Short-term Results of Medial Displacement Calcaneal Osteotomy for Flexible Flatfoot (유연성 편평족에 대한 내측 전위 종골 절골술의 단기 추시 결과)

  • Purpose: To evaluate the short-term results of medial displacement calcaneal osteotomy without flexor digitorum longus transfer for flexible flatfoot deformity. Materials and Methods: Twenty four patients (25 feet) who had undergone medial displacement calcaneal osteotomy without flexor digitorum longus transfer for flexible flatfoot between July 2004 and May 2007 were included. The mean age was 43.6 years (16~78 years). The mean follow-up was 26 months (14~50 months). Clinical outcomes were assessed using American Orthopaedic Foot and Ankle Society (AOFAS) score and visual analogue scale (VAS). Six radiographic parameters were measured from weightbearing radiographs to assess the difference between preoperative and postoperative radiographs. Results: The mean AOFAS score improved from 57.9 points preoperatively to 89.2 points at latest follow-up (p=0.000). The mean VAS improved from 62 points preoperatively to 23 points at latest follow-up (p=0.000). The mean talonavicular coverage angle on anteroposterior (AP) view changed from 20.2 degrees to 16.0 degrees (p=0.002). The mean calcaneal pitch angle on lateral view changed from 12.6 degrees preoperatively to 14.5 degrees at latest follow-up (p=0.001). Regarding these radiographic parameters, the difference between interobserver measurements was larger than that between pre- and post-operative measurements. The calcaneus was transferred medially by average 11.8 mm (p=0.003), which was 27.9% of the width of calcaneal tuberosity (p=0.000). The mean talo-first metatarsal angle on AP and lateral views, and navicular height on lateral view were not significantly changed. Conclusion: Medial displacement calcaneal osteotomy without flexor digitorum longus transfer for flexible flatfoot could lead to improve the clinical outcomes, although the restoration of medial longitudinal arch was not clinically significant.

Recovery of Silicon from Silicon Sludge by Electrolysis (실리콘 슬러지로부터 실리콘의 전해회수(電解回收))

  • As a recovery of elemental silicon from the sludge of Si wafer process, a process of mechanical separation-chlorine roasting-electrolysis has been suggested. The silicon sludge consisted of Si, SiC, machine oil, and metallic impurities. The oil and metal impurities was removed by mechanical separation. The Si-SiC mixture was converted to silicon chloride by chlorine roasting at $1000^{\circ}C$ for 1 hr and the silicon chloride was dissolved into an ionic liquid of $[Bmpy]Tf_2N$ as an electrolyte. Cyclic voltammetry results showed an wide voltage window of pure $[Bmpy]Tf_2N$ and a reduction peak of elemental Si from $[Bmpy]Tf_2N$ dissolved $SiCl_4$ on Au electrode, respectively. The silicon deposits could be prepared on the Au electrode by the potentiostatic electrolysis of -1.9 V vs. Pt-QRE. The elemental silicon uniformly electrodeposited was confirmed by various analytical techniques including XRD, FE-SEM with EDS, and XPS. Any impurity was not detected except trace oxygen contaminated during handling for analysis.

Significance of Intraoperative BAEPs Monitoring during Microvascular Decompression Surgery (미세혈관 감압술중 뇌간청각유발전위 감시장치의 유용성)

