  • The diagnosis and treatment plan for forming ideal occlusal plane in full mouth rehabilitation are difficult because each process is complicated and information exchange between dentist and technician is subjective. The OP finder® system simplifies this process and helps to deliver more objective and accurate information. In this case, full mouth rehabilitation was performed using OP finder® system for patients with old bad fixed prosthesis and severely worn mandibular teeth, and reported that the result of proper occlusal plane setting and masticatory function recovery was obtained.

  • Patients often seek rehabilitative treatment for severely worn dentition. The etiology of noncarious tooth wear due to attrition, abrasion, and erosion is multifactorial. To treat the worn dentition, it is important to identify and eliminate and/or control the factors that contribute to excessive wear. Many situations requiring complete mouth rehabilitation present with the challenge of a lack of restorative space. To establish a correct occlusal plane and space for prostheses, it is necessary to increase vertical dimension. This may require an increase in occlusal vertical dimension. Also clinicians should be able to choose the appropriate restorative materials to achieve excellence in natural esthetics as well as proper biomechanics and durability. This article presents a method for altering occlusal vertical dimension to restore dentitions with limited restorative space due to loss of tooth structure.

  • Patients who miss teeth partially or fully show many changes which make them lose function and esthetics. From the esthetic point of view, loosing teeth makes lower face unharmonized. There are various changes of lower and whole face according as how much change oral cavity is. Restoring the multiple teeth missing properly can make patient's face harmonized. Especially full mouth implant restorations can cause drastic occlusal change affecting masticatory muscles. Because all the muscles are connected closely, the masticatory muscles which is part of lower facial muscles can cause whole muscle change. In full mouth implant restoration case, I will show the whole face muscle change harmonized by meticulous occlusal treatment process. Full mouth restorations installed in right way show whole face muscle changes extending to head and neck muscles.

  • Gradual teeth wear is normal physiological progress, severe tooth wear can causes dysfunction, occlusal plane disharmony and esthetic problems. If additional space is needed for prosthetic restoration due to severe attrition, full mouth rehabilitation with increase of occlusal vertical dimension may be considered. In this case, 72 year old male patient with severe worn dentition, irregular gingival zenith and deep anterior bite was treated by full mouth rehabilitation for regaining the space for restoration, improving relationship and esthetic of anterior teeth. Provisional restoration obtained by systemic analysis, diagnosis and re-evaluation for a sufficient time was replicated to final restorations through double scanning technique using monolithic zirconia blocks. Satisfactory functional and esthetic outcomes were obtained.

  • The progressive attrition of teeth is a normal process by aging. However, excessive tooth wear with decreased vertical dimension of occlusion and collapse of occlusal plane may cause pathologic pulpal condition, occlusal disharmony and functional disorders. In this case, a patient with severely worn dentition was treated. Diagnostic wax-up was performed at the increased vertical dimension. After evaluation of provisional restorations for 12 weeks, final restorations were fabricated with zirconia crown and routine clinical assessments were made. Esthetically and functionally satisfactory results were obtained.

  • Excessive pathological attrition over the entire dentition can cause decreased masticatory function, and pathological problems of the temporomandibular joint and the muscular nervous system. In this case, 71 year-old male patient with severe attrition and fracture across the entire tooth was treated by full mouth rehabilitation for regaining space of restoration. In order to determine the appropriate vertical dimension of the patient's occlusion, facial appearance, aesthetics, restoration space were evaluated, and provisional restoration of the raised vertical dimension was made. After 4 months of evaluation, the entire restoration of monolithic zirconia crown using CAD-CAM was completed. Through the above process, satisfactory aesthetic and functional results were obtained.

  • Severe dental attrition causes pathological changes of the tooth, imbalanced occlusion, and functional and aesthetic complications and can also result in a decrease in occlusal vertical dimension that can incur temporomandibular joint and muscular complications. Before restoring the vertical dimension with full-mouth prosthetic restorations, it is important to determine the amount of vertical lifting through complete diagnosis. In this study, a 59 year-old male patient with generalized attrition and fracture of teeth was treated with full-mouth zirconia prosthetic restoration in order to recover vertical dimension and aesthetics. Through the analysis of physiologic rest position and inter-canine distance, the treatment was planned for lifting 3 mm in vertical dimension. Interim crown were fabricated after full-mouth wax up, having the patient use for 6 months. The final monolithic and bilayered zirconia restorations were completed. The patient showed satisfaction in function and aesthetics for 18 months of follow-up since delivering the final restorations.

  • Excessive tooth wear can lead to decrease in occlusal vertical dimension and can cause pathological changes in the oral environment and masticatory system. When recovering occlusal vertical dimension and occlusion, accurate diagnosis and analysis are essential. This clinical case describes a 75-year-old woman with severely worn dentition due to loss of the posterior support. Full mouth rehabilitation with occlusal vertical dimension increment was planned. Clinical and radiographic examinations, occlusal vertical dimension evaluation, and diagnostic wax-up were performed and patient adaptability was evaluated using provisional restorations. As for definitive restoration, considering economic condition of the patient, removable partial denture was fabricated and solitary implants were placed in the mandibular left and right posterior region to increase support and retention of the removable partial denture. During one year of follow-up, functional and esthetic outcomes were observed satisfactory.
