인간면역결핍바이러스 음성 환자에서 뇌신경 마비, 뇌경색 및 수막척수염으로 발현한 신경매독의 자기공명영상 소견 (Meningovascular and Spinal form of Neurosyphilis Presenting as Multiple Cranial Nerve Palsy, Cerebral Infarction and Meningomyelitis in a Human Immunodeficiency Virus Negative-Patient: MR Imaging Features)

  • 신경매독은 스피로헤타(spirochete)인 Treponema pallidum에 의한 뇌와 척수의 드문 감염증이다. 저자들은 53세 남자환자에서 시신경, 삼차신경, 안면신경, 전정와우신경, 중뇌동맥, 후뇌동맥, 척수와 척수막을 침범한 복합적인 수막혈관성, 수막척수염 형태의 신경매독을 경험하여 이를 보고하고자 한다. 본 증례를 통해 뇌수막염과 더불어 뇌신경염, 뇌혈관염이 있고 수막척수염을 동반하는 복합적인 중추신경계 이상 소견이 있어 임상적인 진단이 어려운 경우 신경매독을 감별진단으로 고려하여 영상 소견을 바탕으로 한 빠른 진단이 필요함을 강조하고자 한다.

  • Vertigo is a subtype of dizziness in which there is an illusion of movement, most commonly rotation. Acute peripheral vestibulopathy is one of the most common clinical neurologic syndromes at my age with the acute onset of vertigo, nausea, and vomiting lasting for several days and not associated with auditory or neurologic symptoms. Most affected patients gradually improve over one to two weaks, but some develop recurrent episode. We treated two patients suffered from vertigo, nausea & vomiting, nystagmus, headache with herbal medicine, acupuncture, cupping-treatment. After treatment we observed the improvement. Based on this study, it is considered that oriental medical treatment can be applied to the management of vestibular neuronitis.

청심연자탕으로 호전된 태음인 전정신경염 환자 치험 1례 (A Case Report of Taeumin Patient with Vestibular Neuronitis Treated with Cheongsimyeonja-tang)

  • Objectives This study was aimed to report significant improvement of vestibular neuronitis treated with Cheongsimyeonja-tang. Methods The patient was treated with Sasang herbal medicine for vestibular neuronitis. Also acupuncture and pharmacopuncture were used to treat neck and shoulder pain. Numeral rating scale(NRS), Romberg test, Spontaneous nystagmus, Induced nystagmus and Head thrust test were used to observe changes of dizziness in vestibular neuronitis. Results and Conclusions After 41days of treatment, the patient showed improvement in NRS. Also, the patient's symptoms of nausea, dyspepsia, sleep disturbance were improved. This study shows that Cheongsimyeonja-tang might be effective in Taeumin patient's vestibular neuronitis.

양성 돌발성 체위 현훈으로 이행된 급성 전정신경염 환자의 한양방 협진 치료 1례 (Case of Combined Eastern-Western Medicine Treatment on a Patient Diagnosed with Benign Paroxysmal Positional Vertigo after Acute Vestibular Neuritis)

  • Objective: This clinical study was performed with a female patient to evaluate the effects of combined Eastern-Western medicine treatment on benign paroxysmal positional vertigo after acute vestibular neuritis.Method: We used acupuncture, herbal medication, Western medication, fluid therapy, and the canalith repositioning maneuver to treat a female patient suffering from dizzy spells. The vertigo scale was checked to assess any improvement in symptoms.Results: The patient had originally recovered from acute vestibular neuritis, but after the first discharge, she came down with benign paroxysmal positional vertigo and had to be rehospitalized. She eventually got better, however, and her score on the vertigo scale improved.Conclusion: This study demonstrates that combined Eastern-Western medicine treatment may be an effective option for treating benign paroxysmal positional vertigo after acute vestibular neuritis, despite the difficulty of the treatment in this case.

비위허약으로 변증된 전정신경염 환자의 삼출건비탕가감방을 포함한 한의 치료 치험 1례 (A Case Report of Korean Medicine Treatment Including Samchulgunbi-tang-gagambang in a Patient with Vestibular Neuritis)

  • Background: Vestibular neuritis is a common cause of acute unilateral peripheral vestibulopathy. Vestibular neuritis is the second most common disease among patients with dizziness. Clinical symptoms of vestibular neuritis include the sudden onset of vertigo with spontaneous nystagmus, unsteady gait, nausea, and vomiting that last from days to weeks. However, even after the vertigo disappears, difficulty maintaining balance while walking may persist for weeks to months. Antihistamines, serotonin receptor blockers, and benzodiazepine vestibular suppressants are widely used as symptomatic treatments to reduce the severity of symptoms that occur in the acute phase. Case Summary: A patient diagnosed with acute vestibular neuritis was treated with acupuncture, moxibustion, and herbal medicine. We used the visual analog scale (VAS) to assess each symptom and the vertigo score to observe the effect of treatment. After treatment, the VAS scores for each symptom and the vertigo score decreased, and the severity of nystagmus was reduced. Conclusion: This study suggests that Korean medicine treatments, including Samchulgunbi-tang-gagam, could be effective in improving the clinical symptoms of vestibular neuritis.