  • Objective : Surgery for the microvascular decompression is mostly concerned with injury to the cranial nerves or brain stem by cerebellar retraction. Intraopeartive brain stem auditory evoked potentials(BAEPs) has been continuously monitored on surgery to evaluate the extent of injury, recovery of the nerves and prognosis. Methods : Of the 161 cases of CP angle surgery from Feb. 1996 to Apr. 1998, 103 cases were monitored during operation. Thirty five patients who had undergone similar surgery were selected and evaluated ; 23 patients were monitored and 12 were not during surgery. If monitor showed more than 0.5 mSec delay of latency, surgeon was given a warning not to retract brain any more. If more than 1mSec delay, surgeon was informed to stop surgery and wait for the returning of evoked potentials. The level of amplitudes and delay of latencies during the initial stage of operation, opening the dura, insertion of teflon patches, and closing the dura and recovery were then compared. Resuls : Twenty patients were male and 15 were female. Their average age was 50.26 years. Mean amplitude during the initial stage of operation was $0.60{\pm}0.25mV$, at opening the dura $0.56{\pm}0.26$, after teflon patches insertion $0.49{\pm}0.20$, and after closure of dura $0.47{\pm}0.28mV$. Mean latency during the early stage of operation was $6.08{\pm}0.67mSec$, at opening of dura $6.38{\pm}0.55$, insertion of teflon $6.97{\pm}0.59$, and closing the dura $6.17{\pm}0.54$. There was statistical significance in the difference of amplitudes between each procedures, and in the difference of latencies. For the complete recovery of amplitude and latency, it usually took average 5.65 minutes(0-20 min). In monitored group, only one patient required more than 20 minutes to recover and suffered from hearing disturbance after surgery. Others were recovered within 10 minutes without complications. However, 4 out of 12 patients who were not monitored showed hearing disturbance, and 1 patient had temporary facial palsy and dizziness(p=0.000). Conclusion : The results indicate that continuous intraoperative monitoring of BAEPs during CP angle surgery is seen mandatory procedure to prevent operative complications.

  • This research is aimed for the better understanding of corrosion behavior of carbon steel in ETA or $NH_3$ solution at high temperature. To minimize the corrosion it is important to select proper pH control agent(s), which also let it maintain basic or reductive environment inside the steel pipe. This work will provide the practical guideline which can be applied to the nuclear power plant for developing the life extension method of carbon steel. Experiments were carried out by measuring the corrosion rate using the potentiodynamic polarization curve of carbon steel in ETA and NH3 at different temperatures. The corrosion rate was estimated by using the Tafel curve measured under various test conditions: i) $NH_3$ is less than ETA as a pH agent. ii) the corrosion was reduced at high pH, and iii) the corrosion was maximum in the temperature range of $150\sim200^{\circ}C$.

Arthroscopic Cannulated Screw Fixation Technique for Avulsion Fracture of the Intercondylar Eminence of the Tibia (삽관 나사못(Cannulated screw)을 사용한 경골 과간 융기부 견열 골절의 관절경적 치료 기법)

  • Avulsion fractures of the intercondylar eminence of the tibia are not uncommon. In the displaced avulsion fracture, anatomical reduction and firm fixation of fracture fragments are needed but the most of the conventional operative techniques including arthroscopic technique are relatively complex and need. The results were not always satisfactory due to the risk of postoperative complications such as wound infection, premature epiphyseal closure and loss of fixation after early motion etc. So we describe a simple and safe modified method of arthroscopic reduction and fixation for avulsion fractures of the intercondylar eminence of the tibia. In our thirteen cases, we achieved anatomical reduction and secure fixation using cannulated screw through the three arthroscopic portals (anterolateral, medial mid-patellar and central). Postoperatively, immediate limited range of motion of the knee and partial weight bearing were possible. Additional use of the washer afforded safe fixation of comminuted avulsion fracture. The advantage of this technique includes its technical simplicity, easy removal of hardware, ability to treat comminuted type IV fracture with washer, no additional skin incision, no damage to growing plate in growth children and less morbidity.

Hetzer Technique for Surgical Correction of Ebstein's Anomaly (Hetzer 술기를 이용한 엡스타인 기형의 수술적 교정)

  • Background: The Hetzer procedure for the correction of Ebstein's anomaly has the advantages of technical feasibility and incorporation of the atrialized right ventricle (RV) into the functional RV. Material and Method: We preformed a retrospective review of 11 patients with Ebstein's anomaly and they had undergone a Hetzer procedure between March 2002 and December 2006. Result: The median age at operation was 19.8 years (range: 6 months ${\sim}56$ years). There were 4 males and 7 females. All patients showed severe tricuspid regurgitation (TR) preoperatively, and arterial desaturation (<95%) was present in 3 patients. The original Hetzer technique was employed in 6 patients with the Carpentier type A anomaly. In the remaining 5 patients with the Carpentier type B or C anomalies, valve competence was restored at the level of the displaced tricuspid valve mechanism. Adjunct bidirectional cavo-pulmonary shunt, or one and a half ventricle repair strategy was employed for all the patients. The median follow-up was 8.6 months (range: $0.8{\sim}51.9$ months). There was no early or late death, and the immediate postoperative TR was trivial to mild in 8 patients. The median cardio-thoracic ratios on chest X-ray at the preoperative period and at postoperative 0, 1 and 6 months were 65%, 62%, 55% and 55%, respectively. Conclusion: The original or modified Hetzer procedure for Ebstein's anomaly shows excellent intermediate-term outcomes.

