.