Arthroscopic Reconstruction of Posterior Cruciate Ligament with Achilles Tendon Allograft (동종 이식 아킬레스 건을 이용한 관절경적 후방 십자 인대 재건술)

  • Purpose : We have evaluated the outcome of the operative treatment of PCL rupture using the achilles tendon allograft. Materials and Methods : A retrospective study was completed for 30-PCL reconstruction cases, using the achilles tendon allograft from september 1996 to march 1998. There was an average follow up of 24 months, with range of 12 to 50 months. Results : The active range of motion was improved postoperatively. The Lysholm Knee Score was improved from a mean of 54 points preoperatively to 87 points postoerative 18 months. The "Cybex 340 isometric test" revealed satisfactory results compared with autograft. Conclusions : Use of allograft for recostruction of the PCL is an attractive option because it precludes the harvesting of autogenous tissue in a knee that is already at risk for patellofemoral and tibiofemoral osteoarthrosis.

  • Soft carbons are prepared by heat-treatment of cokes with different amounts of phosphoric acid (2, 4.5, and 10 wt% vs. cokes) at $900^{\circ}C$ to be used as anode materials for lithium ion batteries. From electrochemical measurements combined with structural analyses, we confirm that abundant nano-pores are existed in the microstructure of soft carbons prepared with the phosphoric acid, which are responsible for further lithium ion storage. Significant increase in reversible capacity of soft carbon is attained in proportion to added amount of the phosphoric acid. We also demonstrate the effect of structural modification with phosphoric acid on electrochemical performance of soft carbon to elucidate the origin of additional capacity.

  • Learning for thinking says the behavior of the organism changes as a result of practice or experience. It is very difficult to identify focusing ability objectively when students study. But, brain of the body is not so. EEG signal means continuously electric records of brain potential variation between two points on the scalp when brain activities take place. In types of EEG, there are delta(0~4Hz), theta(4~8Hz), alpha(8~13Hz), beta(13~30Hz) and gamma waves(30~50Hz). SMR waves and Mid-beta waves appear when focused for studying. Part for the most influence on concentrating reported that Mid-beta waves. In relation to brain activities, EEG has been actively researched for evaluating brain focus index system during learning and study. So, By using Biopac system for this study, measured brain wave was converted into FFT for extracting Mid-beta domain signals that are related to learning after giving focus invoked subjects to a small number of people. When concentrating, we measured the change in the power of the Mid-beta frequency domain and presented a correlation. Based on these results, we analyzed whether students are concentrated objectively on learning or not. and hope to offer more efficient learning method.

Electrochemical Reduction of Perchlorate Using Mercury Film Electrode (수은 막전극을 이용한 수용액 중 과염소산이온의 전기화학적 환원)

  • A method for electrochemical degradation of the perchlorate anion ($ClO_4{^-}$) using mercury film electrode has been studied. Electrochemical method has relatively simple pre-treatment. However, electrochemical method should avoid interference from hydrogen evolution at the applied potential to degradation of perchlorate ion, and thus applied electrode should have large hydrogen overvoltage which suppressed the hydrogen evolution at the working reduction potential to prevent hydrogen evolution. In this study, we used mercury film electrode as a working electrode which has a large overvoltage. Ag / AgCl (sat. NaCl) was used as a reference electrode, and platinum was used as a counter electrode. Mercury film electrode was made by cyclic voltammetry (CV) method. The deposition time was decided as 10 minute, and the stability of the mercury electrode in perchlorate solution was confirmed by CV. The reduction potential of perchlorate was checked by using CV method, and decomposition of perchlorate was performed by using chronoamperometric (CA) method. Also, ion chromatography (IC) was used to confirm the degradation rates of perchlorate.
